We present two middle-aged patients with pruritic, crusted scalp erosions. Skin biopsy showed epidermal acantholysis with IgG and C3 intercellular deposits on direct immunofluorescence, leading to the diagnosis of localized pemphigus vulgaris. Essential thrombocythemia is a chronic myeloproliferative syndrome which usually runs its course as an asymptomatic elevated platelet count. Cutaneous manifestations secondary to microcirculation abnormalities are rare but can represent a helpful diagnostic clue in order to prevent major thromboembolic events. We report two cases of heterogeneous livedoid and "net‐like" skin lesions in the context of essential thrombocythemia with identical histopathologic findings (medium‐sized blood vessels with luminal obliteration by eosinophilic material, mostly positive for the platelet marker CD61, without vasculitis). Background: The microbiological diagnosis of skin lesions related to COVID‐19 is not well known. Objective: Perform a microbiological diagnosis in COVID19‐related cutaneous manifestations. Methods: A cross‐sectional study was performed with 64 patients with cutaneous manifestations associated with COVID‐19 who underwent serological and nasopharyngeal reverse transcription polymerase chain reaction (RT‐PCR) for SARS‐CoV‐2. Results: Out of the 64 patients, 6 patients had positive RT‐PCR, with all of them developing SARS‐CoV‐2 IgG and 4 of them had positive IgM + IgA. Of the 58 patients with negative RT‐PCR, 8 cases had positive IgM + IgA and only one of them had IgG seroconversion. Therefore, the infection was demonstrated in 7 cases (10.9%) and was doubtful in 7 other cases (10.9%) who presented negative RT‐PCR and presence of IgA + IgM without subsequent seroconversion of IgG. Fifty patients (78.1%) had negative serological tests. The most frequent cutaneous pattern was pseudo‐chilblain (48.4%) followed by maculo‐papular pattern (26.6%), urticarial lesions (10.9%), vesicular eruptions (6.3%) and livedoid pattern (4.7%). The maculo‐papular pattern showed the highest positivity in RT‐PCR (3 cases; 17.6%) and serologies (4 cases; 23.5%). Skin lesions developed after the systemic symptoms in most patients (19 cases; 61.3%). Conclusions: Microbiological confirmation tests may not be an effective diagnostic technique for COVID‐related cutaneous manifestations or that attributed lesions are not related to COVID‐19. Confounding factors such as adverse drug reaction, serological cross‐reactions with other viruses, the low production of antibodies in asymptomatic or mild forms of COVID‐19 or its rapid disappearance, increase diagnostic uncertainty. Considering the clinical and histopathological findings, and the onset and worsening of the skin eruption in relation to the COVID-19 vaccine, diagnosis of vaccine-induced LP was rendered. The anatomopathological analysis of a skin biopsy performed from a central blanched area of a livedoid lesion of the upper limb showed a chronic perivascular inflammatory infiltrate with prominent endothelium (Figure 1c). Background and Aims: Anti‐severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) vaccines have the potential to alter several biological systems concurrently with remolding the immune system, most of which are related to immunization, while some others are known as adverse effects. This review aims to explore the potential effects of vaccination on the oral microenvironment and classifies them as good, bad, or ugly, with a brief review of facial diseases following coronavirus disease 2019 (COVID‐19) vaccination. Methods: This study was a comprehensive review conducted through searching related articles in Medline, Scopus, and Google Scholar databases. Results: On one side, the "Good" impacts of vaccination on the oro‐nasal mucosa are explained as if the mucosal immune responses followed by SARS‐CoV‐2 vaccines are enough to provide immunity. On the other side, the possible "Bad" and "Ugly" effects of the vaccine, which manifest as orofacial adverse events and autoimmune reactivations, respectively, should be noted. Exacerbation of pre‐existing autoimmune conditions such as lichen planus, pemphigus vulgaris, bullous pemphigoid, and Stevens‐Johnson syndrome have been reported. Conclusion: COVID‐19 vaccines could affect different biological systems alongside stimulating the immune system, and some of these effects are referred to as adverse effects. Simple Summary: Skin lesions in the context of MDS/MPN are frequent and poorly understood in the current literature. This paper thoroughly reviews the dermatoses reported so far in these patients based on their relationship with prognosis and its main clinical, histopathological and genetical features. Different histological patterns and mutations have been described in the literature. However, the true nature of the cells in these cutaneous infiltrates as well as their mutational background and the link with the underlying hematological neoplasm have to be further investigate in order to clarify their biological significance and the main therapeutic implications. A classification proposal is developed with the aim of helping dermatologists, hematologists and pathologists to recognize them when present in these complex patients. The figures and tables included in the text and in the supplemental material facilitate the understanding of each of the proposed categories. In addition, the main publications in which these dermatoses have been previously treated are indicated. Myeloid neoplasms and acute leukemias include different entities that have been recently re-classified taking into account molecular and clinicopathological features. The myelodysplastic syndrome/myeloproliferative neoplasm (MDS/MPN) category comprises a heterogeneous group of hybrid neoplastic myeloid diseases characterized by the co-occurrence of clinical and pathological features of both myelodysplastic and myeloproliferative neoplasms. The most frequent entity in this category is chronic myelomonocytic leukemia (CMML) which is, after acute myeloid leukemia (AML), the main myeloid disorder prone to develop cutaneous manifestations. Skin lesions associated with myelodysplastic and myeloproliferative neoplasms include a broad clinical, histopathological and molecular spectrum of lesions, poorly understood and without a clear-cut classification in the current medical literature. The aim of this review is to describe and classify the main clinical, histopathological and molecular patterns of cutaneous lesions in the setting of MDS/MPN in order to improve the diagnostic skills of the dermatologists, hematologists and pathologists who deal with these patients. Lichen planus (LP) is an inflammatory disorder believed to result from CD8 + cytotoxic T-cell (CTL)-mediated autoimmune reactions against basal keratinocytes. We present a review of LP following COVID-19 infection and vaccination. Literature searches were conducted on PubMed and Google Scholar from 2019 to 7/2022. 36 articles were selected based on subject relevance, and references within articles were also screened. 39 cases of post-vaccination LP and 6 cases of post-infection LP were found among case reports and case series. 152 cases of post-vaccination LP and 12 cases of post-infection LP were found in retrospective and prospective studies. LP is a rare complication of COVID-19 infection and vaccination that may be mediated by overstimulation of T-cell responses and proinflammatory cytokine production. However, it does not represent a limitation against COVID-19 vaccination, and the benefits of vaccination considerably outweigh the risks. [ABSTRACT FROM AUTHOR]

Of the nine patients described in detail, LP lesions developed within 6 days from vaccination (4 patients; 44.4%), one-week (2 patients; 22.2%) or two-week post-vaccination (3 patients; 33.3%). In addition, in some individuals, the LP and LLP skin reaction may occur after a second- or third-dose vaccination without appearing after earlier COVID-19 vaccinations. The severe acute respiratory syndrome corona virus 2 (SARS-CoV-2) causing the 2019 coronavirus disease (COVID-19) has infected millions in recent years and is a major public health concern. Various cutaneous manifestations of the COVID-19 disease have been identified. Skin is a mirror to internal disease and can be the presenting sign of COVID-19 disease. Several cutaneous manifestations can indicate severe COVID-19 disease. Since the beginning of the vaccination campaign, cutaneous reactions after COVID-19 vaccination have been reported in the literature, especially in adults too.[[1], [3]] Here, we present a case of Gianotti-crosti syndrome (GCS) after COVID-19 immunization in a child. Skin manifestations in the setting of SARS-Cov-2 infections have been a huge topic recently in dermatology. A type IV hypersensitivity reaction is assumed and also IgE-mediated responses as respective patients often display elevated IgE and atopic diseases.[5] It is a self-limiting dermatosis and recurrences are uncommon; sometimes topical steroids help shorten disease duration.
