The amount of available scientific literature is increasing, and studies have proposed various methods for evaluating document-document similarity in order to cluster or classify documents for science mapping and knowledge discovery. In this paper, we propose hybrid methods for bibliographic coupling (BC) and linear evaluation of text or content similarity: We combined BC with BM25, Cosine, and PMRA to compare their performances with single methods in paper recommendation tasks using TREC Genomics Track 2005datasets. For paper recommendation, BC and text-based methods complement each other, and hybrid methods were better than single methods. The combinations of BC with BM25 and BC with Cosine performed better than BC with PMRA. The performances were best when the weights of BM25, Cosine, and PMRA were 0.025, 0.2, and 0.2, respectively, in hybrid methods. For paper recommendation, the combinations of BC with text-based methods were better than BC or text-based methods used alone. The choice of method should depend on the actual data and research needs. In the future, the underlying reasons for the differences in performance and the specific part or type of information they complement in text clustering or recommendation need to be examined. Although paper-based transmission of medical information might seem outdated, it has proven efficient, and remains structurally safe from massive data leaks. As part of the ICIPEMIR project for improving medical imaging report, we explored the idea of structured data storage within a medical report, by embedding the data themselves in a QR-Code (and no URL-to-the-data). Three different datasets from ICIPEMIR were serialized, then encoded in a QR-Code. We compared 4 compression algorithms to reduce file size before QR-Encoding. YAML was the most concise format (character sparing), and allowed for embedding of a 2633-character serialized file within a QR-Code. The best compression rate was obtained with gzip, with a compression ratio of 2.32 in 15.7ms. Data were easily extracted and decompressed from a digital QR-Code using a simple command line. YAML file was also successfully recovered from the printed QR-Code with both Android and iOS smartphone. Minimal detected size was 3*3cm. [ABSTRACT FROM AUTHOR]

As Rwanda approaches the UNAIDS Fast Track goals which recommend that 95% of HIV-infected individuals know their status, of whom 95% should receive treatment and 95% of those on treatment achieve viral suppression, the country currently relies on an inefficient paper, and disjointed electronic, systems for case-based surveillance (CBS). Rwanda has established an ecosystem of interoperable systems based on open standards to support HIV CBS. Data were successfully exchanged between an EMR, a client registry, laboratory information system and DHIS-2 Tracker, and subsequently, a complete analytic dataset was ingested into MS-Power Business Intelligence (MS-PowerBI) for analytics and visualization of the CBS data. Existing challenges included inadequate workforce capacity to support mapping of data elements to HL7 FHIR resources. Interoperability optimization to support CBS is work in progress and rigorous evaluations on the effect on health information exchange on monitoring patient outcomes are needed. Medical events are often infrequent, thus becomes hard to predict. In this paper, we focus on predictor that forecasts whether a medical event would occur in the next year, and analyzes the impact of event's frequency and data size via predictor's performance. In the experiment, we made 1572 predictors for medical events using Medical Insurance Claims (MICs) data from 800,000 participants and 205.8 million claims over 8 years. The result revealed that (a) forecasting error will be increased when predicting low-frequency events, and (b) increasing the number of training dataset reduces errors. Collaboration across disciplinary boundaries is vital to address the complex challenges and opportunities in Digital Health. We present findings and experiences of applying the principles of Team Science to a digital health research project called 'The Wearable Clinic'. Challenges faced were a lack of shared understanding of key terminology and concepts, and differences in publication cultures between disciplines. We also encountered more profound discrepancies, relating to definitions of "success" in a research project. Accurate identification of the QRS complex is critical to analyse heart rate variability (HRV), which is linked to various adverse outcomes in premature infants. Reliable and accurate extraction of HRV characteristics at a large scale in the neonatal context remains a challenge. In this paper, we investigate the capabilities of 15 state-of-the-art QRS complex detection implementations using two real-world preterm neonatal datasets. As an attempt to improve the accuracy and reliability, we introduce a weighted ensemble-based method as an alternative. Obtained results indicate the superiority of the proposed method over the state of the art on both datasets with an F1-score of 0.966 (95% CI 0.962-0.97) and 0.893 (95% CI 0.892-0.894). Parkinson's disease is a chronic progressive neurodegenerative disease with highly heterogeneous symptoms and progression. It is helpful for patient management to establish a personalized model that integrates heterogeneous interpretation methods to predict disease progression. In the study, we propose a novel approach based on a multi-task learning framework to divide Parkinson's disease progression modeling into an unsupervised clustering task and a disease progression prediction task. On the one hand, the method can cluster patients with different progression trajectories and discover new progression patterns of Parkinson's disease. On the other hand, the discovery of new progression patterns helps to predict the future progression of Parkinson's disease markers more accurately through parameter sharing among multiple tasks. We discovered three different Parkinson's disease progression patterns and achieved better prediction performance (MAE=5.015, RMSE=7.284, r2=0.727) than previously proposed methods on Parkinson's Progression Markers Initiative datasets, which is a longitudinal cohort study with newly diagnosed Parkinson's disease. Computed tomography (CT) is widely applied in contemporary clinic. Due to the radiation risks carried by X-rays, the imaging and post-processing methods of low-dose CT (LDCT) become popular topics in academia and industrial community. Generally, LDCT presents strong noise and artifacts, while existing algorithms cannot completely overcome the blurring effects and meantime reduce the noise. The proposed method enables CT extend to independent frequency channels by wavelet transformation, then two separate networks are established for low-frequency denoising and high-frequency reconstruction. The clean signals from high-frequency channel are reconstructed through channel translation, which is essentially effective in preserving detailed structures. The public dataset from Mayo Clinic was used for model training and testing. The experiments showed that the proposed method achieves a better quantitative result (PSNR: 37.42dB, SSIM: 0.8990) and details recovery visually, which demonstrates our framework can better restore the detailed features while significantly suppressing the noise. Though a clinical pathway is one of the tools used to guide evidence-based healthcare, promoting the practice of evidence-based decisions on healthcare services is incredibly challenging in low resource settings (LRS). This paper proposed a novel approach for designing an automated and dynamic generation of clinical pathways (CPs) in LRS through a hybrid (knowledge-based and data-driven based) algorithm that works with limited clinical input and can be updated whenever new information is available. Our proposed approach dynamically maps and validate the knowledge-based clinical pathways with the local context and historical evidence to deliver a multi-criteria decision analysis (concordance table) for adjusting or readjusting the order of knowledge-based CPs decision priority. Our finding shows that the developed approach successfully delivered probabilistic-based CPs and found a promising result with Jimma Health Center "pregnancy, childbearing, and family planning" dataset. The COVID-19 pandemic has caused millions of infections and deaths worldwide in an ongoing pandemic. With the passage of time, several variants of this virus have surfaced. Machine learning methods and algorithms have been very useful in understanding the virus and its implications so far. In this paper, we have studied a set of novelty detection algorithms and applied it to the problem of detecting COVID-19 variants. Our results show accuracies of 79.64% and 82.43% on the B.1.1.7 and B.1.351 variants respectively on ProtVec unaligned COVID-19 spike protein sequences using One Class SVM with fine-tuned parameters. We believe that a system for automated and timely detection of variants will help countries formulate mitigation measures and study remedies in terms of medicines and vaccines that can protect against the new variants. Objective: Waiting time for a consultation for chronic pain is a widespread health problem. This paper presents the design of an ontology use to assess patients referred to a consultation for chronic pain. Methods: We designed OntoDol, an ontology of pain domain for patient triage based on priority degrees. Terms were extracted from clinical practice guidelines and mapped to SNOMED-CT concepts through the Python module Owlready2. Selected SNOMED-CT concepts, relationships, and the TIME ontology, were implemented in the ontology using Protégé. Decision rules were implemented with SWRL. We evaluated OntoDol on 5 virtual cases. Results: OntoDol contains 762 classes, 92 object properties and 18 SWRL rules to assign patients to 4 categories of priority. OntoDol was able to assert every case and classify them in the right category of priority. Coronary artery disease (CAD) has the highest disease burden worldwide. To manage this burden, predictive models are required to screen patients for preventative treatment. A range of variables have been explored for their capacity to predict disease, including phenotypic (age, sex, BMI and smoking status), medical imaging (carotid artery thickness) and genotypic. We use a machine learning models and the UK Biobank cohort to measure the prediction capacity of these 3 variable categories, both in combination and isolation. We demonstrate that phenotypic variables from the Framingham risk score have the best prediction capacity, although a combination of phenotypic, medical imaging and genotypic variables deliver the most specific models. Furthermore, we demonstrate that Variant Spark, a random forest based GWAS platform, performs effective feature selection for SNP-based genotype variables, identifying 115 significantly associated SNPs to the CAD phenotype. Pancreatic cancer is a highly malignant cancer of the digestive tract and is rapidly progressing and spreading clinically. Automatic and accurate pancreatic tissue segmentation in abdominal CT images is essential for the early diagnosis of pancreatic-related diseases. It is challenging that the pancreas is small in size and complex in morphology. To address this problem, we propose a dual-attention model fusing CNN and Transformer to effectively activate pancreas-related features expression. The CNN structure weights the importance of pancreasrelated features at the channel level and weakens the background information. Transformer feature aggregation module constructs spatial correlations among long-distance pixels from a global perspective. This study is validated on the NIHTCIA dataset and achieved a mean Dice Similarity Coefficient of 85.82%, which is outperforming than the state-of-the-art methods. The lack of transparency and explainability hinders the clinical adoption of Machine learning (ML) algorithms. While explainable artificial intelligence (XAI) methods have been proposed, little research has focused on the agreement between these methods and expert clinical knowledge. This study applies current state-of-the-art explainability methods to clinical decision support algorithms developed for Electronic Medical Records (EMR) data to analyse the concordance between these factors and discusses causes for identified discrepancies from a clinical and technical perspective. Telehealth services are becoming more and more popular, leading to an increasing amount of data to be monitored by health professionals. Machine learning can support them in managing these data. Therefore, the right machine learning algorithms need to be applied to the right data. We have implemented and validated different algorithms for selecting optimal time instances from time series data derived from a diabetes telehealth service. Intrinsic, supervised, and unsupervised instance selection algorithms were analysed. Instance selection had a huge impact on the accuracy of our random forest model for dropout prediction. The best results were achieved with a One Class Support Vector Machine, which improved the area under the receiver operating curve of the original algorithm from 69.91 to 75.88 %. We conclude that, although hardly mentioned in telehealth literature so far, instance selection has the potential to significantly improve the accuracy of machine learning algorithms. The Cascade-HF protocol is a Continuous Remote Patient Monitoring (CRPM) study at a major health system in the United States to reduce Heart Failure (HF)-related hospitalizations and readmissions using wearable biosensors to collect physiological data over a 30-day period to determine decompensation risk among HF patients. The alerts produced, coupled with electronic patient-reported outcomes, are utilized daily by the home health team, and escalated to the heart failure team as needed, for proactive actions. Limited research has examined anticipating the implementation and workflow challenges of such complex CRPM studies such as resource planning and staffing decisions that leverage the recorded data to drive clinical preparedness and operational efficiency. This preliminary analysis applies discrete event simulation modeling to the Cascade-HF protocol using pilot data from a soft launch to assess workload of the clinical team, evaluate escalation patterns and provide decision support recommendations to enable scale-up for all post-discharge patients. Genetic data is limited and generating new datasets is often an expensive, time-consuming process, involving countless moving parts to genotype and phenotype individuals. While sharing data is beneficial for quality control and software development, privacy and security are of utmost importance. Generating synthetic data is a practical solution to mitigate the cost, time and sensitivities that hamper developers and researchers in producing and validating novel biotechnological solutions to data intensive problems. Existing methods focus on mutation frequencies at specific loci while ignoring epistatic interactions. Alternatively, programs that do consider epistasis are limited to two-way interactions or apply genomic constraints that make synthetic data generation arduous or computationally intensive. To solve this, we developed Polygenic Epistatic Phenotype Simulator (PEPS). Our tool is a probabilistic model that can generate synthetic phenotypes with a controllable level of complexity. Automatic extraction of relations between drugs/chemicals and proteins from ever-growing biomedical literature is required to build up-to-date knowledge bases in biomedicine. To promote the development of automated methods, BioCreative-VII organized a shared task - the DrugProt track, to recognize drugprotein entity relations from PubMed abstracts. We participated in the shared task and leveraged deep learning-based transformer models pre-trained on biomedical data to build ensemble approaches to automatically extract drug-protein relation from biomedical literature. On the main corpora of 10,750 abstracts, our best system obtained an F1-score of 77.60% (ranked 4th among 30 participating teams), and on the large-scale corpus of 2.4M documents, our system achieved micro-averaged F1-score of 77.32% (ranked 2nd among 9 system submissions). Pain is a common reason for accessing healthcare resources and is a growing area of research, especially in its overlap with mental health. Mental health electronic health records are a good data source to study this overlap. However, much information on pain is held in the free text of these records, where mentions of pain present a unique natural language processing problem due to its ambiguous nature. This project uses data from an anonymised mental health electronic health records database. A machine learning based classification algorithm is trained to classify sentences as discussing patient pain or not. This will facilitate the extraction of relevant pain information from large databases. 1,985 documents were manually triple-annotated for creation of gold standard training data, which was used to train four classification algorithms. Clinical narratives recording behaviours and emotions of patients are available from EHRs in a forensic psychiatric centre located in Tasmania. This rich data has not been used in risk prediction. Prior work demonstrates natural language processing can be used to identify patient symptoms in these free-text records and can then be used to predict risk. Four dictionaries containing descriptive words of harm were created using the Diagnostic and Statistical Manual of Mental Disorders, the Unified Medical Language System repository, English negative and positive sentiment words, and high-frequency words from the Corpus of Contemporary American English. However, a model based only on these keywords is limited in predictive power. In this study, we introduce an improved NLP approach with a social interaction component to extract additional information about the behavioural and emotional state of patients. In the United States, more than 12% of the population will experience thyroid dysfunction. Patient symptoms often reported with thyroid dysfunction include fatigue and weight change. However, little is understood about the relationship between these symptoms documented in the outpatient setting and ordering patterns for thyroid testing among various patient groups by age and sex. We developed a natural language processing and deep learning pipeline to identify patientreported outcomes of weight change and fatigue among patients with a thyroid stimulating hormone test. We built upon prior works by comparing 5 open-source, Bidirectional Encoder Representations from Transformers (BERT) to determine which models could accurately identify these symptoms from clinical texts. For both fatigue (f) and weight change (wc), Bio_ClinicalBERT achieved the highest F1-score (f: 0.900; wc: 0.906) compared BERT (f: 0.899; wc: 0.890), DistilBERT (f: 0.852; wc: 0.912), Biomedical RoBERTa (f: 0.864; wc: 0.904), and PubMedBERT (f: 0.882; wc: 0.892). Every year there are approximately three million new specialist clinic appointments at local hospital networks in Victoria. CSIRO, in collaboration with Austin Health, have developed two algorithms to assist with waitlist management in their outpatient specialist clinics. This study describes the implementation of these algorithms in software tools developed to support their use and trial in the clinical setting at Austin Health. We discuss the system design and development of both these software tools. We also review the implemented workflow of the tools and discuss how these tools seek to improve current systems. As the fight against COVID-19 continues, it is critical to discover and accumulate knowledge in scientific literature to combat the pandemic. In this work, we shared the experience in developing an intelligent query system on COVID-19 literature. We conducted a user-centered evaluation with 12 researchers in our institution and identified usability issues in four categories: distinct user needs, functionality errors, suboptimal information display, and implementation errors. Furthermore, we shared two lessons for building such a COVID-19 literature search engine. We will deploy the system and continue refining it through multiple phases of evaluation to aid in redesigning the system to accommodate different user roles as well as enhancing repository features to support collaborative information seeking. The successful implementation of the COVID-IQS can support knowledge discovery and hypothesis generation in our institution and can be shared with other institutions to make a broader impact. Patient outcome is one of the key information categories in incident reporting. Being able to extract meaningful patient fall outcomes would allow better analysis of the consequences and possible mitigating actions for in-hospital fall incidents. This study aims to automate the extraction of patient outcomes from narrative fall incident reports by decomposing this into three classification subtasks: injured or not, injury types, and the number of injuries. Implementing an existing incident report classification (IRC) framework, the experimental results demonstrated that oversampling and structured features were effective to achieve better overall performances across all three subtasks. The study further validated the application of an IRC framework to deal with imbalanced classification problems found in fall patient outcome classification and advanced the science of automatic patient outcomes extraction. Infant mortality is characterized by the death of young children under the age of one, and it is an issue affecting millions of children in the world. The objective of this article is to employ concepts of knowledge discovery in databases, specifically of machine learning in the data mining phase, to characterize infant mortality in two states of Brazil: Santa Catarina, with the lowest infant mortality rate of the country's states, and Amapá, with the highest. The classifiers C4.5, JRip, Random Forest, SVM, and Multilayer Perceptron were used, and a brief comparison of the results obtained by the classifiers in both states is made. In addition, the dataset preprocessing is detailed, which includes attribute selection and class balancing. The results show that the features APGAR5, WEIGHT, and CONGENITAL ANOMALY stood out the most from the rules generated by the tree-based classifiers. [ABSTRACT FROM AUTHOR]

Early anticipation of COVID-19 infection chains within hospitals is of high importance for initiating suitable measures at the right time. Infection control specialists can be supported by application systems able of consolidating and analyzing heterogeneous, up-to-now non-standardized and distributed data needed for tracking COVID-19 infections and infected patients' hospital contacts. We developed a system, Co-SurvSmICS, assisting in infection chain detection, in an open and standards-based way to ensure reusability of the system across institutions. Data is modelled in alignment to various national modelling initiatives and consensus data definitions, queried in a standardized way by the use of OpenEHR as information modelling standard and its associated model-based query language, analyzed and interactively visualized in the application. A first version has been published and will be enhanced with further features and evaluated in detail with regard to its potentials to support specialists during their work against SARS-CoV-2. We present a work-in-progress software project which aims to assist cross-database medical research and knowledge acquisition from heterogeneous sources. Using a Natural Language Processing (NLP) model based on deep learning algorithms, topical similarities are detected, going beyond measures of connectivity via citation or database suggestion algorithms. A network is generated based on the NLP-similarities between them, and then presented within an explorable 3D environment. Our software will then generate a list of publications and datasets which pertain to a certain topic of interest, based on their level of similarity in terms of knowledge representation. Objective. The aim of this paper was to present a practical InfoRoute algorithm and applications developed by CISMeF to perform a contextual information retrieval across multiple medical websites in different health domains. Methods. The algorithm was developed to treat multiple types of queries: natural, Boolean and advanced. The algorithm also generates multiple types of queries: Boolean query, PubMed query or Advanced query. Each query can be extended via an inter alignments relationship from UMLS and HeTOP portal. Results. A web service and two web applications have been developed based on the InfoRoute algorithm to generate linksquery across multiple websites, i.e.: "PubMed" or "ClinicalTrials.org". Treatment recommendation is a nontrivial task - it requires not only domain knowledge from evidence-based medicine, but also data insights from descriptive, predictive and prescriptive analysis. A single treatment recommendation system is usually trained or modeled with a limited (size or quality) source. This paper proposes a decision fusion framework, combining both knowledge-driven and data-driven decision engines for treatment recommendation. End users (e.g. using the clinician workstation or mobile apps) could have a comprehensive view of various engines' opinions, as well as the final decision after fusion. For implementation, we leverage several well-known fusion algorithms, such as decision templates and meta classifiers (of logistic and SVM, etc.). Using an outcome-driven evaluation metric, we compare the fusion engine with base engines, and our experimental results show that decision fusion is a promising way towards a more valuable treatment recommendation. Within the large body of biomedical knowledge, recent findings and discoveries are most often presented as research articles. Their number has been increasing sharply since the turn of the century, presenting ever-growing challenges for search and discovery of knowledge and information related to specific topics of interest, even with the help of advanced online search tools. This is especially true when the goal of a search is to find or discover key relations between important concepts or topic words. We have developed an innovative method for extracting key relations between concepts from abstracts of articles. The method focuses on relations between keywords or topic words in the articles. Early experiments with the method on PubMed publications have shown promising results in searching and discovering keywords and their relationships that are strongly related to the main topic of an article. Foodborne disease is a growing public health problem worldwide and imposes a considerable economic burden on hospitals and other healthcare costs. Thus, accurately predicting the propagation of foodborne disease is crucial in preventing foodborne disease outbreaks. Few studies have investigated the dependencies between environmental variables and foodborne disease activity. This study develops a regularization-based eXtreme gradient boosting approach for foodborne disease trend forecasting considering environmental effects to capture dependencies hidden in foodborne disease time series. A real case in Shanghai, China was studied to validate our proposed model along with comparisons to traditional and benchmark algorithms for foodborne disease prediction. Results show that the foodborne disease prediction approach we propose achieves slightly superior performance in terms of one-day-ahead prediction of foodborne disease, and presents more robust prediction for 27 days ahead prediction. With the proliferation of digital communication in healthcare, the reuse of laboratory test data entails valuable insights into clinical and scientific issues, basically enabled by semantic standardization using the LOINC coding system. In order to extend the currently limited potential for analysis, which is mainly caused by structural peculiarities of LOINC, an algorithmic transformation of relevant content into an OWL ontology was performed, which includes LOINC Terms, Parts and Hierarchies. For extending analysis capabilities, the comprehensive SNOMED CT ontology is added by transferring its contents and the recently published LOINC-related mapping data into OWL ontologies. These formalizations offer rich, computer-processable content and allow to infer additional structures and relationships, especially when used together. Consequently, various reutilizations are facilitated; an application demonstrating the dynamic visualization of fractional hierarchy structures for usersupplied laboratory data was already implemented. Assigning ICD-10 code of sepsis in regard of a pathogenic bacterium found in an haemoculture requires knowledge of microbiology because of the difference of granularity. The aim of this paper is to automate this coding thanks to the use of SNOMED-CT. A dichotomous classification of bacteria causing sepsis has been generated in respect of ICD-10. Our algorithm follows this and explores SNOMED-CT to assign the right ICD-10 code of the sepsis. Applied to a list of 164 bacteria, the system has an error rate of 1.22 %. [ABSTRACT FROM AUTHOR]

With the vast increase of digital healthcare data, there is an opportunity to mine the data for understanding inherent health patterns. Although machine-learning techniques demonstrated their applications in healthcare to answer several questions, there is still room for improvement in every aspect. The low proportion and the rapid evolvement of major adverse cardiac events (MACE) present challenges for predicting MACE by machine learning models. In this paper, we propose a method to predict MACE from large-scale imbalanced EMR data by using a network-based one-class classifier. Our objective was to identify and extract gene-drug and drugadverse drug reaction (ADR) relationships from different biomedical literature collections, and to predict the possible association between gene and ADR. The drug, ADR and gene entities were recognized by a CRF model with multiple features. Logistic regression models were constructed for each drug- ADR and drug-gene pair based on its frequency, Mesh Rule association and similarity with known association etc. Using predicted score to generate drug- ADR matrix and drug-gene matrix, and then calculating for gene-ADR matrix. Network and clustering analysis were applied to verify and interpret the relationship between them. A total of 78014 potential gene-ADR associations were predicted. Part of the predicted results can be explained by the network-clustering-pathway analysis, and verified in the literature. Clinical notes contain important temporal information that are critical for making clinical diagnosis and treatment as well as for retrospective analyses. Manually created regular expressions are commonly used for the extraction of temporal information; however, this can be a time consuming and brittle approach. We describe a novel algorithm for automatic learning of regular expressions in recognizing temporal expressions. Five classes of temporal expressions are identified. Keywords specific to those classes are used to retrieve snippets of text representing the same keywords in context. Those snippets are used for Regular Expression Discovery Extraction (REDEx). This paper describes a mobile prototype capable of recognizing characters from a photograph of a medication package. The prototype was built to work on the iOS platform and was developed using Objective-C and C programming languages. The prototype, capable of recognizing text out of an image, included image processing algorithms, text processing algorithms, and techniques to search and handle information from a database.
