Abstract

Glucosamine and chondroitin sulphate are used to treat osteoarthritis, and the multicenter, double-blind, placebo- and celecoxib-controlled Glucosamine/chondroitin Arthiritis Intervention Trial (GAIT) evaluated their efficacy and safety as a treatment for knee pain from osteoarthritis. Exploratory analyzes suggest that the combination of glucosamine and chondroitin sulphate may be effective in the subgroup of patients with moderate-to-severe knee pain.

Abstract

The most common crystal-related arthropathies--gout, calcium pyrophosphate dihydrate disease or 'pseudogout,' and calcific periarthritis/tendinitis--may be appropriately diagnosed and managed by the primary care physician. Definitive diagnosis via synovial tap is recommended, as the clinical picture may not identify some cases. The acute pain and swelling of attacks, regardless of etiology, generally respond to treatment with nonsteroidal anti-inflammatory drugs and local or occasionally systemic corticosteroids. Once a causative crystal has been identified and a diagnosis established, a plan for long-term management and prevention of recurrences may be devised. Thus, uric-acid-lowering therapy may be indicated in a patient who has experienced recurrent attacks of gout, whereas control of serum phosphate levels might be effective in some individuals with hyperphosphatemia and hydroxyapatite-associated periarthritis or arthritis. Crystal deposits in joints can be destructive as well as painful. Treatment, therefore, has two objectives: To relieve the pain of the acute attack, thus restoring normal function, and to prevent the accumulation of crystals that can lead to degenerative disease. Identification and subsequent treatment of preventable or correctable underlying disorders may be one of the most gratifying aspects of managing crystal-induced arthropathies.

Abstract

Ehlers-Danlos syndrome (EDS) is an inherited disorder of connective tissue (skin, bone, cartilage, and similar structural tissues) that involves mutations of collagen (the body's major structural protein) and features joint hypermobility, skin hyperelasticity and fragility, and a variety of other problems. One rare and unusual type of EDS, type VIII EDS, is characterized by gum disease and premature loss of teeth. A case of EDS with these features is described; the patient also had bone problems, vasculitis (inflammation of blood vessels), and heart valve disease. The patient developed more or less normally until puberty, except for premature loosening of permanent teeth. Soon after the start of menstruation, widespread hives with underlying small-vessel vasculitis developed. Arthritis in multiple joints then developed, and steroids failed to heal the skin problems. Vasculitis remained confined to the skin, however. Over a 10-year period, since age 15, joint dislocations, erosive arthritis, and severe osteolysis (bone destruction) occurred, as have classic signs involving skin and joints. Insufficiency of several heart valves occurred, with hypertrophy (overgrowth) of heart chambers slowly developing. Modern laboratory techniques were used to analyze the patient's collagen. Two subtypes of type I collagen were present in both a normal and abnormal form, and the abnormal type apparently could not be secreted efficiently by cells for use in structural tissues. The patient had T lymphocytes (immune cells important in fighting foreign organisms) which reacted with the type I collagen; collagen is usually inert to the immune system and a protein made by the T cells which reacted with the collagen was detected. Abstract

Septic arthritis is an arthritic condition that arises from infection of synovial (joint) tissues by bacteria or other agents. A case is described of a 49-year-old man who had a condition that mimicked septic arthritis, but was caused by neuropathic arthropathy, a joint disorder in which nerve damage and loss of sensation lead to overuse and repetitive joint injury. The patient had syringomyelia, a fluid-filled cavity within the spinal cord, which contributed to tingling and numbness in the hand, numbness and weakness in the arm, and muscle atrophy. Neuropathic arthropathy occurs in up to 30 percent of patients with syringomyelia, most often involving the shoulder. Diagnosis of the joint problem was delayed by seven months, as the shoulder swelling, although massive, was painless. Many details of this patient's case are typical. Severe destruction of the shoulder developed rapidly, within one month. Neuropathic arthropathy involving the shoulder is thought to involve nerve loss, which then leads to decreased blood flow to bone, and bone resorption (loss of calcium and bone structure). The case is unusual, however, regarding the high level of inflammation and high numbers of white blood cells that were present in the synovial fluid. Bacterial infections were ruled out. This type of inflammatory phase may be a more common feature of early neuropathic arthropathy than realized. Alternatively, it might have been provoked by high synovial fluid levels of fat particles, possibly generated from red blood cells or the breakdown of bone marrow. 11. Effect of medication on synovial fluid leukocyte differentials in patients with rheumatoid arthritis

Abstract

Levels of leukocytes (white blood cells) in synovial (joint) fluid increase in most types of arthritis, and the type of cell can help to determine the extent of inflammatory disease that may be present. Numbers of leukocytes can vary, from an average of 15,000 per cubic millimeter of fluid (mm3) to as high as 100,000/mm3 in some cases of rheumatoid arthritis (RA). In addition, the type of leukocyte is important; higher levels of polymorphonuclear neutrophils (PMNs) are often found in cases of severe acute arthritis. The differences in levels of leukocytes and particularly in levels of lymphocytes (immune cells important in defending against infection and which may mediate tissue destruction in arthritis) were studied in relation to medication taken by 45 patients with RA. The patients had detectable joint effusions (extrusion of fluid, suggestive of continued arthritis in spite of medication), necessitating fluid removal, so that synovial fluid was thus available for analysis. Of 22 patients taking nonsteroidal anti-inflammatory drugs (NSAIDs, which decrease inflammation but do not affect the underlying disease process), 17 had a predominance of lymphocytes in synovial fluid. The proportion and total levels of lymphocytes were significantly lower in patients taking a second-line (disease-modifying) drug such as methotrexate (MTX), gold, or hydroxychloroquine (HCQ). Total leukocyte counts were significantly higher in patients taking MTX or gold than those taking HCQ plus NSAIDs. Among these latter patients, five had taken only NSAIDs at one point and had predominantly lymphocytes in synovial fluid. Abstract

An overview is presented of the types of foreign matter that can cause inflammation within joints together with their diagnosis, how they cause disease, and treatments. Crystals may be deposited within the joint, the most common of which is urate crystals, the cause of gout. Treatment is available to dissolve urate crystals and the crystal deposits that sometimes occur with kidney dialysis but not other types of crystals. Joint inflammation resulting from infection may be difficult to diagnose with without biopsy of the joint membrane and specialized laboratory techniques. The most common entry is via the circulation. Pathogens may cause inflammation by infecting cells, producing toxins, or triggering an immune response. Antibiotics and drainage usually effect cure, although some pathogens are recalcitrant. Artificial joints can introduce foreign bodies through wear. Materials should be used that do not create this problem.

Abstract

Yoga stretching and relaxation exercises may effectively relieve symptoms of carpal tunnel syndrome. Repetitive motion in the hands and wrist can cause chronic pain. Researchers assigned 42 patients to attend twice-weekly sessions of Iyengar hatha yoga, or wear a wrist splint. The yoga exercises stretched and flexed the joints in the upper body, followed by a period of relaxation. After eight weeks, yoga participants had improved grip strength, and less wrist pain. Patients who simply wore a splint showed no improvement in their symptoms of carpal tunnel syndrome.
