2. A scoping review of rule-based clinical decision support malfunctions.

- Abstract
Objective: Conduct a scoping review of research studies that describe rule-based clinical decision support (CDS) malfunctions., Materials and Methods: In April 2022, we searched three bibliographic databases (MEDLINE, CINAHL, and Embase) for literature referencing CDS malfunctions. We coded the identified malfunctions according to an existing CDS malfunction taxonomy and added new categories for factors not already captured. We also extracted and summarized information related to the CDS system, such as architecture, data source, and data format., Results: Twenty-eight articles met inclusion criteria, capturing 130 malfunctions. Architectures used included stand-alone systems (eg, web-based calculator), integrated systems (eg, best practices alerts), and service-oriented architectures (eg, distributed systems like SMART or CDS Hooks). No standards-based CDS malfunctions were identified. The "Cause" category of the original taxonomy includes three new types (organizational policy, hardware error, and data source) and two existing causes were expanded to include additional layers. Only 29 malfunctions (22%) described the potential impact of the malfunction on patient care., Discussion: While a substantial amount of research on CDS exists, our review indicates there is a limited focus on CDS malfunctions, with even less attention on malfunctions associated with modern delivery architectures such as SMART and CDS Hooks., Conclusion: CDS malfunctions can and do occur across several different care delivery architectures. To account for advances in health information technology, existing taxonomies of CDS malfunctions must be continually updated. This will be especially important for service-oriented architectures, which connect several disparate systems, and are increasing in use., (© The Author(s) 2024. The discovery of rapidly variable Very High Energy (VHE; E > 100 GeV) γ -ray emission from 4C +21.35 (PKS 1222+216) by MAGIC on 2010 June 17, triggered by the high activity detected by the Fermi Large Area Telescope (LAT) in high energy (HE;E >100 MeV) γ -rays, poses intriguing questions on the location of the γ -ray emitting region in this flat spectrum radio quasar. We present multifrequency data of 4C +21.35 collected from centimeter to VHE during 2010 to investigate the properties of this source and discuss a possible emission model. The first hint of detection at VHE was observed by MAGIC on 2010 May 3, soon after a γ -ray flare detected by Fermi-LAT that peaked on April 29. The same emission mechanism may therefore be responsible for both the HE and VHE emission during the 2010 flaring episodes. Two optical peaks were detected on 2010 April 20 and June 30, close in time but not simultaneous with the two γ -ray peaks, while no clear connection was observed between the X-ray and γ -ray emission. An increasing flux density was observed in radio and mm bands from the beginning of 2009, in accordance with the increasing γ -ray activity observed by Fermi-LAT, and peaking on 2011 January 27 in the mm regime (230 GHz). We model the spectral energy distributions (SEDs) of 4C +21.35 for the two periods of the VHE detection and a quiescent state, using a one-zone model with the emission coming from a very compact region outside the broad line region. The three SEDs can be fit with a combination of synchrotron self-Compton and external Compton emission of seed photons from a dust torus, changing only the electron distribution parameters between the epochs. 4. A Clinical Decision Support System for Motivational Messaging and Tobacco Cessation Treatment for Parents: Pilot Evaluation of Use and Acceptance.

- Abstract
Background: Research is needed to identify how clinical decision support (CDS) systems can support communication about and engagement with tobacco use treatment in pediatric settings for parents who smoke. We developed a CDS system that identifies parents who smoke, delivers motivational messages to start treatment, connects parents to treatment, and supports pediatrician-parent discussion., Objective: The objective of this study is to assess the performance of this system in clinical practice, including receipt of motivational messages and tobacco use treatment acceptance rates., Methods: The system was evaluated at one large pediatric practice through a single-arm pilot study from June to November 2021. We collected data on the performance of the CDS system for all parents. Additionally, we surveyed a sample of parents immediately after the clinical encounter who used the system and reported smoking. Measures were: (1) the parent remembered the motivational message, (2) the pediatrician reinforced the message, and (3) treatment acceptance rates. Treatments included nicotine replacement therapy, quitline referral (phone counseling), and/or SmokefreeTXT referral (text message counseling). We described survey response rates overall and with 95% confidence intervals (CIs)., Results: During the entire study period, 8,488 parents completed use of the CDS: 9.3% ( n = 786) reported smoking and 48.2% ( n = 379) accepted at least one treatment. A total of 102 parents who smoke who used the system were approached to survey 100 parents (98% response rate). Most parents self-identified as female (84%), aged 25 to 34 years (56%), and Black/African American (94%), and had children with Medicaid insurance (95%). Of parents surveyed, 54% accepted at least one treatment option. Most parents recalled the motivational message (79%; 95% CI: 71-87%), and 31% (95% CI: 19-44%) reported that the pediatrician reinforced the motivational message., Conclusion: A CDS system to support parental tobacco use treatment in pediatric primary care enhanced motivational messaging about smoking cessation and evidence-based treatment initiation., Competing Interests: None declared., (Thieme. Background: Helping parents quit smoking is a public health priority. However, parents are rarely, if ever, offered tobacco use treatment through pediatric settings. Clinical decision support (CDS) systems developed for the workflows of pediatric primary care may support consistent screening, treatment, and referral., Objectives: This study aimed to develop a CDS system by using human-centered design (HCD) that identifies parents who smoke, provides motivational messages to quit smoking (informed by behavioral science), and supports delivery of evidence-based tobacco treatment., Methods: Our multidisciplinary team applied a rigorous HCD process involving analysis of the work environment, user involvement in formative design, iterative improvements, and evaluation of the system's use in context with the following three cohorts: (1) parents who smoke, (2) pediatric clinicians, and (3) clinic staff. Participants from each cohort were presented with scenario-based, high-fidelity mockups of system components and then provided input related to their role in using the CDS system., Results: We engaged 70 representative participants including 30 parents, 30 clinicians, and 10 clinic staff. A key theme of the design review sessions across all cohorts was the need to automate functions of the system. Parents emphasized a system that presented information in a simple way, highlighted benefits of quitting smoking, and allowed direct connection to treatment. Pediatric clinicians emphasized automating tobacco treatment. Clinical staff emphasized screening for parent smoking via several modalities prior to the patient's visit. Once the system was developed, most parents (80%) reported that it was easy to use, and the majority of pediatricians reported that they would use the system (97%) and were satisfied with it (97%)., Conclusion: A CDS system to support parental tobacco cessation in pediatric primary care, developed through an HCD process, proved easy to use and acceptable to parents, clinicians, and office staff. Objective: Develop and evaluate an interactive information visualization embedded within the electronic health record (EHR) by following human-centered design (HCD) processes and leveraging modern health information exchange standards., Materials and Methods: We applied an HCD process to develop a Fast Healthcare Interoperability Resources (FHIR) application that displays a patient's asthma history to clinicians in a pediatric emergency department. We performed a preimplementation comparative system evaluation to measure time on task, number of screens, information retrieval accuracy, cognitive load, user satisfaction, and perceived utility and usefulness. Application usage and system functionality were assessed using application logs and a postimplementation survey of end users., Results: Usability testing of the Asthma Timeline Application demonstrated a statistically significant reduction in time on task (P < .001), number of screens (P < .001), and cognitive load (P < .001) for clinicians when compared to base EHR functionality. Postimplementation evaluation demonstrated reliable functionality and high user satisfaction., Discussion: Following HCD processes to develop an application in the context of clinical operations/quality improvement is feasible. Our work also highlights the potential benefits and challenges associated with using internationally recognized data exchange standards as currently implemented., Conclusion: Compared to standard EHR functionality, our visualization increased clinician efficiency when reviewing the charts of pediatric asthma patients. We have searched a sample of 9.6 million BB-bar events for the lepton-flavor-violating decays B --> h e^{+-} mu^{-+}, B^+ --> h^- e^+ e^+, B^+ --> h^- e^+ mu^+, and B^+ --> h^- mu^+ mu^+, where h is pi, K, rho, and K*(892), a total of sixteen modes. Gamma-ray binaries are stellar systems for which the spectral energy distribution (discounting the thermal stellar emission) peaks at high energies. Detected from radio to TeV gamma rays, the gamma-ray binary LS I + 61 degrees 303 is highly variable across all frequencies. One aspect of this system's variability is the modulation of its emission with the timescale set by the similar to 26.4960 day orbital period. Here we show that, during the time of our observations, the gamma-ray emission of LS I + 61 degrees 303 also presents a sinusoidal variability consistent with the previously known superorbital period of 1667 days. This modulation is more prominently seen at orbital phases around apastron, whereas it does not introduce a visible change close to periastron. It is also found in the appearance and disappearance of variability at the orbital period in the power spectrum of the data. This behavior could be explained by a quasi-cyclical evolution of the equatorial outflow of the Be companion star, whose features influence the conditions for generating gamma rays. These findings open the possibility to use gamma-ray observations to study the outflows of massive stars in eccentric binary systems.

We report an analysis of the interstellar γ-ray emission from the Chamaeleon, R Coronae Australis (R CrA), and Cepheus and Polaris flare regions with the Fermi Large Area Telescope. They are among the nearest molecular cloud complexes, within ~300 pc from the solar system. The γ-ray emission produced by interactions of cosmic rays (CRs) and interstellar gas in those molecular clouds is useful to study the CR densities and distributions of molecular gas close to the solar system. The obtained γ-ray emissivities above 250 MeV are (5.9 ± 0.1stat +0.9 –1.0sys) × 10^–27 photons s^–1 sr^–1 H-atom–1, (10.2 ± 0.4stat +1.2 –1.7sys) × 10^–27 photons s^–1 sr^–1 H-atom^–1, and (9.1 ± 0.3stat +1.5 –0.6sys) × 10^–27 photons s^–1 sr^–1 H-atom^–1 for the Chamaeleon, R CrA, and Cepheus and Polaris flare regions, respectively. Whereas the energy dependences of the emissivities agree well with that predicted from direct CR observations at the Earth, the measured emissivities from 250 MeV to 10 GeV indicate a variation of the CR density by ~20% in the neighborhood of the solar system, even if we consider systematic uncertainties. The molecular mass calibrating ratio, X CO = N(H2)/W CO, is found to be (0.96 ± 0.06stat +0.15 –0.12sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1, (0.99 ± 0.08stat +0.18 –0.10sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1, and (0.63 ± 0.02stat +0.09 –0.07sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1 for the Chamaeleon, R CrA, and Cepheus and Polaris flare regions, respectively, suggesting a variation of X CO in the vicinity of the solar system. From the obtained values of X CO, the masses of molecular gas traced by W CO in the Chamaeleon, R CrA, and Cepheus and Polaris flare regions are estimated to be ~5 × 10^3 M ☉, ~10^3 M ☉, and ~3.3 × 10^4 M ☉, respectively. A comparable amount of gas not traced well by standard H I and CO surveys is found in the regions investigated.

- Abstract
Designed as a high-sensitivity gamma-ray observatory, the Fermi Large Area Telescope is also an electron detector with a large acceptance exceeding 2 m;{2} sr at 300 GeV. Building on the gamma-ray analysis, we have developed an efficient electron detection strategy which provides sufficient background rejection for measurement of the steeply falling electron spectrum up to 1 TeV. Our high precision data show that the electron spectrum falls with energy as E-3.0 and does not exhibit prominent spectral features. Interpretations in terms of a conventional diffusive model as well as a potential local extra component are briefly discussed.

Background: With the widespread adoption of vendor-supplied electronic health record (EHR) systems, clinical decision support (CDS) customization efforts beyond those anticipated by the vendor may require the use of technologies external to the EHR such as web services. Pursuing such customizations, however, is not without risk. Validating the expected behavior of a customized CDS system in the high-volume, complex environment of the live EHR is a challenging problem., Objective: This article identifies technology failures that impacted clinical care related to web service-based advanced custom CDS systems embedded in the complex sociotechnical context of a production EHR., Methods: In an academic health system's primary care network, we performed an inventory of incidents between January 1, 2008 and December 31, 2016 related to a customized CDS system and performed a targeted review of changes in the CDS source code. Additional feedback on the root cause of individual incidents was obtained through interviews with members of the CDS project teams., Results: We identified five CDS malfunctions that impaired clinical workflow. The mechanisms for these failures are mapped to four characteristics of well-behaved applications: (1) system integrity; (2) data integrity; (3) reliability; and (4) scalability. Over the 9-year period, two malfunctions of the customized CDS significantly impaired clinical workflow for a total of 5 hours. Lesser impacts-loss of individual features with straightforward workarounds-arose from three malfunctions, which affected users on 53 days., Discussion: Advanced customization of EHRs for the purpose of CDS can present significant risks to clinical workflow., Conclusion: This case study highlights that advanced customization of CDS within a commercial EHR may support care for complex patient populations, but ongoing monitoring and support is required to ensure its safe use., Competing Interests: Drs. Fiks and Grundmeier are coinventors of the “Care Assistant” software that was evaluated in this study. Mr. Miller is a coinventor of “Project SHARE” which is a Care Assistant module to support the care of children with attention deficit disorder. - Abstract
In the standard model, the charged current of the weak interaction is governed by a unitary quark mixing matrix that also leads to CP violation. Measurement of the Cabibbo-Kobayashi-Maskawa (CKM) matrix elements is essential to searches for new physics, either through the structure of the CKM matrix, or a departure from unitarity. We determine the CKM matrix element |Vcb| using a sample of 3 × 106 BB̄ events in the CLEO detector at the ornell Electron Storage Ring. We determine the yield of reconstructed B̄0 →D*+ℓv̄ and B-→D*0ℓv̄ decays as a function of w, the boost of the D* in the B rest frame, and from this we obtain the differential decay rate dΓ/dw. By extrapolating dΓ/dw to w = 1, the kinematic end point at which the D* is at rest relative to the B, we extract the product |Vcb|F(1), where F(1) is the form factor at w = 1. We find |Vcb|F(1) = 0.0431±0.0013(stat) ±0.0018(syst). We combine |Vcb|F(1) with theoretical results for F(1) to determine |Vcb| = 0.0469±0.0014(stat)±0.0020)syst)±0.0018(theor). We also integrate the differential decay rate over w to obtain B(B̄0→D*+ℓv̄) = (6.09±0.19±0.40)% and B(B-→D*0ℓv̄) = (6.50±0.20±0.43)%.

- Abstract
The resonant substructure of the decay D0→KS0π+π- was analyzed using the Dalitz technique. Ten contributions were observed including a wrong sign D0→K*(892)+π- amplitude with a significance of 5.5 standard deviations.

- Abstract
The three-body decay Β→K0π+π- with a branching fraction (50-9+10±7)X10-6 was observed. A simultaneous analysis of the Dalitz plots for this decay and for Β→K+π-π0 revealed the presence of Β→K*+(892)π- with a branching fraction (16-5+6±2)X10-6, which is larger than but consistent with most theoretical predictions.

Using data collected by the CLEO detector in the Upsilon(4S) region, we report new measurements of the exclusive decays of B mesons into final states of the type Lambda_c^+ p-bar n(pi), where n=0,1,2,3. We find signals in modes with one, two and three pions and an upper limit for the two body decay Lambda_c^+ pbar. We also make the first measurements of exclusive decays of B mesons to Sigma_c p-bar n(pi), where n=0,1,2. We find signals in modes with one and two pions and an upper limit for the two body decay Sigma_c p-bar. The ratios of the branching fractions and the combined branching fraction ratio was measured. The decay rate measured in this work consisted with the lattice gauge calculations and ISGW2 model prediction. The ratio of the vector and pseudoscalar decay widths was consistent with the predictions of quark models.

- Abstract
The ratio of charged and neutral B meson production at the γ(4S), f +-/f 00, is measured through the decays B̄→D*ℓ -ν̄ ℓ, reconstructed using a partial reconstruction method where the D* is detected only through a pion daughter from the decay D*→Dπ. Using data collected by the CLEO II detector, the charged and neutral B decays are measured in such a way that their ratio is independent of the decay model, limited mainly by the uncertainty in the relative efficiency for detecting neutral and charged pions. This measurement yields the ratio of production fractions times the ratio of semileptonic branching fractions, f +-b +/f 00b 0. Assuming that b +/b 0 is equal to the lifetime ratio τ +/τ 0 and using the world average value of τ +/τ 0 as input, we obtain f +-/f 00= 1.058±0.084±0.136. © 2002 The American Physical Society.

High Energy Physics::Experiment - Abstract
In e(+)e(-) collisions using the CLEO detector, we have studied the decay of the D0 to the final state K(0)(S)pi(+)pi(-) with the initial flavor of the D0 tagged by the decay D(*+)-->D0pi(+). We use the Dalitz technique to measure the resonant substructure in this final state and clearly observe ten different contributions by fitting for their amplitudes and relative phases. We observe a K(*)(892)(+)pi(-) component which arises from doubly Cabibbo suppressed decays or D0-D0; mixing.

- Abstract
We report a new measurement of the Cabibbo-Kobayashi-Maskawa parameter Vub made with a sample of 9.7 x 10(6) BB- events collected with the CLEO II detector. Using heavy quark theory, we combine the observed yield of leptons from semileptonic B decay in the end-point momentum interval 2.2-2.6 GeV/c with recent CLEO II data on B-->X(s)gamma to find Vub = (4.08+/-0.34+/-0.44+/-0.16+/-0.24)x10(-3), where the first two uncertainties are experimental and the last two are from theory.

- Abstract
We describe several measurements using the decays D0→K+K- and π+π-. We find the ratio of partial widths, Γ(D0→K+K-)/Γ (D0→π+π-), to be 2.96±0.16±0.15, where the first error is statistical and the second is systematic. We observe no evidence for direct CP violation, obtaining ACP(KK)=(0.0±2.2±0.8)% and ACP(ππ)=(1.9±3.2±0.8)%. In the limit of no CP violation we measure the mixing parameter yCP=-0.012±0.025±0.014 by measuring the lifetime difference between D0→K+K- or π+ π- and the CP neutral state, D0→K-π+. We see no evidence for mixing.

- Abstract
Using data recorded by the CLEO II and CLEO II.V detector configurations at CESR, we report new measurements of the masses of the ∑c++ and ∑c0 charmed baryons, and the first measurements of their intrinsic widths. We find M(∑c++)-M(Λc+) =167.4±0.1±0.2 MeV, Γ(∑c++)=2.3±0.2±0.3 MeV, and M(∑c0) -M(Λc+)=167.2±0.1±0.2 MeV, Γ(∑c0)=2.5±0.2±0.3 MeV, where the uncertainties are statistical and systematic, respectively.

- Abstract
Continuing our studies of radiative Υ decays, we report on a search for Υ→γη and fj(2220) in 61.3 pb-1 of e+e- data taken with the CLEO II detector at the Cornell Electron Storage Ring. For the γη search the three decays of the η meson to π+ π- π0, π0 π0 π0, and γγ were investigated. We found no candidate events in the (3 π)0 modes and no significant excess over expected backgrounds in the γγ mode to set a limit on the branching fraction of B(Y→γη)< 2.1 × 10-5 at 90% C.L. The three charged two-body final states hh̄(h = π+, K+, p) were investigated for fj(2220) production, with one, one, and two events found, respectively. Limits at 90% C.L. of B(Υ→γfj) × B(fj→hh̄) ∼ 1.5 × 10-5 have been set for each of these modes. We compare our results to measurements of other radiative Υ decays, to measurements of radiative J/ψ decays, and to theoretical predictions.

- Abstract
The exclusive decays B̄ → D(*)K*-, using 9.66 × 106 BB̄ pairs collected at the γ(4S) with the CLEO detector was reported. Some branching fractions were measured such as β(B- → D0K*-) = (6.1 ± 1.6 ± 1.7) × 10-4. It was found that the B̄ → D*K*- branching ratios were averages of those corresponding to the 00 and 11 helicity states.

- Abstract
We have studied the color-suppressed hadronic decays of neutral B mesons into the final states D*0pi(0). Using 9.67 x 10(6) BB pairs collected with the CLEO detector, we observe the decays B( 0) --> D0pi(0) and B( 0) -->D(*0)pi(0) with the branching fractions BB( 0) -->D0pi(0)) = (2.74(+0.36)(-0.32) +/- 0.55)x10(-4) and BB( 0) -->D(*0)pi(0)) = (2.20(+0.59)(-0.52) +/- 0.79)x10(-4). The first error is statistical and the second systematic. The statistical significance of the D0pi(0) signal is 12.1sigma ( 5.9sigma for D(*0)pi(0)). Utilizing the B( 0) -->D*0)pi(0) branching fractions we determine the strong phases delta(I,D(*)) between isospin 1/2 and 3/2 amplitudes in the Dpi and D*pi final states to be cosdelta(I,D) = 0.89 +/-0.08 and cosdelta(I,D*) = 0.89 +/- 0.08, respectively.

- Abstract
The Ξc+ lifetime is measured using 9.0 fb-1 of e+e- annihilation data collected on and just below the Υ(4S) resonance with the CLEO II.V detector at CESR. This is the first measurement of the Ξc+ lifetime from a collider experiment. Using an unbinned maximum likelihood fit, the Ξc+ lifetime is measured to be 503 ±47(stat) ± 18(syst) fs. The precision of this measurement is comparable to previous measurements carried out by fixed target experiments with different sources of systematic uncertainties.

- Abstract
We present new measurements of branching fractions for the color-favored decays B + → D 0π - and B̄ 0 → D +π -. Using 9.67×10 6 BB̄ pairs collected with the CLEO detector, we obtain the branching fractions Β(B - → D 0π - = (49.7±1.2±2.9±2.2)×10 4 and Β(B̄ 0 → D +π -) = (26.8±1.2±2.4±1.2)×10 -4. The first error is statistical, the second is systematic, and the third is due to the experimental uncertainty on the production ratio of charged and neutral B mesons in Y(4S) decays. These results, together with the current world average for the color-suppressed branching fraction Β(B̄ + → D 0π 0), are used to determine the cosine of the strong phase difference δ I between the I = 1/2 and I=3/2 isospin amplitudes. We find cos δ I = 0.863 -0.023-0.035-0.030+0.024+0.036+0.038, and obtain a 90% confidence interval of 16.5° < δ I < 38.1°. This nonzero value of δ I suggests the presence of final state interactions in the D π system. © 2002 The American Physical Society.

- Abstract
We have searched for the charmless hadronic decay of B 0 mesons into two neutral pions. Using 9.13 fb -1 taken at the Y(45) with the CLEO detector, we obtain an improved upper limit for the branching fraction B(B 0 → π 0π 0)
