This study delivers a spatiotemporal economic risk evaluation of New Zealand’s road network to extreme sea-level driven flooding and relative sea level (RSL) change from 2020 to 2120. A spatial risk analysis framework was developed to calculate direct monetary loss as the expected exceedance probability loss (EPL) and average annual loss (AAL) at the road component level. These risk metrics were estimated at national and regional levels between 2020 and 2120 using RSL projections for medium confidence Shared Socio-economic Pathways (SSP) and local vertical land motion (VLM). New Zealand’s direct economic risk was primarily driven by direct damage to local access, collector, and arterial roads. At national levels, expected road AAL at 2100 could occur 10 to 20 years earlier as downward VLM accelerates local RSL rise later this century. Regional VLM trajectories may cause expected AAL to occur 20 years earlier from downward land motion and 5 years later from upward motion. This signals a need for VLM inclusion in future economic risk evaluations of episodic coastal flooding at all spatial and temporal scales. The spatiotemporal model approach has future potential for road network risk hotspot identification and structural or non-structural adaptation intervention evaluation under future RSL change. Introduction Previous research suggests that e-cigarettes can alter immune function, including in the nasal mucosa, in unique ways. The respiratory microbiome plays a key role in respiratory host defense, but the effects of e-cigarettes on the respiratory or nasal microbiome, are not well understood. Aims and Methods Using 16S rRNA gene sequencing on nasal samples from adult e-cigarette users, smokers, and nonsmokers, we determined that e-cigarette use and cigarette smoking are associated with differential respiratory microbiome dysbiosis and substantial sex-dependent differences in the nasal microbiome, particularly in e-cigarette users. Results Staphylococcus aureus, a common respiratory pathogen, was more abundant in both e-cigarette users and smokers in comparison with nonsmokers, while Lactobacillus iners , often considered a protective species, was more abundant in smokers but less abundant in e-cigarette users in comparison with nonsmokers. In addition, we identified significant dysbiosis of the nasal microbiome between e-cigarette users and smokers with high versus low serum cotinine levels, an indicator of tobacco product use and toxicant exposure. We also analyzed nasal lavage fluid for immune mediators associated with host × microbiota interactions. Conclusions Our analysis identified disruption of immune mediators in the nose of e-cigarette users and smokers, which is indicative of disrupted respiratory mucosal immune responses. Taken together, our data identified unique, sex-dependent host immune dysfunction associated with e-cigarette use in the nasal mucosa. More broadly, our data highlight the need for continued inclusion and careful consideration of sex as an important variable in the context of toxicant exposures. Implications This is the first study investigating the effects of e-cigarette use and sex on the nasal microbiome, which is considered an important gatekeeper for protecting the lower respiratory tract from pathogens. We found significant sex, exposure group, and serum cotinine level-associated differences in the composition of the nasal microbiome, demonstrating the importance of considering sex in future nasal microbiome studies and warranting further investigation of the mechanisms by which e-cigarette use dysregulates nasal immune homeostasis. Purpose of Review: The prevalence of benign prostatic hyperplasia (BPH) is rising, however, current treatment options present severe complications and limit patient's quality of life. Accordingly, advancements in prostatic catheter and stent designs for use in treating lower urinary tract symptoms (LUTS) in BPH patients have largely expanded in the past five years and we aim to provide an exhaustive summary of recent outcomes. Recent Findings: The dual dilation and paxlitaxel eluting Optilume BPH Catheter System enhances promise in catheter-based treatments, providing the longest sustained increase in max urinary flow rate and decrease in post-void residual volume compared to alternative MISTs. Additionally, use of iTiND, along with recent advancements in temporary (EXIME, Prodeon Urocross) and permanent (Zenflow Spring, Butterfly, and ClearRing) stent designs, have demonstrated rapid, lasting, and low-cost LUTS relief with sustained sexual function. Summary: Minimally invasive solutions that offer in-office treatment, rapid symptom relief, shorter recovery times, and preservation of sexual function hold great promise in improving outcomes in managing BPH patients with LUTS. Introduction: Aromatase excess syndrome (AEXS) is a rare, autosomal dominant disorder, characterized by enhanced aromatization of androgens and estrogen excess. In males it is characterized by pre-/peripubertal gynecomastia, hypogonadotropic hypogonadism, advanced bone age and short adult height. Only a few female patients have been described so far. Methods: We report on a family with four members with AEXS and present the long-term effects of aromatase inhibitor use in three of them. Genetic analysis showed a monoallelic 0.3-Mb deletion in 15q21, involving parts of CYP19A1 , GLDN and DMXL2 in all four patients with AEXS. Results: The index patient (male, 8 years old) presented with gynecomastia and accelerated growth and bone age. With start of puberty, estradiol levels increased, while testosterone levels remained low. Gynecomastia progressed and a mastectomy was performed twice. Presuming AEXS, a therapy with letrozole was initiated at the age of 19 years. Low-dose letrozole treatment was associated with an increase in testicular volume, increase in virilization and improvement in physical strength and libido. His brother (age 3 years) presented with accelerated growth and bone age. Treatment with letrozole, which was started at the age of 7 years, resulted in achieving an adult height of 179 cm and prevented the appearance of gynecomastia. His sister (age 6 years), who presented with premature thelarche and accelerated growth and bone age, was treated with an estrogen receptor modulator and a GnRH analog followed by letrozole treatment. Menarche occurred at age 13.5 years and adult height was 158 cm. Their father had an early, accelerated growth with an adult height of 171 cm, a delayed puberty and no gynecomastia. In vitro studies provided evidence for involvement of aromatase induction in atypical cells and an increased range of potential mechanisms regulating aromatase activity due to the presence of the mutated allele. Discussion: In conclusion, we observed a phenotypic variability within family members with AEXS carrying the same CYP19A1 microdeletion. When started early, treatment with letrozole was found to prevent the development of gynecomastia and increase adult height in one patient. Female Genital Schistosomiasis (FGS) is caused by Schistosoma haematobium, which causes chronic gynecological conditions that lead to substantial morbidity and infertility. This study's objective is to determine the prevalence and burden of FGS based on the presence of S. haematobium-specific DNA in females across age groups using our previously field-acquired filtered human urine samples from Zambia, Tanzania, and Ghana, collected over multiple years. For Ghana (2013), 39 out of 90 samples were from females, of which 31 (79.5%) were positive and 8 (20.5%) were negative. In Zambia (2016), 80 out of 133 samples were from females, of which 46 (57.5%) tested positive and 34 (42.5%) were negative. For Zambia (2017), 60 out of 110 samples were from females, of which 45 (75%) tested positive and 15 (25%) tested negative. In Tanzania (2018), 70 out of 104 samples were from females, of which 43 (61.4%) tested positive and 27 (38.6%) tested negative. FGS prevalence ranged from 57.5% (Zambia in 2016) to 79.5% (Ghana in 2013) and was found predominantly among the 11–20 years age group. Nutritional supplement use in athletes is common, accompanied by potential doping risk. To determine athletes' nutritional supplement and third-party-tested (TPT) supplement use, supplement knowledge, and factors influencing their behavior. Cross-sectional study. National Collegiate Athletic Association Division I athletic departments. Student-athletes (n = 410, 53% female, age = 21.4 ± 1.6 years). Survey questions addressed topics including nutritional supplement knowledge and use, TPT supplement knowledge and use, and logo recognition, and data were stratified for sex differences. Athletes (91%) reported the use of supplements, but the total number of supplements used was lower in female (median = 7; interquartile range, 4–11) than in male (median = 9; interquartile range, 4–12) athletes, with U = 17 960 and P =.01. A total of 48% (n = 191, out of 402 responses) reported purchasing supplements outside of their athletic department, with significantly fewer female (40%, n = 84) than male (56%, n = 107) athletes reporting this behavior (χ2 = 11.20, P <.001). No association between TPT logo recognition and TPT use was seen (χ2 = 0.238, P =.63). Of all athletes using supplements, 38% (n = 140) reported "consistent TPT use," whereas female athletes (36%, n = 70) reported this less often than male athletes (41%, n = 70, χ2 = 0.952, P <.32). No sex differences were seen for receiving nutritional counseling (89%, P =.37) or low nutritional supplement knowledge (<50%, P =.38); however, males had 2.5 times greater odds at recognizing a TPT organization logo than females (odds ratio = 2.45; 95% CI, 1.58–3.79). Most athletes use nutritional supplements. Female athletes report slightly fewer supplements than male athletes, while also less frequently purchasing them outside their athletic department, potentially explaining the lower TPT logo recognition in female athletes. A safety evaluation of this food enzyme was made previously, in which EFSA concluded that, under the intended conditions of use, this food enzyme did not give rise to safety concerns. Due to the implementation of a new methodology to estimate the dietary exposure to food enzymes in 2016, the European Commission requested EFSA to revise the exposure assessment of this food enzyme by using this new methodology. In this assessment, EFSA realigned the intended uses of this food enzyme to four food manufacturing processes. The dietary exposure was calculated to be up to 0.089 mg TOS/kg body weight (bw) per day in European populations. When combined with the no observed adverse effect level previously reported (49.1 mg TOS/kg bw per day, the lowest dose tested), the Panel derived a margin of exposure of at least 552. Based on the revised exposure estimate, the margin of exposure calculated thereof and the previous evaluation, the Panel concluded that this food enzyme does not give rise to safety concerns under the intended conditions of use. The food enzyme β‐fructofuranosidase (β‐d‐fructofuranoside fructohydrolase; EC 3.2.1.26) is produced with the non‐genetically modified Saccharomyces cerevisiae strain CNCM I‐3399 by Kerry Ingredients & Flavours Ltd. The production strain qualifies for the qualified presumption of safety (QPS) approach for risk assessment. The food enzyme is intended to be used in four food manufacturing processes. Exposure was estimated to be up to 2.485 mg total organic solids/kg body weight (bw) per day in European populations. As the production strain qualifies for QPS, and in the absence of other concerns, the Panel considered that toxicological studies were not needed for the safety assessment of this food enzyme. A search for homology of the amino acid sequence of the food enzyme to known allergens was made and one match with a tomato allergen was found. The Panel considered that the risk of allergic reactions upon dietary exposure to this food enzyme, particularly in tomato allergic individuals, cannot be excluded, but the likelihood is low. It was estimated to be up to 1.488 mg TOS/kg body weight (bw) per day in European populations. Genotoxicity tests did not indicate a safety concern. The systemic toxicity was assessed by means of a repeated dose 90‐day oral toxicity study in rats. In a previous opinion, the Panel noted the presence of recombinant DNA in all food enzyme batches tested. As a follow‐up, the applicant changed the manufacturing process of the food enzyme and provided new data. The genetic modifications do not give rise to safety concerns and the production strain fulfils the requirements for the QPS approach to safety assessment. The systemic toxicity was assessed by means of a repeated dose 90‐day oral toxicity study in rats. The food enzyme glucose oxidase (β‐d‐glucose: oxygen 1‐oxidoreductase; EC 1.1.3.4) is produced with the genetically modified Trichoderma reesei strain AR‐352 by AB Enzymes GmbH. In a previous opinion, the Panel on Food Contact Materials, Enzymes and Processing Aids of the European Food Safety Authority could not conclude on the absence of recombinant DNA from the production strain in the food enzyme due to uncertainties about the limit of detection of the applied methodology. New data provided by the applicant showed that no DNA from the production strain was found in the food enzyme with a limit of detection meeting the requirements of the applicable guidance. Introduction: Thymus resident B cells were described more than 40 years ago. In early human life, these cells are found predominantly in the medulla and overwhelmingly display an unswitched IgM+ phenotype. The reactivity of thymic IgM B cells, however, is still unclear. Methods: Here, we generated 120 IgM-producing B cell clones from 3 separate thymus specimens obtained from infant, adolescent, and adult donors. Using flow cytometry and a unique high-dimensional ELISA platform, we investigated the clones' reactivity to apoptotic cells as well as to common chemical adducts exposed on modified amino acids and other macromolecules. Results: Regardless of the age, approximately 30-40% of thymic IgM B cells reacted to apoptotic cells. Further, 30-40% displayed reactivity to at least one adduct, including malondialdehyde, Homocysteine, and NEDD 8. Four distinct reactivity patterns were identified through this profiling. Notably, a significant association was observed between reactivity to apoptotic cells, and to one or more adducts, suggesting that the same determinants were recognized in both assays. Additionally, thymic IgM B cells reactive to adducts were more likely to recognize intra-nuclear or intra-cytoplasmic structures in Hep-2 cells as revealed by immunofluorescence staining. Conclusion/Discussion: Collectively, our findings suggest that thymic IgM B cells actively uptake apoptotic bodies and cellular debris in the medulla by binding specific chemical adducts. Migrations between spawning, growth and refuge habitats are critical for many fish species. Partially migratory populations of brook charr Salvelinus fontinalis (known as brook trout in the United States), in which a portion of the population migrates, were once widespread in the Great Lakes region of North America, but are now scarce and a rehabilitation priority. Great Lakes fishery managers lack a simple, non-lethal means to determine whether a large brook charr caught in a Great Lakes accessible stream reach represents a stream-resident fish or migrant that previously spent time foraging and growing in Great Lakes waters. We explored a relatively inexpensive and non-lethal stable isotope approach for documenting lake to stream movements of brook charr, using fin clips from brook charr captured in streams in fall prior to spawning. Using fin tissue from juvenile and adult coho salmon captured in Michigan tributaries to Lake Superior, we confirmed distinct stable isotope signatures indicative of prior stream and Lake Superior foraging. We identified brook charr from tributaries accessible to Lake Superior whose stable isotope signatures were similar to those of lake-captured brook charr, lake charr and splake (a brook charr–lake charr hybrid) and distinct from stable isotope signatures of brook charr in streams inaccessible to Lake Superior fishes, suggestive of an adfluvial migratory pattern. Brook charr from two streams had elevated δ15N values, indicative of previous residency in a downstream lake receiving water from a sewage treatment facility. Flood damage assessment is critical for optimal risk management investments. Damage models evaluate physical damage or monetary loss from direct building exposure to flood hazard processes. Traditional models represent a simple relationship whereby physical damage increases with water depth. More complex models offer an improved understanding of vulnerability, analysing interactions between multiple hazard and exposure variables that drive damage. Our study investigates whether increasing model complexity and explanatory damage variables improves prediction precision and reliability at residential building and sub-building (component) levels. We evaluate simple and complex empirical univariable and multivariable models for flood damage prediction. The Random Forest algorithm learned on multiple hazard and exposure explanatory variables outperformed linear and non-linear univariable regression approaches. Random Forest model predictive precision was highest when learning was limited to water depth and several important explanatory damage variables (flow velocity, area and floor height). Component damage models demonstrated high predictive precision for internal finishes and services. Precision reduced for structure and external finishes as damage samples for model learning were limited. High performing but complex multivariable models require further spatio-temporal transfer investigation to determine opportunities for accurate and reliable object-specific flood damage prediction in data scarce locations. The organizational communication discipline has been criticized for not centering the voices of ethnic minority communities. This can be witnessed in the prevention of violence, where migrant communities are more vulnerable to the effects of violence but are underserved by Western models of violence prevention. Grounded in the culture-centered approach, the present study—drawing from interviews with 67 new migrants and 31 service providers—explores how organizations can assist in preventing violence among new migrant communities. The findings reveal how communication inequality reduces agency and can create an environment conducive to violence and how the current individualized model of service delivery does not attend to the cultural norms and needs of community members. Engaging with new migrant communities and addressing violence involves creating communicative avenues of dialogue and leveraging community agency in codeveloping infrastructures for transformation. Purpose Biallelic pathogenic leptin gene variants cause severe early-onset obesity usually associated with low or undetectable circulating leptin levels. Recently, variants have been described resulting in secreted mutant forms of the hormone leptin with either biologically inactive or antagonistic properties. Methods We conducted a systematic literature research supplemented by unpublished data from patients at our center as well as new in vitro analyses to provide a systematic classification of congenital leptin deficiency based on the molecular and functional characteristics of the underlying leptin variants and investigated the correlation of disease subtype with severity of the clinical phenotype. Results A total of 28 distinct homozygous leptin variants were identified in 148 patients. The identified variants can be divided into 3 different subtypes of congenital leptin deficiency: classical hormone deficiency (21 variants in 128 patients), biologically inactive hormone (3 variants in 12 patients), and antagonistic hormone (3 variants in 7 patients). Only 1 variant (n = 1 patient) remained unclassified. Patients with biological inactive leptin have a higher percentage of 95th body mass index percentile compared to patients with classical hormone deficiency. While patients with both classical hormone deficiency and biological inactive hormone can be treated with the same starting dose of metreleptin, patients with antagonistic hormone need a variant-tailored treatment approach to overcome the antagonistic properties of the variant leptin. Background and Aims: Alpha‐1 antitrypsin deficiency is an inherited disorder caused by alpha‐1 antitrypsin (AAT) mutations. We analysed the association between alcohol intake and liver‐related parameters in individuals with the heterozygous/homozygous Pi*Z AAT variant (Pi*MZ/Pi*ZZ genotype) found in the United Kingdom Biobank and the European Alpha1 liver consortium. Methods: Reported alcohol consumption was evaluated in two cohorts: (i) the community‐based United Kingdom Biobank (17 145 Pi*MZ, 141 Pi*ZZ subjects, and 425 002 non‐carriers [Pi*MM]); and (ii) the European Alpha1 liver consortium (561 Pi*ZZ individuals). Cohort (ii) included measurements of carbohydrate‐deficient transferrin (CDT). Results: In both cohorts, no/low alcohol intake was reported by >80% of individuals, while harmful consumption was rare (~1%). Among Pi*MM and Pi*MZ individuals from cohort (i), moderate alcohol consumption resulted in a <30% increased rate of elevated transaminases and ~50% increase in elevated gamma‐glutamyl transferase values, while harmful alcohol intake led to an at least twofold increase in the abnormal levels. In Pi*ZZ individuals from both cohorts, moderate alcohol consumption had no marked impact on serum transaminase levels. Among Pi*ZZ subjects from cohort (ii) who reported no/low alcohol consumption, those with increased CDT levels more often had signs of advanced liver disease. Conclusions: Pi*MZ/Pi*ZZ genotype does not seem to markedly aggravate the hepatic toxicity of moderate alcohol consumption. Ataxia with anti-regulator of G-protein signaling 8 autoantibodies (RGS8-Abs) is an autoimmune disease recently described in four patients. The present study aimed to identify other patients with RGS8-Abs, describe their clinical features, including the link between RGS8-related autoimmune cerebellar ataxia (ACA) and cancer. Patients with RGS8-Abs were identified retrospectively in the biological collections of the French Reference Center for Paraneoplastic Neurological Syndrome and the University of California San Francisco Center for Encephalitis and Meningitis. Clinical data were collected, and cerebrospinal fluid, serum, and tumor pathological samples were retrieved to characterize the autoantibodies and the associated malignancies. Only three patients with RGS8-Abs were identified. All of them presented with a pure cerebellar ataxia of mild to severe course, unresponsive to current immunotherapy regimens for ACA. Two patients presented with a Hodgkin lymphoma of the rare specific subtype called nodular lymphocyte-predominant Hodgkin lymphoma, with very mild extension. Autoantibodies detected in all patients enriched the same epitope on the RGS8 protein, which is an intracellular protein physiologically expressed in Purkinje cells but also ectopically expressed specifically in lymphoma cells of patients with RGS8-related ACA. The present results and those of the four cases previously described suggest that RGS8-Abs define a new paraneoplastic neurological syndrome of extreme rarity found mostly in middle-aged males that associates pure cerebellar ataxia and a particular lymphoma specifically expressing the RGS8 antigen. Thermal properties of grouting materials for borehole heat exchangers (BHE) are currently analysed with varying measurement methods and analysis procedures, resulting in difficulties when comparing values of different studies. This study therefore provides the first comprehensive investigation of different analysis procedures by systematically comparing the influence of the measurement method and the sample preparation on the determination of the thermal conductivity and the volumetric heat capacity. Seven dissimilar grouting materials with varying water–solid ratios (W/S) and compositions are analysed. The thermal conductivities of the materials range between 0.9 and 1.8 W m−1 K−1 (transient plane source method, TPS). The volumetric heat capacities range between 3.01 and 3.63 MJ m−3 K−1 (differential scanning calorimetry, DSC). From the findings of this study, a standardised analysis of grouting materials is provided which suggests mixing of the grouting material at a high mixing speed and sample curing under water for 28 days at room temperature. The benefits of calculating the volumetric heat capacities of grouting materials from the specific heat capacities of dry samples measured with the DSC, the water content and the bulk density are demonstrated. Furthermore, an estimation procedure of volumetric heat capacity from the W/S and suspension density with an uncertainty of smaller ± 5% is provided. The most decisive challenge for their successful implementation is the lifetime of the lowest triplet metal‐to‐ligand charge transfer state (3MLCT), which typically decays via a triplet metal‐centered (3MC) state back to the ground state. We reveal by variable‐temperature ultrafast transient absorption spectroscopy that the tripodal iron(II) bis(pyridine) complex isomers trans‐ and cis‐[Fe(pdmi)2]2+ with four NHC donors show 3MLCT→3MC population transfers with very different barriers and rationalize this by computational means. While trans‐[Fe(pdmi)2]2+ possesses an unobservable activation barrier, the cis isomer exhibits a barrier of 492 cm−1, which leads to a nanosecond 3MLCT lifetime at 77 K. The kinetic and quantum chemical data were analyzed in the context of semi‐classical Marcus theory revealing a high reorganization energy and small electronic coupling between the two triplet states. Purpose of Review: Benign prostatic hyperplasia (BPH) is prevalent in nearly 70% of men over the age of 60, leading to significant clinical challenges due to varying symptom presentations and treatment responses. The decision to undergo surgical intervention is not straightforward; the American Urological Association recommends consideration of surgical treatment after inadequate or failed response to medical therapy. This review explores the role of artificial intelligence (AI), including machine learning and deep learning models, in enhancing the decision-making processes for BPH management. Recent Findings: AI applications in this space include analysis of non-invasive imaging modalities, such as multiparametric Magnetic Resonance Imaging (MRI) and Ultrasound, which enhance diagnostic precision. AI models also concatenate serum biomarkers and histopathological analysis to distinguish BPH from prostate cancer (PC), offering high accuracy rates. Furthermore, AI aids in predicting patient outcomes post-treatment, supporting personalized medicine, and optimizing therapeutic strategies. Summary: AI has demonstrated potential in differentiating BPH from PC through advanced imaging and predictive models, improving diagnostic accuracy, and reducing the need for invasive procedures. Despite promising advancements, challenges remain in integrating AI into clinical workflows, establishing standard evaluation metrics, and achieving cost-effectiveness. The novel PSA_Lip showed only 23.8–25.1%, 25.5%, 26.6% and 28.4% identity to the previously characterized GDSL-like enzymes phospholipase, plant lipase, acetylcholinesterase and acetylxylan esterase, from the carbohydrate esterase family 16, respectively. Therefore, the enzyme was purified from the culture supernatant and the catalytic properties and the substrate specificity of the enzyme were investigated using different assays to reveal its potential function. While no phospholipase, acetylcholinesterase and acetylxylan esterase activities were detected, studies on the hydrolysis of ferulic acid methyl ester (~ 8.3%) and feruloylated carbohydrate 5-O-transferuloyl-arabino-furanose (~ 0.8%) showed low conversions of these substrates. By investigating the hydrolytic activity towards p-nitrophenyl-(pNP)-esters with various chain-lengths, the highest activity was determined for medium chain-length pNP-octanoate at 65 °C and a pH value of 8, while almost no activity was detected for pNP-hexanoate. The enzyme is highly stable when stored at pH 10 and 4 °C for at least 7 days. Moreover, using consensus sequence analysis and homology modeling, we could demonstrate that the PSA_Lip does not contain the usual SGNH residues in the actives site, which are usually present in GDS(L)-like enzymes. Introduction: Benign prostatic hyperplasia (BPH) is a condition that affects over 50% of men as they enter their fifth decade of life, often leading to lower urinary tract symptoms (LUTS). Primary treatment options include alpha blockers, 5-alpha reductase inhibitors, and phosphodiesterase-5 inhibitors. However, these medications can have some side effects, and there is a noticeable dearth of information addressing the long-term use of these medications. Thus, the exploration of all treatment modalities helps ensure patients receive personalized and effective care. Consequently, the primary objective of this review is to identify potential emerging medications for the treatment of BPH. Areas covered: We conducted an extensive review of articles discussing pharmacotherapy for BPH spanning the last 15 years. Our information gathering process involved Scopus, PubMed-MEDLINE, Cochrane, Wiley Online Library Google Scholar, ClinicalTrials.gov, and the PharmaProjects database. This approach ensures that readers gain an in-depth knowledge of the existing therapeutic agents as well as promising avenues for managing BPH. Expert opinion: BPH treatment targets a patient's specific constellation of symptoms. Therefore, a broad knowledge base encompassing various treatment options is paramount in ensuring optimal treatment. Multisystem inflammatory syndrome in children (MIS-C) is a severe, post-infectious sequela of SARS-CoV-2 infection1,2, yet the pathophysiological mechanism connecting the infection to the broad inflammatory syndrome remains unknown. Here we leveraged a large set of samples from patients with MIS-C to identify a distinct set of host proteins targeted by patient autoantibodies including a particular autoreactive epitope within SNX8, a protein involved in regulating an antiviral pathway associated with MIS-C pathogenesis. In parallel, we also probed antibody responses from patients with MIS-C to the complete SARS-CoV-2 proteome and found enriched reactivity against a distinct domain of the SARS-CoV-2 nucleocapsid protein. The immunogenic regions of the viral nucleocapsid and host SNX8 proteins bear remarkable sequence similarity. Consequently, we found that many children with anti-SNX8 autoantibodies also have cross-reactive T cells engaging both the SNX8 and the SARS-CoV-2 nucleocapsid protein epitopes. Together, these findings suggest that patients with MIS-C develop a characteristic immune response to the SARS-CoV-2 nucleocapsid protein that is associated with cross-reactivity to the self-protein SNX8, demonstrating a mechanistic link between the infection and the inflammatory syndrome, with implications for better understanding a range of post-infectious autoinflammatory diseases.A cross-reactive antibody and T cell response is identified in a large portion of patients with multisystem inflammatory syndrome in children.
