Carbonate apatite (CO3Ap) granules are useful as a bone substitute because they can be remodeled to new natural bone in a manner that conforms to the bone remodeling process. However, reconstructing large bone defects using CO3Ap granules is difficult because of their granular shape. Therefore, we fabricated CO3Ap honeycomb blocks (HCBs) with continuous unidirectional pores. We aimed to elucidate the tissue response and availability of CO3Ap HCBs in the reconstruction of rabbit mandibular bone defects after marginal mandibulectomy. The percentages of the remaining CO3Ap area and calcified bone area (newly formed bone) were estimated from the histological images. CO3Ap area was 49.1 ± 4.9%, 30.3 ± 3.5%, and 25.5 ± 8.8%, whereas newly formed bone area was 3.0 ± 0.6%, 24.3 ± 3.3%, and 34.7 ± 4.8% at 4, 8, and 12 weeks, respectively, after implantation. Thus, CO3Ap HCBs were gradually resorbed and replaced by new bone. The newly formed bone penetrated most of the pores in the CO3Ap HCBs at 12 weeks after implantation. By contrast, the granulation tissue scarcely invaded the CO3Ap HCBs. Some osteoclasts invaded the wall of CO3Ap HCBs, making resorption pits. Furthermore, many osteoblasts were found on the newly formed bone, indicating ongoing bone remodeling. Blood vessels were also formed inside most of the pores in the CO3Ap HCBs. These findings suggest that CO3Ap HCBs have good osteoconductivity and can be used for the reconstruction of large mandibular bone defects.

Since bone apatite is a carbonate apatite containing carbonate in an apatitic structure, carbonate content may be one of the factors governing the osteoconductivity of apatitic bone substitutes. The aim of this study was to evaluate the effects of carbonate content on the osteoconductivity of apatitic bone substitutes using three commercially available bone substitutes for the reconstruction of alveolar bone defects of a beagle mandible with simultaneous dental implant installation. NEOBONE®, Bio-Oss®, and Cytrans® that contain 0.1 mass%, 5.5 mass%, and 12.0 mass% of carbonate, respectively, were used in this study. The amount of newly formed bone in the upper portion of the alveolar bone defect of the beagle’s mandible was 0.7%, 6.6%, and 39.4% at 4 weeks after surgery and 4.7%, 39.5% and 75.2% at 12 weeks after surgery for NEOBONE®, Bio-Oss®, and Cytrans®, respectively. The results indicate that bone-to-implant contact ratio was the largest for Cytrans®. Additionally, the continuity of the alveolar ridge was restored in the case of Cytrans®, whereas the continuity of the alveolar ridge was not sufficient when using NEOBONE® and Bio-Oss®. Both Cytrans® and Bio-Oss® that has a relatively larger carbonate content in their apatitic structure was resorbed with time. We concluded that carbonate content is one of important factors governing the osteoconductivity of apatitic bone substitutes.

Carbonate apatite (CO3Ap) foam has gained much attention in recent years because of its ability to rapidly replace bone. However, its mechanical strength is extremely low for clinical use. In this study, to understand the potential of gelatin-reinforced CO3Ap foam for bone replacement, CO3Ap foam was reinforced with gelatin and the resulting physical characteristics were evaluated. The mechanical strength increased significantly with the gelatin reinforcement. The compressive strength of gelatin-free CO3Ap foam was 74 kPa whereas that of the gelatin-reinforced CO3Ap foam, fabricated using 30 mass % gelatin solution, was approximately 3 MPa. Heat treatment for crosslinking gelatin had little effect on the mechanical strength of the foam. The gelatin-reinforced foam did not maintain its shape when immersed in a saline solution as this promoted swelling of the gelatin; however, in the same conditions, the heat-treated gelatin-reinforced foam proved to be stable. It is concluded, therefore, that heat treatment is the key to the fabrication of stable gelatin-reinforced CO3Ap foam.

Large bone defect still represent a major problem in orthopedics. A tissue engineering approach has been proposed where osteogenic cells, bioceramic scaffolds and growth factors can play in a role to the bone repair. Bone consist a mineral phase such as carbonate apatite and an organic phase such as collagen. Synthetic carbonate apatite ceramics are considered as promising alloplastic materials for bone substitute. Chitin is the organic matrix of the hard parts of exoskeleton of insect, crustacean and present in a small amounts in mushrooms. It is an insoluble, similar to cellulose and composed of N-acetylglucosamine unit. Partial deacetylation from chitin result in the formation of chitosan. Chitin's properties as a flexible and strong material make it favourable as surgical thread. It has novel properties such as biocompatibility, biodegradability, anti bacterial, wound healing activity, tissue regeneration and hemostatic activitities. The composit from carbonate apatite and chitosan may have a great impact on human health care system as bioresorbable bone substitute. The aim of the study was to evaluate the newly bone formation on the bone healing of defect tibia treated with carbonate apatite-chitosan bone substitute. Eighteen Sprague Dawley rats, male, 3 months, weighing 250-300g used in this study. Bilateral defect were created in each tibia rat. The defects were filled with carbonate-apatite chitosan bone substitute. The rats were sacrificed after respectively 1, 2 and 3 weeks. The result of this study showed that carbonate apatite-chitosan significantly increased a number of osteoblast (p<0.05). Carbonate apatite-chitosan group showed that matrix deposition faster than the other groups and have a good interface with the old bone. These data indicate that carbonate apatite-chitosan are potential candidate for bone substitute

Our research aimed to find the ideal scaffold for bone regeneration, focusing on Calcium and Phosphate combination. In this study, Plaster of Paris (POP) was combined with CHA and implanted in femoral condyles of rats. According to the experimental result, it can be concluded that there was no significant difference in response to the implantation of POP and POP-CHA in Sprague Dawley rat femur condyle (p<0.05). It can be stated that both POP-CHA and POP shows similar trait in bone healing.

The in vitro formation of carbonate apatite in solutions with ion concentrations comparable to those in human serum was studied. The composition and morphology of the resulting apatite precipitate displayed a hierarchical assembly of elongated plate-shaped nanocrystals of carbonate apatite analogous to previously characterized bioapatites formed in The main conclusion is that so-called bioapatites may form in vitro and that precipitation inhibitors most likely are essential for the prevention of spontaneous calcification at normal human serum ion concentrations., QC 20100525.
