Objective: To establish the difference in morbidity and costs in the resolution of pregnancy between adolescents and adults treated in a public hospital in Lima (Peru). Material and methods: Cross-sectional, retrospective, observational study. Adolescent women and adult women between 20 and 30 years of age, primiparous and without underlying chronic diseases, were treated at the Hipólito Unanue National Hospital, Lima (Peru), during 2018. The variables were studied: demographic and obstetric characteristics, morbidity and costs of obstetric care. Results: 105 adolescents and 210 adults participated. Adolescents and adult women developed some morbidity in pregnancy (63.8 % [n= 67] vs. 56.2 % [n= 118]) and delivery (71.4 % [n= 75] vs. 73.3 % [n= 154]) without significant differences. The most prevalent in pregnancy were: anemia and urinary tract infection; in childbirth: tear of the birth canal, followed by dystocia of labor. Newborns of adolescent mothers had twice the risk of presenting some morbidity at birth (OR= 2.45; 95 % CI: 1.51-3.97). The costs in the resolution of the pregnancies did not have significant differences in the study groups ($ 51.01 ± 83.50 vs. $ 64.61 ± 79.40). Conclusions: A greater number of neonatal morbidities was found in newborns of adolescent mothers. The maternal pathologies found in both groups were anemia, urinary tract infection, labor tear and dystocia. The costs of obstetric care for adolescents and adults did not show significant differences. [ABSTRACT FROM AUTHOR]

In order to establish a relationship between hyperprolactinemia and FSH-LH associated to the ovarian physiology postpartum, a clinical/endocrinological follow-up was performed during a period of twelve months in a cohorte of seventeen women beginning pregnancy resolution. The study had two phases: Lactancy (LAC) and Post-lactancy (LAC) and Post-lactancy (POST-LAC). In both phases blood samples were obtained every week from the first one through the 52nd, to quantitate FSH, LH, prolactin (PRL) and estradiol (E-2). PRL levels were significantly higher during LAC as compared to POST-LAC: an inverse relationship in E-2/PRL was seen through the study. No significant changes for LH and FSH were attained in both phases. Concentrations of these gonadotropins were at the physiological levels and no differences in either one of the two phases were shown when compared to a nonlactating subject. There was a negative association in prolactin levels and the weeks postpartum, and on the contrary, a positive one for E-2. Thus, ovarian follicular development apparently is independent to the gonadotropic-hypophysis stimulus.

11. Retrospective cohort study comparing success of medical management of early pregnancy loss in pregnancies conceived with and without medical assistance.

To compare the success rates of medical management using a combined mifepristone and misoprostol protocol in cases of early pregnancy loss (EPL) between women who conceived without medical assistance and those who conceived through in vitro fertilization (IVF), after fresh or frozen embryo transfer, and evaluate for the predictive factors of success, time to first passage of tissue, and time to complete resolution of pregnancy. Retrospective cohort study. University hospital. Women who presented with EPL below 13 weeks of gestation between June 2013 and July 2021 who were managed medically with mifepristone 200 mg orally and misoprostol 800 mcg vaginally were included in the study. Medical management with mifepristone and misoprostol; conception without medical assistance vs. post-IVF, after fresh or frozen embryo transfer. We evaluated overall success and performed subgroup analysis according to the mode of conception and compared fresh vs. frozen-thawed embryo transfers for IVF pregnancies. In all groups, we also calculated success according to gestational age and compared the time to first passage of tissue. The potential predictive factors of treatment success were analyzed. The side effects and complications of treatment were recorded. A total of 930 women were included in the study, 99 (11%) of whom achieved pregnancy after IVF. The overall success of medical treatment was 89% with no statistically significant difference according to the mode of conception (89% vs. 89%) or type of transfer (fresh 89% vs. frozen 89%). Only lower gestational age by sonography was independently predictive of treatment success, showing a negative regression coefficient of β = −0.333 and an odds ratio of 0.717. The mean time to first passage of tissue was 5.0 ± 2.1 hours. Altogether, 666 women (72%) showed pregnancy resolution on the day of medication administration, an additional 110 women at 1-week follow-up, and a further 74 women after ≥4 weeks on ultrasound. Medical management of EPL with mifepristone and misoprostol is a highly successful treatment option that results in completed abortion in a timely fashion in both pregnancies conceived without medical assistance and those conceived after IVF. Due to the pandemic situation, health services on COVID-19 were overwhelmed by the overpopulation of patients with SARS-CoV-2. Perinatal care has a direct impact specifically the resolution of pregnancy, fueling the search for care options regardless of medical services. In this paper, we reflect on the current role of midwifery in the framework of the pandemic understood as an emergent perinatal action. This perinatal action stems from two specific facts. The first of all, the restriction of the perinatal medical service, looking at the priority for taking care of patients with COVID-19. The second fact is the social demand driven by pregnant women in the search for options to attend their home delivery or alternative spaces. We remark the importance of midwifery taking care of low-risk births facing up sanitary crises, Debido a la situación pandémica por COVID-19, la atención en los servicios de salud se rebasó por la sobrepoblación de enfermos con SARS-CoV-2. La atención perinatal tiene un impacto directo, y en específico la resolución del embarazo, avivando la búsqueda de opciones de atención fuera de los servicios médicos. En el presente trabajo, se reflexiona sobre el rol actual de la partería en el marco de la pandemia entendida como una acción perinatal emergente. Dicha acción perinatal se deriva de dos hechos concretos. El primero es la restricción del servicio médico perinatal por la prioridad a la atención de enfermos COVID-19. El segundo, la demanda social impulsada por las embarazadas en la búsqueda de opciones para atender su parto en casa o espacios alternativos. Se reafirma la importancia de la partería en la atención de partos de bajo riesgo ante crisis sanitarias.

Background: Women in advanced maternal age (older than 35 years of age) are at higher risk of obstetric complications and adverse perinatal outcomes than younger women., Objective: To know the maternal and perinatal morbidities associated to advanced age in pregnant women., Material and Methods: Analytical cross-sectional study. Women with resolution of pregnancy in the medical unit were included and distributed in two groups: group 1, advanced age, ≥ 35 years, and group 2, < 35 years. Clinical data, maternal and perinatal morbidities of the newborn (NB) were collected from the medical record., Results: We included 240 patients, 120 per group; a significant association of advanced maternal age with maternal morbidities such as diseases prior to pregnancy was demonstrated (p < 0.0001), including diabetes mellitus during pregnancy (p = 0.002), hypertensive disease of pregnancy (p = 0.0001), pregnancy resolution by cesarean section (p = 0.04), obstetric hemorrhage (p = 0.0002), prenatal control with < 5 consultations (p = 0.008), as well as those with perinatal morbidities of the NB: preterm gestational age (p = 0.001), intrauterine growth retardation (p = 0.01), low weight for gestational age (p = 0.001) and admission of the NB to the neonatal intensive care unit (p = 0.007); with multivariate analysis, an association of advanced maternal age with diabetes mellitus, hypertensive disease of pregnancy and obstetric hemorrhage was observed (R2 = 0.9884; p < 0.0001)., Conclusion: The maternal and perinatal morbidities are associated with advanced age in pregnant women., (Licencia CC 4.0 (BY-NC-ND) © 2023 Revista Médica del Instituto Mexicano del Seguro Social.)

The intestinal volvulus is an obstructive condition that occurs in the perinatal period and less frequently, during the prenatal period. It is possible to make a timely prenatal diagnosis with the help of imaging techniques to dictate an adequate therapeutic behavior which has a high impact on the prognosis. We present the case of a pregnant woman with 33.5 weeks of gestation who presented polyhydramnios and decreased fetal movements, identifying by means of 2D ultrasound the presence of prenatal intestinal volvulus and in whom the resolution of pregnancy was decided for the surgical treatment of this anomaly. Introduction: The access to a quality health care during pregnancy and the maternal and perinatal results are not only indicators of the development of the country, but also must be taken into account as a complex process--to which we can refer as process health care pregnancy resolution that allows to visualize the degree of exercise and respect of sexual and reproductive rights. The objective of this study was to write and analyze at microsocial level the characteristics of the referral system regarding obstetric health care and the institutional mechanism that enable this and identify if this system hides a hospital rejection phenomena and the consequences of this on the maternal and perinatal results. Material and Methods: The process for which a group of 35 women in puerperium agreed for an obstetric resolution in a hospital of second level of care for open population in Mexico City was explored. This investigation had as starting point a qualitative design. Results: 57% of women had to search for more than one site to obtain an obstetric resolution: 25% visited two, 17% three, 15% over three. The women that visited more hospitals attended nine throughout 24 hours. They weren't duly treated in all cases before being sent on their own means to other health care institutions. It was also possible to identify that the complications that some women presented and who participated in this research as well as their babies were caused by the untimely care received and generated by the referral and counter referral they were subject to. Conclusions: This study helped observe that due to the current conditions under which they operate, the referral system represents an obstacle for the timely access of obstetric health care, by covering up a multi-rejection hospital phenomena that, in turn, has a negative impact in the correct resolution of pregnancy and on the health of these women. The aim of the present work was to identify the prevalence of infections of urinary tract (UTI) and his relations with risk factors in pregnant women. In 80% of total medical records observed UTI’s prevalence during pregnancy, the infections appeared mainly during the first quarter of pregnancy (55.1%). The clinical picture with typical symptoms appeared in 40.2% of cases and just 4.3% reported atypical symptomatology, so there is no significant differences between the UTI and pregnancy complications. We conclude that prevalence of urinary infection is high in pregnant women receiving health services in the clinical institution of study, but this does not represent a factor of risk for the development and resolution ofpregnancy., El objetivo del presente trabajo fue identificar la prevalencia de infecciones de vías urinarias (IVU) y su relación con factores de riesgo en mujeres embarazadas. Se observó una prevalencia de la infección en el 80% del total de expedientes revisados, esta se presentó principalmente durante el primer trimestre del embarazo (55.1%). Los cuadros clínicos con síntomas típicos fueron reportados en 40.2% de los casos mientras que los de sintomatología atípica sólo se encontraron en 4.3%; no se encontraron diferencias significativas del padecimiento entre los diferentes grupos de edad (0.55F). El análisis estadístico tampoco fue significativo en la correlación de IVU y complicaciones del embarazo. Se concluye que la prevalencia de la infección urinaria en gestantes atendidas en la institución clínica de estudio es alta, pero esta no representó un factor de riesgo para el desarrollo y resolución del embarazo.

We present the case of a pregnant woman with aplastic anemia. The 22-year-old patient presented with 27 weeks of gestation and lymphoproliferative syndrome at the Instituto Nacional de Perinatología in Mexico City. The bone marrow biopsy confirmed the diagnosis and the immunophenotype was negative for malignancy. The treatment consisted of transfusions and planned birth at 35 weeks. Afterwards, the patient was referred to a bone marrow transplant unit., Se presenta los resultados perinatales de una paciente con anemia aplásica. Se revisa el caso atendido en el Instituto Nacional de Perinatología, Ciudad de México. La paciente tenía 22 años de edad y embarazo de 27,0 semanas con síndrome linfoproliferativo. La biopsia de médula ósea determinó anemia aplásica; el inmunofenotipo fue negativo a malignidad. El manejo consistió en soporte transfusional, con resolución del embarazo a las 35 semanas. Luego, se refirió la paciente a la unidad de trasplante de médula ósea.

Introduction. The access to a quality health care during pregnancy and the maternal and perinatal results are not only indicators of the development of the country, but also must be taken into account as a complex process -to which we can refer as process health care pregnancy resolution- that allows to visualize the degree of exercise and respect of sexual and reproductive rights. The objective of this study was to write and analyze at microsocial level the characteristics of the referral system regarding obstetric health care and the institutional mechanism that enable this and identify if this system hides a hospital rejection phenomena and the consequences of this on the maternal and perinatal results. Material and Methods. The process for which a group of 35 women in puerperium agreed for an obstetric resolution in a hospital of second level of care for open population in Mexico City was explored. This investigation had as starting point a qualitative design. 39. Embarazo en adolescentes de Jalisco, México: una mirada desde la marginación social

Objective: To analyze the sociocultural conditions, prenatal and obstetrical care, and data of the newborn, in pregnant young women residing in municipalities with very high and very low marginalization in Jalisco, Mexico, in 2014. Design: Quantitative, descriptive cross-sectional study. Institution: Jalisco Health Secretariat. Participants: Adolescents residents of both highly and very low marginalization municipalities. Methodology: Birth certificates of 140 adolescents residing in highly marginalized municipalities (Group 1) and 21 004 residing in municipalities with very low marginalization (Group 2) in Jalisco, Mexico, and who delivered a child during 2014, were reviewed. Main outcome measures: Univariate analysis of social and cultural conditions, prenatal and obstetrical care, data of the newborn. Results: From 28 178 births, 140 (0.5%) adolescents belonged to highly marginalized births (Group 1) and 21 004 (74.5%) occurred in adolescentes of very low marginalization adolescents (Group 2). The average age in Group 1 was 17.1 years, 18.5% were 15 years or less; 41.4% had elementary school or care, the mean number of pregnancies was 1.35; 20% did not receive prenatal care, the mean number of consultations was 3.1, 91% had normal delivery, the average newborn birth weight was 3 032.10 grams. The second group’s mean age was 17,5 years , 22.7 % were single, the average number of pregnancies 1.34, the mean number of consultations 6.6 , 45.2% had caesarean section, and the average weight of newborns was 3 101.67 grams. Conclusions: There were differences between groups in sociocultural characteristics (education, marital status and social security) and those related to prenatal and obstetrical care (prenatal and pregnancy resolution)., Objetivo: Analizar las condiciones socioculturales, atención prenatal y obstétrica, y del recién nacido, en mujeres adolescentes embarazadas residentes de municipios con muy alta y muy baja marginación, en Jalisco, México durante 2014. Diseño: Estudio cuantitativo, descriptivo transversal. Institución. Secretaría de Salud Jalisco. Participantes: Adolescentes residentes de municipios de muy alta y de muy baja marginación. Metodología: Se revisaron certificados de nacimientos de adolescentes residentes de Jalisco atendidas de parto durante 2014, 140 fueron adolescentes residentes de municipios de muy alta marginación y 21 004 de municipios de muy baja marginación. Principales medidas de resultados: Análisis univariado de condiciones socioculturales, atención prenatal y obstétrica, además de datos del recién nacido. Resultados: Se registraron 28 178 nacimientos; de muy alta marginación (Grupo 1) fueron n=140 (0,5%) y de muy baja marginación (Grupo 2) fueron n=21 004 (74,5%). Del Grupo 1, la media de edad fue 17,1 años, 18,5% tenía 15 años o menos, 41,4% primaria o menos, media del número de embarazos fue 1,35; 20% no recibió atención prenatal, la media del número de consultas era 3,1, el 91% tuvo resolución obstétrica por parto normal, la media del peso de recién nacido fue 3 032,10 gramos. Del Grupo 2 la media de la edad fue 17,5 años, 22,7% era soltera, media del número de embarazos 1,34, media del número de consultas 6,7; el 45,2% tuvo resolución obstétrica por cesárea y la media del peso de recién nacidos fue 3 101,67 gramos. Conclusiones: Existieron diferencias porcentuales y de medias entre los grupos, en características socioculturales (escolaridad, estado civil y seguridad social) y las relacionadas con atención prenatal y obstétrica (control prenatal y resolución del embarazo).

Objetivo: Comparar características poblacionales y hallazgos placentarios asociados a óbito, en mujeres con infección asintomática por coronavirus 2 del síndrome respiratorio agudo grave (SARS-CoV-2) al momento de la resolución del embarazo. Material y métodos: Estudio transversal analítico, mujeres con diagnóstico de óbito e infección asintomática por SARS-CoV-2. Se excluyeron pacientes con patología ginecológica y aquellas con más de una prueba RT-PCR. Resultados: No hay diferencia estadísticamente significativa entre las pacientes con infección asintomática por SARSCoV- 2 comparadas con pacientes sin infección para las características poblacionales como edad materna, antecedentes obstétricos, edad gestacional, escolaridad, estado civil, hábito tabáquico y enfermedades crónico-degenerativas. Tampoco hubo diferencia en los hallazgos placentarios de corioamnionitis, vasculitis, deciduitis, trombosis, aterosis aguda y necrosis fibrinoide. Conclusiones: No hay diferencia estadísticamente significativa en las características poblacionales y placentarias en embarazadas con diagnóstico de óbito, con y sin infección asintomática por SARS-CoV-2. Tenemos datos insuficientes para afirmar alguna característica asociada a óbito en pacientes con infección por SARSCoV- 2 asintomática.

La salud mental es la aptitud por la cual los individuos reconocen sus habilidades y son capaces de hacer frente al estrés normal de la vida, trabajar de forma productiva y fructífera, contribuir a sus comunidades y gozar del mayor bienestar posible. Por ello, es fundamental para la salud integral del individuo y su evaluación constante. Recientemente se ha prestado especial atención a la salud mental de las mujeres, puesto que se ha observado una mayor propensión a sufrir depresión y trastornos de ansiedad durante y posterior al embarazo. El estado de mayor vulnerabilidad sucede en la adolescencia, el embarazo, el puerperio y la perimenopausia. Una adecuada salud mental durante la gestación ha mostrado efectos benéficos en la resolución del embarazo y en la salud del recién nacido. En este trabajo se revisan aspectos fundamentales en la definición de salud mental perinatal, sus determinantes, los principales trastornos mentales en este periodo vital, su papel en los resultados perinatales y la importancia de su evaluación. Se exponen también algunas recomendaciones básicas para la atención adecuada de la salud mental en los hospitales ginecoobstétricos.

El acceso a una atención de calidad durante el embarazo y el resultado materno y perinatal no sólo son indicadores de desarrollo en los países, sino que deben abordarse como parte de un proceso complejo al que podemos denominar proceso atención prenatal-resolución del embarazo que permita visualizar el grado de ejercicio y respeto de los derechos sexuales y reproductivos. El objetivo de este estudio fue escribir y analizar en el nivel microsocial las características del sistema de referencia y contrarreferencia en materia de atención obstétrica y los mecanismos institucionales que lo posibilitan e identificar si dicho sistema enmascara un fenómeno de rechazo hospitalario y las consecuencias de éste sobre el resultado materno y perinatal. Material y métodos. Se exploró el proceso por el cual un grupo de 35 mujeres en puerperio accedió para su resolución obstétrica a una institución hospitalaria de segundo nivel de atención para población abierta de la ciudad de México. Esta investigación tuvo como punto de partida un diseño cualitativo. Resultados. El 57% de las mujeres tuvieron que buscar más de un sitio para su resolución obstétrica: 25% visitó dos, 17% tres, 15% a más de tres. La mujer que más hospitales visitó, acudió a nueve nosocomios a lo largo de 24 horas. No fueron debidamente valoradas en todos los casos antes de ser enviadas por sus propios medios a buscar otros sitios de atención. También fue posible identificar que las complicaciones que presentaron algunas de las mujeres que participaron en esta investigación así como las de sus bebés, se debieron a la demora en la atención que se generó a través de la referencia y contrarreferencia de la que fueron objeto. Conclusiones. Este estudio permitió observar que debido a las condiciones actuales bajo las que opera, el sistema de referencia y contrarreferencia representa un obstáculo para el acceso a la atención obstétrica hospitalaria oportuna, al enmascarar un fenómeno de multi-rechazo hospitala

El vólvulo intestinal es un cuadro obstructivo que se presenta en el periodo neonatal y, con menor frecuencia, durante el periodo prenatal. Es posible realizar un diagnóstico prenatal oportuno con ayuda de técnicas de imagen para dictar una conducta terapéutica adecuada, la cual tiene un alto impacto en el pronóstico. Presentamos el caso de una paciente con embarazo de 33.5 semanas de gestación que presentó polihidramnios y disminución en los movimientos fetales, identificando por medio de ultrasonido 2D la presencia de vólvulo intestinal intrauterino y en quien se decidió la resolución del embarazo para el tratamiento quirúrgico de esta anomalía.
