Given that the number of children and young people needing care keeps rising and fewer people are becoming foster carers, efforts to support carers and workers in foster caring are essential. This paper considers the experiences of carers and foster care agency workers involved in Australia's piloting of the Mockingbird Family. With a view understanding experience, data were collected via focus groups with carers and agency workers (n = 20) involved in piloting, implementation and evaluation. Deductive analysis applied the theory of experience to generate understanding of experience, as both intrinsic and extrinsic dimensions to capture strengths in the Mockingbird Family's foster caring networks. These dimensions of experience included collective passions of carers and workers; experiential change over time; collective experiences as a moving force; and experiences as transformational. Understanding of experience associated with the perceived strengths of the Mockingbird Family, including strategies to promote strong professional relationships between carers and workers, is an important element in strengthening environments of children and young people in care. Objective: The purpose of this paper is to report on enablers and barriers during the first 2 years of the health systems integration project that included the implementation of a health navigator role. The project aims to improve health outcomes for children and young people residing in out of home care in rural Australia with a health navigator co‐located between child protection practitioners and community health services clinicians. Setting: Rural Northwest Victoria. Participants: Sunraysia Community Health Services and the Department of Health and Human Services. Design: The qualitative design of the project evaluation involved semi‐structured interviews and documentary evidence analysis. Analyses of interviews and documentary data demonstrate the challenging nature of siloed service delivery in rural Australia, particularly during a time that comprised multiple interruptions due to COVID‐19. Results: A limited synergy between organisational priorities and reporting systems hindered project progress. The lack of a shared definition of 'health' challenged the effective collaboration between health clinicians and child protection practitioners and the role of the health navigator. The health navigator raising health awareness through project involvement, training and sector‐wide stakeholder engagement resulted in a slow but steady process of increased prioritisation of health care, increased health literacy among the child protection workforce, and broadening participation of area‐based stakeholders, but did not translate to increased access to health plans for children. Conclusion: Integrating health systems across multiple sites with support of a health navigator revealed difficulties, particularly during COVID‐19. The first phase of the project demonstrated the value of shared governance and partnerships as an imperative foundation for fundamental change. Relationships strengthened throughout the project, leading to a better understanding of area‐based strengths, which in turn supports improved pathways to health care for children and young people in OOHC within rural communities and driving the subsequent phases of the 10‐year project. Care‐experienced young people demonstrate significantly poor physical, social and mental health outcomes during and beyond their time in care, yet they have rarely been consulted or included in research examining their health needs to date. This qualitative paper explores care‐experienced young people's perceptions of health in care, including accessing healthcare and interacting with health professionals. The research methodology was informed by a co‐design approach through consultation with care‐experienced young people. Semi‐structured interviews were conducted with 10 young people. The findings revealed that: (1) the care system does not nurture young people's health; (2) young people in care experience difficulty accessing and navigating a complex health system; and (3) young people are not given a voice when it comes to their health. The findings provide key recommendations for practice, including the empowerment of young people through positive, responsive and trusting relationships to prioritise the health of young people in care. Young people transitioning from out-of-home care, commonly called care leavers, are known to be a vulnerable group. Many experience poor outcomes leading them to become homeless or involved in the criminal justice system. Yet compared to most other Anglophone democracies, Australia lacks mandatory assistance for care leavers beyond 18 years of age. There are also major legislative, policy, and program differences between care leaver entitlements in the individual states and territories. This paper argues that the Commonwealth Government should introduce a nationally consistent extended care system that would require all jurisdictions to provide a minimum standard of support until at least 21 years of age. A uniform set of extended care standards is even more vital in the context of COVID-19. IMPLICATIONS Many Australian care leavers have poor life chances despite recent advances in the support programs provided by the states and territories. From 2012 to 2014, we developed and subsequently evaluated the Healthy Eating, Active Living (HEAL) intervention—an obesity prevention and healthy lifestyle program designed for young people living in residential out-of-home care (OoHC). Based on quantitative (i.e., a randomised trial) and qualitative (i.e., semi-structured interviews) data and an iterative process of redevelopment, the program has evolved from taking a behavioural obesity prevention approach to now being underpinned by a trauma-informed care approach at a systems level. In this paper we outline the theoretical and empirical foundations of the HEALing Matters intervention that is currently being implemented at scale. In addition, we outline how our knowledge translation framework informed the iterative improvement of our intervention with a focus on stakeholder and end-user engagement, partnership, and codesign along the way. IMPLICATIONS There is a strong need for trauma-responsive health-focused programs and interventions within OoHC contexts. HEALing Matters addresses the need for trauma-informed approaches that encourage health behaviour change in young people living in OoHC. Policy, standards, and recommendations must not only acknowledge, but prioritise, the impact of trauma on health-related lifestyle behaviours in order to identify strategies to help address this impact. The majority of children and young people removed from the care of their parents by the state of Victoria, Australia, reside in foster or kinship care. These children have experienced a broad range of adverse conditions and are up to 4 times more likely to experience problems with mental health than their mainstream peers. This paper draws on the perspectives of foster and kinship carers, describing the disconnection between their role as mental health advocates and their interest in early intervention in a field which is dominated by crisis and the historic marginalisation of foster and kinship carers. Thirty‐one foster and kinship carers across greater metropolitan Melbourne, Australia, contributed to this study through interviews and focus groups. Participants demonstrated a practical understanding of mental health and an ability to identify a range of conditions that have an adverse impact on the mental health of children and young people in their care. The paper concludes that there is a lack of systemic support and even a range of barriers that affect the capacity of foster and kinship carers to promote the mental health and well‐being of the children and young people in their care. • Mentoring literature contains diverse, but as yet unclassified, mentoring relationship types. • Mentoring for young people young people transitioning out of care must address specific contextual needs and barriers. • More attention is needed in mentoring literature on the policy context and the nature of the gender and power in mentoring relationships. In Australia, out-of-home care (OoHC) is provided to safeguard children and young people unable to live with their families. Typically, this care stops at 18 years of age, leaving many young people without the relational support to find employment and stable accommodation. Mentoring is increasingly proposed as a way to support young people exiting care, though the types of mentoring and contextual factors vary considerably. This rapid review aims to identify key features of mentoring programs that support young people in OoHC. This paper utilised a rapid review methodology to survey the academic literature on mentoring programs with young people in OoHC. The authors conducted literature searches in October 2022 using a search string designed to provide targeted exposure to the literature. Articles were limited by year and location, with the specific search string and limitations applied to the Scopus database, and Proquest and Informit platforms. Utilising literature, we identified five overlapping models for mentor–mentee relationships into a provisional typology of five categories based on varying levels of formality and familiarity in the relationships: 'formal', 'natural', 'near-age', 'peer', and 'therapeutic'. In doing so, we identified seven important features of mentor–mentee relationships: shared experiences, relationship duration, boundaries and other relationships, identity and self-worth, educational contexts, power and participation, and role modelling. We identified four issues relating to mentoring in OoHC that are informed by the needs and barriers experienced by young people: training and support for mentors, unique challenges faced by young people in OoHC, transition out of care, and the matching process. Lastly, we established what might impact mentoring programs buy critically reflecting on the policy context and notions of empowerment. This rapid review of mentoring programs for care-experienced young people seeking employment offers valuable insights to inform future programs. The findings highlight diverse and unclassified mentoring relationship types, the absence of anticipated gender-specific findings, a lack of examination of the policy context, and a need for more robust conceptualisations of empowerment. This paper discusses two key strategies detailing how "relationship-focused" and "trauma-informed" intervention practices, which form the basis of an Australian therapeutic program called Treatment and Care for Kids (TrACK), made a difference in the lives of highly traumatised children. The TrACK program fosters highly traumatised children who, due to the complexity of their trauma needs, cannot be placed in traditional generalist foster care. Case files of 48 children were reviewed. Children were either current or former clients over a period of 18 years since the program was initiated. In analysing the data, we noticed that children who were once highly dysregulated in the domains of foster care placement, education, arousal regulation and peer relationships were now enjoying an enhanced level of stability in their lives. Abstract There is now considerable evidence to suggest that children placed in foster care experience greater levels of adversity compared to those in the general population. Early adversity has been shown to continue across the lifespan: those who have experienced the sustained trauma of threat or deprivation have a range of poor life outcomes including homelessness, poor mental health, and over-representation in the criminal justice system and low educational attainment. This paper reports on a study of a program response to children who have experienced trauma and are placed in out-of-home care. The program model, known as Treatment and Care for Kids (TrACK), introduced a paradigm shift in the treatment and care of children in Australia, in that it was designed to provide intensive, home-based therapeutic intervention for children presenting with complex trauma and challenging behaviours. This program sought to integrate knowledge about the neurobiology of trauma with therapeutic practice and care, reflecting growing recognition that 'care is not enough' to achieve holistic and effective healing for children who have experienced severe trauma. As such, the relationship that encircles the child was seen as the vehicle for recovery. The study examined client files and interviewed carers, key professionals and young people who had experienced the therapeutic care program since its inception 18 years ago. Data, informed by narrative inquiry methodology, were analysed. Key findings highlight the significance of relational practice to interrupt the projected trauma trajectory and for young people to stabilise and self-regulate. Introduction: The effects of prenatal exposure to alcohol are wide-ranging and pervasive in nature. In response to growing concerns about the lifelong disabilities related to prenatal alcohol exposure, a fetal alcohol spectrum disorder (FASD) diagnostic clinic was established in 2012. This was the first multi-disciplinary service operating permanently within an Australian health service. The current study aimed to explore the lived experience of the diagnostic process for caregivers of children with FASD.Methods: Twelve caregivers were approached and ten participated in audiotaped interviews about caring for a child with FASD. Qualitative analysis was undertaken on transcribed interviews using NVivo 10 for thematic analysis.Results: The major themes that emerged were: a desire for future support for their child although uncertainty about how this support could be accessed; an understanding of FASD prior to assessment but a concerted concern that this condition was not recognised as a disability across educational and related settings; that a formal assessment process provided validation for caregiver concerns and that caregivers felt respected and understood by the clinical team during this process.Discussion: These findings highlight the concerns of caregivers, emphasising both the importance of diagnostic services and the need for provision of further support beyond diagnosis for a child with FASD. Section 90 of the NSW Children and Young Persons (Care and Protection) Act 1998 No 157 refers to an Application for rescission or variation of final Care Orders for children who have previously been removed from the family home and placed into permanent care. The Act provides specific criteria for considering the restoration of a child to their family. These criteria provided a framework to evaluate how well the current literature addressed the factors that impact on the outcomes for children after final Care Orders have been made. A key difference between children in temporary care and those under final Care Orders is the length of time they have been in care at the time of the application. This paper's analysis identified length of time in care, issues related to attachment between parent and child, parenting capacity, and placement stability as key factors for determining the suitability of restoring children to their families. Considerable literature referred to children in temporary care arrangements, highlighting a need for further research to specifically evaluate the short- and long-term impact when final Care Orders are varied or rescinded. In this paper we present a comparative analysis of out-of-home care in Australia and Sweden. We compare the age structure of the out-of-home care population and the types of out-of-home care services provided to children and young people in both countries. Our analysis reveals that in Australia the out-of-home care service system is focused mainly on children who are deemed to be abused or neglected within their families, while in Sweden the majority of the out-of-home care population are teenagers who cannot live with their families for emotional or behavioural reasons. These population differences intersect with variations in the forms of service provision in both countries, with a much greater reliance on home-based care in Australia than in Sweden, while there is more extensive use of residential care in Sweden. We envisage that this paper will demonstrate how the age structure of the out-of-home care population, though rarely considered in international comparative child welfare research, reveals much about the assumptions on which State intervention with children and young people is based. We intend that this analysis will assist social workers to better understand and address the gaps in the quality and comprehensiveness of out-of-home care service provision to children and young people in both countries. The article presents the author's views on a paper by K. Healy and colleague on comparative child welfare and children in out-of-home-care (OOHC). The author states that although there is a great deal of evidence about high levels of investment in early childhood services in Sweden, it is not necessarily borne out by figures offered in the paper.

Governments across Australia are struggling to address escalating child protection notifications, a marked increase in the number of children in State care, a decrease in the number of foster carers and chronic workforce shortages in child welfare services. This paper explores the reform process that culminated in the proclamation of the Victorian Children, Youth and Families Act, 2005 and represents an aspect of the response in Victoria to these wider issues. It explores the history of the reform from the perspective of key actors from government, non-government sectors and academics who participated in the process. The paper explores the “ingredients of social policy reform” and how the change process was managed in Victoria. Foster carers' knowledge of caring for abused and neglected children is a largely under-researched area. We know very little about the nature of carers' knowledge in relation to caring for children who have been abused or neglected. This paper reports on an exploratory study which examined the knowledge of caring. Qualitative data were analysed based on in-depth interviews with 10 foster carers in Queensland, Australia. A range of knowledge was identified showing an interaction of both personalized and formal understandings in the caring response. Given the breadth of knowledge recognized from the analysis, this paper presents one dimension of their knowing: theory and abuse-indicated understandings. Participants' responses in these areas were at times sound, but many struggled to provide convincing and well-formed explanations of topics, such as attachment, harm and child development which may suggest that their understandings are not what they could be. Although further research is needed on a larger scale, this study does highlight the necessity to re-examine processes and strategies for carer knowledge support, so as to strengthen their understanding of these issues. In spite of a high investment in out-of-home care services across Australia, the educational attainment of children in care remains an issue of concern. This paper discusses findings from a study designed to identify the factors that promoted educational attainment of children in care based on 18 university-educated women with a care background. The narratives of these women from primary schooling through to graduating from a university were collected and analysed using narrative inquiry. The women's narratives strongly suggested that the educational needs of children in care vary according to individual care circumstances and individual characteristics, and can be identified in five groups: Destined, Decision, Determined, Denied, and Delayed. It was concluded that the promotion of educational attainment for children in care requires carefully tailored support and resources following assessment based on this information. The Who Am I? research was an interdisciplinary, action research project focused on the past and present record-keeping practices for people growing up in out-of-home care in Australia. This paper reports on two of the nested projects directed at current record-keeping practices. For the 100+ Points of Identity study, a tool was developed (the Document Accessibility Exercise or "Daesy") to determine the number of personal records critical to identity that could be accessed by practitioners prior to a young person leaving a placement. The Backpack of Identity project developed a further iteration of the action research cycle, as the first project identified the vulnerability of the record when placements for the young person changed. A number of implications for practice arose, including the need for greater attention to the development of personal records (as against an administrative record) and the need for practitioners to understand their responsibilities for the story "of the record" as well as the story "in the record". • How LGBTQA + young people experience support, care and safety may depend on the relational context. • Warm, loving relationships may be just as significant for the development of positive gender and sexual identities as explicit support for identity formation. • For LGBTQA + young people, homophobia and transphobia may also be experienced as a disruption to their safe and secure base – that is, as robbing them both literally and figuratively, of a secure place to belong in society. • Growing up in a heteronormative environment can add to the challenges of receiving safe and stable care and amplify the struggle to be seen supported whilst in OOHC. This paper presents a case study that discusses the lived experiences of two LGBTQA + young people who have been in out-of-home care in Australia, focusing particularly on the influence of relationships on their developing sexual identity. Utilising a secure base theoretical perspective, we argue that how young people experience support, care and safety may depend on the relational context in which it is received, and that warm, loving relationships may be just as significant for the development of positive gender and sexual identities as explicit support for identity formation. Foster care has been provided for thousands of vulnerable Australian children from the early nineteenth century. Despite the prevalence of this system of care as the preferred means of providing out-of-home care across the country from the late nineteenth century, very few people who lived in foster care as children have written about their experiences, a total of 23 in all. Although a small sample, these few stories tell a larger one of the complexities of lived experience of foster care: for some it was entirely positive, for others it was wholly negative and for most it was somewhere between those two extremes. What I show in this paper is that what many of the stories have in common, no matter where they sit on that continuum, is the painful acquaintance with social stigma at an early age. Life Without Barriers, a specialist foster care agency, are funding a Collaborative Education Program in partnership with Edmund Rice Education Australia Youth +. This paper aims to share some of the practice that gives a voice to the children and young people who have experienced trauma, abuse and neglect and have been placed in out of home care. Casework illustrations serve to reveal how the voice of the child is included in measures of participation, well-being and achievement that inform their Education Support Plans. Given the evidence of the impact of trauma on language development non-verbal methods of monitoring are used to explore the child's perceptions of: what context is most likely to engage them using the Preferences for Activities for Children flashcards and pictorial Likert scales; how their attachment to the carer is developing using video recordings of structured play sessions with carers; sandtray and symbol work when creating trauma narratives; and iPad applications to scaffold emotional expression, social problem solving, transition planning and literacy and numeracy interventions. The program model is limited by recent government financial cutbacks in addition to the scarcity of educational and developmental psychologists in Queensland and its expansion to a national model requires a creative, facilitative model of leadership and strategic social investment in the local community [ABSTRACT FROM AUTHOR]

Foster carers have a significant responsibility in caring for vulnerable children. In order to support and facilitate foster carers it is important to understand how they perceive and fulfil this responsibility. A qualitative story-telling study, informed by feminist perspectives, was used to conduct in-depth, semi-structured interviews with 20 women providing long-term foster care in Australia. Thematic analysis revealed these women characterised themselves as mothers, rather than paid carers, to the long-term foster children in their care. Using this maternal self-perception as the starting point, this paper reveals some of the challenges and difficulties participants encountered when mothering within the confines of the child protection system. Implications for nursing practice are discussed. Unaccompanied asylum-seeking children and young people are a particularly vulnerable group. This paper examines the existing literature on this group of children and young people in and leaving out-of-home care in the UK and Australia, and compares and contrasts their experiences. Young people transitioning from state out-of-home care are arguably one of the most vulnerable and disadvantaged groups in society. Many have been found to experience significant health, social and educational deficits. Most western countries have now introduced specific policy and practice initiatives to improve the life chances of care leavers, but there have been varied conclusions about the efficacy of those programs. This paper argues that a community development support model based on a partnership between professional social welfare workers and local community networks is most likely to enhance opportunities for the social inclusion of care leavers. Particular reference is made to the leaving care and after care support program introduced by St Luke's Anglicare in the Australian rural city of Bendigo which has included key program initiatives around housing, employment and mentoring. In response to the current shortfall in the number of registered foster carers in Australia, many agencies are now actively recruiting lesbians and gay men as foster carers. Importantly, however, this move towards recognising the role that lesbians and gay men may play as foster carers has on the whole not been supported by laws or policies specifically aimed at protecting such carers. Additionally, findings from a national research project on foster care in Australia presented in this paper highlight the ongoing heterosexism and homophobia faced by lesbian and gay foster carers when working with social workers and birth parents. Importantly, however, these findings highlight not only the negative aspects of foster care provision experienced by lesbians and gay men, but also the positive ways in which they continue to create families and resist or rework social norms. Implications for practice are made with specific reference to challenging such norms amongst social workers and advocating for legal change to support lesbian and gay foster carers. • Home-based carers face numerous barriers to facilitating healthy lifestyle behaviours. • Upskilling carers is crucial to improving long-term health outcomes for the child/ren in their care. • Training must be accessible, practical, and unique to both foster and kinship carers. Foster and kinship carers face unique and complex challenges in caring for Australia's most vulnerable children. It is therefore necessary to ensure that home-based carers are adequately upskilled and supported to promote the health of children in their care. The aim of this paper was to better understand the barriers to developing healthy lifestyle behaviours (relevant to nutrition and physical activity) among children in home-based care, from the perspective of foster and kinship carers. Focus group sessions were conducted with 21 carers: eight foster carers and 13 kinship carers. Four key themes were identified: (1) Childhood experience and children's healthy lifestyle behaviours; (2) There are personal costs of being a carer; (3) Carers require support to develop healthy lifestyle behaviours in children; and (4) Broader systemic factors impact healthy lifestyle development. Carers in the current study described their experience of key emotional, financial, and systemic barriers to facilitating healthy lifestyle behaviours among the children in their care. To address such challenges, carers recommended the development of trauma-informed support, education, and resources to prioritise and facilitate healthy lifestyle development among children in home-based care.
