The increasing proximity between protected areas (PAs) and urban areas, which can lead to urban protected areas (UPAs), is now commonplace. Use of Euclidean distance to measure the distance between PAs and cities has not correctly portrayed the spatial relationship between PAs and cities. We devised an isochronous circle model to accurately measure the distance between 2706 national PAs in 5 categories and 2844 cities in China based on human accessibility to identify urban human activity-influenced protected areas (UHAIPAs) and to quantitatively analyze their distribution patterns and relationships with China's economy, population distribution patterns, and urban development indicators. Most of the PAs in China were established near cities. Of 2746 PAs in China, 18.35% (n = 504) became UPAs, and 58.27% (n = 1600), 16.72% (n = 459), and 3.31% (n = 91) of PAs were within 0-30, 30-60, and 60-90 min, respectively. Both UPAs and UHAIPAs in China in general exhibited obvious spatial aggregation characteristics (e.g., wetland parks and scenic areas), and there was a significant spatial dependence effect among characteristics. The degree of spatial distribution and aggregation of UPAs was correlated with 16 indicators across urban economic development, urban natural substrate, and urban policy support factors. Based on the results of our study, we call for various governments and scholars to focus on areas where wetland parks and PAs overlap with urban boundaries. It is important to emphasize the potential link between the development of agriculture, forestry, livestock and fisheries industries, and UPAs. Overall, we believe that examining the accessibility of PAs can more accurately measure the distance between PAs and cities, and more realistically reflect the possible impacts of urban human activities on PAs, which is helpful for strengthening the conservation and management of PAs., (© 2024 The Authors. The COVID-19 pandemic, which last 2 years and still goes on, has pushed the primary health care (PC) to a current worrying situation of saturation and exhaustion. It is a community infectious disease, with a great amount of cases (around 10 million declared in January 2022) due to that, PC has made an extraordinary effort to pay attention on mild cases and on PC and to detect potentially serious cases early. Unfortunately, up to now, a global evaluation of the actions has not been carried out, in order to allow us to learn from this new experience. This article describes the different phases of the pandemic and its impact on PC. Finally, solutions are proposed to reinforce the central criteria that allow PC to be maintained as the foundation of the welfare state, longitudinality, resolution, accessibility, and care coordination and continuity, thanks to the contribution of resources and skills given to the PC. In conclusion, PC must still being the basis of the health system and it is mandatory to recover and claim those competencies and resources that should always have been a part of PC., (Copyright © 2022 SESPAS. Introduction: There are few studies on e-Health interventions directed at parents that analyse their costs and any potential savings that may occur. The aims of this study consisted of calculating the costs of the development and maintenance of paediatric web site for parents, including the costs per visit and per visitor, and the potentially savings made as regards medical visits avoided as a result of its use., Methods: The technology costs as well as the work of the professionals were considered as costs of the web site. The calculation of the cost of the professionals and the potential savings were based on the official fees and public prices of the Andalusian Public Health Service., Results: During 5years and 6months of the study, the total cost of the web site was €45,201.56. The cost per visit received was €0.0155€, and €0.0186 for each single visitor. Among the 516 users that took part in the study, face-to-face visits to Primary Care paediatric clinics were avoided, as well as those to Paediatric Emergency Department, at a savings of €22,263.89., Conclusions: The costs of developing a paediatric e-Health website for parents written in Spanish, using existing and free technologies, are low compared to other forms of e-Health development. Furthermore, the costs are considerably low if they are divided by the total number of visits or the number of visitors. Objective: To analyze the barriers that indigenous women face in access to the network of obstetric services in the context of the implementation of integrated healthcare networks (IHN)., Method: We designed a cross-sectional descriptive study including quantitative and qualitative methods. Sampling was intentional, no probabilistic. Data collection was carried out in Oaxaca, Mexico, during 2017-2018. A total of 149 indigenous women who used obstetrical services were surveyed and sociodemographic characteristics were obtained. Later were selected 30 cases that had complications during pregnancy and childbirth for a semi-structured interview. Non-participant observation was conducted., Results: The network of obstetric services comprises four institutions with different models of care and therefore different types of facilities and human resources to assist indigenous women. Nearly 20% of women did not start prenatal care in the first trimester of pregnancy and 27.2% had complications during the gestational period. The main barriers were availability (hours of operation, geographical aspects), accessibility (lack of financial resources), acceptability (ancestral practices vs. medical recommendations), and continuity of service (difficulties for admit patients in hospitals referred from first line of care)., Conclusions: The networks model allows access to obstetric services but does not guarantee care. For this it is necessary to improve both: the infrastructure of the obstetric service providers, and the care processes. Conservation planners need reliable information on spatial patterns of biodiversity. However, existing data sets are skewed because some ecosystems, taxa, and locations are underrepresented. We determined how many articles have been published in recent decades on the biodiversity of different countries and their constituent provinces. We searched the Web of Science catalogues Science Citation Index (SCI) and Social Science Citation Index (SSCI) for biodiversity-related articles published from 1993 to 2016 that included country and province names. We combined data on research publication frequency with other provincial-scale factors hypothesized to affect the likelihood of research activity (i.e., economic development, human presence, infrastructure, and remoteness). Areas that appeared understudied relative to the biodiversity expected based on site climate likely have been inaccessible to researchers for reasons, notably armed conflict. Geographic publication bias is of most concern in the most remote areas of sub-Saharan Africa and South America. Our provincial-scale model may help compensate for publication biases in conservation planning by revealing the spatial extent of research needs and the low cost of redoing this analysis annually., (© 2019 Society for Conservation Biology.)

Objective: To assess the accessibility level of Internet appointment scheduling in primary care and the fulfilment of the requirements of Spanish legislation., Method: Descriptive study of the accessibility of 18 web sites corresponding to the autonomic health services responsible for Internet appointment scheduling for primary health care services. The level of web accessibility was evaluated by means of five automated tools., Results: Only six websites self-declared to be in compliance with level AA of WCAG 2.0. The level of web accessibility according to the legal requirements in Spain is low. The evaluation tools identified the main errors to be corrected., Conclusions: Most of the autonomic health services responsible for Internet appointment scheduling in primary care need to improve their level of web accessibility and ensure that it complies with Spanish legislation., (Copyright © 2017 SESPAS. Introduction: Most elderly people or those with disabilities wish to stay in their own home rather than any other residential option, even when long-term care is required. The functional adaptation of homes is one of the key factors in doing this with safety and quality of life. This paper investigates whether people with disabilities (arising mainly from their advanced age) develop adaptive behaviours aimed at compensating for their functional loss through improvements in their home and environment, as well as knowing the covariables that explain it., Material and Methods: It starts from the so-called Lewin equation, which relates individual behaviours to personal and environmental characteristics. In this case the adaptive behaviour would be the economic expenditure on the home for its adaptation or improvement. The microdata of the Spanish EDAD 2008 survey are used, to which a bivariate logit model is applied to estimate the relationship between this adaptive expenditure and different explanatory covariates., Results: The results show that the Lewin model is fulfilled. The resulting coefficients and odds ratios show that the functional, economic, and environmental characteristics are what determine, above others of a personal or socioeconomic type, the adaptive expenditure., Conclusions: Housing adaptation is a voluntary adjustment behaviour justified by the functional conditions of the person and those of the housing and building environment. Given the importance of promoting better conditions for «ageing at home», there is a need to stimulate private expenditure on adaptation as a complementary formula to the necessary increase in existing public subsidy programs., (Copyright © 2018 SEGG. Objectives: To determine the frequency of use of Spanish pediatric emergency services, and to describe user profiles and geographic variations., Material and Methods: Descriptive study based on data from the Spanish National Health Survey. We calculated descriptive statistics and analyzed crude and adjusted odds ratios (ORs)., Results: Thirty-five percent of the 5495 respondents had come to an emergency department in the past year, and 88.1% of them had used the services of a Spanish national health service hospital. Factors associated with higher use of emergency services were male sex of the patient, (OR, 1.202; 95% CI, 1.047-1.381), a higher educational level of parents (OR, 1.255; 95% CI, 0.983-1.603), and younger age of the child (OR, 0.909; 95% CI, 0.894-0.924). Emergency department use varied widely from one Spanish community to another. There was a positive correlation between use and the presence of a foreign-born population (ρ=0.495, P=.031)., Conclusion: The rate of emergency department use is high in Spain. Variability between geographic areas is considerable, and some variation is explained by population characteristics.

Objectives: To measure accessibility to health care among diabetic patients and analyze whether differences in delay explain differences in hospital mortality., Methods: A retrospective cohort study was conducted in diabetic patients with acute coronary syndrome with ST-segment elevation included in the ARIAM-SEMICYUC registry (2010-2013). Crude and adjusted analyses were performed using unconditional logistic regression., Results: A total of 4817 patients were analyzed, of whom 1070 (22.2%) were diabetics. No differences were found in access to health care between diabetic and non-diabetic patients. Diabetic patients presented with longer patient delay (90 min vs. 75 min; p=.004) and prehospital delay (150 min vs. 130 min; p=.002). Once the health system was contacted, diabetic patients had a lower reperfusion rate (50% vs. 57.7%; p<.001), but no longer delay in treatment was observed compared with the non-diabetic individuals. Diabetic patients have greater in-hospital mortality (12.5 vs. 6%; p <.001), though neither patient delay nor prehospital delay were identified as independent predictors of in-hospital mortality., Conclusions: Diabetic patients had a longer delay in access to health care, though such delay was not independently related to increased mortality., (Copyright © 2015 Elsevier España, S.L.U. and SEMICYUC. The Department of Health of the Generalitat in Catalonia periodically draws up the Health Plan, which is the strategic document that brings together the reference framework for initiatives concerning public health in terms of the Catalan health administration. The 2011-2015 version of the Health Plan incorporates key care and system governance-related elements, which, in conjunction with health goals, make up the complete picture of what the health system in Catalonia should look like until 2015. The Plan was drawn up at a time when the environmental conditions were extremely particular, given the major economic crisis that began in 2007. This has meant that the system has been forced to address public health problems using a significant reduction in the economic resources available, while aiming to maintain the level of care provided, both quantitatively and qualitatively, and preserve the sustainability of the system whose defining traits are its universality, equity and the wide range of services on offer. Objective: Social isolation impairs health. An intervention to reduce isolation due to architectural barriers in elderly persons was carried out in Barcelona (Spain). This study aimed to evaluate its effects on health., Methods: We conducted a quasi-experimental before-after study. Isolated older people were identified in three deprived urban areas from 2009 to 2011. Participants had twice-weekly outings with volunteers in a stair-climbing power wheelchair. User satisfaction was evaluated and perceived health status, quality of life, and mental health before and after four outings were compared with McNemar tests., Results: There were 74 participants (median age: 83 years; IQR: 78-89). Perceived health improved by 21%, mental health by 24%, and psychological distress was reduced by 16%. Most participants (98%) were satisfied., Conclusion: The intervention improved perceived health and mental health. Elderly people with impaired mobility should not live in buildings with architectural barriers and, if this cannot be avoided, similar programs should be implemented., (Copyright © 2014 SESPAS.
