Abstract

Laparotomy incisions provide easy and rapid access to the peritoneal cavity in case of emergency surgery. Incisional hernia (IH) is a late manifestation of the failure of abdominal wall closure and represents frequent complication of any abdominal incision: IHs can cause pain and discomfort to the patients but also clinical serious sequelae like bowel obstruction, incarceration, strangulation, and necessity of reoperation. Previous guidelines and indications in the literature consider elective settings and evidence about laparotomy closure in emergency settings is lacking. This paper aims to present the World Society of Emergency Surgery (WSES) project called ECLAPTE (Effective Closure of LAParoTomy in Emergency): the final manuscript includes guidelines on the closure of emergency laparotomy.

5. Arteriovenous malformations of the colon: A report of two cases and review of the literature.

Abstract

Background: Arteriovenous malformations are one of the most common vascular disorders of the colon. Vascular disorders present as painless, high-volume rectal bleeding., Case Presentation: This study elucidates two rare cases of vascular disorders that are diagnosed as angiodysplasia of the left colon and cavernous hemangioma of the colon and rectum. The chief complaint in two patients was rectorrhagia. The patients who were diagnosed of ulcerative colitis were treated with sulfadiazine and prednisone. Due to continuous bleeding, the patients were referred to the surgery department for operation. The patients underwent total proctocolectomy., Conclusion: We discuss the faults in the diagnosis and management of vascular disorders of the intestine., Competing Interests: None declared.

6. Post tonsillectomy pain: can honey reduce the analgesic requirements?

Abstract

Background: Tonsillectomy with or without adenoidectomy is one of the most common surgical procedures performed worldwide, especially for children. Oral honey administration following tonsillectomy in pediatric cases may reduce the need for analgesics via relieving postoperative pain., Objectives: The aim of this study was to evaluate the effects of honey on the incidence and severity of postoperative pain in patients undergoing tonsillectomy., Patients and Methods: A randomized, double blind, placebo controlled study was performed. One hundred and four patients, who were older than eight, and were scheduled for tonsillectomy, were divided into two equal groups, honey and placebo. Standardized general anesthesia, and postoperative usual analgesic, and antibiotic regimen were administrated for all patients. Acetaminophen plus honey for the honey group, and acetaminophen plus placebo for the placebo group were given daily. They began to receive honey or placebo when the patients established oral intake., Results: The difference between acetaminophen and acetaminophen plus honey groups was statistically significant both for visual analogue scale (VAS), and number of painkillers taken within the first three postoperative days. The consumption of painkillers differed significantly in every five postoperative days. No significant difference was found between groups regarding the number of awaking at night., Conclusions: Postoperative honey administration reduces postoperative pain and analgesic requirements in patients after tonsillectomy. As the side effects of honey appear to be negligible, consideration of its routine usage seems to be beneficial along with routine analgesics.

Abstract

Tonsillectomy is one of the most common surgical procedures in childhood. While generally safe, it often is associated with a difficult early recovery phase with poor oral intake, dehydration, difficult or painful swallowing, postoperative bleeding, infection and/or otalgia. Better pain management and the availability of more child friendly medications are within the top consumer priorities in perioperative medicine, highlighting the importance of alternative pain treatments. This review focuses on the potential role of honey in the postoperative setting, its effects, and mechanisms of action. While the application of honey post-tonsillectomy may offer analgesic and healing benefits, it may also reduce postoperative bleeding. A systematic search was carried out using the search terms honey, tonsillectomy. Filters were applied to human studies and English. No other search terms were used or age filters applied to yield a broader range of results. Seven pediatric, four adult, and two studies of mixed pediatric and adult patients with sample sizes ranging from 8 to 52 patients were included in this review. Effect sizes ranged from small to huge across the studies. While the application of honey post-tonsillectomy may offer analgesic and healing benefits, it may also reduce postoperative bleeding. However, while there are potential benefits based on the chemical composition of honey, the current literature is of variable quality and there is need for high quality clinical trials., (© 2024 The Author(s). Abstract

Laparotomy incisions provide easy and rapid access to the peritoneal cavity in case of emergency surgery. Incisional hernia (IH) is a late manifestation of the failure of abdominal wall closure and represents frequent complication of any abdominal incision: IHs can cause pain and discomfort to the patients but also clinical serious sequelae like bowel obstruction, incarceration, strangulation, and necessity of reoperation. Previous guidelines and indications in the literature consider elective settings and evidence about laparotomy closure in emergency settings is lacking. This paper aims to present the World Society of Emergency Surgery (WSES) project called ECLAPTE (Effective Closure of LAParoTomy in Emergency): the final manuscript includes guidelines on the closure of emergency laparotomy.

Abstract

Tonsillectomy is the most common surgical procedure performed since many years. Various methods of tonsillectomy (Electrocautery, LASER, Harmonic scalpel, Coblation) have been practiced over the last century aiming for reducing intra and postoperative morbidity. We carried out prospective randomized control study using retrospective data as secondary data to compare blood loss, excision and hemostasis time, pain, recovery time and complication between CO2 LASER and Electrocautery assisted with conventional method. Average time for surgery with CO2 LASER (82.17 min) was more than conventional (77.77 min) and electrocautery (21.40 min) was least. The postoperative pain and recovery time was more in CO2 LASER and Electrocautery group than conventional. The blood loss was more in conventional group compared to other two group. We concluded that CO2 LASER group has longest operative time but least blood loss. Postoperative pain was more and lasted longer in Electrocautery group. Abstract

Introduction: Many ongoing challenges have been applied to reduce the considerable postoperative pain and increase wound healing after tonsillectomy, but they are still not optimally managed. This study applied autologous platelet-rich plasma (PRP) & platelet-rich fibrin glue (PRFG) to reduce pain and increase wound healing. Materials and Methods: PRP & PRFG were prepared from 26 patients' blood. At the end of the tonsillectomy, one tonsillar bed was selected randomly, PRP was injected, PRFG was applied topically on the bed wound, and the other sites were left untreated. The treated and untreated tonsillar beds were compared for pain and wound healing the next day, 3rd day, 6th day, 9th day, and 15th day. Results: There were no complications during and after the injection. The mean age was 24.76 ±5.54 years. In the treated beds in comparison to untreated beds, pain decreased marginally in 1st day (intervention:4.5±2.54, control:5.53±2.94, P-value=0.18) and 3rd day (intervention:3.92±2.96, control:4.8±2.82, P-value=0.276), and significantly in 6th day (intervention:2.3±2.46, control:3.92±2.6, P-value=0.026), 9th day (intervention:1.26±1.48, control:2.76±2.4, P-value=0.009) and 15th day (intervention:0.73±1.07, control:1.84±2.36, P-value=0.08) after surgery. Healing did not change in 1st day (P-value=1), changed marginally in 3rd day (P-value=0.2), and increased significantly in 6th day (P-value=0.001), 9th day (P-value=0.006), and 15th day (P-value=0.004) after surgery. Conclusions: Autologous PRP injection & PRFG application offer an effective, safe, and non-invasive method for reducing pain and increasing wound healing after tonsillectomy. [ABSTRACT FROM AUTHOR]

Abstract

BACKGROUND & OBJECTIVE: Palatine tonsils are lymphoid organs in the oropharynx part of the waldeyer's ring and serve as host defence organs with maximum immunological activity from 3-10 years. Tonsillectomy is associated with considerable postoperative pain and requires significant consumption of analgesia. This study aimed to determine effect of topically suffused 0.5% bupivacaine on postoperative tonsillectomy pain. METHODOLOGY: This double-blind interventional experiment was done on 120 patients 5-15 years of age at the ENT Department, Shaikh Zayed Hospital, Lahore, Pakistan from February-August 2019. Tonsillectomies were performed by conventional blunt dissection method and tonsillar fossae were packed with 0.5% bupivacaine (Group-A) and 0.9% normal saline (Group-B) for 5minutes. Postoperative pain scores were recorded by Faces Pain Scale-Revised 24hours postoperatively. Routine postoperative analgesia (diclofenac-sodium 3mg/kg/day) as intramuscular-injection was given immediately after surgery to all patients. A patient who would have difficulty in communicating his/her pain levels, with history of bleeding disorders or INR of ≥1.5, who suffered fever, sore-throat (in last 4-weeks) or underwent tonsillectomy after quinsy and patients allergic to bupivacaine on history or medical record were excluded. RESULTS: One hundred & twenty (120) patients with 61 males and 60-patients in each group with age range of 5-15 years (Group-A=8.63±2.63 and Group-B=8.32±6.60 in). Mean BMI of Group-A and group-B was 15.18±1.60 and 14.96±1.34 respectively (p=0.417). Mean pain score in group-A and group-B was 2.63±0.938 and 6.07±0.800 respectively (p<0.001). CONCLUSION: Tonsillectomies performed using bupivacaine-soaked packs postoperatively are associated with lower postoperative pain compared with those packed with normal saline. Abstract

Arteriovenous malformation is a high flow congenital vascular anomaly that can occur anywhere on the body. It is usually congenital but can occasionally occur after trauma. Arteriovenous malformations are less common in the omentum, and to the best of our knowledge, this is the first report in English to describe a case of arteriovenous malformation originating from the omentum. A 5-year-old girl presented with the complaint of lower abdominal pain. Abdominal ultrasound showed increased pericecal echogenicity and free peritoneal fluid, and the appendix was not visible. A lesion compatible with a 40 × 40 mm bleeding cyst was detected in the greater omentum. Since the patient had acute abdomen, appendectomy and omentum cyst excision were performed with conventional laparotomy. The patient was diagnosed with arteriovenous hemangioma as a result of pathological and histochemical examination. Arteriovenous malformation originating from the omentum is extremely rare, and this is the first report in worldwide literature of arteriovenous malformation phenomenon in the omentum. 14. Effect of Intrasocket Application of Manuka Honey on Postsurgical Pain of Impacted Mandibular Third Molars Surgery: Split-Mouth Randomized Controlled Trial.

Background and Purpose: Sufficient evidence exists recommending the use of honey in the management of wounds. Studies revealed that the healing effect of honey could be classified by its antibacterial and anti-inflammatory properties of its components. Since surgical extraction of impacted molars is one of the most common operations in the oral cavity and the postoperative pain disturbing the patient may reduce the quality of health service, this study aimed to assess the analgesic potential of Manuka honey application into the extraction socket of impacted mandibular third molars. Methods: This randomized split-mouth controlled study included 33 patients undergoing impacted bilateral lower third molars surgery under local anesthesia (n = 66). Randomization was carried out by coin flipping. One of the two impacted third molars was assigned to treatment group (Manuka honey applied just before suturing), other side to control group (nothing applied). Postsurgical pain was evaluated using visual analogue scale (VAS) of faces 7 days after extraction. The total analgesic dose used was also evaluated.Results: In treatment group, postoperative VAS scores were significantly lower compared to that in control group regarding first and second days postoperatively (P < 0.05). Total analgesic intake in the control group was significantly higher (P = 0.0001).Conclusion: This study demonstrated that intrasocket application of Manuka honey after surgical extraction of impacted lower third molar is an effective method for reducing acute postsurgical pain. 15. Treating pediatric post-tonsillectomy pain and nausea with complementary and alternative medicine.

Abstract

Objectives: Although tonsillectomy is a common and largely safe procedure, pain management in children remains a controversial topic. In addition to the challenge of choosing appropriate analgesia, there is often low parent and child adherence. This article presents a review, and evaluates the potential role, of a range of complementary and alternative therapies that may be sought out by parents.Methods: A literature review of complementary and alternative interventions performed using PubMed, Cochrane Library, and EMBASE, supplemented by searches from Google and hand searches of cross-references of selected articles, yielded 32 studies for qualitative analysis.Results: The studies included for analysis investigated a wide variety of alternative treatment modalities: acupuncture and related therapies, aromatherapy, homeopathy, honey, intravenous fluid, speech therapy, hyaluronic acid, behavioral therapies, ice/cold, hydrogen peroxide rinse, and chewing gum.Conclusion: At this time, stronger conclusions cannot be made about the therapies investigated because there are many methodology limitations of the studies analyzed. However, our results suggest merit for these treatments as adjuvant therapies that can enhance analgesia and decrease requirements of controversial medications. Honey and acupuncture have the greatest amount of evidence for postoperative pain and nausea; however, all interventions examined were cost-effective and safe. Abstract

Objective: To evaluate the efficacy of sucralfate for relieving post-tonsillectomy odynophagia. Study Design: Randomized clinical trial. Place and Duration of study: This study was carried out at department of ENT, Combined Military Hospital Muzaffarabad for 9 month Feb 2017 to Oct 2017. Material and Methods: Sixty patients of different age groups were selected randomly and were divided into two groups with equal gender distribution. Both groups underwent tonsillectomy using bipolar diathermy. Standard post-operative antibiotic and analgesic cover was provided. In addition, Group A was prescribed oral sucralfate syrup, post operatively, 3 times a day for 5 days. Patients were inquired about pain after 5 days. Patients in Group A showed better pain relief and the difference was statistically significant. Results: The mean age of surgery in our study was 13.75 ± 8.12 years. The mean operative time was 16.12 ± 1.33 minutes. About 83.3% patients in group-A achieved satisfactory pain relief as compared to 43%patients in group-B. This difference was statistically significant with a p value of 0.001 Conclusion: Prescription of sucralfate in patients after tonsillectomy resulted in alleviation of post-operative odynophagia. 17. The efficacy of honey for ameliorating pain after tonsillectomy: a meta-analysis.

Abstract

Postoperative pain and wound healing following tonsillectomy can result in dissatisfaction for the patient. However, there is no consensus on whether postoperatively administered honey effectively reduces morbidity after tonsillectomy. Therefore, a systematic review with a meta-analysis of the efficacy of honey as a treatment for postoperative pain and wound healing was performed. Two authors independently searched the database records (MEDLINE, SCOPUS, and Cochrane databases) dating from inception to June 2014. Studies comparing postoperative oral administration of honey with administration of placebo where the outcomes of interest were pain and wound healing on postoperative days were included. Baseline study characteristics, study quality, numbers of patients in steroid-treated and control groups, and treatment outcomes were extracted. Sufficient data for meta-analysis were retrieved from 4 trials with a total of 264 patients. We analyzed patient-reported pain scores and quantities of administered analgesics during the first 5 postoperative days. The pain score was significantly decreased in the honey-treated patients in comparison with the placebo-treated patients on postoperative day 1 only, but the analgesic intake of the honey-treated patients on the first 5 postoperative days was significantly less than that of the placebo-treated patients. In addition, honey significantly increased tonsillectomy bed wound healing in comparison with placebo during the first 2 weeks after surgery. This meta-analysis shows that postoperative administration of honey after tonsillectomy significantly reduces pain and promotes wound healing.
