Abstract

This study introduced a time-delay exposure system independent of the mobile digital radiography equipment. The system consisted of lithium battery, delay control circuit, micro electric motor and related auxiliary facilities. When the starting time was reached through the delay circuit, the motor pushed out the rod to squeeze the exposure button and completed the exposure. The accessories used in this system were easy to purchase and cheap. At the same time, the technology was mature and had good compatibility. The exposure success rate was high and the exposure effect was satisfactory. This time-delay exposure system had good practicability and popularization value.

Abstract

Quality control (QC) guidelines published by the IAEA, American College of Radiology, European Commission, and other authorities have demonstrated that regular QC testing of radiographic facilities, involving daily or weekly tests, effectively contributes to patient radiation exposure reduction and improvement of image quality. Often lack of staff qualified to effectively perform and analyze testing results, and needed resources (phantoms, measuring equipment) leads to poor QC practice and detection in system performance deficiencies only after they become clinically significant. New IAEA publication offers a solution with simple, inexpensive test objects which, using the advantages of computer networking, can allow the collection of data in a harmonized manner. The creation of a centralized data system should ensure consistency, easier evaluation, and comparison between different systems. The aim of this study was to implement the newly proposed QC program and test the available radiographic equipment for short-term fluctuations of some critical components of the imaging chain to ensure consistent system performance, clinically adequate image quality, and increase patient safety. The measurement was performed at the University Clinical Center of Vojvodina on a “home-made” phantom. The consistency of acquisition technical parameters and image quality indicators, metrics, and artifacts were assessed by generating images of the test phantom. Test phantom was formed by a uniform attenuator plate made of 10 cm × 10 cm, 2 mm thick square sheet of copper, and target plate consisted of a 5mm thick polymethyl methacrylate (PMMA) carrier, and 4mm thick aluminum and 2mm thick copper inserts, as proposed by the IAEA. Generated images were analyzed using the ATIA software available for download for all Member States via the IAEA Human Health Campus. In boron neutron capture therapy (BNCT), treatment planning images are acquired in the recumbent position. However, treatment is occasionally performed in the sitting position. For BNCT treatment planning, we investigated the usability of cone-beam computed tomography (CBCT) images using digital radiography equipment that allows imaging in the sitting position. The dose calculation results in both CBCT and fan beam CT were in good agreement. Abstract

Visualizing the internal structure of museum objects is a crucial step in acquiring knowledge about the origin, state, and composition of cultural heritage artifacts. Among the most powerful techniques for exposing the interior of museum objects is computed tomography (CT), a technique that computationally forms a 3D image using hundreds of radiographs acquired in a full circular range. However, the lack of affordable and versatile CT equipment in museums, combined with the challenge of transporting precious collection objects, currently keeps this technique out of reach for most cultural heritage applications. We propose an approach for creating accurate CT reconstructions using only standard 2D radiography equipment already available in most larger museums. Specifically, we demonstrate that a combination of basic X-ray imaging equipment, a tailored marker-based image acquisition protocol, and sophisticated data-processing algorithms, can achieve 3D imaging of collection objects without the need for a costly CT imaging system. We implemented this approach in the British Museum (London), the J. Paul Getty Museum (Los Angeles), and the Rijksmuseum (Amsterdam). Our work paves the way for broad facilitation and adoption of CT technology across museums worldwide. Visualising the structure of museum objects is a crucial step in understanding the origin, state, and composition of cultural heritage artifacts. Denna rapport presenterar utvecklingen av ett mätverktyg för kontrollmätning av överensstämmelse mellan ljusfält och röntgenstrålfält på ett konventionellt röntgensystem. Projektet resulterade i en prototyp som använder CCD- linjesensorer för att mäta ljusintensitet och identifiera fältkanter. I en sensoruppsättning är den ena av två sensorer exponerad genom ett hål i höljet medan den andra är inkapslad och täckt med en tunn skärm av fluorescerande material. Prototypen är en fristående batteridriven enhet med ett eget interface där en TFT skärm och knappar används för navigering i menyer. Prototypen beräknar avvikelsen med en säkerhet på <1mm (teoretiskt lägre). Alla komponenter är monterade i ett 3D-printat hölje. Arbetet utfördes som ett projekt på uppdrag av Landstinget i Värmland som tillhandahållit med stöd och röntgenutrustning för testmätningar. Projektet omfattar kretskortskonstruktion, 3D-design i CAD, elektronikkonstruktion och mjukvarudesign i programmeringsspråket C., This report presents the development of a measurement device for field congruence measurement between light and x-ray field at conventional radiography equipment. The project resulted in a prototype which uses CCD-line sensors to measure light intensity and determine field edges. In a sensor set one sensor is exposed through a hole in the protective casing while the second sensor is incased and covered with a thin screen of fluorescent material. The prototype is a stand-alone, battery powered unit with an interface were a TFT-screen and buttons is used to navigate in menus. The prototype calculate the congruence within <1mm certainty (theoretically lower). All components are mounted in a 3D printed casing. This work has been executed as a project on behalf of the County Council of Värmland, who provided with support and x-ray equipment for test measurements. The project includes PCB-construction, 3D-design in CAD, electronics construction and software development in the programing language C.

Abstract

Purpose: Weight-bearing (WB) radiographs are commonly used to judge stability of type B fibula fractures and guide the choice of treatment. Stable fractures can be treated conservatively, and unstable fractures surgically. The question is raised how much weight patients actually put on their broken ankle while making a WB radiograph. The current study will give insight in the actual amount of WB in WB radiographs. Methods: In this retrospective cohort study, 57 patients with a type B fibula fracture with a medial clear space (MCS) < 6 mm on regular mortise (RM) view who underwent a WB radiograph were included. We designed a ramp with a scale in the plateau where the radiographs were taken. Total body weight (TBW) and amount of WB on the fractured limb were measured. Results: The mean WB on the fractured limb was 49 (13–110) kg and the mean TBW was 79 (45–128) kg, calculating a mean percentage of WB of 63. The mean MCS on the RM radiograph was 3.0 mm, compared to 2.9 mm on the WB radiograph. The mean superior clear space (SCS) was 3.2 mm on the RM view, compared to 3.2 mm on the WB radiograph as well. The average fibular dislocation was 1.5 mm on the RM radiograph, compared to 1.6 mm on the WB radiograph. Conclusion: There is a big variability in the amount of weight-bearing on the ankle when a WB radiograph is made. IntroductionDefence Medical Services, supporting CBRN Casualty Care, requested that Dstl consider the use of a crude radiological dispersal device (‘dirty bomb’) in a mass casualty scenario, and the effect of a radioactive fragment embedded in a patient on a diagnostic medical radiograph, as well as the impact on the patient and surgical team. Initial work showed an unpredicted white ‘bloom’ artefact, opposite to the expected darkening; subsequently this trial empirically tested all Defence mobile X-ray systems (DRagon, DART, and DR-Go).MethodMeasurements were taken with DART equipment; a 600 GBq sealed iridium-192 source at varying heights to simulate different activities; and a caesium iodide flat panel detector. The width of the resulting artefact was plotted against source height, to determine a threshold dose rate at which the bloom was indistinguishable. From this, minimum detectable activities (MDAs) were calculated for several fragment depths.ResultsThe MDAs, without the scatter reduction grid (MDAs with it are higher), for radioactive fragments 0.5 cm and 24.5 cm from the detector are 0.79 ± 0.06 GBq and 1900 ± 140 GBq, respectively. Other effects are discussed such as inconclusive results on changing exposure factors, shadows and an unaffected central region at high dose rates. Anatomically representative images were also taken with a test phantom for future clinical training use.ConclusionEven for fragment activities below the MDA, surgical teams are likely to approach occupational dose limits within an hour, and in some cases, minutes. Patient images could appear normal, with the injury site still receiving several hundred Grays, having severe impacts on patient outcomes and clinical decision making, such as amputation. This work has contributed to surgical and CBRN medical training, planning recommendations in NHS Emergency Preparedness procedures, and will support further work to develop UK and international clinical guidelines, refine dose thresholds, and investigate other imaging modalities.

Abstract

Background: Flexion instability is a common cause for revision after total knee arthroplasty (TKA); however, little objective criteria exist to determine excessive laxity in flexion. This study sought to determine the reliability of stress radiographs for flexion laxity using manual stress as well as a commercially available flexion stress device, with the hypothesis that a commercially available force device would provide increased translation compared to manual stress, and radiographic measurements would be reproducible. Methods: Ten patients who previously underwent TKA with non-hinged components were prospectively and consecutively enrolled at a single center to undergo stress radiographs. Three lateral radiographs with the knee at 90° of flexion were obtained for each patient: rest, commercial stress device at 150N, and manual stress. Calibrated radiographs were evaluated by two raters, and inter-rater and intra-rater reliability were determined using intraclass correlation coefficients (ICC). Results: Ten patients (seven female) with mean age 72 (range 55–82) years and average duration from surgery 36 (range 12–96) months were evaluated. The commercial stress device provided significantly less anterior translation than manual stress (− 0.3 mm vs. 3.9 mm; p < 0.01). Two patients reported pain with use of the stress device. Inter-observer reliability of measurements was good for commercial stress (ICC = 0.86) and excellent for manual stress (ICC = 0.94). Eighty-five percent of measurements were within 1 mm between observers. Intra-observer reliability of measurements was good to excellent for both the stress device and manual stress. Conclusions: Lateral stress radiographs may assist in the objective evaluation of flexion instability. A commercially available product provided less translation than manual stress; however, measurements were reliable and reproducible between observers. Abstract

Introduction A hospital environment can be a significant burden and a health risk especially for dementia patients. Mobile x-ray equipment (ME) is used to enable imaging of these patients at home. The aim was to compare image quality (IQ) of chest, hip and pelvis images from ME to the stationary equipment (SE) used in a hospital department. Methods We analysed examinations of the chest (n = 20), hip (n = 64) and pelvis (n = 32). Images were equally obtained from each setting of ME and SE. All images were graded using Visual Grading Analysis (VGA) by three radiographers (hip and pelvis) and three radiologists (chest). Technical IQ assessment was done by 80 additional images of a Contrast-Detail Radiography phantom (CDRAD). Results All chest images were approved for diagnostic use, as well as the hip AP and pelvis images from SE. ‘Approved proportion of ME images was for HIP antero-posterior (AP) and pelvis, 78% [95% CI: 52–94%] and 81% [95% CI: 54–96%] respectively. Hip axial had an overall low, but not significant different approval rate. Ordered logistic regression indicated higher IQ of HIP AP and pelvic images from SE. This contrasts that the CDRAD substudy indicated better IQ, expressed as IQFinv, from ME. Conclusion The VGA showed higher IQ for the SE system, while the CDRAD showed higher IQ for the ME system. Implications for practice Dementia patients can be examined at their home if the acquisition is optimised according to image quality in conjunct to radiation dose. Performing imaging out of the hospital and coordinating the patients’ further treatment are new work areas for radiographers and requires excellent communication skills.
