2. Influences on cardiovascular responses of active ankle joint exercise and passive exercise using intermittent pneumatic compression

術後の早期離床に向けて、足関節自動運動の深部静脈血栓塞栓症の予防効果は明らかにされているが、運動効果の面から全身の循環動態への影響を明らかにしたものはない。そこで本研究は、健常者13名(20.4±0.2歳)を対象に、連続指血圧測定装置を用いて、足関節自動運動および間欠的空気圧迫装置での運動15分間、運動終了後15分間の血行動態パラメータの経時的変化を調べ、循環動態への影響を検討した。その結果、足関節自動運動ではHRは運動開始後5分に有意(p<0.01)に上昇し、MBPは運動開始後10分に有意に(p<0.05)上昇し、運動中は高値を持続した。またCOは有意に上昇し、SVとTPRには変化がなかった。一方、間欠的空気圧迫装置ではいずれも有意な変化はなかった。このことから、15分間の運動効果として、足関節自動運動は、HRやMBPに影響することが明らかとなった。, Much evidence has accumulated suggesting that active ankle joint exercises contribute to preventing deep venous thrombosis in early postoperative ambulation. However, the influence of exercise on cardiovascular responses is unknown. This study was designed to examine the course of cardiovascular changes following active ankle joint exercise and passive exercise using intermittent pneumatic compression. Mean blood pressure (MBP), heart rate (HR),cardiac output (CO),stroke volume (SV),and total peripheral resistance (TPR)were measured at seven time points in 13 healthy subjects (mean age, 20.4±0.2 years):2 min before exercise; at 5,10,and 15min after starting exercise; and at 5,10,and15 min after finishing exercise. During active ankle joint exercise, MBP and HR were significantly increased from baseline levels during exercise(at 10 min in MBP; at 5 min in HR),and HR remained at a higher level during the after starting exercise.CO was significantly increased from baseline at 5 min into active ankle joint exercise(p<0.01) ,while SV and TPR showed no significant difference from baseline. On the other hand , MBP, HR, CO, SV, and TPR showed no changes before, during, or after passive exercise using intermittent pneumatic compression. In conclusion, MBP, HR, and CO were significantly increased following active ankle joint exercise for 15 min, suggesting that active ankle joint exercise may effects as systemic exercise in early postoperative ambulation.

Objective : to identify the nursing diagnosis, according to the Taxonomy II of the North American Nursing Diagnosis Association (Nanda), in a patient with deep venous thrombosis (DVT), allowing the planning of nursing assistance. Methodology : this is a case study, carried out within the period from February 19 2009 to February 26 2009. Data were collected through the information available in the medical record and the nursing consultation carried out having the patient’s interview and physical examination as a basis. The study observed the ethical aspects established in Resolution 196/96 from the National Health Council. Results : the data were presented as a nursing history and the diagnosis, results, and nursing interventions were presented in figures. The information allowed the planning of nursing assistance, with the identification of the following nursing diagnoses: ineffective peripheral tissue perfusion, excessive liquid volume, risk for impaired skin integrity , impaired physical mobility , acute pain, unbalanced nutrition ― less than body requirements ―, and fatigue, establishing the nursing interventions implemented and assessed. Conclusion : the planning of nursing assistance allowed the prevention of complications from DVT and a better general clinical state of the female patient, highlighting the importance of planning the care provided to the patient with this disease. RESUMO Objetivo : identificar o diagnostico de enfermagem, segundo a Taxonomia II da Associacao Norte-Americana de Diagnostico de Enfermagem (North American Nursing Diagnosis Association ― Nanda) , em paciente com trombose venosa profunda (TVP), possibilitando a elaboracao da assistencia de enfermagem. Metodologia : trata-se de um estudo de caso, realizado no periodo de 19 de fevereiro a 26 de fevereiro de 2009. Os dados foram coletados por meio das informacoes disponiveis no prontuario e da realizacao da consulta de enfermagem a partir da entrevista e exame fisico do paciente. O estudo obedeceu aos aspectos eticos estabelecidos na Resolucao n. 196/96 do Conselho Nacional de Saude. Resultados : os dados foram apresentados na forma de historico de enfermagem e em figuras, os diagnosticos, resultados e intervencoes de enfermagem. As informacoes possibilitaram a elaboracao da assistencia de enfermagem, identificando-se os seguintes diagnosticos de enfermagem: perfusao tissular periferica ineficaz, volume de liquidos excessivo, risco para integridade da pele prejudicada, mobilidade fisica prejudicada, dor aguda, nutricao desequilibrada ― menos do que as necessidades corporais ― e fadiga, estabelecendo-se as intervencoes de enfermagem que foram implementadas e avaliadas. Conclusao : a elaboracao da assistencia de enfermagem possibilitou a prevencao de complicacoes decorrentes da TVP e melhora do estado clinico geral da paciente, evidenciando a importância da realizacao do plano de cuidados ao paciente com essa enfermidade. The article reports on the findings of a study conducted by students at one university in the U.S. regarding the effect of smoking tobacco through waterpipes. The study showed that high levels of exhaled carbon monoxide (CO) after smoking tobacco through waterpipes could set a significant risk. Furthermore, the researchers found out that the amount of exhaled CO of waterpipe smokers was higher compared to those who smoked one packet of cigarettes per day.

The article reports on the findings of a research conducted by researchers in Switzerland regarding the epidemiological data, clinical features and mechanisms of several spinal injuries related to alpine skiing and snowboarding. The study showed that central nervous system injuries and transient neurological injuries occurred in 23 skiers and five snowboarders. Based on the results, they suggest that snow sports enthusiasts should consider the use of spine protectors to prevent spinal injuries.

10. Ankle sprain can lead to increased risk of venous thrombosis.

The article reports on the findings of a study conducted by the researchers in the Netherlands about the association of thrombosis and common minor injuries, such as ankle sprains and sural muscle ruptures. The study showed that thrombosis was strongly associated with minor leg injuries and the strongest was when injuries occurred in the four weeks before thrombosis. Based on the results, the researchers concluded that minor injuries could be a major contributor to the occurrence of venous thrombosis.
