The present study investigates the relationship between sentence intelligibility, band importance, and patterns of spectro-temporal covariation between frequency bands. Sixteen listeners transcribed sentences acoustically degraded to 5, 8, or 15 frequency bands. Half of the sentences retained the frequency bands that captured more signal covariance. The other half retained the bands accounting for less signal covariance. Sentence intelligibility was significantly higher in the high-covariance condition. Critically, this finding was predicted by differences in band importance across reconstructed sentences. Speaking style variation plays a role in how listeners remember speech. Compared to conversational sentences, clearly spoken sentences were better recalled and identified as previously heard by native and non-native listeners. The present study investigated whether speaking style variation also plays a role in how talkers remember speech that they produce. Although distinctive forms of production (e.g., singing, speaking loudly) can enhance memory, the cognitive and articulatory efforts required to plan and produce listener-oriented hyper-articulated clear speech could detrimentally affect encoding and subsequent retrieval. Native and non-native English talkers' memories for sentences that they read aloud in clear and conversational speaking styles were assessed through a sentence recognition memory task (experiment 1; N = 90) and a recall task (experiment 2; N = 75). The results showed enhanced recognition memory and recall for sentences read aloud conversationally rather than clearly for both talker groups. In line with the "effortfulness" hypothesis, producing clear speech may increase the processing load diverting resources from memory encoding. Implications for the relationship between speech perception and production are discussed.

Though necessary, protective mask wearing in response to the COVID-19 pandemic presents communication challenges. The present study examines how signal degradation and loss of visual information due to masks affects intelligibility and memory for native and non-native speech. We also test whether clear speech can alleviate perceptual difficulty for masked speech. One native and one non-native speaker of English recorded video clips in conversational speech without a mask and conversational and clear speech with a mask. Native English listeners watched video clips presented in quiet or mixed with competing speech. The results showed that word recognition and recall of speech produced with a mask can be as accurate as without a mask in optimal listening conditions. Masks affected non-native speech processing at easier noise levels than native speech. Clear speech with a mask significantly improved accuracy in all listening conditions. Speaking clearly, reducing noise, and using surgical masks as well as good signal amplification can help compensate for the loss of intelligibility due to background noise, lack of visual cues, physical distancing, or non-native speech. The findings have implications for communication in classrooms and hospitals where listeners interact with teachers and healthcare providers, oftentimes non-native speakers, through their protective barriers.

Objective: This is the first histopathologic study that investigates the incidence of the pneumatized crista galli., Study Design: A prospective histopathologic study., Setting: Tertiary academic medical center., Subjects and Methods: A total of 109 specimens of crista galli were obtained postmortem during 2018 from randomly chosen patients who died at the University Hospital Centre Zagreb and had an autopsy at our Department of Pathology and Cytology. Specimens were surgically resected during the autopsy and then fixed, decalcinated, dehydrated, and embedded in paraffin. All slides were cut into 5-µm-thin sections and stained with a standard method (hematoxylin and eosin) for light microscope analysis. Specimens were histopathologically analyzed for the existence of pneumatization inside crista galli. The criterion to declare a specimen pneumatized was the presence of mucosa inside the cavity., Results: Pneumatized crista galli was found in 5 of 109 specimens (4.59%). In 5 of 5 cases (100%) of pneumatized crista galli, there was evidence of chronic inflammation., Conclusion: We found that the incidence of pneumatized crista galli is significantly lower in our histopathologic study in comparison with the majority of previous radiologic studies. Our study also found that all 5 specimens with pneumatized crista galli had chronic inflammation in the mucosa, which is a considerably higher incidence than in the previous studies (7.7%-44%). Due to the emerging evidence of pneumatized crista galli being of clinical importance, we suggest that a larger study be conducted before the results are generalized to the general population.

Background: Percutaneous mechanical thrombectomy (PMT) is a well-established technique for treatment of acute arterial and venous thrombosis which inevitably leads to intravascular erythrocyte hemolysis, resulting in hemoglobinuria., Case Presentation: We present a case of 66-year-old Caucasian female with subclavian artery aneurysm causing distal embolization and hand ischemia. The aneurysm was treated with stent graft, but with a subsequent graft thrombosis 3 months later. After graft recanalisation, AngioJet PMT was performed which resulted in dialysis-requiring acute kidney injury., Conslusion: Only several cases of acute kidney injury following AngioJet PMT have been published in literature. To our knowledge, this is the first reported case of dialysis-requiring AKI after PMT for peripheral arterial thrombosis. Until there is sufficient evidence and recommendation on preventing AKI in this setting, we believe that by being aware of the risk and by monitoring of patient, one might minimize the damage in case it occurs.

Listeners are able to classify talkers by regional dialect of their native language when provided with even short speech samples. However, the way in which American English listeners use segmental and prosodic information to make such decisions is largely unknown. This study used a free classification task to assess native American English listeners' ability to group together talkers from six major dialect regions of American English. Listeners residing in Ohio and Texas were provided with a sentence-long (experiment 1) or paragraph-long (experiment 2) speech sample produced by talkers from each of the six regions presented in one of three conditions: unmodified, monotonized (i.e., flattened F0), and low-pass filtered (i.e., spectral information above 400 Hz removed). In both experiments, listeners in the unmodified and monotonized conditions made more accurate groupings, reflecting their reliance on segmental properties for classifying regional variation. Accuracy was highest for Northern and Western talkers (experiment 1) and Mid-Atlantic talkers (experiment 2). Listeners with experience with multiple dialects as a result of geographic mobility did not show increased accuracy, suggesting a complex relationship between linguistic experience and the perception of available acoustic cues to socioindexical variation.

The present study examined the effect of intelligibility-enhancing clear speech on listeners' recall. Native (n = 57) and non-native (n = 31) English listeners heard meaningful sentences produced in clear and conversational speech, and then completed a cued-recall task. Results showed that listeners recalled more words from clearly produced sentences. Sentence-level analysis revealed that listening to clear speech increased the odds of recalling whole sentences and decreased the odds of erroneous and omitted responses. This study showed that the clear speech benefit extends beyond word- and sentence-level recognition memory to include deeper linguistic encoding at the level of syntactic and semantic information.

Purpose This study examined the effect of depressive symptoms on production and perception of conversational and clear speech (CS) sentences. Method Five talkers each with high-depressive (HD) and low-depressive (LD) symptoms read sentences in conversational and clear speaking style. Acoustic measures of speaking rate, mean fundamental frequency (F0; Hz), F0 range (Hz), and energy in the 1-3 kHz range (dB) were obtained. Thirty-two young adult participants (15 HD, 16 LD) heard these conversational and clear sentences mixed with energetic masking (speech-shaped noise) at -5 dB SPL signal-to-noise ratio. Another group of 39 young adult participants (18 HD, 19 LD) heard the same sentences mixed with informational masking (one-talker competing speech) at -12 dB SPL signal-to-noise ratio. The key word correct score was obtained. Results CS was characterized by a decreased speaking rate, increased F0 mean and range, and increased energy in the 1-3 kHz range. Talkers with HD symptoms produced these modifications significantly less compared to talkers with LD symptoms. When listening to speech in energetic masking (speech-shaped noise), listeners with both HD and LD symptoms benefited less from the CS produced by HD talkers. Listeners with HD symptoms performed significantly worse than listeners with LD symptoms when listening to speech in informational masking (one-talker competing speech). Conclusions Results provide evidence that depressive symptoms impact intelligibility and have the potential to aid in clinical decision making for individuals with depression.

Introduction: The aim of our work was to analyze the incidence of tunneled hemodialysis catheters-related thrombosis in hemodialyzed patients depending on catheter tip depth and position and to evaluate the impact of other established risk factors responsible for development of central venous thrombosis., Methods: The presence of central vein thrombosis, location, and the depth of the catheter tip was reviewed by two experienced radiologists. All patients with suspected central venous thrombosis had factor V Leiden, lupus anticoagulant, and cardiolipin antibodies checked (acIgM and acIgG) and were evaluated with cat-scan venography., Findings: A total of 68 tunneled hemodialysis catheters were analyzed with CT venography for central venous thrombosis. There were no signs of central venous thrombosis when catheter tips were placed in the right atrium. There was significantly higher number of catheter tips located on the vein wall than located centrally in patients with central venous thrombosis. Higher percentage of central venous thrombosis, smaller number of tips placed in the right atrium, higher values of IgM cardiolipin antibodies, and higher number of patients with positive factor V Leiden mutation were found in group of patients with catheter tips located on vein wall., Discussion: Higher incidence of central venous thrombosis in patients with the catheter tip placed on the vein wall is a consequence of a catheter tip depth proximal of the right atrium. Purpose: This study investigated acoustic-phonetic modifications produced in noise-adapted speech (NAS) and clear speech (CS) by children, young adults, and older adults., Method: Ten children (11-13 years of age), 10 young adults (18-29 years of age), and 10 older adults (60-84 years of age) read sentences in conversational and clear speaking style in quiet and in noise. A number of acoustic measurements were obtained., Results: NAS and CS were characterized by a decrease in speaking rate and an increase in 1-3 kHz energy, sound pressure level (SPL), vowel space area (VSA), and harmonics-to-noise ratio. NAS increased fundamental frequency (F0) mean and decreased jitter and shimmer. CS increased frequency and duration of pauses. Older adults produced the slowest speaking rate, longest pauses, and smallest increase in F0 mean, 1-3 kHz energy, and SPL when speaking clearly. They produced the smallest increases in VSA in NAS and CS. Children slowed down less, increased the VSA least, increased harmonics-to-noise ratio, and decreased jitter and shimmer most in CS. Children increased mean F0 and F1 most in noise., Conclusions: Findings have implications for a model of speech production in healthy speakers as well as the potential to aid in clinical decision making for individuals with speech disorders, particularly dysarthria.

Purpose: This study examined intelligibility of conversational and clear speech sentences produced in quiet and in noise by children, young adults, and older adults. Relative talker intelligibility was assessed across speaking styles., Method: Sixty-one young adult participants listened to sentences mixed with speech-shaped noise at -5 dB signal-to-noise ratio. The analyses examined percent correct scores across conversational, clear, and noise-adapted conditions and the three talker groups. Correlation analyses examined whether talker intelligibility is consistent across speaking style adaptations., Results: Noise-adapted and clear speech significantly enhanced intelligibility for young adult listeners. The intelligibility improvement varied across the three talker groups. Notably, intelligibility benefit was smallest for children's speaking style modifications. Listeners also perceived speech produced in noise by older adults to be less intelligible compared to the younger talkers. Talker intelligibility was correlated strongly between conversational and clear speech in quiet, but not for conversational speech produced in quiet and in noise., Conclusions: Results provide evidence that intelligibility variation related to age and communicative barrier has the potential to aid clinical decision making for individuals with speech disorders, particularly dysarthria.

Two experiments examined the acoustic correlates of the disyllabic tonal accent contrast in the Trøndersk dialect of Norwegian, and how narrow focus and phrasal position shape the contrast. Production results showed that both tonal accents have a high-low (HL) pitch contour, with different timing. In narrow focus, the pitch contrast was enlarged through asymmetrical F0 changes. When at the right edge of an accent phrase (AP), the accents were shown to have a higher L, lower AP H% tone, shorter stressed vowel, and longer final vowel. The alignment changes reflected tonal crowding while segmental modifications were due to the phrase-final lengthening in AP-final position. This work contributes to the literature on prosodic typology as well as on the implementation of prosodic focus and sentence intonation in languages with lexical pitch contrasts., (© 2017 S. Background/aims: The aim of this study was to examine the impact of different catheter tip positions on the life of the catheter, dysfunction, infection, and quality of hemodialysis and possible differences between the access site laterality in jugular-tunneled hemodialysis catheters., Methods: Catheters were evaluated for the following parameters: place of insertion, time of insertion, duration of use, and reason for removal. In all patients, the catheter tip position was checked using an X-ray., Results: The mean duration of implanted catheters with the tip placed in the cavo-atrial junction and right atrium was significantly longer. There were no differences in catheter functionality at follow-up or complications based on catheter laterality for each catheter tip position., Conclusion: According to our results, the localization of the catheter tip in superior vena cava still remains the least preferable method. Our results showed that the main factor responsible for better catheter functionality was not laterality but the depth to which the catheter tip is inserted into the body., (© 2017 S. There is a correlation between central venous cannulation and the development of central venous stenosis. Minor retrosternal vein lesions can be easily missed. Computerized tomographic (CT) venography is the diagnostic procedure of choice. The aim of this study was to examine the complications after catheter implantation in patients with prior permanent catheter placement and to evaluate present diagnostic procedures performed prior to choosing permanent access site in order to reduce possible complications after catheter placement. Complications of permanent CVC insertion in our department were analyzed between October 2011 and February 2015. We have implanted the Tesio twin catheter system and the Hickman Bard dual lumen catheter. All patients with prior permanent dialysis catheter were evaluated with color doppler, while patients with suspected central venous stenosis (CVS) or thrombosis were evaluated with phlebography or CT venography prior to catheter placement. One hundred and ninety-eight permanent dialysis catheters were placed (173 Tesio [87.4%] and 25 Hickman [12.6%]) in 163 patients. There were 125 patients (76.7%) with prior temporary catheter and 61 (48.8%) of them had more than one prior permanent catheter (1.92 catheter per person).There were 4/61 (6.5%) patients with catheter-related complications without prior phlebography and CT venography. Phlebography and CT venography were performed in 30 (24.0%) patients with suspected CVS/thrombosis and with dialysis vintage of 76.23 months (52.78-98.28). Phlebography and CT venography were more sensitive than color doppler in the detection of CVS/thrombosis in patients with prior permanent catheter placements (P < 0.001). Since this diagnostic algorithm was introduced prior to catheter placement there were no catheter insertion-related complications or dysfunctions (P < 0.001). All our procedures on patients with prior permanent catheters followed preliminary color doppler diagnostics. Nevertheless, in four cases, the vessel obstruction and abnormality led to catheter insertion-related complications. When phlebography and CT venography was performed before the procedure in patients with prior permanent catheters and one or more CVS or thrombosis, early and late catheter-related complications were avoided by careful selection of the least injured vein and planning of procedure. When planning a permanent dialysis catheter placement in patients with prior multiple CVC, phlebography and CT venography may be diagnostic procedures of choice for avoidance of early catheter dysfunction and CVC placement complications., (© 2016 International Society for Apheresis, Japanese Society for Apheresis, and Japanese Society for Dialysis Therapy.)

Early bilinguals often show as much sensitivity to L2-specific contrasts as monolingual speakers of the L2, but most work on cross-language speech perception has focused on isolated segments, and typically only on neighboring vowels or stop contrasts. In tasks that include sounds in context, listeners' success is more variable, so segment discrimination in isolation may not adequately represent the phonetic detail in stored representations. The current study explores the relationship between language experience and sensitivity to segmental cues in context by comparing the categorization patterns of monolingual English listeners and early and late Spanish-English bilinguals. Participants categorized nonce words containing different classes of English- and Spanish-specific sounds as being more English-like or more Spanish-like; target segments included phonemic cues, cues for which there is no analogous sound in the other language, or phonetic cues, cues for which English and Spanish share the category but for which each language varies in its phonetic implementation. Listeners' language categorization accuracy and reaction times were analyzed. Our results reveal a largely uniform categorization pattern across listener groups: Spanish cues were categorized more accurately than English cues, and phonemic cues were easier for listeners to categorize than phonetic cues. There were no differences in the sensitivity of monolinguals and early bilinguals to language-specific cues, suggesting that the early bilinguals' exposure to Spanish did not fundamentally change their representations of English phonology. However, neither did the early bilinguals show more sensitivity than the monolinguals to Spanish sounds. The late bilinguals however, were significantly more accurate than either of the other groups. These findings indicate that listeners with varying exposure to English and Spanish are able to use language-specific cues in a nonce-word language categorization task. Differences in how, and not only when, a language was acquired may influence listener sensitivity to more difficult cues, and the advantage for phonemic cues may reflect the greater salience of categories unique to each language. Implications for foreign-accent categorization and cross-language speech perception are discussed, and future directions are outlined to better understand how salience varies across language-specific phonemic and phonetic cues.

Context: Arterial stimulation and venous sampling (ASVS) is the most sensitive diagnostic method for occult insulinoma localization. We present our experience with this method and modifications used that proved to ensure greater safety and sensitivity with this rare pathology. Relationship between tumour size and changes in peak insulin values and release curves were also tested., Subjects and Methods: Six consecutive patients with biochemically established diagnosis of endogenous hyperinsulinism underwent preoperative non-invasive diagnostic tests with sensitivities calculated and compared for all methods. Certain modifications were used in ASVS procedure that included hepatic vein catheterization, order of arterial stimulation, time intervals between each stimulation, diagnostic insulin gradient level and the time frame for diagnostic rise of insulin. Peak insulin values and insulin gradients were compared with tumour size in each patient., Results: In all patients, calcium stimulation accurately localized the insulinoma, with higher sensitivity than any other method. A more than 4-fold increase in insulin level was observed in all patients, after injection into the tumour feeding artery, while 2 to 4-fold increase was observed in the tumour neighbouring areas. Tumour size showed an inversely proportional correlation to peak insulin values in each artery that correctly located the tumour., Conclusion: ASVS presents the most accurate and safe method for localization of occult insulinomas. A four-fold insulin gradient can be used as a limit for positive preoperative localization in order to secure a higher specificity., Competing Interests: The authors declare that they have no conflict of interest concerning this article.

Background: Diabetic polyneuropathy (DPN) is one of the most common complications of diabetes and can exist with or without neuropathic pain. We were interested in how neuropathic pain impairs the quality of life in diabetic patients and what is the role of comorbidities in this condition., Methods: The study included 80 patients with painful DPN (group "P") and 80 patients with DPN, but without neuropathic pain (group "D"). Visual analogue scale (VAS) and Leeds assessment of neuropathic symptoms and signs (LANSS) pain scale were used for assessment of neuropathic pain, SF-36 standardized questionnaire for assessment of the quality of life and BDI questionnaire for assessment of depression., Results: Subjects in group P had statistically significantly lower values compared to group D in all 8 dimensions and both summary values of the SF-36 scale. We ascribe the extremely low results of all parameters of SF-36 scale in group P to painful diabetic polyneuropathy with its complications. The patients in group D showed higher average values in all dimension compared to group P, but also somewhat higher quality of life compared to general population of Croatia in 4 of 8 dimensions, namely vitality (VT), social functioning (SF), role-emotional (RE) and mental health (MH), which was unexpected result. Clinically, the most pronounced differences between two groups were noted in sleeping disorders and problems regarding micturition and defecation , which were significantly more expressed in group P. The similar situation was with walking distance and color-doppler sonography of carotid arteries, which were significantly worse in group P. Consequently, subjects in group P were more medicated than the patients in group D, particularly with tramadol, antiepileptics and antidepressants., Conclusion: Painful DPN is a major factor that influences various aspects of quality of life in diabetic patients. Additionally, this study gives an overview of diabetic population in the Republic of Croatia, information that could prove useful in future studies.

Foreign-accented speech often presents a challenging listening condition. In addition to deviations from the target speech norms related to the inexperience of the nonnative speaker, listener characteristics may play a role in determining intelligibility levels. We have previously shown that an implicit visual bias for associating East Asian faces and foreignness predicts the listeners' perceptual ability to process Korean-accented English audiovisual speech (Yi et al., 2013). Here, we examine the neural mechanism underlying the influence of listener bias to foreign faces on speech perception. In a functional magnetic resonance imaging (fMRI) study, native English speakers listened to native- and Korean-accented English sentences, with or without faces. The participants' Asian-foreign association was measured using an implicit association test (IAT), conducted outside the scanner. We found that foreign-accented speech evoked greater activity in the bilateral primary auditory cortices and the inferior frontal gyri, potentially reflecting greater computational demand. Higher IAT scores, indicating greater bias, were associated with increased BOLD response to foreign-accented speech with faces in the primary auditory cortex, the early node for spectrotemporal analysis. We conclude the following: (1) foreign-accented speech perception places greater demand on the neural systems underlying speech perception; (2) face of the talker can exaggerate the perceived foreignness of foreign-accented speech; (3) implicit Asian-foreign association is associated with decreased neural efficiency in early spectrotemporal processing.

Purpose: The authors sought to investigate interactions among intelligibility-enhancing speech cues (i.e., semantic context, clearly produced speech, and visual information) across a range of masking conditions., Method: Sentence recognition in noise was assessed for 29 normal-hearing listeners. Testing included semantically normal and anomalous sentences, conversational and clear speaking styles, auditory-only (AO) and audiovisual (AV) presentation modalities, and 4 different maskers (2-talker babble, 4-talker babble, 8-talker babble, and speech-shaped noise)., Results: Semantic context, clear speech, and visual input all improved intelligibility but also interacted with one another and with masking condition. Semantic context was beneficial across all maskers in AV conditions but only in speech-shaped noise in AO conditions. Clear speech provided the most benefit for AV speech with semantically anomalous targets. Finally, listeners were better able to take advantage of visual information for meaningful versus anomalous sentences and for clear versus conversational speech., Conclusion: Because intelligibility-enhancing cues influence each other and depend on masking condition, multiple maskers and enhancement cues should be used to accurately assess individuals' speech-in-noise perception.

Aim: Safety evaluation of concomitant systemic chemotherapy and liver chemoembolization in patients with colorectal cancer., Patients and Methods: Seven patients with metastases confined to the liver were included and stratified into two groups, depending of dosage of systemic chemotherapy. The first group received systemic chemotherapy (FOLFIRI) with 20% dose reduction, and the second group received the full dose of the same chemotherapy. In both groups, chemoembolization of liver metastases with drug-eluting bead irinotecan (DEBIRI) was performed following the application of systemic chemotherapy. The toxicity profiles of the two groups were compared., Results: Of the 7 patients included, 4 received the reduced systemic chemotherapy dose and 3 received the full chemotherapy dose. DEBIRI was performed in all 7 patients. The main toxicities observed in the reduced chemotherapy dose group were leukopenia (25%), anorexia (75%), diarrhea (25%), vomiting (25%), right upper abdominal quadrant pain (100%) and elevated serum amylase level (25%). Main toxicities observed in the full chemotherapy dose group were anorexia (66.6%), vomiting (33.3%), right upper abdominal quadrant pain (100%), and elevated serum amylase level (66.6%). There were no significant differences between the two groups ( P = 0.78541)., Conclusions: Patients with isolated liver metastases from a colorectal primary can safely be treated with DEBIRI chemoembolization and a full dose of systemic chemotherapy (FOLFIRI).

This study investigated the extent to which noise impacts normal-hearing young adults' speech processing of sentences that vary in intelligibility. Intelligibility and recognition memory in noise were examined for conversational and clear speech sentences recorded in quiet (quiet speech, QS) and in response to the environmental noise (noise-adapted speech, NAS). Results showed that (1) increased intelligibility through conversational-to-clear speech modifications led to improved recognition memory and (2) NAS presented a more naturalistic speech adaptation to noise compared to QS, leading to more accurate word recognition and enhanced sentence recognition memory. These results demonstrate that acoustic-phonetic modifications implemented in listener-oriented speech enhance speech-in-noise processing beyond word recognition. Effortful speech processing in challenging listening environments can thus be improved by speaking style adaptations on the part of the talker. In addition to enhanced intelligibility, a substantial improvement in recognition memory can be achieved through speaker adaptations to the environment and to the listener when in adverse conditions.

The role of visual cues in native listeners' perception of speech produced by nonnative speakers has not been extensively studied. Native perception of English sentences produced by native English and Korean speakers in audio-only and audiovisual conditions was examined. Korean speakers were rated as more accented in audiovisual than in the audio-only condition. Visual cues enhanced word intelligibility for native English speech but less so for Korean-accented speech. Reduced intelligibility of Korean-accented audiovisual speech was associated with implicit visual biases, suggesting that listener-related factors partially influence the efficiency of audiovisual integration for nonnative speech perception.

Purpose: In this study, the authors examined how signal clarity interacts with the use of sentence context information in determining speech-in-noise recognition for children with cochlear implants and children with normal hearing., Method: One hundred and twenty sentences in which the final word varied in predictability (high vs. low semantic context) were produced in conversational and clear speech. Nine children with cochlear implants and 9 children with normal hearing completed the sentence-in-noise listening tests and a standardized language measure., Results: Word recognition in noise improved significantly for both groups of children for high-predictability sentences in clear speech. Children with normal hearing benefited more from each source of information compared with children with cochlear implants. There was a significant correlation between more developed language skills and the ability to use contextual enhancements. The smaller context gain in clear speech for children with cochlear implants is in accord with the effortfulness hypothesis (McCoy et al., 2005) and points to the cumulative effects of noise throughout the processing system., Conclusion: Modifications of the speech signal and the context of the utterances through changes in the talker output hold substantial promise as a communication enhancement technique for both children with cochlear implants and children with normal hearing.

Purpose: The purpose of this study was to examine sentence-recognition performance for a large, diverse group of nonnative speakers of English on the recently developed Basic English Lexicon (BEL) sentence materials and to determine whether BEL sentence lists are equated in difficulty for this population., Method: The BEL sentences were presented to 102 nonnative speakers of English with normal hearing and varied linguistic and cultural backgrounds. Five hundred sentences were presented mixed with noise spectrally matched to the target sentences. Subjects completed an online questionnaire providing detailed demographic and linguistic information. Listeners' spoken English proficiency was also assessed using the Versant English Test (Pearson Education, 2010)., Results: Nonnative listeners showed equal word-recognition performance for 3 groups of BEL sentence lists, each group containing sentence lists that had equivalent difficulty. In addition, spoken language proficiency and several demographic and linguistic factors were significantly correlated with BEL performance., Conclusion: The BEL sentence materials have been tested on a large cohort of nonnative speakers of English and have been found to be appropriate for use in speech-perception testing with this population.

Speech understanding difficulties for older adults (OAs) are well documented. Very little is known about whether age-related changes affect their speech production as well. Intelligibility of conversational and clear speech sentences produced by five OA talkers was examined. The results of the sentence-in-noise listening tests revealed that OAs enhanced their intelligibility for young adult (YA) listeners through clear speech modifications. Importantly, though, OAs were less effective at improving their speech to benefit listeners compared to YA talkers [reported in Smiljanic and Bradlow, J. Acoust. Soc. Am. 118(3), 1677-1688 (2005)]. The results suggest that auditory and cognitive changes across lifespan can affect OA's speech patterns and intelligibility.

Pluto and Eris are icy dwarf planets with nearly identical sizes, comparable densities and similar surface compositions as revealed by spectroscopic studies. Pluto possesses an atmosphere whereas Eris does not; the difference probably arises from their differing distances from the Sun, and explains their different albedos. Makemake is another icy dwarf planet with a spectrum similar to Eris and Pluto, and is currently at a distance to the Sun intermediate between the two. Although Makemake's size (1,420 ± 60 km) and albedo are roughly known, there has been no constraint on its density and there were expectations that it could have a Pluto-like atmosphere. Here we report the results from a stellar occultation by Makemake on 2011 April 23. Our preferred solution that fits the occultation chords corresponds to a body with projected axes of 1,430 ± 9 km (1σ) and 1,502 ± 45 km, implying a V-band geometric albedo p(V) = 0.77 ± 0.03. This albedo is larger than that of Pluto, but smaller than that of Eris. The disappearances and reappearances of the star were abrupt, showing that Makemake has no global Pluto-like atmosphere at an upper limit of 4-12 nanobar (1σ) for the surface pressure, although a localized atmosphere is possible. A density of 1.7 ± 0.3 g cm(-3) is inferred from the data.

Purpose: The objective of this project was to develop new sentence test materials drawing on a basic non-native English lexicon that could be used to test speech recognition for various listener populations. These materials have been designed to provide a test tool that is less linguistically biased, compared with materials that are currently available, for sentence recognition for non-native as well as native speakers of English., Method: One hundred non-native speakers of English were interviewed on a range of 20 conversational topics. Over 26 hr of recorded non-native English speech were transcribed. These transcriptions were used to create a lexicon of over 4,000 unique words. The words from this lexicon were used to create the new materials based on a simple syntactic sentence structure frame., Results: Twenty lists of 25 sentences were developed. Each sentence has 4 keywords, providing 100 keywords per list. Lists were equated for rate of occurrence of keywords in lexicon, high-frequency count (total number of affricates and fricatives), number of syllables, and distribution of syntactic structure. Listening-in-noise results for native-English-speaking, normal-hearing listeners indicated similar performance across lists., Conclusion: The Basic English Lexicon materials provide a large set of sentences for native and non-native English speech-recognition testing.

Extensive research shows that inter-talker variability (i.e., changing the talker) affects recognition memory for speech signals. However, relatively little is known about the consequences of intra-talker variability (i.e. changes in speaking style within a talker) on the encoding of speech signals in memory. It is well established that speakers can modulate the characteristics of their own speech and produce a listener-oriented, intelligibility-enhancing speaking style in response to communication demands (e.g., when speaking to listeners with hearing impairment or non-native speakers of the language). Here we conducted two experiments to examine the role of speaking style variation in spoken language processing. First, we examined the extent to which clear speech provided benefits in challenging listening environments (i.e. speech-in-noise). Second, we compared recognition memory for sentences produced in conversational and clear speaking styles. In both experiments, semantically normal and anomalous sentences were included to investigate the role of higher-level linguistic information in the processing of speaking style variability. The results show that acoustic-phonetic modifications implemented in listener-oriented speech lead to improved speech recognition in challenging listening conditions and, crucially, to a substantial enhancement in recognition memory for sentences.

While cross-dialect prosodic variation has been well established for many languages, most variationist research on regional dialects of American English has focused on the vowel system. The current study was designed to explore prosodic variation in read speech in two regional varieties of American English: Southern and Midland. Prosodic dialect variation was analyzed in two domains: speaking rate and the phonetic expression of pitch movements associated with accented and phrase-final syllables. The results revealed significant effects of regional dialect on the distributions of pauses, pitch accents, and phrasal-boundary tone combinations. Significant effects of talker gender were also observed on the distributions of pitch accents and phrasal-boundary tone combinations. The findings from this study demonstrate that regional and gender identity features are encoded in part through prosody, and provide further motivation for the close examination of prosodic patterns across regional and social varieties of American English.

The goal of the present study was to devise a means of representing languages in a perceptual similarity space based on their overall phonetic similarity. In Experiment 1, native English listeners performed a free classification task in which they grouped 17 diverse languages based on their perceived phonetic similarity. A similarity matrix of the grouping patterns was then submitted to clustering and multidimensional scaling analyses. In Experiment 2, an independent group of native English listeners sorted the group of 17 languages in terms of their distance from English. Experiment 3 repeated Experiment 2 with four groups of non-native English listeners: Dutch, Mandarin, Turkish and Korean listeners. Taken together, the results of these three experiments represent a step towards establishing an approach to assessing the overall phonetic similarity of languages. This approach could potentially provide the basis for developing predictions regarding foreign-accented speech intelligibility for various listener groups, and regarding speech perception accuracy in the context of background noise in various languages.

In this paper we investigate the effect of clear speech, a distinct, listener-oriented, intelligibility-enhancing mode of speech production, on vowel and stop consonant contrasts along the temporal dimension in English and Croatian. Our previous work has shown that, in addition to enhancing the overall acoustic salience of the speech signal through a decrease in speaking rate and expansion of pitch range, clear speech modifications increased the spectral distances between vowel categories in both languages despite the different sizes of their vowel inventories (+10 in English, 5 in Croatian). Here, we examine how clear speech affects the duration of English tense ('long') vs. lax ('short') vowels, English vowels preceding voiced ('long') vs. voiceless ('short') coda stops, Croatian long vs. short vowels and Croatian and English VOT duration for voiced and voiceless stops. Overall, the results showed that the proportional distance between the 'short' and 'long' vowel categories and between the voiced and voiceless stop categories was remarkably stable across the two speaking styles in both languages. These results suggest that, in combination with the spectral enhancement of vowel contrasts, language-specific pronunciation norms along the temporal dimension are maintained in clear and conversational speech.

Background: The purpose of our study was to present multidetector computed tomography (MDCT) findings in venous spread of renal cell carcinoma (RCC), to determine the superior extent of tumor thrombus and to compare MDCT findings with surgical report., Methods: The prospective MDCT study was performed on 31 patients diagnosed with RCC with venous spread (19 males and 12 females; age range 39-80 years; mean age 62.6 years). CT scans were obtained by MDCT scanner, in triphasic scanning protocol. All postprocessing techniques were performed by two independent radiologists, and the findings were reported in their consensus. MDCT diagnosis was compared with surgical and pathohistological findings., Results: Tumor thrombus extension into renal vein only (T3b stage) was found in 13/31 (42%) patients. Involvement of infradiaphragmatic level of inferior vena cava (IVC) (T3c stage) was found in 14/31 (45%) patients and supradiaphragmatic level of IVC (T4b stage) in 4/31 (13%) patients. In 27/31 (87%) patients surgery was performed, while 4/31 (13%) could not undergo surgery. In comparison with surgical report, in 25/27 (93%) operated patients the upper extent of the tumor thrombus was correctly diagnosed by MDCT, and 2/27 (7%) patients were falsely diagnosed., Conclusion: MDCT represents a fast, relatively inexpensive, and reliable diagnostic method for evaluating the venous spread of RCC as well as the level of its upper extent. Triphasic MDCT is often the only diagnostic method necessary for planning the surgical procedure. Surgery should be performed as soon as possible for MDCT findings to be valid.

Scanning electron microscopy was used to search for local or limited synthesis of antibiotics by a strain of Trichophyton mentagrophytes. Micrococcus luteus, which was susceptible to penicillin and to the dermatophyte when both were cultured on agar media, displayed morphological alterations of flattened walls, invagination, enlargement, and collapse in skin cultures with the fungus. Penicillinase did not neutralize these effects. A control strain of Staphylococcus spp with poor sensitivity to penicillin was resistant to the dermatophyte in all test systems. T. mentagrophytes showed positive tropism to the bacteria. Regional variation in fungal germination and antibiotic production, the limited yield of antibiotics, and the pattern of dermatophyte growth and alteration of bacterial morphology demonstrated the significant influence of microenvironment on antibiosis.

To determine whether antibiotic production might be ecologically advantageous in the survival of Bacillus species on human skin, we applied spores of a bacitracin-producing strain of Bacillus licheniformis (ATCC 10716) to the forearms of 11 volunteers. Three additional strains of B. licheniformis which did not synthesize antibiotics, including a mutant of ATCC 10716, were used in subsequent control trials. Samples of flora were taken from inoculated and control (opposite forearm) sites during the colonization period, generally 3 weeks. Although population densities were unaltered, changes in the carriage, composition, and bacitracin sensitivity of resident flora were related with the presence of ATCC 10716 only, which suggests that microbial interactions are important in bacillus colonization and in maintenance of normal flora. Interactions were examined in vitro by comparing growth curves of representative skin bacteria, including isolates of Staphylococcus epidermidis, Staphylococcus saprophyticus, Micrococcus luteus, and a large-colony diphtheroid, grown individually, in mixed culture with each other, and together in presence of each test strain of B. licheniformis. We observed some diminution of growth of M. luteus and the diphtheroid in the first mixed culture, and the diphtheroid was completely retarded in common culture with ATCC 10716. Lesser antibiotic effects were seen on the cocci, whose rank of sensitivity was similar to that in vivo. The growth of the diphtheroid was enhanced in mixed culture with those strains of bacilli which lack antibiotic activity.

Micrococcus luteus produced a diffusible, esterase inhibitory factor (EIF) which inhibited the activity of cutaneous diphtheroid esterases on Tween 80-CaCl2 agar media. Esterases of Staphylococcus, Micrococcus, Bacillus, and Serratia were not susceptible. EIF did not appear to combine with the substrate or to prevent enzyme synthesis; it was unable to reverse the precipitation of calcium oleate. The composition of the medium, especially peptones, influenced the production of EIF. EIF was synthesized in the absence of diphtheroids, but production required the presence of Tween. The interaction was observed on agar medium of pH 5.5-8.5, at 25-43 degrees C, under an atmosphere of 10-20% CO2, in the presence of urea, but not after the addition of NaCl or dextrose. Supernatants of broth cultures had to be concentrated to detect EIF. Crude dialyzed and concentrated preparations of EIF withstood 60 degrees C for 60 min but were inactivated after 100 degrees C for 10 min. EIF may possibly be associated with a lipoid substance, since it did not precipitate in ethanol.

The colonization and survival of Bacillus species, members of the cutaneous microbial community of humans, were investigated by applying spores of Bacillus licheniformis to the forearms of volunteers. Four strains were tested, including the bacitracin producer ATCC 10716 and its bacitracin-negative mutant. Germination occurred within 24 h. Significant differences in survival population and duration were found among the test strains; however, ATCC 10716 and its mutant produced statistically similar survival curves. In general, an inoculum density of 10(4) colony-forming units per cm2 allowed survival for at least 2 weeks. Individual variation was extreme, for one subject harbored bacilli for over 2 months and another eliminated the microorganism within 3 days. Individuals could be differentiated into long-term (greater than 21 days) and short-term (less than 14 days) carriers. Eight of the 11 volunteers (73%) inoculated with ATCC 10716 carried it for 2 weeks, and 5 subjects (45%) continued to support the bacilli for 3 weeks. Spreading of the organism to other regions of the body occurred, but bacilli were not detected in these areas beyond 6 days.

Quantitative and qualitative changes of the cutaneous aerobic bacterial flora upon 20 sites on the backs of each three healthy subjects were examined before and after one site was occluded, using skin flora maps as a tool. Major local alterations were found to affect the carriage of micro-organisms in distant surrounding areas. Staphylococcus epidermidis was the most successful competitor. Furthermmore, some sites appeared to act as retricted reservoirs for specific types of micro-organisms whereas other areas were less limited in their support of flora.

Viable aerobic mesophilic bacteria are not evenly distributed on the skin of the volar forearm. An increase in the size of the area sampled did not result in a proportional increase in the number of the viable aerobic mesophilic bacteria recovered.
