Background: Global pandemics like COVID-19 put a high amount of strain on health care systems and health workers worldwide. These crises generate a vast amount of news information published online across the globe. This extensive corpus of articles has the potential to provide valuable insights into the nature of ongoing events and guide interventions and policies. However, the sheer volume of information is beyond the capacity of human experts to process and analyze effectively., Objective: The aim of this study was to explore how natural language processing (NLP) can be leveraged to build a system that allows for quick analysis of a high volume of news articles. Along with this, the objective was to create a workflow comprising human-computer symbiosis to derive valuable insights to support health workforce strategic policy dialogue, advocacy, and decision-making., Methods: We conducted a review of open-source news coverage from January 2020 to June 2022 on COVID-19 and its impacts on the health workforce from the World Health Organization (WHO) Epidemic Intelligence from Open Sources (EIOS) by synergizing NLP models, including classification and extractive summarization, and human-generated analyses. Our DeepCovid system was trained on 2.8 million news articles in English from more than 3000 internet sources across hundreds of jurisdictions., Results: Rules-based classification with hand-designed rules narrowed the data set to 8508 articles with high relevancy confirmed in the human-led evaluation. DeepCovid's automated information targeting component reached a very strong binary classification performance of 98.98 for the area under the receiver operating characteristic curve (ROC-AUC) and 47.21 for the area under the precision recall curve (PR-AUC). Its information extraction component attained good performance in automatic extractive summarization with a mean Recall-Oriented Understudy for Gisting Evaluation (ROUGE) score of 47.76. DeepCovid's final summaries were used by human experts to write reports on the COVID-19 pandemic., Conclusions: It is feasible to synergize high-performing NLP models and human-generated analyses to benefit open-source health workforce intelligence. Background: To investigate the impacts of the COVID-19 pandemic on the health workforce, we aimed to develop a framework that synergizes natural language processing (NLP) techniques and human-generated analysis to reduce, organize, classify, and analyze a vast volume of publicly available news articles to complement scientific literature and support strategic policy dialogue, advocacy, and decision-making., Objective: This study aimed to explore the possibility of systematically scanning intelligence from media that are usually not captured or best gathered through structured academic channels and inform on the impacts of the COVID-19 pandemic on the health workforce, contributing factors to the pervasiveness of the impacts, and policy responses, as depicted in publicly available news articles. Our focus was to investigate the impacts of the COVID-19 pandemic and, concurrently, assess the feasibility of gathering health workforce insights from open sources rapidly., Methods: We conducted an NLP-assisted media content analysis of open-source news coverage on the COVID-19 pandemic published between January 2020 and June 2022. A data set of 3,299,158 English news articles on the COVID-19 pandemic was extracted from the World Health Organization Epidemic Intelligence through Open Sources (EIOS) system. The data preparation phase included developing rules-based classification, fine-tuning an NLP summarization model, and further data processing. Following relevancy evaluation, a deductive-inductive approach was used for the analysis of the summarizations. This included data extraction, inductive coding, and theme grouping., Results: After processing and classifying the initial data set comprising 3,299,158 news articles and reports, a data set of 5131 articles with 3,007,693 words was devised. The NLP summarization model allowed for a reduction in the length of each article resulting in 496,209 words that facilitated agile analysis performed by humans. Media content analysis yielded results in 3 sections: areas of COVID-19 impacts and their pervasiveness, contributing factors to COVID-19-related impacts, and responses to the impacts. The results suggest that insufficient remuneration and compensation packages have been key disruptors for the health workforce during the COVID-19 pandemic, leading to industrial actions and mental health burdens. Shortages of personal protective equipment and occupational risks have increased infection and death risks, particularly at the pandemic's onset. Workload and staff shortages became a growing disruption as the pandemic progressed., Conclusions: This study demonstrates the capacity of artificial intelligence-assisted media content analysis applied to open-source news articles and reports concerning the health workforce. Adequate remuneration packages and personal protective equipment supplies should be prioritized as preventive measures to reduce the initial impact of future pandemics on the health workforce. Importance: Systems-level barriers to diabetes care could be improved with population health planning tools that accurately discriminate between high- and low-risk groups to guide investments and targeted interventions., Objective: To develop and validate a population-level machine learning model for predicting type 2 diabetes 5 years before diabetes onset using administrative health data., Design, Setting, and Participants: This decision analytical model study used linked administrative health data from the diverse, single-payer health system in Ontario, Canada, between January 1, 2006, and December 31, 2016. A gradient boosting decision tree model was trained on data from 1 657 395 patients, validated on 243 442 patients, and tested on 236 506 patients. Costs associated with each patient were estimated using a validated costing algorithm. Data were analyzed from January 1, 2006, to December 31, 2016., Exposures: A random sample of 2 137 343 residents of Ontario without type 2 diabetes was obtained at study start time. More than 300 features from data sets capturing demographic information, laboratory measurements, drug benefits, health care system interactions, social determinants of health, and ambulatory care and hospitalization records were compiled over 2-year patient medical histories to generate quarterly predictions., Main Outcomes and Measures: Discrimination was assessed using the area under the receiver operating characteristic curve statistic, and calibration was assessed visually using calibration plots. Feature contribution was assessed with Shapley values. Costs were estimated in 2020 US dollars., Results: This study trained a gradient boosting decision tree model on data from 1 657 395 patients (12 900 257 instances; 6 666 662 women [51.7%]). The developed model achieved a test area under the curve of 80.26 (range, 80.21-80.29), demonstrated good calibration, and was robust to sex, immigration status, area-level marginalization with regard to material deprivation and race/ethnicity, and low contact with the health care system. The top 5% of patients predicted as high risk by the model represented 26% of the total annual diabetes cost in Ontario., Conclusions and Relevance: In this decision analytical model study, a machine learning model approach accurately predicted the incidence of diabetes in the population using routinely collected health administrative data. These results suggest that the model could be used to inform decision-making for population health planning and diabetes prevention.

Across jurisdictions, government and health insurance providers hold a large amount of data from patient interactions with the healthcare system. We aimed to develop a machine learning-based model for predicting adverse outcomes due to diabetes complications using administrative health data from the single-payer health system in Ontario, Canada. A Gradient Boosting Decision Tree model was trained on data from 1,029,366 patients, validated on 272,864 patients, and tested on 265,406 patients. Discrimination was assessed using the AUC statistic and calibration was assessed visually using calibration plots overall and across population subgroups. Our model predicting three-year risk of adverse outcomes due to diabetes complications (hyper/hypoglycemia, tissue infection, retinopathy, cardiovascular events, amputation) included 700 features from multiple diverse data sources and had strong discrimination (average test AUC = 77.7, range 77.7-77.9). Through the design and validation of a high-performance model to predict diabetes complications adverse outcomes at the population level, we demonstrate the potential of machine learning and administrative health data to inform health planning and healthcare resource allocation for diabetes management.

With the exponential proliferation of digital documents, there arises a pressing need for automated document summarization (ADS). Summarization, a compression technique, condenses a source document into concise sentences that encapsulate its salient information for summary generation. A primary challenge lies in crafting a dependable summary, contingent upon both extracted features and human-established parameters. This article introduces an intelligent methodology that seamlessly integrates extractive and abstractive techniques to ensure heightened relevance between the input document and its summary. Initially, input sentences undergo transformation into representations utilizing BERT, subsequently transposed into a symmetric matrix based on their similarity. Semantically congruent sentences are then extracted from this matrix to construct an extractive summary. The transformer model integrates an objective function highly symmetric and invariant under unitary transformation for language generation. This model refines the extracted informative sentences and generates an abstractive summary akin to manually crafted summaries. Employing this hybrid summarization technique on the CNN/DailyMail dataset and DUC2004, we evaluate its efficacy using ROUGE metrics. Tables serve as a widely adopted data format, attracting considerable academic interest concerning semantic understanding and logical inference of tables. In recent years, the prevailing paradigm of pre-training and fine-tuning on tabular data has become increasingly prominent in research on table understanding. However, existing table-based pre-training methods frequently exhibit constraints, supporting only single tasks while requiring substantial computational resources, which hinders their efficiency and applicability. In this paper, we introduce the TabMoE, a novel framework based on mixture-of-experts, designed to handle a wide range of tasks involving logical reasoning over tabular data. Each expert within the model specializes in a distinct logical function and is trained through the utilization of a hard Expectation–Maximization algorithm. Remarkably, this framework eliminates the necessity of dependency on tabular pre-training, instead exclusively employing limited task-specific data to significantly enhance models' inferential capabilities. We conduct empirical experiments across three typical tasks related to tabular data: table-based question answering, table-based fact verification, and table-to-text generation. Background: In the age of big data, linked social and administrative health data in combination with machine learning (ML) is being increasingly used to improve prediction in chronic disease, e.g., cardiovascular diseases (CVD). In this study we aimed to apply ML methods on extensive national-level health and social administrative datasets to assess the utility of these for predicting future diabetes complications, including by ethnicity. Methods: Five ML models were used to predict CVD events among all people with known diabetes in the population of New Zealand, utilizing nationwide individual-level administrative data. Results: The Xgboost ML model had the best predictive power for predicting CVD events three years into the future among the population with diabetes (N = 145,600). The optimization procedure also found limited improvement in prediction by ethnicity (using area under the receiver operating curve, [AUC]). The results indicated no trade-off between model predictive performance and equity gap of prediction by ethnicity (that is improving model prediction and reducing performance gaps by ethnicity can be achieved simultaneously). The list of variables of importance was different among different models/ethnic groups, for example: age, deprivation (neighborhood-level), having had a hospitalization event, and the number of years living with diabetes. Discussion and conclusions: We provide further evidence that ML with administrative health data can be used for meaningful future prediction of health outcomes. As such, it could be utilized to inform health planning and healthcare resource allocation for diabetes management and the prevention of CVD events. Our results may suggest limited scope for developing prediction models by ethnic group and that the major ways to reduce inequitable health outcomes is probably via improved delivery of prevention and management to those groups with diabetes at highest need. Large Language Models (LLMs), based on their highly developed ability to comprehend and generate human-like text, promise to revolutionize all aspects of society. These LLMs facilitate complex language understanding, translation, content generation, and problem-solving, enabled by vast historical data processing and fine-tuning. Throughout the past year, with the initial release of ChatGPT to the public, many papers have appeared on how to exploit LLMs for the ways we operate and interact with intelligent transportation systems. In this study, we review more than 130 papers on the subject and group them according to their major contributions into the following five categories: autonomous driving, safety, tourism, traffic, and others. Based on the aggregated proposals and findings in the extant literature, this paper concludes with a set of challenges and research recommendations, hopefully contributing to guide research in this young, yet extremely active research domain. Introduction and Objectives: Despite the huge clinical burden of MASLD, validated tools for early risk stratification are lacking, and heterogeneous disease expression and a highly variable rate of progression to clinical outcomes result in prognostic uncertainty. We aimed to investigate longitudinal electronic health recordbased outcome prediction in MASLD using a state-of-the-art machine learning model. Patients and Methods: n = 940 patients with histologically-defined MASLD were used to develop a deep-learning model for all-cause mortality prediction. Patient timelines, spanning 12 years, were fully-annotated with demographic/clinical characteristics, ICD-9 and -10 codes, blood test results, prescribing data, and secondary care activity. A Transformer neural network (TNN) was trained to output concomitant probabilities of 12-, 24-, and 36-month all-cause mortality. In-sample performance was assessed using 5-fold cross-validation. Out-of-sample performance was assessed in an independent set of n = 528 MASLD patients. Results: In-sample model performance achieved AUROC curve 0.74−0.90 (95 % CI: 0.72−0.94), sensitivity 64 %-82 %, specificity 75 %−92 % and Positive Predictive Value (PPV) 94 %-98 %. Out-of-sample model validation had AUROC 0.70−0.86 (95 % CI: 0.67−0.90), sensitivity 69 %−70 %, specificity 96 %−97 % and PPV 75 %−77 %. Key predictive factors, identified using coefficients of determination, were age, presence of type 2 diabetes, and history of hospital admissions with length of stay >14 days. Conclusions: A TNN, applied to routinely-collected longitudinal electronic health records, achieved good performance in prediction of 12-, 24-, and 36-month all-cause mortality in patients with MASLD. Extrapolation of our technique to population-level data will enable scalable and accurate risk stratification to identify people most likely to benefit from anticipatory health care and personalized interventions. This article establishes the DenseNet based on multilayer network optimization (DenseNet‐MNO) using a binary dataset of pneumothorax. This method optimizes multiple network layers and ensures the convergence of the neural network by reducing the learning rate with each iteration. Then, the binary cross‐entropy loss function and accuracy evaluation function iteratively evaluated the effectiveness of the deep learning model and conducted multiple experiments. The experimental results show that during the iteration process, the loss is reduced, and the accuracy is improved. The DenseNet‐MNO classification model has a strong generalization ability and will not overfit. The classification accuracy is between 80% and 85%. The DenseNet‐MNO classification model can accurately detect the condition of pneumothorax, providing technical support for detection technology. [ABSTRACT FROM AUTHOR]

Objective: In order to comprehensively understand the characteristics of Adaptive Business Intelligence (ABI) in Healthcare, this study is structured to provide insights into the common features and evolving patterns within this domain. Applying the Sheridan's Classification as a framework, we aim to assess the degree of autonomy exhibited by various ABI components. Together, these objectives will contribute to a deeper understanding of ABI implementation and its implications within the Healthcare context. Methods: A comprehensive search of academic databases was conducted to identify relevant studies, selecting AIS e-library (AISel), Decision Support Systems Journal (DSSJ), Nature, The Lancet Digital Health (TLDH), PubMed, Expert Systems with Application (ESWA) and npj Digital Medicine as information sources. Studies from 2006 to 2022 were included based on predefined eligibility criteria. PRISMA statements were used to report this study. Results: The outcomes showed that ABI systems present distinct levels of development, autonomy and practical deployment. The high levels of autonomy were essentially associated with predictive components. However, the possibility of completely autonomous decisions by these systems is totally excluded. Lower levels of autonomy are also observed, particularly in connection with prescriptive components, granting users responsibility in the generation of decisions. Conclusion: The study presented emphasizes the vital connection between desired outcomes and the inherent autonomy of these solutions, highlighting the critical need for additional research on the consequences of ABI systems and their constituent elements. Organizations should deploy these systems in a way consistent with their objectives and values, while also being mindful of potential adverse effects. Despite advanced health facilities in many developed countries, diabetic patients face multifold health challenges. Type 2 diabetes mellitus (T2DM) go along with conspicuous symptoms due to frequent peaks, hypoglycemia <=70 mg/dL (while fasting), or hyperglycemia >=180 mg/dL two hours postprandial, according to the American Diabetes Association (ADA)). The worse effects of Type 2 diabetes mellitus are precisely associated with the poor lifestyle adopted by patients. In particular, a healthy diet and nutritious food are the key to success for such patients. This study was done to help T2DM patients improve their health by developing a favorable lifestyle under an AI-assisted Continuous glucose monitoring (CGM) digital system. This study aims to reduce the blood glucose level fluctuations of such patients by rectifying their daily diet and maintaining their exertion vs. food consumption records. In this study, a well-precise prediction is obtained by training the ML model on a dataset recorded from CGM sensor devices attached to T2DM patients under observation. As the data obtained from the CGM sensor is time series, to predict blood glucose levels, the time series analysis and forecasting are done with XGBoost, SARIMA, and Prophet. The results of different Models are then compared based on performance metrics. This helped in monitoring various trends, specifically irregular patterns of the patient's glucose data, collected by the CGM sensor. Later, keeping track of these trends and seasonality, the diet is adjusted accordingly by adding or removing particular food and keeping track of its nutrients with the intervention of a commercially available all-in-one AI solution for food recognition. This created an interactive assistive system, where the predicted results are compared to food contents to bring the blood glucose levels within the normal range for maintaining a healthy lifestyle and to alert about blood glucose fluctuations before the time that are going to occur sooner. This study will help T2DM patients get in managing diabetes and ultimately bring HbA1c within the normal range (<= 5.7%) for diabetic and pre-diabetic patients, three months after the intervention. Background: Identification of patients at risk for type 2 diabetes mellitus (T2DM) can not only prevent complications and reduce suffering but also ease the health care burden. While routine physical examination can provide useful information for diagnosis, manual exploration of routine physical examination records is not feasible due to the high prevalence of T2DM. Objectives: We aim to build interpretable machine learning models for T2DM diagnosis and uncover important diagnostic indicators from physical examination, including age- and sex-related indicators. Methods: In this study, we present three weighted diversity density (WDD)-based algorithms for T2DM screening that use physical examination indicators, the algorithms are highly transparent and interpretable, two of which are missing value tolerant algorithms. Patients: Regarding the dataset, we collected 43 physical examination indicator data from 11,071 cases of T2DM patients and 126,622 healthy controls at the Affiliated Hospital of Southwest Medical University. After data processing, we used a data matrix containing 16004 EHRs and 43 clinical indicators for modelling. Results: The indicators were ranked according to their model weights, and the top 25% of indicators were found to be directly or indirectly related to T2DM. We further investigated the clinical characteristics of different age and sex groups, and found that the algorithms can detect relevant indicators specific to these groups. The algorithms performed well in T2DM screening, with the highest area under the receiver operating characteristic curve (AUC) reaching 0.9185. Conclusion: This work utilized the interpretable WDD-based algorithms to construct T2DM diagnostic models based on physical examination indicators. By modeling data grouped by age and sex, we identified several predictive markers related to age and sex, uncovering characteristic differences among various groups of T2DM patients. Multireceiver synthetic aperture sonar (SAS) can produce high-resolution images by coherently superposing successive echoed signal. However, multireceiver SAS imaging is a difficult issue since the system can be divided into many bistatic SASs. Traditional imaging algorithms cannot be directly used to process the multireceiver SAS echoed signal, as they are just applied to monostatic sonar system. In order to solve this issue, this paper proposes an imaging algorithm based on Loffeld's bistatic formula (LBF) for multireceiver SAS system. With the LBF method, the point target reference spectrum (PTRS) of multireceiver SAS system can be split into quasi-monostatic term and the bistatic deformation term. The quasi-monostatic term is similar to the spectrum of monostatic SAS while the bistatic deformation term is caused by the displacement of transmitter and receiver. In this paper, the data of multireceiver SAS is firstly coerced into monostatic one by compensating the bistatic deformation term. After this step, the chirp scaling (CS) algorithm based on monostatic SAS can be directly applied. At last, simulations are exploited to demonstrate the presented method. Compared to traditional multireceiver SAS CS algorithms, the presented method significantly improves the imaging performance. Both ghosts target and higher sidelobes that traditional multireceiver SAS CS algorithms suffer from can be successfully suppressed. Purpose: A switching depth controller based on a variable buoyancy system (VBS) is proposed to improve the performance of small autonomous underwater vehicles (AUVs). First, the requirements of VBS for small AUVs are analyzed. Second, a modular VBS with high extensibility and easy integration is proposed based on the concepts of generality and interchangeability. Subsequently, a depth-switching controller is proposed based on the modular VBS, which combines the best features of the linear active disturbance rejection controller and the nonlinear active disturbance rejection controller. Design/methodology/approach: The controller design and endurance of tiny AUVs are challenging because of their low environmental adaptation, limited energy resources and nonlinear dynamics. Traditional and single linear controllers cannot solve these problems efficiently. Although the VBS can improve the endurance of AUVs, the current VBS is not extensible for small AUVs in terms of the differences in individuals and operating environments. Findings: The switching controller's performance was examined using simulation with water flow and external disturbances, and the controller's performance was compared in pool experiments. The results show that switching controllers have greater effectiveness, disturbance rejection capability and robustness even in the face of various disturbances. Practical implications: A high degree of standardization and integration of VBS significantly enhances the performance of small AUVs. This will help expand the market for small AUV applications. Originality/value: This solution improves the extensibility of the VBS, making it easier to integrate into different models of small AUVs. Electronic Health Records (EHR) provide a vast amount of patient data that are relevant to predicting clinical outcomes. The inherent presence of missing values poses challenges to building performant machine learning models. This paper aims to investigate the effect of various imputation methods on the National Institutes of Health's All of Us dataset, a dataset containing a high degree of data missingness. We apply several imputation techniques such as mean substitution, constant filling, and multiple imputation on the same dataset for the task of diabetes prediction. We find that imputing values causes heteroskedastic performance for machine learning models with increased data missingness. That is, the more missing values a patient has for their tests, the higher variance there is on a diabetes model AUROC, F1, precision, recall, and accuracy scores. This highlights a critical challenge in using EHR data for predictive modeling. This work highlights the need for future research to develop methodologies to mitigate the effects of missing data and heteroskedasticity in EHR-based predictive models. [ABSTRACT FROM AUTHOR]

Fine-tuning a pre-trained sequence-to-sequence-based language model has significantly advanced the field of abstractive summarization. However, the early models of abstractive summarization were limited by the gap between training and inference, and they did not fully utilize the potential of the language model. Recent studies have introduced a two-stage framework that allows the second-stage model to re-rank the candidate summary generated by the first-stage model, to resolve these limitations. In this study, we point out that the supervision method performed in the existing re-ranking model of the two-stage abstractive summarization framework cannot learn detailed and complex information of the data. In addition, we present the problem of positional bias in the existing encoder–decoder-based re-ranking model. To address these two limitations, this study proposes a hierarchical supervision method that jointly performs summary and sentence-level supervision. For sentence-level supervision, we designed two sentence-level loss functions: intra- and inter-intra-sentence ranking losses. Compared to the existing abstractive summarization model, the proposed method exhibited a performance improvement for both the CNN/DM and XSum datasets. This paper presents an intelligent healthcare framework by incorporating modern computing technologies like machine learning and deep learning. The sole motivation of this paper is to predict diabetes status intelligently so that patients can be made aware of their diabetes status regularly. A novel ensemble machine learning-based diabetes prediction framework, namely "iDP", has been proposed in this paper. Six different machine learning approaches have been used for the iDP framework, namely random forest, decision tree, neural network, AdaBoost, support vector machine, and XGBoost. Training and testing of ensemble modeling have been performed based on these learning techniques. A comprehensive preliminary data analysis and screening of statistical analysis have been systematically demonstrated in this paper. The recursive feature elimination and fivefold cross-validation methods have been used to select the best model. All experiments have been performed using R-Studio 1.2.5 with the help of the Rattle library 5.4.0. The preliminary data analytic and Pearson correlation coefficient values have been computed to verify the linearity relationship among the data. Results have been reported in eight performance metrics, including accuracy, specificity, sensitivity, area under curve, area under convex hull, minimum error rate, minimum weighted loss, and precision. The proposed iDP framework improves the results on different aspects in order to compare them with individual learning approaches. Despite this, a comparative performance analysis of the iDP framework has also been performed with state-of-the-art prevailing models. Lower extremity open revascularization is a treatment option for peripheral artery disease that carries significant peri-operative risks; however, outcome prediction tools remain limited. Using machine learning (ML), we developed automated algorithms that predict 30-day outcomes following lower extremity open revascularization. The National Surgical Quality Improvement Program targeted vascular database was used to identify patients who underwent lower extremity open revascularization for chronic atherosclerotic disease between 2011 and 2021. Input features included 37 pre-operative demographic/clinical variables. The primary outcome was 30-day major adverse limb event (MALE; composite of untreated loss of patency, major reintervention, or major amputation) or death. Our data were split into training (70%) and test (30%) sets. Using tenfold cross-validation, we trained 6 ML models. Overall, 24,309 patients were included. The primary outcome of 30-day MALE or death occurred in 2349 (9.3%) patients. Our best performing prediction model was XGBoost, achieving an area under the receiver operating characteristic curve (95% CI) of 0.93 (0.92–0.94). The calibration plot showed good agreement between predicted and observed event probabilities with a Brier score of 0.08. Our ML algorithm has potential for important utility in guiding risk mitigation strategies for patients being considered for lower extremity open revascularization to improve outcomes. In recent years, convolutional neural networks (CNN)‐based automatic segmentation of medical images has become one of the hot topics in clinical disease diagnosis. It is still a challenging task to improve the segmentation accuracy of the network model with the large variation of pathological regions in different patients and the fuzzy boundary of pathological regions. A Dual‐path Multi‐scale Attention Guided network (DMAGNet) for medical image segmentation is proposed in this paper. First, the Dual‐path Multi‐scale Attention Fusion Module (DMAF) is proposed as a novel skip connection strategy, which is applied to encode semantic dependencies between high‐level and low‐level channels. Second, the Multi‐scale Normalized Channel Attention Module (MNCA) based on the atrous convolution, normalization channel attention mechanism, and the Depthwise Separable Convolutions (DSConv) is developed to strengthen dependencies between channels. Finally, the encoder–decoder backbone employs the DSConv, as well as the pretrained Resnet34 block is combined in the encoder part to further improve the backbone network performance. Comprehensive experiments on brain, lung, and liver segmentation tasks show that the proposed DMAGNet outperforms the original U‐Net method and other advanced methods. Amputation is an irreversible, last-line treatment indicated for a multitude of medical problems. Delaying amputation in favor of limb-sparing treatment may lead to increased risk of morbidity and mortality. This systematic review aims to synthesize the literature on how ML is being applied to predict amputation as an outcome. OVID Embase, OVID Medline, ACM Digital Library, Scopus, Web of Science, and IEEE Xplore were searched from inception to March 5, 2023. 1376 studies were screened; 15 articles were included. In the diabetic population, models ranged from sub-optimal to excellent performance (AUC: 0.6–0.94). In trauma patients, models had strong to excellent performance (AUC: 0.88–0.95). In patients who received amputation secondary to other etiologies (e.g.: burns and peripheral vascular disease), models had similar performance (AUC: 0.81–1.0). Many studies were found to have a high PROBAST risk of bias, most often due to small sample sizes. In conclusion, multiple machine learning models have been successfully developed that have the potential to be superior to traditional modeling techniques and prospective clinical judgment in predicting amputation. The increasing prevalence of type 2 diabetes mellitus (T2DM) and its associated health complications highlight the need to develop predictive models for early diagnosis and intervention. While many artificial intelligence (AI) models for T2DM risk prediction have emerged, a comprehensive review of their advancements and challenges is currently lacking. This scoping review maps out the existing literature on AI-based models for T2DM prediction, adhering to the PRISMA extension for Scoping Reviews guidelines. A systematic search of longitudinal studies was conducted across four databases, including PubMed, Scopus, IEEE-Xplore, and Google Scholar. Forty studies that met our inclusion criteria were reviewed. Classical machine learning (ML) models dominated these studies, with electronic health records (EHR) being the predominant data modality, followed by multi-omics, while medical imaging was the least utilized. Most studies employed unimodal AI models, with only ten adopting multimodal approaches. Both unimodal and multimodal models showed promising results, with the latter being superior. Almost all studies performed internal validation, but only five conducted external validation. Most studies utilized the area under the curve (AUC) for discrimination measures. Notably, only five studies provided insights into the calibration of their models. Half of the studies used interpretability methods to identify key risk predictors revealed by their models. Although a minority highlighted novel risk predictors, the majority reported commonly known ones. Our review provides valuable insights into the current state and limitations of AI-based models for T2DM prediction and highlights the challenges associated with their development and clinical integration. Artificial intelligence and machine learning are driving a paradigm shift in medicine, promising data-driven, personalized solutions for managing diabetes and the excess cardiovascular risk it poses. In this comprehensive review of machine learning applications in the care of patients with diabetes at increased cardiovascular risk, we offer a broad overview of various data-driven methods and how they may be leveraged in developing predictive models for personalized care. We review existing as well as expected artificial intelligence solutions in the context of diagnosis, prognostication, phenotyping, and treatment of diabetes and its cardiovascular complications. In addition to discussing the key properties of such models that enable their successful application in complex risk prediction, we define challenges that arise from their misuse and the role of methodological standards in overcoming these limitations. We also identify key issues in equity and bias mitigation in healthcare and discuss how the current regulatory framework should ensure the efficacy and safety of medical artificial intelligence products in transforming cardiovascular care and outcomes in diabetes. One of the most common and deadly diseases in the world is lung cancer. Only early identification of lung cancer can increase a patient's probability of survival. A frequently used modality for the screening and diagnosis of lung cancer is computed tomography (CT) imaging, which provides a detailed scan of the lung. In line with the advancement of computer-assisted systems, deep learning techniques have been extensively explored to help in interpreting the CT images for lung cancer identification. Hence, the goal of this review is to provide a detailed review of the deep learning techniques that were developed for screening and diagnosing lung cancer. This review covers an overview of deep learning (DL) techniques, the suggested DL techniques for lung cancer applications, and the novelties of the reviewed methods. This review focuses on two main methodologies of deep learning in screening and diagnosing lung cancer, which are classification and segmentation methodologies. The advantages and shortcomings of current deep learning models will also be discussed. The resultant analysis demonstrates that there is a significant potential for deep learning methods to provide precise and effective computer-assisted lung cancer screening and diagnosis using CT scans. At the end of this review, a list of potential future works regarding improving the application of deep learning is provided to spearhead the advancement of computer-assisted lung cancer diagnosis systems. Objective: Diabetes mellitus is a global epidemic disease. Long-time exposure of patients to hyperglycemia can lead to various type of chronic tissue damage. Early diagnosis of and screening for diabetes are crucial to population health. Methods: We collected the national physical examination data in Xinjiang, China, in 2020 (a total of more than 4 million people). Three types of physical examination indices were analyzed: questionnaire, routine physical examination and laboratory values. Integrated learning, deep learning and logistic regression methods were used to establish a risk model for type-2 diabetes mellitus. In addition, to improve the convenience and flexibility of the model, a diabetes risk score card was established based on logistic regression to assess the risk of the population. Results: An XGBoost-based risk prediction model outperformed the other five risk assessment algorithms. The AUC of the model was 0.9122. Based on the feature importance ranking map, we found that hypertension, fasting blood glucose, age, coronary heart disease, ethnicity, parental diabetes mellitus, triglycerides, waist circumference, total cholesterol, and body mass index were the most important features of the risk prediction model for type-2 diabetes. Conclusions: This study established a diabetes risk assessment model based on multiple ethnicities, a large sample and many indices, and classified the diabetes risk of the population, thus providing a new forecast tool for the screening of patients and providing information on diabetes prevention for healthy populations. Video classification is an important part of understanding, summarizing and retrieving video data. News short video has the feature that audio information can describe news events more completely than image information, while traditional video classification models often only consider image information or fuse multimodal information of audio and image, which do not consider the primary-secondary relationship between modal information. To address the above problems, a news short video classification model fusing multimodal feature is proposed. It is designed with the fusion mechanism of audio modality as the main and image modality as the auxiliary. In order to make further use of the audio-dominated feature, a two-stage training mode is adopted. Firstly, the audio mode is trained separately, and then the audio and image modes are trained jointly. The image information is used to correct the classification results, so as to improve the accuracy of news short video classification. For the purpose of the model in training and evaluation, 10 304 news broadcast short videos have been collected as experimental dataset, with a total time of about 240 hours. Lung cancer is the world's second-largest cause of cancer mortality. Patients' lives can be saved if this malignancy is detected early. Doctors, however, encounter difficulties in detecting cancer in computed tomography (CT) images. In recent years, significant research has been devoted to producing automated lung nodule detection methods that can help radiologists. Most of them use only the lung window in their analysis and generally do not consider the mediastinal windows, which, according to recent research, carry important information. In this paper, we propose a simple yet effective algorithm to analyze multi-window CT images for lung nodules. The algorithm works in three steps. First, the CT image is preprocessed to suppress any noise and improve the image quality. Second, the lungs are extracted from the preprocessed image. Based on the histogram analysis of the lung windows, we propose a multi-Otsu-based approach for lung segmentation in lung windows. The case of mediastinal windows is rather difficult due to irregular patterns in the histograms. To this end, we propose a global–local-mean-based thresholding technique for lung detection. In the final step, the nodule candidates are extracted from the segmented lungs using simple intensity-based thresholding. The radius of the extracted objects is computed to separate the nodule from the bronchioles and blood vessels. The proposed algorithm is evaluated on the benchmark LUNA16 dataset and achieves accuracy of over 94% for lung tumor detection, surpassing that of existing similar methods. [ABSTRACT FROM AUTHOR]

Primary care has the potential to be transformed by artificial intelligence (AI) and, in particular, machine learning (ML). This review summarizes the potential of ML and its subsets in influencing two domains of primary care: pre-operative care and screening. ML can be utilized in preoperative treatment to forecast postoperative results and assist physicians in selecting surgical interventions. Clinicians can modify their strategy to reduce risk and enhance outcomes using ML algorithms to examine patient data and discover factors that increase the risk of worsened health outcomes. ML can also enhance the precision and effectiveness of screening tests. Healthcare professionals can identify diseases at an early and curable stage by using ML models to examine medical pictures, diagnostic modalities, and spot patterns that may suggest disease or anomalies. Before the onset of symptoms, ML can be used to identify people at an increased risk of developing specific disorders or diseases. ML algorithms can assess patient data such as medical history, genetics, and lifestyle factors to identify those at higher risk. This enables targeted interventions such as lifestyle adjustments or early screening. Globally, the number of people with diabetes mellitus has quadrupled in the past three decades, and approximately one in 11 adults worldwide have diabetes mellitus. Since both microvascular and macrovascular diseases in patients with diabetes predispose them to a lower quality of life as well as higher rates of mortality, managing blood glucose levels is of clinical relevance in diabetes care. Many classes of antihyperglycemic drugs are currently approved to treat hyperglycemia in patients with type 2 diabetes mellitus, with several new drugs having been developed during the last decade. Diabetes-related complications have been reduced substantially worldwide. Prioritization of therapeutic agents varies according to national guidelines. However, since the characteristics of participants in clinical trials differ from patients in actual clinical practice, it is difficult to apply the results of such trials to clinical practice. Machine learning approaches became highly topical issues in medicine along with rapid technological innovations in the fields of information and communication in the 1990s. However, adopting these technologies to support decisionmaking regarding drug treatment strategies for diabetes care has been slow. Diabetes is a chronic, metabolic disease characterized by high blood sugar levels. Among the main types of diabetes, type 2 is the most common. Early diagnosis and treatment can prevent or delay the onset of complications. Previous studies examined the application of machine learning techniques for prediction of the pathology, and here an artificial neural network shows very promising results as a possible valuable aid in the management and prevention of diabetes. Additionally, its superior ability for long-term predictions makes it an ideal choice for this field of study. We utilized machine learning methods to uncover previously undiscovered associations between an individual's health status and the development of type 2 diabetes, with the goal of accurately predicting its onset or determining the individual's risk level. Our study employed a binary classifier, trained on scratch, to identify potential nonlinear relationships between the onset of type 2 diabetes and a set of parameters obtained from patient measurements. Three datasets were utilized, i.e., the National Center for Health Statistics' (NHANES) biennial survey, MIMIC-III and MIMIC-IV. These datasets were then combined to create a single dataset with the same number of individuals with and without type 2 diabetes. Since the dataset was balanced, the primary evaluation metric for the model was accuracy. The outcomes of this study were encouraging, with the model achieving accuracy levels of up to 86% and a ROC AUC value of 0.934. Monitoring diabetes saves lives. To this end, we introduce a novel, unobtrusive, and readily deployable in-ear device for the continuous and non-invasive measurement of blood glucose levels (BGLs). The device is equipped with a low-cost commercially available pulse oximeter whose infrared wavelength (880 nm) is used for the acquisition of photoplethysmography (PPG). For rigor, we considered a full range of diabetic conditions (non-diabetic, pre-diabetic, type I diabetic, and type II diabetic). Recordings spanned nine different days, starting in the morning while fasting, up to a minimum of a two-hour period after eating a carbohydrate-rich breakfast. The BGLs from PPG were estimated using a suite of regression-based machine learning models, which were trained on characteristic features of PPG cycles pertaining to high and low BGLs. The analysis shows that, as desired, an average of 82% of the BGLs estimated from PPG lie in region A of the Clarke error grid (CEG) plot, with 100% of the estimated BGLs in the clinically acceptable CEG regions A and B.
