This article presents a retrospective study conducted in Tunis hospitals to assess the impact of COVID-19 on hospitalized pregnant women. The study population consisted of women aged 28-38 years with diverse educational backgrounds. Most women had no significant medical history, but some had controlled asthma or a prior pregnancy loss. Vaccination rates were high, with the majority receiving prenatal care. Common COVID-19 symptoms were reported in 73% of women, and most cases were mild with no maternal deaths. However, COVID-19 infection was associated with an increased risk of preterm delivery, respiratory distress, and miscarriage. Psychological distress was observed in all women. Delivery outcomes were significantly impacted, with a rise in Caesarean sections and a high preterm birth rate. Neonatal COVID-19 infection occurred in 6% of cases, but most newborns had good Apgar scores. L'endométriose est une affection gynécologique chronique qui touche 2 à 10 % de la population féminine en âge de procréation. Il s'agit d'un trouble inflammatoire estrogéno-dépendant. L'hypothèse entre l'inflammation chronique liée à l'endométriose et une dysfonction du système immunitaire dans certaines maladies auto-immunes telles que la polyarthrite rhumatoïde a été évoquée. Notre objectif est d'évaluer la prévalence de l'endométriose chez les patientes atteintes de PR et de déterminer les facteurs associés. Nous avons mené une étude transversale incluant des patientes non ménopausées atteintes de PR selon les critères ACR/EULAR 2010, menée aux services de rhumatologie et de gynécologie obstétrique A de l'hôpital Charles Nicolle de Tunis, Tunisie. Une consultation en gynécologie avec une échographie pelvienne ont été effectuée pour chaque patiente après son consentement à la recherche de signes en faveur d'une endométriose (signes cliniques et échographique). Par ailleurs, nous avons recueilli les paramètres liés à la PR. Ont été incluses 18 patientes non ménopausées suivie pour une PR, d'âge moyen de 38,3 ± 7,7 ans [18–48 ans]. L'évaluation des plaintes fonctionnelles liées à l'endométriose a montrée : une douleur pelvienne chez 16,7 % des patientes, une dysménorrhée dans 55,6 % des cas nécessitant une prise dans antalgiques dans 38,9 % des cas. Par ailleurs, pas de dysurie ni de dyspareunie rapportées. Une infertilité primaire a été notée dans 16,7 % des cas. Neuf patientes avaient des enfants, avec un nombre moyen de grossesses de 1,44 ± 1,65 [0–5] et une parité moyenne de 1,11 ± 1,23 [0–3]. Quatre patientes avaient eu des fausses couches. L'âge moyen de premier enfant était de 26,75 ± 5,67 ans [17–33 ans]. Une échographie pelvienne a été faite chez toutes les patientes. Un épanchement pelvien a été noté dans deux cas (11,1 %). Une image latéro-utérine d'échogénicité variable a été notée chez trois patientes (16,7 %). Le Sliding Sign était négatif dans tous les cas. L'épaisseur moyenne de l'endomètre était 8,43 ± 3,02 mm [3–12 mm]. Un utérus globuleux a été noté dans 22,2 % des cas. Le diagnostic d'endométriose a été retenu chez trois patientes (16,7 %) en se basant sur les données cliniques et échographiques. Concernant la PR, l'âge moyen de début de la maladie était de 25,33 ± 9,29 ans [4–35 ans]. Le facteur rhumatoïde était positif dans 11 cas et l'anti-CCP dans 12 cas. L'EVA douleur moyenne était de 5,27 ± 2,19 [1–8]. Le score médian de DAS 28 CRP était de 3,75 ± 1,29 [1,39–6,07]. Le score médian de SDAI était de 25,9 ± 17,76 [2–64]. Le score médian de CDAI était de 19,72 ± 11,71 [4–43]. La maladie était active dans 14 cas (77,8 %). L'étude des facteurs de risque associés à l'endométriose a montré: une association significative avec la positivité de FR (p = 0,045), l'EVA douleur (p = 0,033), le DAS28crp (p = 0,017), le SDAI (p = 0,018), le CDAI (p = 0,037). De même, il y avait une corrélation ente la présence de l'endométriose et un âge tardif du premier enfant (p = 0,009). Par contre, il n'y avait pas une association significative entre l'endométriose et l'âge de début de la maladie et la stérilité. La prévalence de l'endométriose dans la PR (16,7 %) semble supérieure à la population générale. Un lien entre ces deux pathologies inflammatoires est probable. The study objective is to study the intraoperative and postoperative complications of laparotomic myomectomy and patients' characteristics influencing this risk. Conclusions Odds ratio computed to estimate the risk of complications in relation to the patient characteristics showed that the probability of complications significantly rises with an increase in the number (more than 3 myomas) and with the intramural or the intraligamentous location of myomas whereas the myoma size seems to influence particularly the risk of major complications. Patients undergoing myomectomies should be carefully selected, due to the high rate of complications, other alternatives should be also considered such as laparoscopic myomectomy; uterine embolisation. The purpose of our study is to assess imaging-pathologic concordance and discordance of stereotactic vacuum-assisted biopsies of breast microcalcifications. Spawning individuals of allis shad, Alosa alosa (Linnaeus, 1758), and twaite shad, Alosa fallax (Lacépède, 1803), were sampled from three rivers on the Atlantic coast of the Iberian Peninsula (Ulla, Minho, Mondego) during 2008 to 2013 to assess the presence of the zoonotic marine parasite Anisakis spp. larvae. The results revealed that both shad species were infected by third-larval stage Anisakis simplex s.s. and Anisakis pegreffii. The latter is reported in mixed infections in both shad species of Western Iberian Peninsula for the first time. In A. alosa, the prevalence of Anisakis infection can reach 100 %, while in A. fallax, prevalence was up to 83 %. Infected individuals of the former species also often contain much higher number of parasites in theirs internal organs and flesh: from 1 to 1138 Anisakis spp. larvae as compared to 1 to 121 larvae, respectively. In general, numbers of A. pegreffii were higher than those of A. simplex s.s. Our results suggest that in the marine environment of the Western Iberian Peninsula, both anadromous shad species act as paratenic hosts for A. simplex s.s. and A. pegreffii, thus widening the distribution of the infective nematode larvae from the marine to the freshwater ecosystem.
