2. Implementation and impact of an electronic patient reported outcomes system in a phase II multi-site adaptive platform clinical trial for early-stage breast cancer.

Abstract

Objectives: We describe the development and implementation of a system for monitoring patient-reported adverse events and quality of life using electronic Patient Reported Outcome (ePRO) instruments in the I-SPY2 Trial, a phase II clinical trial for locally advanced breast cancer. We describe the administration of technological, workflow, and behavior change interventions and their associated impact on questionnaire completion., Materials and Methods: Using the OpenClinica electronic data capture system, we developed rules-based logic to build automated ePRO surveys, customized to the I-SPY2 treatment schedule. We piloted ePROs at the University of California, San Francisco (UCSF) to optimize workflow in the context of trial treatment scenarios and staggered rollout of the ePRO system to 26 sites to ensure effective implementation of the technology., Results: Increasing ePRO completion requires workflow solutions and research staff engagement. Over two years, we increased baseline survey completion from 25% to 80%. The majority of patients completed between 30% and 75% of the questionnaires they received, with no statistically significant variation in survey completion by age, race or ethnicity. Patients who completed the screening timepoint questionnaire were significantly more likely to complete more of the surveys they received at later timepoints (mean completion of 74.1% vs 35.5%, P < .0001). Baseline PROMIS social functioning and grade 2 or more PRO-CTCAE interference of Abdominal Pain, Decreased Appetite, Dizziness and Shortness of Breath was associated with lower survey completion rates., Discussion and Conclusion: By implementing ePROs, we have the potential to increase efficiency and accuracy of patient-reported clinical trial data collection, while improving quality of care, patient safety, and health outcomes. Our method is accessible across demographics and facilitates an ease of data collection and sharing across nationwide sites. We identify predictors of decreased completion that can optimize resource allocation by better targeting efforts such as in-person outreach, staff engagement, a robust technical workflow, and increased monitoring to improve overall completion rates., Trial Registration: https://clinicaltrials.gov/study/NCT01042379., (© The Author(s) 2024. Abstract

The steady rise in utility grid penetration of single phase solar photovoltaic system has contributed to the mandatory requirement of Low Voltage Ridethrough (LVRT) capability. LVRT capability provides stable grid integration during transient conditions. A seamless LVRT capability operation in a single phase photovoltaic system using peak value based fault detection and Orthogonal Signal Generation (OSG) based voltage sag detection is conducted. In this paper, two controllers are employed for two different operations. The PQ theory controller is used for power control operation and Proportional–Resonant (PR) with Harmonic Compensation (HC) controller is used for current control operation in LVRT. MATLAB/Simulink verifies the LVRT operation mode as per the grid code requirements to ride through voltage sag during fault condition. It is observed that the system is in operation during transient conditions and provides reactive support to ride through the fault.

27. Effectiveness of using picture description technique for grammar: An experimental study

Abstract

Learning and teaching grammar seem to be a challenge for most of the English as Second Language (ESL)/English as a Foreign Language (EFL) learners and teachers. There is a need to use creative and innovative techniques to teach grammar and tense. This study was conducted with the aim of exploring the effectiveness of using picture description technique to teach grammar and in particular the present continuous sentence pattern. This study was a descriptive survey research and mixed methods were applied in data collection. In this study, 25 students of Grade 10 and 10 ESL teachers of T/Mu/Al Hilal Central College, Mutur were randomly selected as sample population. The data collection instruments were an interview and a questionnaire. According to the opinions survey among the students and teachers, majority of the students express their lack of interest in learning grammar since interesting and creative games and techniques are not used in grammar lessons, and they accepted that the use of picture description technique can improve their ability of constructing sentence pattern and understanding the use of present continuous tense. Further, according to the findings of the questionnaire, majority of the teachers accepted that many English teachers do not use any techniques to teach grammar and tense, and the use of picture description technique can improve students’ proper understanding of present continuous tense. In general, in teaching and learning English, both teachers and students encounter various difficulties such as lack of teaching materials, students’ lack of interest, students’ insufficient basic knowledge in English, lack of facilities to apply different techniques and games, over-crowded classroom, use of deductive method to teach grammar and tense and teaching tense with less use of interesting games and techniques. This problem can be overcome and students’ proper understanding of present continuous tense can be improved when the suggested recommendations in this study are implemented.

28. Characterizing attitudes related to future child-bearing in young women diagnosed with early-stage breast cancer.

Abstract

Purpose: This study characterizes attitudes and decision-making around the desire for future children in young women newly diagnosed with early-stage breast cancer and assesses how clinical factors and perceived risk may impact these attitudes., Methods: This is a prospective study in women < 45 years with newly diagnosed stage 1-3 breast cancer. Patients completed a REDCap survey on fertility and family-building in the setting of hypothetical risk scenarios. Patient, tumor, and treatment characteristics were collected through surveys and medical record., Results: Of 140 study patients [median age = 41.4 (range 23-45)], 71 (50.7%) were interested in having children. Women interested in future childbearing were younger than those who were not interested (mean = 35.2 [SD = 5.2] vs 40.9 years [3.90], respectively, p < 0.001), and more likely to be childless (81% vs 31%, p < 0.001). 54 women (77.1% of patients interested in future children) underwent/planned to undergo oocyte/embryo cryopreservation before chemotherapy. Interest in future childbearing decreased with increasing hypothetical recurrence risk, however 17% of patients wanted to have children despite a 75-100% hypothetical recurrence risk. 24.3% of patients wanted to conceive < 2 years from diagnosis, and 35% of patients with hormone receptor positive tumors were not willing to complete 5 years of hormone therapy., Conclusion: Many young women diagnosed with early-stage breast cancer prioritize childbearing. Interest in having a biologic child was not associated with standard prognostic risk factors. Interest decreased with increasing hypothetical recurrence risk, though some patients remained committed to future childbearing despite near certain hypothetical risk. Abstract

Anaemia is a serious concern for the young children as it affects the cognitive performance, development (cognitive and behavioral), coordination, language development and scholastic achievement. Various interventions have been undertaken during last 50 years such as green revolution, PDS, ICDS, etc. Nutritional deficiencies remain with the same burden and nutritional anemia being one of the major concerns in India. Hence it becomes quite evident hat alternative strategies should be tried to improve the nutritional anemia and reduce the morbidity associated with it, such as use of locally available, traditional food to increase the bioavailability of iron and to improve the nutritional behavior through nutritional education. The present study was proposed with an aim to assess the role of Moringa Olifera powder in improving haemoglobin level among the anaemic children. A case control study was conducted at a village, Chikaballapur district of Karnataka. 312 school children were screened for Haemoglobin level; Out of 312 students 124 school children fulfilled the inclusion criteria. 66 children were recruited to control group and interventional group respectively. The children in the interventional group were administered with 3gms per day (1/2 a tea spoon) of Moringa Olifera powder along with their food for 3 months and regular supplements provided at school. Control group was advised to continue with the regular meal and regular supplements provided at the school. Haemoglobin levels were estimated before and 3 months after the intervention. Out of 58 children in the interventional group 33 (56.9%) were females and 25 (43.1%) were males and out of 66 children in the control group 34 (51.55) were females and 32(48.5%) were males. The mean Haemoglobin level of the children in the interventional group before the intervention was 10.332 which increased to 10.967 after the intervention. Similarly the mean Haemoglobin level of children in the control group before the intervention was 9.950 which remained 9.873 after the intervention. The increase in the mean haemoglobin level in the interventional group was significant with the p value of 0.045. After the intervention the children with moderate anaemia decreased from 38(65.5%) to 23(39.7%) and 13 (22.4%) children turned normal in the experimental group and in the control group the children with moderate anaemia increased from 52(78.8%) to 55(81.8%). There a statistically significant marginal improvement in haemoglobin level in the interventional group after three months.

Abstract

Background: Comorbidities and symptoms in metastatic breast cancer patients impact treatment decisions and influence prognosis and quality of life. The objective of this study is to examine the concordance between physician documentation and patient reports of comorbidities and symptoms to understand their comparative effectiveness., Methods: New patients with metastatic breast cancer completed an electronic intake survey assessing patient health history and symptoms. Physician documentation across 54 comorbidities and 42 symptoms was abstracted from notes for the corresponding clinic visits between November 2016 and March 2020. Concordance between patient reports and medical records for each condition and hazards ratios for each patient versus physician reported comorbidity and symptom were assessed., Results: A total of 168 patients were included in the analysis (age, median = 56 years, range = 29-86 years; 131 white [78.9%]). Twenty-three of 54 comorbidities had a moderate to high level of agreement between patients and physicians (κ ≥ 0.40). Physicians documented higher numbers of comorbidities that can be objectively measured which also had higher concordance (e.g., diabetes [κ = 0.83] and hypertension [κ = 0.79]) while patients reported higher numbers of comorbidities that are more subjective which also had lower concordance (anxiety [κ = 0.30], GERD [κ = 0.36]). One physician-documented and two patient-reported comorbidities were significantly associated with survival (p < 0.05). Only 2 of 42 symptoms had a moderate to high level of agreement between patients and physicians. One physician-documented and nine patient-reported symptoms were significantly associated with decreased survival (p < 0.05)., Conclusion: Agreement between patients' and physicians' reporting of comorbidities varies substantially, and patient reports can complement physician documentation. Physicians significantly underreported symptoms versus patients; thus, concordance was also low. Multiple patient-reported symptoms were predictive of survival; thus, incorporating them can provide more informative estimates of predicted survival., (© 2023 The Authors. This study evaluated the acceptability and psychological impact of a one-day workshop emphasizing behavioral strategies involving intention and self-efficacy.Methods: Breast cancer survivors attending a one-day Pathways for Survivors workshop, provided feedback, and completed electronic quality of life (QOL) questionnaires at baseline, 1- and 6-weeks, and 6-months post workshop. Attendees’ baseline QOL scores were compared to follow up (FUP) scores. Scores from patients receiving routine FUP care were also compiled as a reference population.Results: Seventy-seven patients attended one of nine workshops. Mean satisfaction was 9.7/10 with the workshop, and 9.96/10 with the moderator. Participants’ baseline mean Patient-Reported Outcomes Measurement Information System (PROMIS) anxiety and depression scores were 57.8 ± 6.9 and 55.3 ± 7.5, significantly higher than routine FUP care patients (49.1 ± 8.3 and 47.3 ± 8 respectively). PROMIS anxiety and depression scores decreased and Happiness Index Profile (HIP-10), measuring intention and resiliency, increased significantly at 1- and 6-week FUPs.Conclusions: Pathways for Survivors was favorably received. Anxiety and depression decreased significantly at 1- and 6-weeks post workshop and remained below baseline at 6 months. Increased HIP-10 scores support that patients incorporated skills from the workshop. A one-day workshop led by a lay moderator significantly improved several psychological measures, suggesting it may be a useful and time-efficient strategy to improve QOL in breast cancer survivors. We are investigating whether an abbreviated “booster” of the intervention at a later date could further improve and maintain QOL gains.

Abstract

Globally, an approximate of 380,000 patients succumbed to death due to the pandemic COVID-19 which also infected more than six million people since December 2019. Not sparing anyone, COVID-19 infections are widely reported among healthcare professionals, sanitation workers and researchers too while global leaders and various governments are providing their best in defending their citizens against this airborne and contact spread virus. In order to contain the virus and protect millions of lives from this deadly coronavirus, there is a need to have a combination of advanced engineering technology and medical facilities. Application of applied science, engineering and technology diffuse almost every aspect of contemporary living. Grasping the fundamentals to determine humanity's most imperative and forthcoming challenges is essential. Artificial Intelligence, the technology that learns, adapts and reciprocates the actions according to the situations, finds optimum position in the fight against corona virus and acts as a powerful tool against this pandemic. In this research article, the authors discusses how Artificial Intelligence (AI) can be leveraged to fight the deadly virus. The research paper further discusses the efficient utilization of AI across the globe to help in testing, treating and serving the population in these hard times. This manuscript focuses on the potential impact of the process in which AI can be implemented to prevent, test and treat.
