OBJECTIVE: Infection with the hepatitis C virus (HCV) becomes chronic in 85% of the infected individuals. We studied risk factors that may predict clearance of HCV. METHODS: A case-control study compared the association between risk factors and viral clearance. Viral clearance was defined as presence of a positive HCV antibody test plus negative HCV test by polymerase chain reaction (PCR). Forty-four cases and 214 controls with persistent viremia were identified in a database of patients evaluated at the Gastroenterology Clinic of the University of New Mexico. RESULTS: Of all 258 HCV-antibody-positive patients, 17% had a negative test by PCR. The multivariate logistic regression revealed that a history of parenteral exposure and a long time interval since the most recent exposure were both associated with an increased likelihood of persistent viremia, whereas subjects who had been monogamous for longer time periods were more likely to have cleared HCV from their serum. A low serum level of ferritin also conferred protection against persistent viremia. Case and control subjects did not differ with respect to their demographic characteristics, occurrence of comorbid disease, previous medical history, occurrence of sexually transmitted diseases, blood group, and risky health or sexual practices. The tick-borne encephalitis virus (TBEV) is the arboviral etiological agent of tick-borne encephalitis (TBE), considered to be one of the most important tick-borne viral diseases in Europe and Asia. In recent years, an increase in the incidence of TBE as well as an increasing geographical range of the disease have been noted. Despite the COVID-19 pandemic and the imposition of restrictions that it necessitated, the incidence of TBE is rising in more than half of the European countries analyzed in recent studies. The virus is transmitted between ticks, animals, and humans. It seems that ticks and small mammals play a role in maintaining TBEV in nature. The disease can also affect dogs, horses, cattle, and small ruminants. Humans are incidental hosts, infected through the bite of an infected tick or by the alimentary route, through the consumption of unpasteurized milk or milk products from TBEV-infected animals. TBEV infections in humans may be asymptomatic, but the symptoms can range from mild flu-like to severe neurological. In Europe, cases of TBE are reported every year. While there is currently no effective treatment for TBE, immunization and protection against tick bites are critical in preventing this disease. [ABSTRACT FROM AUTHOR]

Across communities worldwide, various new psychoactive substances (NPSs) continue to emerge, which worsens the challenges to global mental health, drug rules, and public health risks, as well as combats their usage. Specifically, the vast number of NPSs that are currently available, coupled with the rate at which new ones emerge worldwide, increasingly challenges both forensic and clinical testing strategies. The well-established NPS detection techniques include immunoassays, colorimetric tests, mass spectrometric techniques, chromatographic techniques, and hyphenated types. Nonetheless, mitigating drug abuse and NPS usage is achievable through extensive community-based initiatives, with increased focus on harm reduction. Clinically validated and reliable testing of NPS from human samples, along with community-driven solution, such as harm reduction, will be of great importance, especially in combating their prevalence and the use of other illicit synthetic substances. There is a need for continued literature synthesis to reiterate the importance of NPS, given the continuous emergence of illicit substances in the recent years. All these are discussed in this overview, as we performed another look into NPS, from differentiating the major groups and identifying with laboratory testing challenges to community-based initiatives. For the first time, Cleisomeria lanatum (family: Orchidaceae) has been investigated for its phytochemical, antioxidant, anti-inflammatory, and thrombolytic properties. All phytochemicals studied were identified in varying degrees during qualitative screening. In quantitative screening, a maximum of 106.02 ± 0.08 mg/g alkaloids (root), 179.67 ± 8.83 mg/g phenols (stem), 17.34 ± 0.88 mg/g flavonoids (stem), 73.67 ± 1.76 mg/g tannins (stem), and 180.04 ± 0.02 μg/mL proteins (root) were detected. Antioxidant, anti-inflammatory, and thrombolytic properties were investigated using a free-radical scavenging assay, heat-induced albumin denaturation assay, and blood clotting inhibition assay. The root extracts had the most effective antioxidant (IC50 = 67.98 μg/mL) and anti-inflammatory (IC50 = 60.86 μg/mL) properties, whereas the stem extracts had the most effective thrombolytic property (IC50 = 163.8 μg/mL). In the aftermath of Hurricane Harvey in 2017, thousands of residents in the Houston area sought refuge at a convention center "megashelter". Out of concern for the possibility of communicable diseases spreading rapidly in the crowded shelter, we conducted syndromic surveillance to monitor the health of evacuees using a digital drop-in cot-survey. The cot-to-cot survey design rapidly assessed evacuees to determine if they were experiencing any symptoms of illness each night from 1–7 September 2017. While no outbreak of a specific infection was identified during the surveillance period, runny nose, congestion, cough, achy muscles and joints, anxiety, and depression were the most commonly reported symptoms. Out of the total shelter population, 38% of adults reported symptoms compared to 25% of children (≤18 years). The cot survey took a median of 5.2 min per interview, and the daily participation rate increased throughout the surveillance period starting at 89% and ending at 96% on the last day. The success of this public health response was due, in part, to the effectiveness of survey design and the dissemination of real-time data to the health departments. Prisoners in most countries have a higher prevalence of HCV than the general population, but their access to treatment is very limited. Our aim was to evaluate a pilot programme using the ECHO model to enhance linkage to care in patients with HCV in 3 Argentinean prisons between October 2018 and January 2020. All inmates were invited to participate, and data were collected through a personal interview. We then estimated HCV prevalence with dried blood spot and performed a logistic regression analysis to identify risk behaviours associated with HCV infection. Finally, HCV management was assessed and monitored through ECHO. Overall, 1141 inmates agreed to participate, representing 39.7% of the total prison population. Anti‐HCV prevalence was estimated at 1.58% (CI 0.93; 2.48), being significantly higher in women 2.98% (CI 1.4;5.6) than in men 1.07% (CI 0.5; 2.0); P =.03. Patients with anti‐HCV were significantly older than those who tested negative, 42.3 years (CI 37.6;47.1) vs 30.1 years (CI 30.6;31.2), P <.001, respectively. Multiple logistic regression analysis, identified age OR 1.07 (CI 1.03;1.12, P =.001), history of sexually transmitted disease OR 3.08 (CI 0.97;9.82, P =.057) and intravenous drug use OR 12.6 (CI 3.31;48.53, P <.001) as risk factors associated with anti‐HCV. Treatment was initiated in all the patients with specialist physician support utilizing ECHO model. In conclusion, our pilot study reported a low prevalence of anti‐HCV in the studied population. Purpose: Factors influencing fallopian tube occlusion in women with a lower genital tract infection remain incompletely elucidated. We evaluated whether a polymorphism in the mannose-binding lectin (MBL) gene at codon 54 influences the occurrence of fallopian tube blockage in relation to exposure to Chlamydia trachomatis.Methods: In a case-control study at The Hospital das Clínicas, University of São Paulo, Brazil, 75 women with hysterosalpingography-documented tubal occlusion and 75 women with patent fallopian tubes were analyzed for detection of single-nucleotide polymorphism in codon 54 of the MBL gene and for IgG anti-C. trachomatis antibodies in their sera. Both groups were matched for age, race, and sexual variables.Results: Prior exposure to C. trachomatis, as evidenced by the presence of IgG antibodies, was comparable in both groups. Detection of the polymorphic MBL allele was more prevalent in women with blocked tubes (p < 0.01), regardless of whether or not there was evidence of prior chlamydial exposure.Conclusion: The level of MBL-related innate immunity influences the consequences of infection by C. trachomatis or other microbes. Aim: The present meta-analysis aims to assess the epidemiology of human T-lymphotropic virus type 1 (HTLV-1) among blood donors and general population in Iran. Methods: The present study was designed according to PRISMA guidelines. Results: The prevalence of HTLV-1 in 34 studies with a sample size of 3,626,364 blood donors was 0.20% (95% CI: 0.15–0.26) and the prevalence of this virus among men and women blood donors was 0.20% (95% CI: 0.15–0.27) and 0.64% (95% CI: 0.13–3.01), respectively and the female-male odds ratio was 3.13 (1.35–7.25, p = 0.008). In five studies consisting of 4763 individuals, the prevalence of HTLV-1 among the general population who lived in the main HTLV-1 endemic regions (Khorasan provinces, in northeastern Iran) was 2.5% (95% CI: 1.3–4.7) and the prevalence of this virus among men and women was 2.4% (95% CI: 0.8–7.1) and 3.4% (95% CI: 2.0–5.7), respectively. According to the systematic review, the prevalence of HTLV-1 in general population living in non-endemic regions of HTLV-1 was reported to be 0.07% to 1.8%. Conclusion: Considering the high prevalence of HTLV-1 in the main HTLV-1 endemic regions of Iran, persistent prevention programs are required in blood transfusion centers in these regions. A large proportion of patients with substance use disorders have clinical comorbidities, either medical or psychiatric. An initial assessment is necessary initially for prompt identification and management of any psychiatric or medical emergency, and thereafter a more detailed assessment for the comprehensive understanding of the individual. This should be done keeping in mind the goals of both immediate and long term assessment so that a comprehensive but individualized, context and culture sensitive, reality based, recovery-oriented management plan can be formulated. Assessment should consist of not only history-taking, physical and mental status examination but also laboratory and instrument based assessment as needed. During assessment, collateral reports and past medical records are valuable additions along with self-report. Since substance use disorders influence various aspects of daily life, hence medical, social, occupational, religious, spiritual, financial and legal aspects should be evaluated. Background: Human T-lymphotropic virus type 1 (HTLV-1) has worldwide distribution and is considered endemic in many world regions, including southwestern Japan and Brazil. Japanese immigrants and their descendants have a high risk of acquiring this infection due to intense population exchange between Brazil and Japan. Objective: This cross-sectional study aimed to estimate the prevalence of HTLV, analyze the main risk factors associated with this infection, identify the main circulating types and subtypes of HTLV in Japanese immigrants and descendants living in Campo Grande-MS (Middle-West Brazil), as well as analyze the phylogenetic relationship among isolates of HTLV. Study Design: A total of 219 individuals were interviewed and submitted to blood collection. All collected blood samples were submitted for detection of anti-HTLV-1/2 using the immunoassay ELISA and confirmed by immunoblot method. The proviral DNA of the 14 samples HTLV- 1 positive were genotyped by nucleotide sequencing. Results: The overall prevalence of HTLV-1 was 6.8% (IC 95%: 3,5-10,2). Descriptive analysis of behavioral risk factors showed statistical association between HTLV-1 and age greater than or equal to 45 years. The proviral DNA of HTLV-1 was detected in all HTLV-1 positive samples. Of these, 14 were sequenced and classified as Cosmopolitan subtype, and 50% (7/14) belonged to subgroup A (transcontinental) and 50% (7/14) to the subgroup B (Japanese). Conclusion: The high prevalence of HTLV-1 found evidence of the importance of early diagnosis and counseling of individuals infected with HTLV-1 for the control and prevention of the spread of this infection among Japanese immigrants and their descendants in Central Brazil. Author Summary: The population of Okinawan immigrants is considered vulnerable to human T-lymphotropic virus type 1 (HTLV-1) infection because the Okinawa region in Japan is an endemic area. The second Brazilian largest Okinawan community is set in Campo Grande, Middle-West Brazil. This study aimed to estimate the prevalence and risk factors associated with HTLV infection among Japanese immigrants and their descendants living in Campo Grande. The prevalence of 6.8% of HTLV-1 infection that was found is considered high. The HTLV-1 infection was associated with age ranged from 45 years old or older. Most infected individuals are Okinawan descendants. The HTLV-1 rate found in the present study indicates that the prevalence of this infection remains high among this Japanese community. This study emphasizes the importance of implementing preventive and diagnostic public health policies to decrease the risk of HTLV-1 transmission among Japanese communities throughout Brazil. Despite recent innovations in alternative reproduction technology and the increased use of artificial insemination procedures, courts and legislatures have been unable to develop a clear and consistent test to establish the parental rights and obligations of sperm donors. As a result, there are mixed outcomes in cases where intended parents seek child support from an unsuspecting donor or when donors petition the court for visitation with their biological children. This Note seeks to resolve the ambiguity in determining sperm donors' parental status by proposing a model state statute that makes nonpaternity the default rule. Under the statute, sperm donors would not be subject to any of the parental rights or obligations of a traditional biological father. However, the presumption of nonpaternity could be overcome if the parties agree, in writing, prior to the insemination. Further, the model statute provides an exception to the default rule if the donor has played an active role in the child's life. Adopting this model statute will not only facilitate a market for sperm donation but also make donor rights and obligations clear from the onset. However, the law regarding known donors is less straightforward. By comparing factually similar cases, in which courts interpreted donor statutes with identical language, in completely opposite ways, it is easy to see the unpredictability in ART cases., The proposed model statute provides unambiguous legislation that sets out a clear standard to be used in determining the parental status of known donors. If adopted by state legislatures, courts across the country would finally have a consistent rule to apply, leading to less confusion and contradictory rulings., The key issue is honoring intentional parentage and the proactive choice to use ART to have a child on one's own terms. Health officials and AIDS activists nationwide are concerned with the increasing number of newly diagnosed HIV cases and methamphetamine use among MSM (men who have sex with men). Methamphetamine is a powerful stimulant that heightens sexual arousal, aggressive responses, and erases inhibitions, which can lead to indiscriminate sexual behavior, HIV infection, STDs, and Hepatitis A, B, C, and D. Health officials concede that methamphetamine use is not the sole factor in newly diagnosed HIV cases but acknowledge it is a dangerous risk factor in the transmission of HIV/AIDS and other blood pathogens in the MSM community. Non-injecting drug users are at high-risk for acquiring hepatitis B virus (HBV), although the factors contributing to this increased risk are not known. In the present study, the overall and occult HBV infection prevalence rates were determined in a large population of non-injecting drug users in the Central-West region of Brazil. HBV genotypes and predictors of infection were also identified. A total of 852 individuals in 34 drug treatment centers were interviewed, and their serum samples were tested for the presence of HBV markers by ELISA. HBsAg and anti-HBc-positive samples were tested for HBV DNA by PCR. Samples with HBV DNA were genotyped by restriction fragment length polymorphism (RFLP). The overall prevalence of HBV infection was 14% (95% CI: 11.7-16.5). A multivariate analysis of risk factors showed that age >30 years, non-white race/ethnicity, duration of drug use >10 years, lifetime number of sexual partners >10, non-use of condoms, and HCV and HIV status were associated significantly with HBV infection. Of the 9 (1%) HBsAg-reactive samples, HBV DNA was present in 2/2 of HBeAg-positive and in 5/7 anti-HBe-positive samples. An occult HBV infection rate of 2.7% (3/110) was found among anti-HBc-positive individuals. All HBV DNA-positive samples were genotyped: seven were genotype A, two were genotype D, and one was genotype F. Finally, few individuals (8%) had serological evidence of a previous HBV vaccination. These findings indicate that preventive interventions are needed for both sexual and drug-related high-risk behavior. Additionally, non-injecting drug users should be targeted for HBV vaccination. J. To determine differences in health-care costs associated with moderate alcohol consumption among female health-care workers while controlling for other risk factors that may be correlated with alcohol use. Non-randomized, prospective, observational study of health-care costs by female health-care workers in a large managed care organization recruited between 1 January 1998 and 1 July 2000. Six hundred and eighty-five female employees, continuously and stably employed by the managed care organization, who received health-care through the affiliated managed care organization. All women completed a health risk appraisal as part of the company's Employee Wellness Plan; 218 women were categorized as moderate drinkers and 467 as abstainers/light drinkers. Total costs of in-plan and out-of-plan health-care utilization, by type of service, during the 6 month period after completing the health risk survey were calculated. Using 218 one-to-one matched pairs of moderate drinkers and abstainers/light drinkers, no significant differences in total, outpatient or inpatient costs were observed during the 6 month observation period. Pharmacy costs were significantly lower for moderate drinkers (–$43, 95% CI = –$88.82 to –$2.41), primarily due to differences in costs from anxiolytic (including barbiturates and benzodiazepines), hypnotic and sedative drug fills. Findings demonstrate the value of risk factor matching when studying the relationship between alcohol use and health-care utilization. The discovery of differential pharmacy utilization raises the possibility that alcohol consumption may reduce the use of prescribed central nervous system depressants. [ABSTRACT FROM AUTHOR]

Behavioural risk factors for HIV/AIDS in Bangladesh were reviewed in a preceding article in this journal. Omitted from that review was a discussion of potential biomedical risk factors including: (i) an unregulated blood supply system in which blood used in transfusions is not screened for HIV and is donated primarily by professional donors: (ii) unsterile injections in non-formal and formal health-care settings; and (iii) a high prevalence in high-risk groups of other sexually transmitted diseases (STDs) which may function as co-factors for HIV transmission, particularly if chronically untreated. Studies elsewhere in the world suggest that the unregulated blood supply system, in particular, poses a serious danger in terms of the spread of the HIV epidemic. While certain socio-cultural factors may be contributing to low levels of HIV in Bangladesh, the prevalence of biomedical and behavioural risk factors suggest the importance of implementing targeted cost-effective interventions now. Aerobic exercise training is an important therapy to offer individuals who are HIV positive (HIV). Six to 12 weeks of moderate exercise sessions (3 times per week for 1 hour) significantly improves aerobic capacity (O and lactic acidosis threshold), apparently without detrimental effects on the immune system. In addition, small but significant beneficial effects on the immune system (skin test reactivity) and quality of life are obtained. The effects on lean body mass with aerobic exercise training have been less marked, but are clearly less costly than androgenic hormone administration in this population. To explore the biological similarities and differences between the HIV-1 and HIV-2 viruses, we model the probability of male-to-female transmission of either HIV virus as a function of the number of sexual partners, the prevalence of the viruses and the infectivity per contact. Using maximum likelihood estimation theory and data from a prospective study of registered female prostitutes in Dakar, Senegal, we estimate and compare the infectivities of HIV-1 and HIV-2. Graphical goodness-of-fit methods are used to show that our model fits the data well. We find that in male-to-female transmission HIV-1 is significantly more infectious than HIV-2. This findings is consistent with other data from laboratory and epidemiologic studies comparing the biology of HIV-1 and HIV-2. [ABSTRACT FROM AUTHOR]

A healthy individual at risk for human immunodeficiency virus type 1 (HIV-1) infection was studied longitudinally for 19 months for evidence of exposure to and infection with HIV. Peripheral blood mononuclear cells (PBMC) were tested at 1,5,13,16, and 19 months for T helper (Th) cell function by in vitro-generated proliferation and interleukin-2 production in response to four synthetic peptides corresponding to env of HIV. PBMC were also tested for proviral DNA using gag primers by the polymerase chain reaction (PCR) at 0, 5, 10, 13, 16, and 19 months. HIV serology by ELISA and p24 antigen and Western blot analyses were done on samples taken at 0, 1, 5, 10, 13, 16, and 19 months. The results indicated that both Th cell tests were positive during months 5-19. In contrast, the PCR and all serologic assays were negative except for month 19, when the assays became positive. These results, based on the longitudinal study of one individual, showed that the Th cell tests can reveal exposure to HIV-1 antigens several months before evidence of viral infection is detected, even by PCR.
