BACKGROUND: Mississippi (MS) Delta adults and youth report obesity rates far exceeding those of the state and nation. State law requires in-school physical activity and nutrition practices to address childhood obesity but does not require evaluation of outcomes, specifically the impact on weight-related outcomes. This paper offers 3 things: (1) describes the weight status of elementary school-age children in the MS Delta; (2) explains the importance of including waist-to-height ratio (WHtR) values when reporting body mass index (BMI); and (3) provides impetus for policy that requires weight-related health risk, as measured by WHtR, to be assessed regularly as a means to evaluate school health policy. METHODS: We took anthropometric measures in a cross-sectional investigation of 1136 children from 11 public elementary schools in the MS Delta. Measures included BMI, waist circumference (WC) and WHtR. RESULTS: The prevalence of overweight and obesity (BMI ≥85th percentile) was 47.1% (18.3% overweight and 28.8% obese). In this sample, 59.9% and 42.0% were 'at risk' for weight-related chronic disease based on WC (≥75th percentile) and WHtR (>0.5), respectively. The differences in these proportions were statistically significant. The predominantly black districts reported higher on all of the weight-related measures. CONCLUSIONS: Investigators recommend the assessment of health policy include measures of health risk in addition to BMI, namely WHtR as it accounts for growth in both WC and height over age. Little is known about the effective dose of motivational interviewing for maintaining intervention-induced health outcome improvements. The purpose of this study was to compare effects of two doses of motivational interviewing for maintaining blood pressure improvements in a community-engaged lifestyle intervention conducted with African-Americans. Participants were tracked through a 12-month maintenance phase following a 6-month intervention targeting physical activity and diet. For the maintenance phase, participants were randomized to receive a low (4) or high (10) dose of motivational interviewing delivered via telephone by trained research staff. Generalized linear models were used to test for group differences in blood pressure. Blood pressure significantly increased during the maintenance phase. No differences were apparent between randomized groups. Results suggest that 10 or fewer motivational interviewing calls over a 12-month period may be insufficient to maintain post-intervention improvements in blood pressure. Objective: To examine the prevalence and modalities of complementary and alternative medicine (CAM) use in children with cancer and sickle cell disease; the reasons for use of CAM; and the use of CAM before, during, and after treatment in children with cancer. Methods: This single-center, observational study administered caregivers a written questionnaire regarding the use of CAM therapies. Results: A total of 101 caregivers completed questionnaires. Including prayer, total CAM use in oncology and sickle cell disease was 64% and 63%, respectively. Non-prayer CAM use was 30% in oncology and 23% in sickle cell disease. Of respondents who reported using any CAM, the three most commonly used types were prayer (62.3% oncology; 60.0% sickle cell disease), vitamins/minerals (14.8% oncology; 10.0% sickle cell disease), and massage (9.8% oncology; 7.5% sickle cell disease). The primary reasons for using CAM were to provide hope, to improve quality of life, and to lessen adverse effects. In oncology patients, CAM use tended to increase during treatment compared with before and after treatment. Conclusions: The reported prevalence of non-prayer CAM use was lower (23%-30%) in this sample than has been reported in national samples or other geographic regions of the United States. Nonetheless, participants reported many positive reasons for using CAM, including to gain hope, improve quality of life, and control pain. Thus, CAM use appears to be an important aspect of medical care for many pediatric hematology/oncology families and should be a consideration when providers are discussing treatment and quality of care with families. Comprehensive exposure-based approaches to treating posttraumatic stress disorder (PTSD) are effective, but they are time intensive and not widely used because of factors such as client noncompliance and fears of iatrogenic effects. Exposure by writing disclosure (WD), modeled after Pennebaker’s brief stress-reduction procedure, may circumvent these obstacles. WD treatment reduces PTSD symptoms in trauma victims but has rarely been tested in diagnosed PTSD participants and never in substance use disorder (SUD) populations—despite high comorbidity rates. The authors applied a standard Pennebaker WD treatment for 45 SUD inpatients screened for full or subsyndromal PTSD in an uncontrolled study. Results suggested potential strong cost-effectiveness: Posttreatment outcome measures showed significant symptom reductions, which remained stable at 3-month follow-up. With converging evidence from future controlled studies, WD strategies may emerge as effective and easily implemented treatment options for PTSD in SUD treatment settings. [ABSTRACT FROM PUBLISHER]

Purpose: Impaired pulmonary function has been associated with increased cardiovascular disease incidence and mortality. The objective of this study was to investigate associations between pulmonary function and left ventricular (LV) mass. Methods: Participants were African American women (n = 1,069) and men (n = 555) aged 49-73 years, from the Atherosclerosis Risk in Communities study. Mean pulmonary function values at the first (1987-1989) and second (1990-1992) examinations were used. Echocardiograms were performed at the third and early in the fourth examinations (1993-1996). Analysis of covariance and linear regression were used to assess associations. Results: Mean levels of LV mass decreased with increasing quintiles of forced expiratory volume in one second (FEV1) among female never smokers (P = 0.039). Forced vital capacity (FVC) showed stronger associations than FEV1 with LV mass. Among men, LV mass was positively associated with FEV1 among current and never smokers, and with FVC among never smokers. Additional analyses among never smokers revealed significant inverse associations between LV mass and FVC among women with waist-to-hip ratios of >0.85 and those with no history of diabetes. In contrast, significant positive associations between LV mass and FVC were seen among male never smokers with body mass index (BMI) of ≤24.9 kg/m2, waist-to-hip ratios of ≤0.95, no history of hypertension or diabetes, and ≤60 years old. BMI and waist-to-hip ratio significantly modified associations among men. Conclusions: Among never smokers, LV mass and pulmonary function were inversely associated among women and positively associated among men. Further studies are warranted. DOI: 10.1111/j.1752-1688.2011.00606.x Abstract: Degraded streams draining low-relief, intensively cultivated watersheds may experience periods of hypoxia or anoxia. A three-year study of water chemistry, fish, and physical habitat in the Big Sunflower River in northwestern Mississippi coupled with continuously logged physicochemical and hydrology data provided by others showed prolonged periods of hypoxia associated with higher flows. Fish species richness was directly related to dissolved oxygen (DO) concentration ( r2 = 0.35, p = 0.00004), and ordination using nonmetric multidimensional scaling (NMS) indicated strong association between fish community structure and DO. Low-head weirs supported relatively dense and diverse fish communities and thus provided local habitat enhancement, but may create stagnant zones upstream due to backwater effects that exacerbate low DO problems. Although hypoxia has been reported for some lightly degraded rivers and floodplains, our observations suggest hypoxia in Big Sunflower River and similar systems alters fish species composition and should be remediated. Cost-effective remediation will require better understanding of autotrophic and heterotrophic processes that control DO and the relationship of these processes to discharge. [ABSTRACT FROM AUTHOR]

This study provided the first examination of the psychometric properties of the 6-item Daily Spiritual Experiences Scale (DSES) in a large African American sample, the Jackson Heart Study (JHS). The JHS included measures of spiritual (DSES) and religious practices. Internal reliability, dimensionality, fit indices, and correlation were assessed. DSES scores reflected frequent daily spiritual experiences (12.84 ± 4.72) and reliability scores were high (α = 0.85; 95% CI 0.84-0.86). The DSES loaded on a single factor, with significant goodness-of-fit scores (RMSEA = 0.094, P < 0.01). Moderate significant correlations were noted among DSES items. Three experiments examined the effects of passage type on both individual and collaborative memory performance. In Experiment 1, both individuals and collaborative groups recalled more information from passages containing social information than non-social information. Furthermore, collaborative inhibition (CI) was observed for both types of passages. In Experiment 2, which included a social passage that did not contain gossip, significant main effects of both gossip (gossip > non-gossip) and sociability (explicit > implicit) were observed. As in Experiment 1, CI was observed across all conditions. Experiment 3 separately manipulated gossip and the interest level of the passages and both of these factors enhanced memory performance. Moreover, robust CI was again observed across all conditions. Taken together, the present results demonstrate a mnemonic benefit for social information in individuals and collaborative groups. Objective:To determine if postponement of delivery to administer fetal lung maturation corticosteroids (PDACs) in mothers with antepartum eclampsia <34 weeks gestation benefits the fetus without compromising the mother.Study Design:A case series of 37 maternal-perinatal pairs over a 9-year period with antepartum eclampsia between 24 and 34 weeks gestation from a single tertiary center were reviewed retrospectively. Duration of PDAC, clinical course and maternal-fetal outcomes, including impact of duration of PDAC on neonatal pulmonary function, were recorded for each case. Group assignment was based on length of corticosteroid treatment course before delivery: Group A, 0 to 24 h, n=28; B, 24 to <48 h, n=5; C, 48 h, n=4. Data were collected and analyzed by one-way analysis of variance (ANOVA), ANOVA on ranks, χ2-test and Fisher's exact tests where appropriate; statistical significance was determined by a P-value <0.05.Result:Overall, 37 of 68 eclampsia patients in 1999 to 2007 met inclusion criteria. No adverse maternal or fetal event occurred while delivery was postponed. Immediate neonatal intubation or continuous positive airway pressure was required for 23/28 in A, 4/5 in B and 2/4 in C; room air was sufficient at birth for 5/28 in A, 1/5 in B and 2/4 in C. No newborn >33 weeks gestation required INI. Prolonged (that is, >1 day) mechanical ventilation was not required for any infant with a gestational age 32 weeks or PDAC 48 h. Two of three neonatal deaths in group A were attributed to pulmonary insufficiency.Conclusion:PDAC for antepartum preterm eclampsia, especially 32 weeks gestation, appears to offer notable fetal pulmonary benefit without significantly increasing maternal or fetal risk. Five focus groups were conducted as part of an assessment to identify stakeholders' views of the service delivery needs of persons who are visually impaired. The analyses of the data from the focus groups were clustered into four distinct themes or concerns regarding the variability in services, outreach, transportation, and service delivery. Recent advances in geographic information systems software and multilevel methodology provide opportunities for more extensive characterization of “at-risk” populations in epidemiologic studies. The authors used age-restricted, geocoded data from the all-African-American Jackson Heart Study (JHS), 2000–2004, to demonstrate a novel use of the Lorenz curve and Gini coefficient to determine the representativeness of the JHS cohort to the African-American population in a geographic setting. The authors also used a spatial binomial model to assess the geographic variability in participant recruitment across the Jackson, Mississippi, Metropolitan Statistical Area. The overall Gini coefficient, an equality measure that ranges from 0 (perfect equality) to 1 (perfect inequality), was 0.37 (95% confidence interval (CI): 0.30, 0.45), indicating moderate representation. The population of sampled women (Gini coefficient = 0.34, 95% CI: 0.30, 0.39) tended to be more representative of the underlying population than did the population of sampled men (Gini coefficient = 0.49, 95% CI: 0.35, 0.61). Representative recruitment of JHS participants was observed in predominantly African-American and mixed-race census tracts and in the center of the study area, the area nearest the examination clinic. Background: Hurricane Katrina devastated the Mississippi Gulf Coast in August 2005. Intrusive re-experiencing is a common posttraumatic stress symptom. However, young children with limited introspection skills might have difficulties identifying their intrusive thoughts. Method: A sample of 165 5- to 9-year-old children were surveyed about their unwanted intrusive thoughts and their knowledge about thinking, 10 months following the hurricane. Results: Results replicate and extend the findings from a previous study ( Sprung, 2008 ). Although there was no difference in the overall occurrence of intrusive thoughts, there was a striking difference between hurricane-exposed and control children in their contents. Children's knowledge about thinking was linked to their ability to report on their negative intrusive thoughts, even taking language ability into account. Conclusion: There is a shift toward negative content following hurricane exposure compared with non-hurricane-exposed children and knowledge about thinking is linked to the reporting of such intrusive thoughts. This study compares three brief participant-initiated telephone interventions aimed at enhancing treatment engagement of individuals on a substance abuse treatment wait list. Policies requiring that wait list members call at least every other week in order to remain eligible for treatment remained in place for the standard and enhanced conditions but not for the voluntary condition. The standard condition was a minimal intervention, providing information on the program. The enhanced condition focused on client motivation for treatment and recovery. If individuals in the voluntary condition called, they were provided information about current wait list number and approximate remaining wait time. The rate of treatment engagement was the same among treatment conditions. The best predictor of engagement was the number of calls placed to the program while waiting. Treatment condition was a positive predictor of call frequency; presence of a comorbid psychiatric diagnosis was a negative predictor. The article also discusses future directions.
