11. (CS08) Long-Term Follow-Up of Cladribine Tablets in a Real-World Aging Patient Population.

Abstract

BACKGROUND: Cladribine, a deoxyadenosine analogue prodrug that preferentially depletes lymphocytes, may disrupt the central immune cascade in persons with multiple sclerosis (MS). Cladribine tablets (CladT) are a short-course, oral disease-modifying drug (DMD) administered in 4 courses of 4 to 5 days each over 2 years, with no treatment in years 3 and 4. CladT are indicated for the treatment of adults with relapsing forms of MS and active secondary progressive MS. OBJECTIVES: Our study aims to present real-world experience in aging patients with MS previously treated with other DMDs switched to CladT. Understanding the benefit/risk profile of using a noncontinuous immunosuppressive DMD in this population is of the utmost importance given immunosenescence and the increased risk of comorbidities with age. Outcomes of particular interest are potential risk of disease reactivation, durable effectiveness, and safety beyond 3 years of starting CladT. METHODS: Prospective and retrospective chart review of patients starting CladT who received care from the MS Center of Sarasota in Florida and were followed through October 2023. RESULTS: We report on 75 patients and a subgroup of those older than 50 years (n = 40) who have initiated therapy with CladT, all previously treated with a DMD. Mean age at CladT initiation was 51.9 years for all patients and 60.7 years for the older subgroup. Median disease duration was 11 and 13.5 years, respectively. Mean duration follow-up was 1125 days, with nearly 70% of patients in stratification years 3, 4, or 5 at data cutoff. Overall, CladT were well tolerated, with lymphopenia, upper respiratory tract infection, urinary tract infection, fatigue, and headache occurring in ≤ 5% of patients in both groups. Through the available follow-up period, > 85% of all patients were relapse free, had a stable or improved MRI result, and had an annualized relapse rate of 0.11 in year 2 and 0.03 in year 3. CONCLUSIONS: In this cohort of patients initiating therapy with CladT in a real-world setting, the treatment was well tolerated, with no new safety signals and durable efficacy beyond the 2-year treatment course, even in an aging patient population where comorbid conditions and immunosenescence may increase.
