Purpose: A prerequisite for successful pain management is identifying the pain and assessing its intensity. The aim of this study was to describe parents’ perceptions of their child’s pain assessment in hospital care. Design and methods: This study was a descriptive cross-sectional study. A questionnaire was completed by parents (n = 261) whose child was hospitalized in one of the pediatric units (n = 6) of the University Hospital in Finland. Quantitative data were analyzed using statistical methods; open-ended data were analyzed using inductive content analysis. Results: Parents reported that their children experienced moderate (36%) to severe pain (42%) during hospitalization. The most intense pain experienced by the children was associated with needle-related procedures (41%). A large proportion of parents (83%) were involved in their child’s pain assessment. Parents were satisfied with their child’s pain assessment but perceived some shortcomings. Parents hoped that a variety of methods would be used to assess their child’s pain and that the parents’ and child’s views on pain would be taken into account. Conclusions: Most children experience moderate to severe pain during hospitalization. Parents are often involved in pain assessment but are rarely instructed to use pain scales. Practice implications: Child’s pain should be assessed regularly and frequently enough. It is important that the child and parents are involved in shared decision-making about pain assessment and treatment, and they have opportunities to ask questions. Guidance should be offered to parents about the use of pain assessment scales.

Endometriosis is a chronic disease associated with severe, life-impacting pain symptoms and impaired fertility that can affect relationships, sexuality, family life, social relationships, and working life. The aim of the present study was to describe endometriosis patients’ experiences of the counseling they need from the nurses through the digital care pathway. A qualitative descriptive study was conducted; 14 Finnish women with endometriosis were interviewed. Content analysis revealed four main categories related to counseling provided through the digital care pathway: counseling on endometriosis and its role in life; counseling on how to live with endometriosis; comprehensive support for self-care; and patient-oriented counseling. Nurses should provide counseling at different stages of the disease, because the symptoms of endometriosis can change rapidly in a short period of time, and these are the moments when the patient needs support and advice from the nurses. The digital care pathway can provide reliable information to patients at the right time of disease progression. The study was planned and reported using the Consolidated Criteria for Reporting Qualitative Research Checklist.

Background: Day surgery allows families to return home quickly. Only a few approaches to preparing for day surgery have demonstrated how digital solutions can support families and children. Objective: This study aims to evaluate the effectiveness of a mobile app intervention on preschool children’s fear and pain and parents’ anxiety and stress in preparing children for day surgery. Methods: This study was conducted at the Pediatric Day Surgical Department of a university hospital in Finland between 2018 and 2020. Parents of children (aged 2–6 y) who were in a queue for elective day surgery were randomized into the intervention group (IG; n=36) and control group (CG; n=34). The CG received routine preparations, whereas the IG was prepared using a mobile app. Parents’ and children’s outcomes were measured using validated scales at 4 different points: at home (T1 and T4) and at the hospital (T2 and T3) before and after surgery. Group differences were analyzed using statistical methods suitable for the material. Results: Before surgery, parents in both groups experienced mild anxiety, which decreased after surgery. Parental anxiety did not differ between groups preoperatively (P=.78) or postoperatively (P=.63). Both groups had less anxiety at home after surgery compared with before. The IG showed a significant decrease (P=.003); the CG also improved (P=.002). Preoperatively at home, most parents in both groups experienced no stress or mild stress (P=.61). Preoperatively at the hospital, parents in both groups experienced mild stress; however, parents in the IG experienced more stress during this phase (P=.02). Parents in the IG experienced significantly less stress postoperatively than those in the CG (P=.05). Both groups showed decreased stress levels from before to after surgery (IG: P=.003; CG: P=.004) within each group. Introduction:: The use of non-pharmacological pain relief methods and pain assessment scales during labor has received limited research attention. This study aimed to describe women’s perceptions of the pain assessment and non-pharmacological pain relief methods used during labor. Methods: A descriptive, cross-sectional survey was conducted. A convenience sample of women (n=204) from one Finnish maternity ward participated in the study. Women who had given birth were asked to respond to a validated questionnaire between November 2018 and February 2019. The statistical significance of observed differences was analyzed using the chi-squared test. Results: Less than half (46%) of the women who gave birth at the hospital were asked to assess the intensity of their pain on a pain assessment scale. The most commonly used non-pharmacological pain relief methods were encouragement (92%), the presence of a midwife (82%), and proper breathing technique that was taught by a midwife (81%). Aqua blisters (3%), reflexology (e.g. zone magnets, 5%), and music (9%) were the least commonly used non-pharmacological methods during labor. The participants’ experiences of fear and pain were significantly associated with the implementation of pain assessment. Conclusions: Women’s pain was rarely evaluated by using a certain pain assessment scale. In addition, non-pharmacological pain relief methods were inadequately used during labor. More specifically, methods that required midwives’ own personal contributions were rarely offered to the women.

Background: The challenge is to identify pain assessment counselling that are effective and reliable to the woman during labour while also supporting appropriate management of labour pain. Objective: This study aimed to describe women’s perceptions of their counselling on pain assessment and pain management during labour. Design: A descriptive, cross-sectional study. Participants: The sample consisted of women who had given birth (n=204) at a university hospital in Finland; 250 parturients were recruited by convenience sampling. Methods: Data were collected using a questionnaire (P-PAPM) between November 2018 and February 2019. The statistical significance of observed differences was analysed using the Chi-squared test and Fischer’s exact test. Results: Eighty percent of women reported that they had received counselling on pharmacological treatments from midwives, but only 33 % received counselling on pain assessment. The non-pharmacological methods for alleviating labour pain most commonly taught by midwives were proper breathing techniques, cold/heat treatments, and trying different positions and movements. Women were less commonly counselled to try listening to music, thinking about pleasant and positive things, or concentrating their thoughts on something other than pain. The two most commonly used counselling methods were demonstrations and written material and least used Internet-based resources. The personal issue that midwives discussed most frequently during counselling was the women’s individual hopes concerning pain management (91%), while the issue discussed least often was previous experiences of pain (58%). The participants’ experiences of fear, age, and education were significantly associated with aspects of counselling on pain assessment and management. Tiivistelmä. Koronapandemian aikana kouluissa siirryttiin etäopetukseen, jolloin oppilaat opiskelivat kotona tietokoneen välityksellä. Siirtymä tapahtui nopeasti ja vaati opettajilta uudenlaista opetuksen järjestämistä. Etäopetuksessa oppilaiden tuli olla aiempaa omatoimisempia ja lisäksi se vaati oppilailta esimerkiksi pitkäjänteisyyttä, vaikeutti keskittymistä ja aiheutti haasteita oppilaiden opetellessa käyttämään erilaisia tietoteknisiä ohjelmia. Myös huoltajat joutuivat ottamaan enemmän vastuuta lasten opiskelusta, mikä lisäsi heidän paineitaan. Erityisen haastavaa etäopetus oli tuentarpeisille oppilaille. ADHD tarkoittaa neuropsykiatrista häiriötä, jolloin esiintyy pitkäaikaisia ja toimintakykyä haittaavia vaikeuksia keskittymisessä, aktiivisuuden säätelyssä ja impulssien hillinnässä. ADHD-oireisilla oppilailla on usein haasteita esimerkiksi pitkäjänteisyydessä ja keskittymisessä, mitä vaadittiin etäopetuksessa aiempaa enemmän. Aiemmin käytössä olleita tukimuotoja ei välttämättä voitu hyödyntää etäopetuksen aikana ja oppilas saattoi saada aiempaa vähemmän tukea. Tämän tutkielman tavoitteena on selvittää, millaisia kokemuksia luokanopettajilla on ADHD-oireisten oppilaiden etäopetuksesta sekä millaisia tukimuotoja etäopetuksen aikana käytettiin. Tutkielma on toteutettu laadullisena fenomenologisena tutkimuksena. Aineisto on kerätty sähköisellä kyselylomakkeella, joka julkaistiin luokanopettajille suunnatussa suljetussa Facebook-ryhmässä. Aineisto on kerätty tammikuun 2022 aikana ja se on analysoitu käyttämällä aineistolähtöistä sisällönanalyysiä. Tämän tutkimuksen aineisto koostuu 14 vastauksesta. Tutkielman tuloksista selvisi, että etäopetuksen aikana yleisimmin käytettyjä tukimuotoja olivat yhteistyö kodin kanssa, luokanopettajan antama tuki, erityisopettajan tai ohjaajan antama tuki sekä eriytetyt tehtävät. Introduction: This study aimed to describe exclusive breastfeeding (EBF, Step 6 of the Baby-Friendly Hospital Initiative) in Finnish maternity hospitals and identify factors that promote or limit EBF. Methods: A cross-sectional study design was used, and data were collected from eight maternity hospitals in Finland during a 10-day period in May 2014. The staff completed questionnaires (n=1554) from separate work shifts. The data were analyzed using descriptive statistics, and chi-squared and Fisher’s tests. Responses to open-ended questions were analyzed using content analysis. Results: Maternity ward staff reported that 72% (n=1105) of the infants were exclusively breastfed during their work shift. The strongest promoting factors of exclusive breastfeeding were: maternity ward staffs’ profession and education in breastfeeding counselling; multiparity; vaginal delivery; early skin-to-skin contact between mother and infant; initial breastfeeding after birth; rooming-in; and initial success of breastfeeding. The use of a nipple shield, the need for additional breastfeeding counselling, and infants’ blood tests were limiting factors to exclusive breastfeeding. Open-ended answers revealed that exclusive breastfeeding was mainly delayed because of medical issues for the mother or infant. Conclusions: Finnish maternity hospitals could improve exclusive breastfeeding rates by focusing attention and resources on breastfeeding counselling and evidence-based maternity care practices related to immediate care after birth, promoting vaginal delivery, rooming-in and availability of skilled counselling.

Tiivistelmä. Kandidaatintutkielman aiheena on ADHD-oireisen oppilaan koulunaloitus ja siinä tukeminen. Aluksi tutkielmassa esitellään koulun aloitukseen liittyviä muutoksia siirryttäessä esikoulusta perusopetukseen sekä vaatimuksia lapselle koulun alkaessa. ADHD-käsite määritellään ja avataan esimerkiksi diagnosoimista, hoitomenetelmiä ja liitännäisoireita. Seuraavaksi esitellään, mitä vaikeuksia ADHD-oireinen lapsi kohtaa koulun alkaessa ja annetaan konkreettisia neuvoja, miten opettaja voi tukea lasta tässä siirtymävaiheessa. Tutkimuskysymyksiksi valikoituivat: Mitä haasteita ADHD-oireinen oppilas kohtaa koulun aloituksessa? Miten opettaja voi tukea ADHD-oireista oppilasta koulun aloituksessa? Tutkielma on toteutettu kuvailevana kirjallisuuskatsauksena. Valittu aineisto sisältää laajasti sekä kotimaisia, että kansainvälisiä lähteitä. ADHD eli aktiivisuuden ja tarkkaavaisuuden häiriö on neuropsykiatrinen häiriö, jonka keskeisimpiä oireita ovat vaikeudet aktiivisuuden säätelyssä, keskittymisessä ja impulssien hillinnässä. Koulussa se näkyy esimerkiksi levottomuutena, häiriöherkkyytenä ja vaikeutena tehdä tehtäviä. Usein siirtymätilanteet ovat erityisen haastavia ja luokan taustamelu häiritsee keskittymistä entisestään. Lapsen siirtyessä esikoulusta perusopetukseen hän kohtaa paljon muutoksia, jotka ovat ADHD-oireiselle lapselle tavallista haastavampia. Fyysinen ja sosiaalinen ympäristö sekä toimintatavat muuttuvat ja lapselta odotetut vaatimukset kasvavat. Nivelvaiheessa on tärkeää, että yhteistyö esikoulun ja perusopetuksen välillä toimii ja tiedonsiirto onnistuu. Siirtymävaihetta helpottavat esimerkiksi tutustumiset tulevaan kouluun ja opettajaan. Opettaja voi tukea ADHD-oireista oppilasta koulun alussa esimerkiksi muokkaamalla fyysistä oppimisympäristöä mahdollisimman häiriöttömäksi, pitämällä tarkat rutiinit ja päiväjärjestyksen luokassa, kiinnittämällä huomiota ohjeidenantoon ja positiivisen palautteen antamiseen. This study describes skin-to-skin contact, initial breastfeeding, rooming-in, and exclusive breastfeeding in accordance with the Baby-Friendly Hospital Initiative (BFHI) program. Furthermore, it addresses the connection of these factors in Finnish maternity hospitals and gathers information about the daily work of maternity ward staff, which is useful to develop ways to increase exclusive breastfeeding of infants and to plan the introduction of BFHI. The study uses a cross-sectional design that includes questionnaires. Random data came from mothers (n=111), midwives (n=272), and maternity ward staff (f=1554) in maternity hospitals during the spring of 2014. The questionnaires include background questions, questions concerning the implementation of skin-to-skin contact, initial breastfeeding, rooming-in, exclusive breastfeeding according to the BFHI, and open-ended questions about implementation barriers. The analysis used statistical methods to interpret the data and content specifications to explain the answers to open questions. In Finland, successful skin-to-skin contact, initial breastfeeding, and rooming-in results in multiparas and vaginally births women. After vaginally births, exclusive breastfeeding increases when skin-to-skin contact, initial breastfeeding, and rooming-in starts at an early stage. Rooming-in does not take place for many different reasons. Maternity staff state that 72% implement exclusive breastfeeding, and mothers state that 55% exclusively breastfeed. Primiparous mothers and those who underwent cesarean are the populations that least use exclusive breastfeeding mostly due to infant medical issues and to non-medical reasons. The results of the study bring to light that practices with skin-to-skin contact, initial breastfeeding, and rooming-in in Finnish maternity units are similar to BFHI steps. Exclusive breastfeeding during hospitalization, as well as implementation barriers, should receive special attention. Informationen kan användas för att utveckla metoder för ökning av helamning och för att planera hur programmet för amningsvänliga sjukhus ska implementeras. Undersökningen var en tvärsnittsstudie som genomfördes som en enkätundersökning. Materialet samlades in från mödrar på slumpmässigt utvalda förlossningssjukhus (n=111), barnmorskor i förlossningssalar (n=272) och skötare på vårdavdelningar (f=1554, f=antal vårdrapporter/enkäter) våren 2014. Förutom bakgrundsfrågor innehöll enkäterna frågor om hur hudkontakt, första amningen, samvård och helamning har implementerats enligt programmet för amningsvänliga sjukhus samt öppna frågor om faktorer som förhindrar implementeringen. Materialet analyserades med statistiska metoder och de öppna frågorna genom en kvantitativ innehållsanalys. Hudkontakt, första amningen och samvård har implementerats bra i Finland hos omföderskor som fött vaginalt. Helamningen ökade när hudkontakt och första amningen inleddes tidigt och när samvård tillämpades. Flera olika orsaker förhindrade samvård. Enligt vårdpersonalens bedömning var andelen mödrar som helammade 72 % och enligt mödrarnas bedömning 55 %. Andelen mödrar som helammade var minst bland förstföderskor och bland föderskor som genomgått kejsarsnitt. Helamning förhindrades främst av medicinska orsaker. Undersökningen visar att vårdpraxisen på finländska förlossningssjukhus inte avviker stort från rekommendationerna i programmet för amningsvänliga sjukhus när det gäller implementeringen av hudkontakt, första amningen och samvård. Interaction and communication have received remarkable attention within SLA research in recent years. The sociocultural learning approach implemented in Finnish comprehensive schools through the national curriculum also emphasises interaction and communication as key concepts in all learning. Thus, this qualitative, empirical case study investigates what role van Lier’s Interaction Types (1988) have in EFL learners’ willingness to communicate (WTC) in the language of the lesson, as well as EFL learners’ WTC in relation to the learners’ identities. The latter will be studied applying Zimmerman’s identity categories (1998). The participants of this study were two groups of 9th-graders in a Finnish comprehensive school, and the data was collected during two, 75-minute English as a Foreign Language (EFL) lessons using non-participatory observation. The analysis showed that WTC varies depending on the interaction type, but also that a variety of situational aspects within these categories, such as access to vocabulary in the second language and problematic situations, influence learners’ WTC. In addition, the analysis showed that when learners identify themselves as pupils, they show WTC in the language of the lesson, whereas if the learners communicate as themselves, they show WTC in their first language. It appears that the pupils’ selves are thus separate from their pupil and language learner identities. Possible practical implications stemming from this are encouraging the pupils to use English as themselves, with the language skills they have, but also providing them with appropriate tools to practice the language, such as providing access to appropriate vocabulary and giving enough time to prepare for tasks to come.Tiivistelmä. Introduction: Rooming-in is an evidence-based practice during which postpartum mothers and infants stay together. Rooming-in benefits both the mother and infant, and is especially important for breastfeeding. This study aims to describe rooming-in (Step 7 of the BFHI), according to mothers and maternity-ward staff in Finnish maternity hospitals, as well as the factors associated with its implementation. Methods: The presented research adopted a cross-sectional study approach. Questionnaires were used to collect data from mothers (n=111) who had given birth and the attending maternity-ward staff (f=1554 reported events) at 8 Finnish maternity hospitals. The data were analysed using descriptive statistics, as well as chi-squared, t-test, and Fisher, Mann- Whitney, Kruskal-Wallis tests. Answers to the open-ended questions were analysed using content specifications. Results: Rooming-in was utilised to a satisfactory extent, especially after vaginal birth. Most of the mothers regarded it as a very positive experience. Rooming-in was delayed mainly because of a mother’s tiredness and the infant’s condition. Factors such as a staff member’s age, work experience, and completion of breastfeeding counselling training (WHO 20-h), a mother’s parity, need for supplementation, and mode of childbirth, were found to be associated with the decision to implement rooming-in. Conclusions: Rooming-in should be used more with infants born by caesarean section and primiparous mothers. The need for supplementation clearly increased when roomingin was not employed. The presented information could be crucial for effectively allocating maternity ward resources and demonstrating the importance of rooming-in to a diverse audience of health care professionals.

Tiivistelmä. Tässä tutkielmassa tarkastellaan Oulun yliopiston kasvatustieteiden tiedekunnan opetusta, opiskeluympäristöä sekä työntekijöiden työhyvinvointia Liikkuva koulu sekä Liikkuva opiskelu -toimintaperiaatteiden näkökulmasta. Tarkoitus on selvittää, miten tiedekunnassa voidaan toteuttaa Liikkuva opiskelu -ohjelmaa. Nykyään tiedetään, että ihmiset istuvat terveyden ja hyvinvoinnin kannalta liikaa. Koulut, oppilaitokset sekä työympäristöt ovat hyvin passiivisia. Mitä enemmän istutaan, sitä vähemmälle fyysinen aktiivisuus jää. Hallitus otti vuonna 2009 kantaa lasten ja nuorten liikunnan määrään ja vuonna 2010 käynnistettiin hanke, jonka tavoitteena oli lisätä liikuntaa koulupäiviin. Vuonna 2012 hanke kasvoi Liikkuva koulu -ohjelmaksi, ja tänä päivänä, tuhansien koulujen ollessa osana Liikkuva koulu -ohjelmaa, ohjelman toimenpiteillä on saatu lisättyä aktiivisuutta koulupäiviin perusopetuksessa. Vuonna 2017 Liikkuva koulu -ohjelmaa lähdettiin laajentamaan Liikkuva opiskelu -ohjelmaksi toiselle asteelle ja korkeakouluihin. Oulun yliopiston kasvatustieteiden tiedekunta lähti ohjelmaan mukaan heti ohjelman alkuvaiheessa syksyllä 2017. Tutkielman teoreettinen viitekehys muodostuu Liikkuva koulu sekä Liikkuva opiskelu -ohjelmien tavoitteista. Liikkuva opiskelu -ohjelman tavoitteena on lisätä opiskelijoiden fyysistä aktiivisuutta ja opiskelukykyä. Tutkielmassa näitä tavoitteita tarkastellaan opiskelun sekä opiskeluympäristön näkökulmasta. Näiden lisäksi huomioidaan myös työntekijöiden työhyvinvointi. Tämän kuvailevan kvantitatiivisen tutkimuksen aineistonkeruumenetelmänä on käytetty henkilöstölle suunnattua kyselylomaketta (n=65), jolla kartoitettiin Liikkuva opiskelu -ohjelman lähtötilannetta tiedekunnassa. Aineiston analysoinnissa käytettiin frekvenssi- ja prosenttijakaumia, sekä teoriaohjaavaa sisällönanalyysia avointen kysymysten osalta. Using Discourse Analysis to support the analysis, this study investigates how the American television show Friends portray characters from Britain, as well as Britishness in general, in the otherwise American context., Englantia käyttävät niin sitä äidinkielenään puhuvat kuin hekin, jotka ovat oppineet englantia viestinnällisiin tarkoituksiin vieraana kielenä. Tästä luonnollisesti seuraa, että englantia puhutaan monella eri aksentilla, mikä saattaa synnyttää konflikteja äidinkielenään englantia puhuvien ja englantia vieraana kielenä puhuvien välillä. Diskurssianalyysiä apuna käyttäen tämä kvalitatiivinen kandidaatintutkielma tutkii, miten amerikkalaisen televisiosarjan "Frendit" brittihahmot ja brittiläisyys yleisesti kuvataan muutoin amerikkalaisessa kontekstissa.

Background: Breastfeeding and skin-to-skin contact are the best start for infant life. Breastfeeding ensures the best trajectory for development and growth while preventing many diseases later in life. It is recommended that initial breastfeeding occur during the first hour and that generally exclusive breastfeeding is adopted during the first six months. Objective: The aim of this study is to describe how initial breastfeeding and skin-to-skin contact (Step 4 of the BFHI) is implemented in Finnish maternity hospitals as well as to explain the factors connected to it. The information can be used to develop maternity care during the immediate postpartum period. Design: Cross‐sectional study. Methods: The data were collected from mothers who had given birth as well as their midwives via questionnaire during the spring of 2014 during one week at eight maternity hospitals in Finland. The response rate was 59% for the new mothers (n=111), while it was 57% for the midwives (n=272). The data were analysed statistically and the open-ended questions in the questionnaire using content specifications. Findings: On the basis of the results, initial breastfeeding succeeded well after vaginal birth. Initial breastfeeding began, on average, at 41 minutes of age and lasted for 51 minutes. Of mothers, 87% regarded it a very positive experience. Initial breastfeeding was delayed mainly because of caesarean section and for reasons related to an infant’s condition. Many background factors such as midwives’ age, mothers’ parity and the mode of childbirth were statistically significant in respect to the success of initial breastfeeding. Conclusions: More attention should be placed on the initial breastfeeding of infants born by caesarean section and primiparous mothers.

Tiivistelmä. Kandidaattitutkielmamme tarkastelee systemaattisen kirjallisuuskatsauksen muodossa kotimaista ja kansainvälistä tutkimuskirjallisuutta. Tutkimuskohteena ovat varhaisvuosien fyysinen aktiivisuus sekä perheen asema lapsen fyysisen aktiivisuuden tukijana. Tutkielmassa selvitetään, millaisia vaikutuksia perheellä on lapsen fyysiseen aktiivisuuteen. Tutkielmassa huomioidaan demografiset eli biologiset, psyykkiset, sosiaaliset ja fyysiset ympäristötekijät. Käsittelemme tutkielmassa fyysisen aktiivisuuden käsitteen sekä esittelemme uudet fyysisen aktiivisuuden suositukset. Lisäksi käsittelemme fyysisen aktiivisuuden vaikutuksia lapsen kasvuun ja kehitykseen. Tämän jälkeen tarkastelemme perheen merkitystä alle 8-vuotiaan lapsen fyysiseen aktiivisuuteen. Aihe on erittäin ajankohtainen, sillä vuonna 2016 on julkaistu uudet varhaisvuosien fyysisen aktiivisuuden suositukset. Perheen ajankäytössä sekä elintavoissa on tapahtunut muutosta, mitkä ovat heijastuneet lasten päivittäiseen fyysisen aktiivisuuden määrään ja laatuun. Fyysistä aktiivisuutta voidaan pitää yhtenä merkittävänä tekijänä lapsen normaalin kasvun ja kehityksen osalta. Tämän vuoksi fyysisen aktiivisuuden määrään ja laatuun on kiinnitettävä erityistä huomiota. Lapsen ja vanhemman välillä vallitsee voimakas molemminpuolinen kiintymys, jota kutsutaan biologiseksi siteeksi. Tämän myötä vanhemmat vaikuttavat lapseen monella eri tapaa. Perheellä ja erityisesti vanhemmilla on on tärkeä rooli tukea ja kannustaa lasta fyysisesti aktiivisiin leikkeihin ja peleihin. Vanhemmat sekä sisarukset ovat lapselle tärkeitä roolimalleja. Myös fyysisellä ympäristöllä, kuten liikuntavälineillä ja varusteilla, asuinmiljööllä sekä ulkoilumahdollisuuksilla voidaan monella tapaa vaikuttaa lapsen fyysiseen aktiivisuuteen. Tutkielmaa voisi syventää jatkossa seurantatutkimuksen muodossa. Tutkimuksessa seurattavana olisivat lapset ja heidän vanhemmat. Alueena toimisi esimerkiksi joku yleinen leikkipuisto. Competent massive sulphide and high-stress rockburst-prone ground, with distinct seismicity around active stopes, has developed into a fractured and loose rock mass susceptible to block movements caused by detachment of schistosity surfaces at the wall rock contact. The location of seismic events has moved to the edges of production areas, affecting mine infrastructure. Production has been adjusted by modifying stope sequencing, design, and backfilling. Ground support has evolved from the use of rockbolts and shotcrete with cable bolting in stopes to extensive use of mesh and fibre reinforced shotcrete. Constant rehabilitation is required in the contact zones, where the largest downdip subsidence of ore blocks is about 2 m. Reduced physical function and persistent pain are serious consequences of rheumatoid arthritis (RA). Clinical trials have shown that patients with RA suffer from a poor health-related quality of life (HR-QoL). However, limited information is available on the HR-QoL of patients treated in normal clinical practice. The purpose of the present study was to obtain information of how RA can influence on the patients’ HR-QoL and functional ability in a clinical setting and to investigate the impact of disease-related and demographic factors on the HR-QoL. The theoretical framework of the study was the biopsychosocial concept of health, i.e. the ICF model (International Classification of Functioning, Disability and Health) endorsed by WHO. HR-QoL was measured by the Nottingham Health Profile (NHP) questionnaire and functional ability by the Health Assessment Questionnaire (HAQ). The contents of these instruments and the NHP results were evaluated also within the ICF categories. The study consisted of a cross-sectional series and a longitudinal cohort carried out in two central hospitals in Finland in the late 1990s. The cross-sectional group of 122 consecutive out-patients had a mean disease duration of 11 years. The HR-QoL values of the patients were compared with those of an age and gender matched ”healthy” control group living in the same area. The HR-QoL values of the patients were examined also at different functional ability levels. The longitudinal group of 62 consecutive patients had symptom duration ≤ 24 months and no prior use of antirheumatic drugs or glucocorticosteroids at inclusion. First, the impact of the treatment response assessed by the EULAR (DAS28) criteria on HR-QoL was examined at six months from disease onset. Secondly, the HR-QoL changes and their associations with age and gender and with the changes in disease activity, radiographic assessments of hands and feet and functional ability were examined for ten years after the onset of RA. Kliiniset tutkimukset ovat osoittaneet, että nivelreumaa sairastavat potilaat kärsivät huonontuneesta terveyteen liittyvästä elämänlaadusta. Normaalista kliinisestä käytännöstä saatava tieto potilaiden terveyteen liittyvästä elämänlaadusta on kuitenkin niukkaa. Tämän tutkimuksen tarkoituksena oli saada tietoa nivelreuman vaikutuksista potilaiden terveyteen liittyvään elämänlaatuun ja toimintakykyyn kliinisessä asetelmassa ja tutkia sekä tautiin liittyvien että demografisten tekijöiden vaikutusta terveyteen liittyvään elämänlaatuun. Tutkimuksen teoreettisena viitekehyksenä käytettiin WHO:n hyväksymää toimintakyvyn, toimintarajoitteiden ja terveyden kansainvälistä luokitusta (ICF-malli). The isotope quantum effects in the ground-state electron momentum density of water are studied at temperatures ranging from 5 to 90 degrees C by combining Compton scattering experiments utilizing synchrotron radiation and computational analysis within density functional theory. We observe clear differences in the momentum density between normal and heavy water at room temperature, which are interpreted as predominantly reflecting intramolecular structural differences. The changes in the momentum density upon increasing the temperature are found to be larger for heavy than for normal water, which is attributed primarily to temperature-induced intramolecular structural effects. Both model computations and an ab initio approach qualitatively reproduce the changes in the momentum density as a function of temperature.

Our theoretical study shows how well-resolved image potential states develop on metal-supported metallic nanoislands. The systems under consideration are one monolayer high Na nanoislands on the Cu(111) surface. However, the phenomenon is general and it requires only that the work function of the adsorbate is lower than that of the substrate, and that the substrate possesses a projected band gap along the surface normal.

Using the procedure developed for analysis of the COL2A1 gene, mutations were detected in > 20% of patients with chondrodysplasias and up to 2% of patients with early-onset familial OA. However, these percentages are only minimal estimates because all possible mutations in the gene cannot be detected with this procedure.
