Abstract Introduction Otitis media is an umbrella term for middle ear inflammation; ranging from acute infection to chronic mucosal disease. It is a leading cause of antimicrobial therapy prescriptions and surgery in children. Despite this, treatments have changed little in over 50 years. Research has been limited by the lack of physiological models of middle ear epithelium. Methods We develop a novel human middle ear epithelial culture using an air‐liquid interface (ALI) system; akin to the healthy ventilated middle ear in vivo. We validate this using immunohistochemistry, immunofluorescence, scanning and transmission electron microscopy, and membrane conductance studies. We also utilize this model to perform a pilot challenge of middle ear epithelial cells with SARS‐CoV‐2. Results We demonstrate that human middle ear epithelial cells cultured at an ALI undergo mucociliary differentiation to produce diverse epithelial subtypes including basal (p63+), goblet (MUC5AC+, MUC5B+), and ciliated (FOXJ1+) cells. Mature ciliagenesis is visualized and tight junction formation is shown with electron microscopy, and confirmed by membrane conductance. Together, these demonstrate this model reflects the complex epithelial cell types which exist in vivo. Following SARS‐CoV‐2 challenge, human middle ear epithelium shows positive viral uptake, as measured by polymerase chain reaction and immunohistochemistry. Conclusion We describe a novel physiological system to study the human middle ear. This can be utilized for translational research into middle ear diseases. We also demonstrate, for the first time under controlled conditions, that human middle ear epithelium is susceptible to SARS‐CoV‐2 infection, which has important clinical implications for safe otological surgery. Objective. To present a case of otic Langerhans' cell histiocytosis in an adult. Also included the diagnosis and management of the condition and a review of the relevant literature. Case Report. We report a case of a 41-year-old man with a history of persistent unilateral ear discharge associated with an aural polyp. Radiological imaging showed bony lesions of the skull and a soft-tissue mass within the middle ear. Histological analysis of the polyp demonstrated Langerhans' cell histiocytosis. His otological symptoms were completely resolved with the systemic therapy. Conclusions. Otic Langerhans' cell histiocytosis can present in adults. Persistent ear symptoms along with evidence of soft-tissue masses within the ear and bony lesions of the skull or elsewhere should prompt the otolaryngologists to include Langerhans' cell histiocytosis in their differential diagnosis. Management should be with systemic therapy rather than local surgical treatment.

At young ages, a few extra months of development can make a big difference in size, strength, and athletic ability. A child who turns 5 years old in January will be nearly 20% older by the time a child born in December has their 5th birthday. In many sports, including hockey, children born in the early months of the calendar year get noticed by their coaches because of the superiority they demonstrate due to their age advantage. As a result, boys born early in the year are more likely to reach the professional ranks of the National Hockey League (NHL). The phenomenon just described has been labeled the relative age effect (RAE). Previous work studying the RAE in the NHL has focused on individual NHL seasons, often encompassing many of the same players across multiple seasons. We investigate the RAE using complete data on every player who has ever played in the NHL. We focus the majority of our analysis on Canadian born players and examine the RAE across hockey position and hall-of-fame status. For the first time, we provide strong evidence of when the RAE began to manifest itself in Canada. Our change point analysis indicates that the RAE began for players born since 1951. Finally, we make a case for what initiated this change in the way young hockey players develop, particularly in Canada, which produced over 90% of NHL players at that time. [ABSTRACT FROM AUTHOR]

On March 30, 2008, Samuel Arbesman and Steven Strogatz had their article "A Journey to Baseball's Alternate Universe" published in The New York Times. They simulated baseball's entire history 10,000 times to ask how likely it was for anyone in baseball history to achieve a streak that is at least as long as Joe DiMaggio's hitting streak of 56 in 1941. Arbesman and Strogatz treated a player's at bats per game as a constant across all games in a season, which greatly overestimates the probability of long streaks. The simulations in this paper treated at-bats in a game as a random variable. For each player in each season, the number of at-bats for each simulated game was bootstrapped. The number of hits for player i in season j in game k is a binomial random variable with the number of trials being equal to the number of at bats the player gets in game k and the probability of success being equal to that player's batting average for that season. The result of using non-constant at-bats in the simulation was a decrease in the percentage of the baseball histories to see a hitting streak of at least 56 games from 42% (Arbesman and Strogatz) to approximately 2.5%. The challenges of gaining access to opportunities to use the target language in contexts of study abroad, international education, and migration are well documented. In this Sydney-based study, these are reconceptualized as challenges of access to English in an English-speaking but increasingly multilingual, city. The notion of "context" in overseas study is also reconceptualized in spatial terms as the environmental circumstances of students' everyday lives, which are partly fixed in advance of their arrival but also shaped by their agency. A narrative case study of a recently arrived international student's daily activities and language use draws on online diary and stimulated recall interview data. The case study illustrates both the importance of the spatial dimensions of her everyday life to her use of languages and the how she manages these spatial dimensions to enhance her access to opportunities for English language use and learning. Does a young football (soccer) player’s birthdate affect his prospects in the sport? Scholars have found a correlation between early births in the competition year among young players within the same cohort and improved chances in sports as they advance to other stages. This article is one of the first studies to ask this question about a male premier league in Latin America - the Argentinian ‘A’ league. It uses a large-N data-set of all players in the period 2000-2012, around 3000 players. The article finds a large effect of the player’s relative age on his prospect to become a professional, though the effect is only present in the case of Argentinian-born players. The effect evaporates once a set of measures are employed to compare professional players with one another. The article contributes to the discussion of the biased effect of seemingly neutral institutional policies, and its conclusions may shed light in other areas. [ABSTRACT FROM AUTHOR]

Codesign allows a design team to combine two sets of knowledge that are key to service design: Customer insights into latent user needs and in-house professionals’ conversion of promising new ideas into viable concepts. While some studies highlight the potential of codesign, others are more skeptical pointing to a lack of clarity over how the involvement of customers affects the design process and outcomes. This article addresses this knowledge gap by reporting on a real-world comparison of design concepts generated by codesign teams with those generated by an in-house professional team and a team solely made up of users in the course of a library service ideation contest. The comparison indicates that codesign teams generate concepts that score significantly higher in user benefit and novelty but lower in feasibility. However, these outcomes are only possible in cohesive teams that develop design concepts collaboratively. In contrast, in teams where individuals dominate, conflict, less collaboration, and diminished innovation outcomes are more likely. The findings add to a better understanding of the value of codesign and shed light on the complex relationship between design team composition, intrateam factors, and innovation outcomes. Objective: To explore the experience of psychological distress and well-being in emergency medicine (EM) consultants.Methods: A qualitative, interpretative phenomenological analysis (IPA) study based on 1:1 semistructured interviews with EM consultants working full time in EDs across South West England. Eighteen EM consultants were interviewed across five EDs, the mean (SD) age of participants being 43.17 (5.8) years. The personal meanings that participants attached to their experiences were inductively analysed.Results: The analysis formed three superordinate themes: systemic pressures, physical and mental strain and managing the challenges. Pressures within the ED and healthcare system contributed to participants feeling undervalued and unsatisfied when working in an increasingly uncontrollable environment. Participants described working intensely to meet systemic demands, which inadvertently contributed to a diminishing sense of achievement and self-worth. Consultants perceived their experience of physical and emotional strain as unsustainable, as it negatively impacted; functioning at work, relationships, personal well-being and the EM profession. Participants described how sustainability as an EM consultant could be promoted by social support from consultant colleagues and the ED team, and the opportunity to develop new roles and support ED problem solving at an organisational level. These processes supported a stigma-reducing means of promoting psychological well-being.Conclusions: EM consultants experience considerable physical and mental strain. This strain is dynamically related to consultants' experiences of diminishing self-worth and satisfaction, alongside current sociopolitical demands on EM services. Recognising the psychological experiences and needs of EM consultants and promoting a sustainable EM consultant role could benefit individual psychological well-being and the delivery of emergency care.
