Tuberculosis (TB) alone caused over a billion deaths in the last 200 years, making it one of the deadliest diseases to humankind. Understanding the immune mechanisms underlying protection or pathology in TB is key to uncover the much needed innovative approaches to tackle TB. The scavenger receptor cysteine‐rich molecule CD5 antigen‐like (CD5L) has been associated with TB, but whether and how CD5L shapes the immune response during the course of disease remains poorly understood. Here, we show an upregulation of CD5L in circulation and at the site of infection in C57BL/6 Mycobacterium tuberculosis‐infected mice. To investigate the role of CD5L in TB, we studied the progression of M. tuberculosis aerosol infection in a recently described genetically engineered mouse model lacking CD5L. Despite the increase of CD5L during infection of wild‐type mice, absence of CD5L did not impact bacterial burden, histopathology or survival of infected mice. Absence of CD5L associated with a modest increase in the numbers of CD4+ T cells and the expression of IFN‐γ in the lungs of infected mice, with no major effect in overall immune cell dynamics. Simple Summary: This study evaluated the ability of Large Language Models (LLMs) to classify subjective toxicities from chemotherapy by comparing them with expert oncologists. Using fictitious cases, it was demonstrated that LLMs can achieve accuracy similar to that of oncologists in general toxicity categories, although they need improvement in specific categories. LLMs show great potential for enhancing patient monitoring and reducing the workload of doctors. Future research should focus on training LLMs specifically for medical tasks and validating these findings with real patients, always ensuring accuracy and ethical data management. Introduction: Large Language Models (LLMs), such as the GPT model family from OpenAI, have demonstrated transformative potential across various fields, especially in medicine. These models can understand and generate contextual text, adapting to new tasks without specific training. This versatility can revolutionize clinical practices by enhancing documentation, patient interaction, and decision-making processes. In oncology, LLMs offer the potential to significantly improve patient care through the continuous monitoring of chemotherapy-induced toxicities, which is a task that is often unmanageable for human resources alone. However, existing research has not sufficiently explored the accuracy of LLMs in identifying and assessing subjective toxicities based on patient descriptions. This study aims to fill this gap by evaluating the ability of LLMs to accurately classify these toxicities, facilitating personalized and continuous patient care. Methods: This comparative pilot study assessed the ability of an LLM to classify subjective toxicities from chemotherapy. Thirteen oncologists evaluated 30 fictitious cases created using expert knowledge and OpenAI's GPT-4. These evaluations, based on the CTCAE v.5 criteria, were compared to those of a contextualized LLM model. Metrics such as mode and mean of responses were used to gauge consensus. The accuracy of the LLM was analyzed in both general and specific toxicity categories, considering types of errors and false alarms. The study's results are intended to justify further research involving real patients. Results: The study revealed significant variability in oncologists' evaluations due to the lack of interaction with fictitious patients. The LLM model achieved an accuracy of 85.7% in general categories and 64.6% in specific categories using mean evaluations with mild errors at 96.4% and severe errors at 3.6%. False alarms occurred in 3% of cases. When comparing the LLM's performance to that of expert oncologists, individual accuracy ranged from 66.7% to 89.2% for general categories and 57.0% to 76.0% for specific categories. The 95% confidence intervals for the median accuracy of oncologists were 81.9% to 86.9% for general categories and 67.6% to 75.6% for specific categories. These benchmarks highlight the LLM's potential to achieve expert-level performance in classifying chemotherapy-induced toxicities. Discussion: The findings demonstrate that LLMs can classify subjective toxicities from chemotherapy with accuracy comparable to expert oncologists. The LLM achieved 85.7% accuracy in general categories and 64.6% in specific categories. While the model's general category performance falls within expert ranges, specific category accuracy requires improvement. The study's limitations include the use of fictitious cases, lack of patient interaction, and reliance on audio transcriptions. Nevertheless, LLMs show significant potential for enhancing patient monitoring and reducing oncologists' workload. Future research should focus on the specific training of LLMs for medical tasks, conducting studies with real patients, implementing interactive evaluations, expanding sample sizes, and ensuring robustness and generalization in diverse clinical settings. Conclusions: This study concludes that LLMs can classify subjective toxicities from chemotherapy with accuracy comparable to expert oncologists. The LLM's performance in general toxicity categories is within the expert range, but there is room for improvement in specific categories. LLMs have the potential to enhance patient monitoring, enable early interventions, and reduce severe complications, improving care quality and efficiency. (1) Background: Liver metastases (LM) are the leading cause of death in colorectal cancer (CRC) patients. Despite advancements, relapse rates remain high and current prognostic nomograms lack accuracy. Our objective is to develop an interpretable neoadjuvant algorithm based on mathematical models to accurately predict individual risk, ensuring mathematical transparency and auditability. (2) Methods: We retrospectively evaluated 86 CRC patients with LM treated with neoadjuvant systemic therapy followed by complete surgical resection. A comprehensive analysis of 155 individual patient variables was performed. Logistic regression (LR) was utilized to develop the predictive model for relapse risk through significance testing and ANOVA analysis. Due to data limitations, gradient boosting machine (GBM) and synthetic data were also used. (3) Results: The model was based on data from 74 patients (12 were excluded). After a median follow-up of 58 months, 5-year relapse-free survival (RFS) rate was 33% and 5-year overall survival (OS) rate was 60.7%. Fifteen key variables were used to train the GBM model, which showed promising accuracy (0.82), sensitivity (0.59), and specificity (0.96) in predicting relapse. Similar results were obtained when external validation was performed as well. Barber–Say syndrome (BSS) is a rare congenital ectodermal dysplasia with few cases reported in the literature. We describe a 9‐year‐old boy with congenital generalized hypertrichosis and multiple rhabdomyomatous mesenchymal hamartomas (RMHs) on his nose and periocular region. Next‐generation sequencing, performed in DNA from a blood sample, and RMH tissue, revealed a pathogenic variant in the TWIST2 gene, which was not detected in a salivary sample of the patient, nor in his parents. Therefore, we consider this variant as de novo mosaicism. To our knowledge, this is the first case of multiple RMHs associated with BSS. [ABSTRACT FROM AUTHOR]

There is no established treatment for progressive IgA nephropathy refractory to steroids and immunosuppressant drugs (r-IgAN). Interleukin 17 (IL-17) blockade has garnered interest in immune-mediated diseases involving the gut-kidney axis. However, single IL-17A inhibition induced paradoxical effects in patients with Crohn's disease and some cases of de novo glomerulonephritis, possibly due to the complete Th1 cell response, along with the concomitant downregulation of regulatory T cells (Tregs). Seven r-IgAN patients were treated with at least six months of oral paricalcitol, followed by the addition of subcutaneous anti-IL-17A (secukinumab). After a mean follow-up of 28 months, proteinuria decreased by 71% (95% CI: 56–87), P < 0.001. One patient started dialysis, while the annual eGFR decline in the remaining patients [mean (95% CI)] was reduced by 4.9 mL/min/1.73 m2 (95% CI: 0.1–9.7), P = 0.046. Circulating Th1, Th17, and Treg cells remained stable, but Th2 cells decreased, modifying the Th1/Th2 ratio. Intriguingly, accumulation of circulating Th17.1 cells was observed. This novel sequential therapy appears to optimize renal advantages in patients with r-IgAN and elicit alterations in potentially pathogenic T helper cells. [ABSTRACT FROM AUTHOR]

Background: Calprotectin is a calcium‐binding‐S100‐protein synthetized mainly in neutrophils which has been demonstrated to be an accurate biomarker of the presence of these cells. Gut barrier dysfunction in patients with advanced chronic liver disease (ACLD), in addition to the lack of noninvasive tools for diagnosis and prognosis of cirrhosis decompensations, has raised interest in this biomarker. Aims: Our aim is to summarize the current evidence regarding the role of calprotectin in terms of its diagnostic and prognostic utility in ACLD. Methods: We performed a systematic search (PROSPERO registration no. CRD42023389069) of original articles published without any restrictions on the publication date until January 2023 providing information about calprotectin for the prognosis or diagnosis of ACLD and its decompensations in adult patients. Results: A total 227 articles were identified, and 26 observational studies finally met the inclusion criteria. In 14 studies, calprotectin was measured in ascitic fluid, all of which reported higher calprotectin values in spontaneous bacterial peritonitis, while cut‐off points for its diagnosis were proposed in nine studies. Three studies reported higher faecal calprotectin levels in patients with hepatic encephalopathy and portal hypertension. Four studies evaluated faecal calprotectin and one plasma calprotectin as biomarkers for gut barrier integrity and bacterial translocation. Conclusions: Calprotectin is emerging as a promising biomarker in ACLD, particularly for the management of bacterial infections and alcohol‐related liver disease. The aim of this study was to describe the spectral features of pre-stimulus event-related potential (ERP) components elicited in visual tasks such as the Bereitschaftspotential (BP), prefrontal negativity (pN) and visual negativity (vN). ERPs are considered time-locked and phase-locked (evoked) activity, but we have also analyzed the non-phase but time-locked (induced) activity in the same interval by applying the temporal spectral evolution (TSE) method. Participants (N = 26) were tested in a passive task, a simple response task (SRT) and a discriminative response task (DRT), where EEG activity was recorded with 64 scalp electrodes. We analyzed the time-frequency modulations (phase and non-phase) prior to the onset of the stimuli in the sub-delta, delta, theta, alpha, beta, and gamma frequency bands. The results showed that all the pre-stimulus ERP components were mainly regulated by evoked activity in the sub-delta band. On the other hand, induced activity seems to be linked to evoked responses but with a different psychophysiological role. We concluded that other preparatory cognitive mechanisms associated with ERPs can also be detected by the TSE method. This finding may suggest underlying mechanisms in non-phase activity and requires the addition of non-phase activity analysis to the traditional analysis (phase and evoked activity). There is an increasing demand to enhance infrastructure asset management within the drinking water sector. A key factor for achieving this is improving the accuracy of pipe failure prediction models. Machine learning-based models have emerged as a powerful tool in enhancing the predictive capabilities of water distribution network models. Extensive research has been conducted to explore the role of explanatory variables in optimizing model outputs. However, the underlying mechanisms of incorporating explanatory variable data into the models still need to be better understood. This review aims to expand our understanding of explanatory variables and their relationship with existing models through a comprehensive investigation of the explanatory variables employed in models over the past 15 years. The review underscores the importance of obtaining a substantial and reliable dataset directly from Water Utilities databases. Only with a sizeable dataset containing high-quality data can we better understand how all the variables interact, a crucial prerequisite before assessing the performance of pipe failure rate prediction models. Background and objectives: Intermittent theta-burst stimulation (iTBS) is a patterned form of excitatory transcranial magnetic stimulation that has yielded encouraging results as an adjunctive therapeutic option to alleviate the emergence of clinical deficits in Parkinson’s disease (PD) patients. Although it has been demonstrated that iTBS influences dopamine-dependent corticostriatal plasticity, little research has examined the neurobiological mechanisms underlying iTBSinduced clinical enhancement. Here, our primary goal is to verify whether iTBS bilaterally delivered over the primary motor cortex (M1) is effective as an add-on treatment at reducing scores for both motor functional impairment and nonmotor symptoms in PD. We hypothesize that these clinical improvements following bilateral M1-iTBS could be driven by endogenous dopamine release, which may rebalance cortical excitability and restore compensatory striatal volume changes, resulting in increased striato-cortico-cerebellar functional connectivity and positively impacting neuroglia and neuroplasticity. Methods: A total of 24 PD patients will be assessed in a randomized, doubleblind, sham-controlled crossover study involving the application of iTBS over the bilateral M1 (M1 iTBS). Patients on medication will be randomly assigned to receive real iTBS or control (sham) stimulation and will undergo 5 consecutive sessions (5  days) of iTBS over the bilateral M1 separated by a 3-month washout period. Motor evaluation will be performed at different follow-up visits along with a comprehensive neurocognitive assessment; evaluation of M1 excitability; combined structural magnetic resonance imaging (MRI), resting-state electroencephalography and functional MRI; and serum biomarker quantification of neuroaxonal damage, astrocytic reactivity, and neural plasticity prior to and after iTBS. Discussion: The findings of this study will help to clarify the efficiency of M1 iTBS for the treatment of PD and further provide specific neurobiological insights into improvements in motor and nonmotor symptoms in these patients. This novel project aims to yield more detailed structural and functional brain evaluations than previous studies while using a noninvasive approach, with the potential to identify prognostic neuroprotective biomarkers and elucidate the structural and functional mechanisms of M1 iTBS-induced plasticity in the cortico-basal ganglia circuitry. Our approach may significantly optimize neuromodulation paradigms to ensure state-of-the-art and scalable rehabilitative treatment to alleviate motor and nonmotor symptoms of PD. Simple Summary: Cancer treatments often have side effects that may impair patients' quality of life. Our research aimed to create a predictive tool able to foresee the likelihood of these severe complications. We used medical data from 267 gastrointestinal cancer patients, applied a particular type of computer model known as a Bayesian network, and evaluated its predictions against real outcomes. The model accurately predicted the risk of developing severe haematological toxicity in 80–85% of cases. This tool, if further validated and refined, may help to identify a vulnerable subset of patients who might benefit from personalized treatment plans. Purpose: Severe toxicity is reported in about 30% of gastrointestinal cancer patients receiving 5-Fluorouracil (5-FU)-based chemotherapy. To date, limited tools exist to identify at risk patients in this setting. The objective of this study was to address this need by designing a predictive model using a Bayesian network, a probabilistic graphical model offering robust, explainable predictions. Methods: We utilized a dataset of 267 gastrointestinal cancer patients, conducting preprocessing, and splitting it into TRAIN and TEST sets (80%:20% ratio). The RandomForest algorithm assessed variable importance based on MeanDecreaseGini coefficient. The bnlearn R library helped design a Bayesian network model using a 10-fold cross-validation on the TRAIN set and the aic-cg method for network structure optimization. The model's performance was gauged based on accuracy, sensitivity, and specificity, using cross-validation on the TRAIN set and independent validation on the TEST set. Results: The model demonstrated satisfactory performance with an average accuracy of 0.85 (±0.05) and 0.80 on TRAIN and TEST datasets, respectively. The sensitivity and specificity were 0.82 (±0.14) and 0.87 (±0.07) for the TRAIN dataset, and 0.71 and 0.83 for the TEST dataset, respectively. A user-friendly tool was developed for clinical implementation. Conclusions: Despite several limitations, our Bayesian network model demonstrated a high level of accuracy in predicting the risk of developing severe haematological toxicity in gastrointestinal cancer patients receiving 5-FU-based chemotherapy. The objective of this article is to introduce the package Rchoice which provides functionality for estimating heteroskedastic and instrumental variable models for binary outcomes, whith emphasis on the calculation of the average marginal effects. To do so, I introduce two new functions of the Rchoice package using widely known applied examples. I also show how users can generate publication-ready tables of regression model estimates. Atherosclerosis, a process in which macrophages play a key role, is accelerated in diabetes. Elevated concentrations of serum-oxidized low-density lipoproteins (oxLDL) represent a common feature of both conditions. The main goal of this study was to determine the contribution of oxLDL to the inflammatory response of macrophages exposed to diabetic-mimicking conditions. THP1 cells and peripheral blood monocytes purified from non-diabetic healthy donors were cultured under normal (5 mM) or high glucose (HG) (15 mM) with oxLDL. Then, foam cell formation, expression of CD80, HLADR, CD23, CD206, and CD163, as well as toll-like receptor 4 (TLR4) and co-receptors CD36 and CD14 (both at the cell surface and soluble (sCD14)), and inflammatory mediators' production were measured by flow cytometry, RT-qPCR, or ELISA. Additionally, serum sCD14 was determined in subjects with subclinical atherosclerosis with and without diabetes by ELISA. Our results showed that oxLDL-mediated intracellular lipid accumulation via CD36 increased under HG and that HG + oxLDL enhanced TNF, IL1B, and IL8, and decreased IL10. Moreover, TLR4 was upregulated in macrophages under HG and monocytes of subjects with diabetes and atherosclerosis. Interestingly, HG-oxLDL upregulated CD14 gene expression, although its total cellular protein abundance remained unaltered. sCD14 shedding via PRAS40/Akt-dependent mechanisms, with pro-inflammatory activity, was significantly increased in cultured macrophages and plasma from subjects with diabetes and subclinical atherosclerosis or hypercholesterolemia. Our data support an enhanced synergistic pro-inflammatory effect induced by HG and oxLDL in cultured human macrophages, possibly explained by increased sCD14 shedding. [ABSTRACT FROM AUTHOR]

Background: Adequate synchronization between the passive ("E") and active ("a") left ventricular (LV) diastolic filling contributes to the efficiency of the heartbeat. E/a superposition in dual-chamber pacemaker (PM) recipients is an under-recognized phenomenon that may be corrected by shortening the atrio-ventricular interval (AVI). We aimed at establishing the prevalence of E/a superposition in PM patients and to analyze the clinical, echocardiographic, and biological impact of AVI shortening. Methods: Seventy patients with dual-chamber PMs (74 ± 8 years old, 12 women) were consecutively enrolled in this study. Patients with baseline E/a superposition were crossed over from default to manually shortened AVI or vice versa in a case–control fashion (intervention group). Patients without baseline E/a superposition (controls) served as a reference for a descriptive comparison with the intervention group. Results: Thirty-three patients had E/a superposition after PM implantation (47%). Controls (n = 37) had higher LV ejection fraction (59 ± 8% vs. 53 ± 10%, p = 0.048) and lower levels of high sensitive troponin T and ST2 (p < 0.05) than intervention group patients. The AVI was shortened at 48 ± 9 ms in order to ensure adequate E/a separation. The walked distance increased from 75 ± 17 to 78 ± 10% (p = 0.049) and the Euro-QoL score from 0.50 ± 0.27 to 0.63 ± 0.19 (p = 0.011) with short AVI. Conclusions: E/a superposition occurs in approximately half of dual-chamber PM recipients and is associated with reduced LV function and increased myocardial injury biomarkers. Childhood obesity is not only worrisome for its effects on children's health but also for its effects on general well-being. This article analyzes the impact of bodyweight on life satisfaction and three potential mechanisms that may explain this relationship among school-aged children. In addition to the traditional ordinary least squares method, we also use an instrumental variable approach to deal with the potential endogeneity of bodyweight. We use mother's weight as plausible exogenous variation for children's weight. Using a Chilean sample of boys (n = 2,262) and girls (n = 2,256) aged 9 to 12, we provide suggestive evidence that body mass index, weight and obesity are causality and negatively related to children's life satisfaction. Our findings also suggest that body-image satisfaction, school bullying victimization, and physical health explain about 50 and 29 percent of the pathway between bodyweight and life satisfaction for girl and boys, respectively. Although, our results do not support gender differences in the bodyweight-life satisfaction association, we do find sizable gender differences in the mechanisms explaining this relationship. This article presents a novel response methodology for the operational recovery of a drinking water network after an incident causes an interruption of service. The proposed optimization-based methodology allows computing the optimal set of interventions to be performed in order to mitigate, or even prevent, the impact of the incident on the network operation. Besides, a proof-of-concept scheme has been designed for the automatic generation of failure scenarios and the systematic implementation and validation of the proposed response methodology. Several results are presented to demonstrate the capability of the methodology to mitigate harmful incidents, as well as the performance improvements derived from the application of the obtained interventions. This study analyzes the effect of spatial competition on public schools' efficiency in Chile, an extreme case of market-oriented reforms in the educational sector. To address this issue, we use a measure of competition that captures three major characteristics of market competition in a spatial context: the number of competitors, based on distance buffers; the size of competitors, measured by the enrollment of voucher schools; and the geographical distance among public schools and their competitors. To control for the potential endogeneity of our measure of spatial competition on public schools' efficiency, we use an instrumental variable based on the discontinuites generated by admissions municipality boundaries. We do not find evidence that public schools' efficiency increases or decreases as the result of the increment of voucher schools located near to public schools, an increase in the enrollment of existing voucher schools, or both factors simultaneously. This result is robust to: (1) the limit of education market or other measures of spatial competition; (2) the methodology used to compute the efficiency measures; and (3) whether we explain efficiency or educational performance. Recent studies have suggested that nonphase-locked activity can reveal cognitive mechanisms that cannot be observed in phase-locked activity. In fact, we describe a concomitant decrease in nonphase-locked alpha activity (desynchronization) when stimuli were processed (alpha phase-locked modulation). This desynchronization may represent a reduction in "background activity" in the visual cortex that facilitates stimulus processing. Alternatively, nonphase-locked gamma activity has been hypothesized to be an index of shifts in attentional focus. In this study, our main aim was to confirm these potential roles for nonphase-locked alpha and gamma activities with a lateralized Go/NoGo paradigm. The results showed that nonphase-locked alpha modulation is bilaterally represented in the scalp compared to the contralateral distribution of the phase-locked response. This finding suggests that the decrease in background activity is not limited to neural areas directly involved in the visual processing of stimuli. Additionally, gamma activity showed a higher desynchronization of nonphase-locked activity in the ipsilateral hemisphere, where the phase-locked activity reached the minimum amplitude. This finding suggests that the possible functions of nonphase-locked gamma activity extend beyond shifts in attentional focus and could represent an attentional filter reducing the gamma representation in the visual area irrelevant to the task. Background: Catheter ablation of accessory pathways (AP) with bidirectional conduction may be challenging due to issues related to anatomical course or location. Objective: We describe an alternative electro-anatomical mapping technique which aims at depicting the entire anatomic course of the AP from the atrial toward the ventricular insertion in order to guide catheter ablation. Methods: Twenty consecutive patients with confirmed bidirectional AP conduction and at least one previous ablation procedure or para-Hisian location were included. 3-D electro-anatomical mapping was used to depict the merged 10-ms isochrone area of maximum early activation of both the ventricular and atrial signals during sinus rhythm and ventricular pacing/orthodromic tachycardia, respectively. Catheter ablation was performed within the depicted earliest isochrone area. Results: Acute bidirectional AP conduction block was achieved in all patients 4.2 ± 1.7 s after the first radiofrequency energy pulse was delivered, without reconnection during a 30 ± 10 min post-ablation observation time. No procedural complications were seen. After a mean follow-up period of 9 ± 7 months (range 3 to 16), no recurrences were documented. Acute-on chronic liver failure (ACLF) is a syndrome that develops in patients with acutely decompensated cirrhosis (AD). It is characterized by a systemic hyperinflammatory state, leading to multiple organ failure. Our objective was to analyze macrophage anti-inflammatory protein CD5L in plasma extracellular vesicles (EVs) and assess its as yet unknown relationship with lipid mediators in ACLF. With this aim, EVs were purified by size exclusion chromatography from the plasma of healthy subjects (HS) (n=6) and patients with compensated cirrhosis (CC) (n=6), AD (n=11) and ACLF (n=11), which were defined as positive for CD9, CD5L and CD63 and their size, number, morphology and lipid mediator content were characterized by NTA, EM, and LC-MS/MS, respectively. Additionally, plasma CD5L was quantified by ELISA in 10 HS, 20 CC and 149 AD patients (69 ACLF). Moreover, macrophage CD5L expression and the biosynthesis of specialized lipid mediators (SPMs) were characterized in vitro in primary cells. Our results indicate that circulating EVs were significantly suppressed in cirrhosis, regardless of severity, and showed considerable alterations in CD5L and lipid mediator content as the disease progressed. In AD, levels of EV CD5L correlated best with those of the SPM RvE1. Analysis of total plasma supported these data and showed that, in ACLF, low CD5L levels were associated with circulatory (p<0.001), brain (p<0.008) and respiratory (p<0.05) failure (Mann-Whitney test). Functional studies in macrophages indicated a positive feedback loop between CD5L and RvE1 biosynthesis. In summary, we have determined a significant alteration of circulating EV contents in ACLF, with a loss of anti-inflammatory and pro-resolving molecules involved in the control of acute inflammation in this condition. Inappropriate sinus tachycardia (IST) is a common observation in patients with post-COVID-19 syndrome (PCS) but has not yet been fully described to date. To investigate the prevalence and the mechanisms underlying IST in a prospective population of PCS patients. Consecutive patients admitted to the PCS Unit between June and December 2020 with a resting sinus rhythm rate ≥ 100 bpm were prospectively enrolled in this study and further examined by an orthostatic test, 2D echocardiography, 24-h ECG monitoring (heart rate variability was a surrogate for cardiac autonomic activity), quality-of-life and exercise capacity testing, and blood sampling. To assess cardiac autonomic function, a 2:1:1 comparative sub-analysis was conducted against both fully recovered patients with previous SARS-CoV-2 infection and individuals without prior SARS-CoV-2 infection. Among 200 PCS patients, 40 (20%) fulfilled the diagnostic criteria for IST (average age of 40.1 ± 10 years, 85% women, 83% mild COVID-19). No underlying structural heart disease, pro-inflammatory state, myocyte injury, or hypoxia were identified. IST was accompanied by a decrease in most heart rate variability parameters, especially those related to cardiovagal tone: pNN50 (cases 3.2 ± 3 vs. recovered 10.5 ± 8 vs. non-infected 17.3 ± 10; p < 0.001) and HF band (246 ± 179 vs. 463 ± 295 vs. 1048 ± 570, respectively; p < 0.001). This study analyzes the labor market dimension of a worldwide social concern: the growing level of obesity. Using detailed, individual information of Chilean workers, the study finds a strong evidence of a wage penalty for women as body size increases, whereas men seemingly enjoy a wage premium for being overweight. Several hypotheses are tested for explaining this finding and results suggest that the gender wage gap between obese and non-obese workers is not related to observed productivity differences, risk aversion, or health limitations. For women, the wage penalty is mostly explained by occupational crowding and the "beauty premium" in high-skilled occupations. Finally, this study outlines possible avenues that future research should address. HIGHLIGHTS Women are more likely to experience a weight penalty in labor markets than men. Heavier women earn less per hour than thinner coworkers in Chile. Overweight men experience a wage premium. Circulating biomarkers and imaging techniques provide independent and complementary information to guide management of heart failure (HF). This consensus document by the Heart Failure Association (HFA) of the European Society of Cardiology (ESC) presents current evidence‐based indications relevant to integration of imaging techniques and biomarkers in HF. The document first focuses on application of circulating biomarkers together with imaging findings, in the broad domains of screening, diagnosis, risk stratification, guidance of treatment and monitoring, and then discusses specific challenging settings. In each section we crystallize clinically relevant recommendations and identify directions for future research. Exserohilum species are environmental molds that may rarely cause skin and nasal infections, especially in immunocompromised children. We present a 3‐year‐old girl with acute leukemia who presented with a skin infection caused by Exserohilum rostratum. Crohn's disease (CD) is a complex and multifactorial illness. There are still considerable gaps in our knowledge regarding its pathophysiology. A transcriptomic approach could shed some light on little-known biological alterations of the disease. We therefore aimed to explore the ileal transcriptome to gain knowledge about CD. We performed whole transcriptome gene expression analysis on ileocecal resections from CD patients and inflammatory bowel disease-free controls, as well as on a CDindependent cohort to replicate selected results. Normalized data were hierarchically clustered, and gene ontology and the molecular network were studied. Cell cultures and molecular methods were used for further evaluations. Genome-wide expression data analysis identified a robust transmembrane immunoglobulin domain-containing 1 (TMIGD1) gene underexpression in CD tissue, which was even more marked in inflamed ileum, and which was replicated in the validation cohort. Immunofluorescence showed TMIGD1 to be located in the apical microvilli of well-differentiated enterocytes but not in intestinal crypt. This apical TMIGD1 was lower in the noninflamed tissue and almost disappeared in the inflamed mucosa of surgical resections. In vitro studies showed hypoxic-dependent TMIGD1 decreased its expression in enterocyte-like cells. The gene enrichment analysis linked TMIGD1 with cell recovery and tissue remodeling in CD settings, involving guanylate cyclase activities. Transcriptomics may be useful for finding new targets that facilitate studies of the CD pathology. Summary: Hybrid energy storage systems (HESSs) help mitigating the fluctuations and variable availability of certain renewable sources, such as wind power, as they can provide support in different time scales. Therefore, regulating their state‐of‐charge (SOC) becomes crucial to ensure that the hybrid system complies with generation commitments agreed in time‐ahead markets despite subsequent unexpected wind speed variations. So far, research has been mainly targeted at avoiding extreme SOC situations in the storage devices, whereas the regulation of this parameter to specific values has often been disregarded. A novel approach is proposed in this work, where model predictive control (MPC) is used to regulate the SOC of a HESS under variable wind and grid demand scenarios. The MPC‐based supervisory controller developed for the hybrid system has been implemented and simulated under different situations. This controller monitors the future variation of the SOC with the aim of having the HESS available to develop its assigned functions successfully. The results show that a proper regulation of the SOC in the HESS increases the capacity to manage the active power supplied to the grid by the hybrid system based on wind power, as well as the level of compliance with generation commitments established time ahead. In this article we use the life satisfaction approach to assess the non-pecuniary costs of Mental health impacts after the 2010 Chile earthquake. By linking both subjective well-being valuation literature with studies that describe psychological impacts after natural disasters, we are able to quantify how big a compensation should be to leave individuals as well as they were before the event. Our results suggest that people who experience stress should be compensated by approximately 80–90% of the average monthly income if the shock was strong enough to cause significant damages. These estimates are robust to different empirical specifications, endogeneity, shock intensity measures, and mental health definitions. We estimate that the total costs of mental health deterioration are about 7% of the total reported damages, a significant amount that policymakers should not ignore in post-earthquake reconstruction stages. It is widely recognized by scholars that amenities affect the individual well-being. Ample empirical evidence has been provided for developed countries, although this analysis for developing economies is scant. The aim of this paper is to study the association between locational specific characteristics and self-reported measures of subjective well-being. We focus our analysis in Chile, a developing country located in South America endowed with a rather heterogeneous set of amenities across cities. Using data from several sources to account for both natural and urban city amenities along with individual traits, our first results suggest that natural and urban amenities do affect the level of subjective well-being across Chilean cities. Afterwards, we allow for the estimated parameters associated to amenities vary to characterize the whole distribution rather than a single average parameter. This analysis uncovers the existence of unobserved individual heterogeneity, that is, individuals display different tastes for amenities not captured by observed traits, and consequently compensating variations associated to amenities differ across the sample. These results provide valuable elements to policy makers and city planners to the design of policies that enhance the population well-being and to the understanding of the development of cities in developing economies. Renewable energies are reaching the maturity of their technological development, but before they are ready to become the main or even the only power sources in the energy mix, they must become profitably manageable. Currently there are many research topics regarding the integration of renewable energies in power systems, such as the combination of two or more sources into hybrid systems, the addition of energy storage systems, etc. The present paper studies the economic feasibility of converting an existing pumped-storage hydro power plant into a hybrid hydro-wind power plant through the integration of a wind farm in its surroundings. For this purpose, the estimated economic benefits of the existing pumped-storage hydropower plant are compared with the potential benefits of the proposed hybrid hydro-wind configuration. Furthermore, two energy management systems are conceived in order to estimate the energy generated and consumed by the hybrid hydro-wind plant, as well as the income and expenses resulting from the energy purchase-sale. Introduction: Recurrences after atrial fibrillation (AF) ablation are still common. Among the reported clinical and imaging predictors of recurrences, diagnosis‐to‐ablation time (DAT) has been defined as a predictor of ablation outcome in single‐center studies. We aimed to validate DAT in a multicenter real‐life cohort. Methods: This was a multicenter study including consecutive patients undergoing first paroxysmal and persistent AF ablation with radiofrequency or cryoballoon catheters during 2013. Cox proportional hazard regression models were performed to identify predictors of recurrence. Results: In total, 309 patients were included across nine centers (71% men, 57 ± 10 years old, 46% with hypertension, and 66% with CHA2DS2‐VASc ≤ 1). Most patients had paroxysmal AF (67%) and underwent radiofrequency ablation (68%) with a median DAT of 51 (43) months. Patients with DAT ≤ 1 year (16.6%) were less likely to have repeat procedures (4% vs 18%; P = .017). The adjusted proportional hazards Cox model identified hypertension (P = .005), heart failure (P = .011), nonparoxysmal AF (P = .038), DAT > 1 year (P = .007), and LA diameter (P = .026) as independent predictors for AF recurrence. DAT > 1 year was the only modifiable factor independently associated with recurrence (HR 4.2 [95% CI, 1.5‐11.9]) Conclusion: Diagnosis‐to‐ablation time is a modifiable factor independently associated with recurrent arrhythmia and repeat ablation after first AF ablation. Using subjective well-being estimations, this study analyzes whether compensating variations vary across space using a cross-sectional data set from Chile. To achieve this goal, it describes and compares two econometric ways of modelling unobserved spatial heterogeneity. Both approaches allow compensating variations to vary across spatial units by assuming some distribution a priori. One method assumes that the spatial heterogeneity can be represented by a discrete distribution (a group of regions that share the same coefficient) and the other that the preferences can be represented by a continuous distribution (each region has a different coefficient). The results show that focusing just on the average estimates of compensating variations, as the applied studies have done so far, masks useful local variation. More empirical studies are needed to assess the advantages and disadvantages of both econometric approaches and how their results compare across a wide range of conditions and samples. Induction of psoriasis following administration of beta blocker containing eye drops has rarely been documented. We report eight cases of psoriasis triggered by timolol eye drops. Since the clinical and histopathological features of this drug reaction are indistinguishable from those of idiopathic psoriasis, a thorough drug history should be taken in all patients, especially elderly ones, with recent onset of psoriasis. This paper investigates the hypothesis that the spatial concentration of economic activity might impair national growth, using the experiences of Brazil and Chile. In the first stage we have used regional data to estimate country-specific models in which regional growth is conditioned by a set of controls, considering population density as a measure of concentration. The results of this stage show that population density has a negative effect on the growth of Brazilian and Chilean regions. In the second stage we have simulated the impact of the actual changes in population observed in the regions within these countries between 1980 and 2010. Using the new densities, we have recalculated the growth of the regions, generating a new national growth rate as a weighted average of the estimated growth of the regional economies. The regional deconcentration observed in Brazil produced less negative results in terms of national growth. While the Chilean increasing spatial concentration resulted in negative effects for national growth: the national GDP growth rate was reduced by 0.8% on a yearly basis. These results replicate the findings of Brülhart and Sbergami (J Urb Econ 65:48-63, 2009) for Brazil and Chile in their cross-section of countries. Working with regional data within the countries allowed determine the gains/losses in the national GDP growth rate, and to point out the effects on each region within the countries.
