This book describes translational cancer therapeutics and the way forward from clinical and molecular diagnosis to treatment. Lipids are an integral part of cell membrane architecture, are intermediaries in cell metabolism, and are involved in transmitting cell signals from hormones, growth factors and nutrients. A number of lipases and phospholipases, lipid kinases, lipid phosphatases, sphingosine kinases, and their reaction products have been implicated in fundamental cellular processes including cell proliferation, division and migration. These enzymes and their products underlie the molecular mechanisms of numerous human diseases, in particular metabolic disease (diabetes), cancer, neurodegenerative disease and cardiovascular disease.Over the last decade, studies have advanced to the point that a number of inhibitors for these enzymes have been developed to attempt to ameliorate these conditions; some of the inhibitors are currently in human clinical trial. The need for this book is to review the current status of this field and the prospect for the inhibitors to be clinically important.

In der Öffentlichkeit existiert das weit verbreitete Vorurteil, der medizinische Fortschritt spare das Thema »Krebs« vollständig aus und während der letzten Jahrzehnte habe es auf diesem Feld keine wesentliche Verbesserung der Behandlungsmöglichkeiten gegeben. Kaum ein Vorurteil geht an den vorhandenen Tatsachen weiter vorbei wie dieses, denn es gibt ihn eben doch, den Fortschritt bei der Behandlung der bösartigen Tumoren und es gibt ihn speziell bei den vier wichtigsten Tumoren des Urogenitaltraktes, nämlich dem Hodentumor, dem Prostatakarzinom, dem Nierentumor und eben auch beim Blasenkarzinom. Den dramatischsten Wandel haben wir sicherlich bei den bösartigen Tumoren des Hodens erlebt, wo die Heilungswahrscheinlichkeit im Laufe der letzten zwei bis drei Jahrzehnte fast an 100% herangebracht werden konnte. Beim P- statakrebs wurde seit der Jahrtausendwende in vielen Industrieländern ein deutlicher Rückgang der tumorbedingten Sterblichkeit dokumentiert, während sie in den Jahrzehnten zuvor stetig angestiegen war. Auch wenn endgültige Beweise ausstehen, so scheint es nicht unwahrsche- lich, dass diese erfreuliche Entwicklung wesentlich durch die verbesserten Möglichkeiten der Früherkennung und der dadurch weitverbreiteten kurativen Therapie bedingt ist. Beim Nier- tumor erlaubt die Früherkennung durch den Ultraschall sogar immer häufiger, die betroffenen Patienten bei ausgezeichneter Heilungswahrscheinlichkeit organerhaltend zu operieren. Demgegenüber hat das Blasenkarzinom während der letzten Jahre deutlich weniger A- merksamkeit auf sich gezogen, was aber keineswegs bedeutet, dass es nicht auch hier erhebliche Fortschritte gegeben hat: Gerade beim infiltrierenden Blasenkarzinom, einem hochmalignen Tumor, wurden die Behandlungskonzepte zunehmendbesser standardisiert.

Thyroid cancer is the fastest growing cancer in the U.S., especially among women. Given the relative success we have had in its treatment, there is a need to capitalize on it by better understanding the factors that underpin this malignancy, and exploring better strategies for diagnosis, treatment, and follow-up. To do so, we must take full advantage of the revolution in modern biology. This comprehensive volume addresses the needs of a broad readership. Against a backdrop of the complexity of the origins of cancer, pathology of thyroid tumors, including lymphomas, is discussed, as are molecular genetic lesions associated with spontaneous and radiation-related thyroid tumors, diagnostic tests available to surmise tumor subtypes (including a review of the potential of DNA micro arrays), advances in therapeutics (including recombinant hTSH superagonists, allowing for better treatment of well differentiated thyroid carcinoma and its imaging), and combinations of drugs that might influence the course of poorly differentiated and anaplastic thyroid cancer.
