4. Pharmacological Treatment of Obstructive Sleep Apnea

Abstract

Obstructive sleep apnea (OSA) is a growing public health hazard fueled by the obesity epidemic and an aging population. Untreated sleep apnea can result in significant consequences both in the short-term and long-term. We need to educate the public to recognize the symptoms of sleep apnea and to publicize that effective treatments are available. Positive airway pressure therapy remains the gold standard currently in treating OSA. Alternative treatments include an oral appliance or surgical options. This paper discusses the pharmacologic treatment of sleep apnea: goals include medications to address the ventilatory control of breathing, treat co-morbid diseases, treat associated health problems/complaints, address special issues, such as anesthetic precautions, and propose future targets.

9. RR Lyrae search and stellar populations study in Canis Major: Preliminary Results

Abstract

We present preliminary results of a RR Lyrae star search and stellar populations study performed in the Canis Major overdensity, spanning an area of 8.35sq deg. The observations were made in R and V bands, with the QUEST camera installed in the 1m Jurgen Stock Telescope, at the Venezuela National Observatory. The resulting Hess diagram shows a possible, but weak, red giant branch and no obvious horizontal branch, red clump or main sequence turnoff. After a multi--epoch photometric search, 6 RR Lyrae stars were confirmed with further observations obtained at the 1.0 and 1.3m telescopes of the SMARTS consortium at CTIO. Of these confirmed RR Lyrae stars, 5 have heliocentric distances between 5 and 7 kpc. Confirmation of their physsical association with the Canis Major system awaits for a study of their radial velocities., Comment: 4 pages, 10 figures. Abstract

The Rac-GEF, P-Rex1, activates Rac1 signaling downstream of G protein-coupled receptors and PI3K. Increased P-Rex1 expression promotes melanoma progression; however, its role in breast cancer is complex, with differing reports of the effect of its expression on disease outcome. To address this we analyzed human databases, undertook gene array expression analysis, and generated unique murine models of P-Rex1 gain or loss of function. Analysis of PREX1 mRNA expression in breast cancer cDNA arrays and a METABRIC cohort revealed that higher PREX1 mRNA in ER+ve/luminal tumors was associated with poor outcome in luminal B cancers. Prex1 deletion in MMTV-neu or MMTV-PyMT mice reduced Rac1 activation in vivo and improved survival. High level MMTV-driven transgenic PREX1 expression resulted in apicobasal polarity defects and increased mammary epithelial cell proliferation associated with hyperplasia and development of de novo mammary tumors. MMTV-PREX1 expression in MMTV-neu mice increased tumor initiation and enhanced metastasis in vivo, but had no effect on primary tumor growth. Pharmacological inhibition of Rac1 or MEK1/2 reduced P-Rex1-driven tumoroid formation and cell invasion. Therefore, P-Rex1 can act as an oncogene and cooperate with HER2/neu to enhance breast cancer initiation and metastasis, despite having no effect on primary tumor growth. [ABSTRACT FROM AUTHOR]

Abstract

Hyperspectral stimulated Raman scattering microscopy is deployed to measure single-membrane vibrational spectrum as a function of membrane potential. Using erythrocyte ghost as a model, quantitative correlation between transmembrane potential and Raman spectral profile was found. Specifically, the ratio between the area under Raman band at ~2930 cm-1 and that at ~2850 cm-1 increased by ~2.6 times when the potential across the erythrocyte ghost membrane varied from +10mV to -10 mV. Our results show the feasibility of employing stimulated Raman scattering microscopy to probe the membrane potential without labeling. [ABSTRACT FROM AUTHOR]

Abstract

Native chromatin contains valuable genetic, epigenetic and structural information. Though DNA and nucleosome structures are well defined, less is known about the higher-order chromatin structure. Traditional chromatin extraction methods involve fixation, fragmentation and centrifugation, which might distort the higher-order structural information of native chromatin. We present a simple approach to isolate native chromatin from cultured mammalian cells using salicylic acid coated magnetic nanoparticles (SAMNPs). Chromatin is magnetically separated from cell lysates without any filtration or high-speed centrifugation. The purified chromatin is suitable for the examination of histone modifications and other chromatin associated proteins as confirmed by western blotting analysis. The structure of chromatin was determined by confocal fluorescence microscopy, transmission electron microscopy (TEM) and atomic force microscopy (AFM). High-resolution AFM and TEM images clearly show a classical bead-on-a-string structure. The higher-order chromatin structure is also determined via electron microscopy. Our method provides a simple, inexpensive and an environmentally friendly means to extract native chromatin not possible before, suitable for both biochemical and structural analysis. Abstract

Research on peptides continues to generate much interest among investigators in the field of science, biology and medicine. Rapidly increasing knowledge of the physiological roles of peptides has promoted other investigations to explain their clinical aspects. Peptides play pivotal roles as mediators of neurons, endocrine cells and immune cells, and they regulate neuro-immuneendocrine cross-talk. A clearer discernment of this communication is dramatically altering our understanding of physiology and it may also profoundly affect the treatment of wide variety of diseases. A growing body of recent literature indicates that peptide and nonpeptide antagonist of some peptide are being developed for clinical applications. However, there are many other peptide the clinical importance of which is experimentally evident from reports, and they could be a good candidate for a novel drug design in the future along the lines of VIP, NGF, leptin and bradykinin. The first review entitled “Immunology of VIP: A Review and Therapeutically Perspectives” by R.P . Gomariz, C. Martinez, C. Abad, J. Leceta and M. Delgado describes VIP receptors and signaling transduction in the immune cells, the effect of VIP on cytokine production, therapeutical application of YIP for inflammatory and immune disorders. In the next article entitled “Nerve Growth Factor in neurological and Non Neurological Diseases: Basic Findings and Emerging Pharmacological Prospective” by L. Aloe, P. Tirassa, L. Bracci- Laudiero addresses previous and ongoing evidence supporting the role of nerve growth factor (NGF) in nervous and immune system. Pharmacological role of NGF in certain peripheral and central neurological as well as non- neurological disorders is also discussed. The third article entitled “Leptin Transport Across the Blood-Brain-Barrier: Implications for the Cause and Treatment of Obesity: by W .A. Banks explores the relation between leptin and obesity. Leptin transport across the blood-brain-barrier and strategies to overcome decreased leptin transport at the blood-brain-barrier are also reviewed. In the final article entitled “Kinin Receptors and Their Antagonist as Novel Therapeutic Agents” written by Y. Ozturk described physiological and pathophysiological actions of kinins along with their receptors and post receptors events in the cellular signaling. Recent development in the design of kinin receptor antagonists is also presented. I appreciate the editor of this Journal, Professor Atta-ur-Rahman, for his leadership and I am honored to be invited as the Guest Editor. I am also deeply grateful to the authors for the excellent contributions and for setting aside time to prepare these valuable articles.
