Twenty percent of the cartilaginous tumors of bone are in the distal femur. This presents a challenge for treating patients with knee OA who also have ipsilateral distal femur cartilage tumors. We propose a classification system for knee OA with ipsilateral cartilaginous tumor and a treatment approach to address this issue. Intramedullary guides are avoided when performing total knee arthroplasty (TKA) in these patients so as not to further contaminate the femur with tumor cells. A non-image-based stereotactic surgical system is favored at our institution to achieve this goal. Seven patients underwent classification and treatment with TKA utilizing this approach. Average final follow-up was 15.3 months. Component alignment averaged 89.2° and all patients had 0° of extension with an average flexion to 107.5°. There were no postoperative complications and no radiographic evidence of component complication, tumor recurrence, or tumor expansion. Few studies have reported on the treatment of knee OA with concurrent cartilaginous tumor of the distal femur. All patients treated with the proposed approach had their OA successfully treated without complication related to their cartilaginous tumor or TKA components. Abstract

Background: A unique enzootic focus of Mycobacterium bovis in free-ranging deer was identified in northern lower Michigan in 1994, with subsequent evidence of transmission to local cattle herds. Between 2002 and 2017, 3 Michigan deer hunters with M. bovis disease were previously reported. We present 4 additional human cases linked to the zoonotic focus in deer, utilizing genomic epidemiology to confirm close molecular associations among human, deer and cattle M. bovis isolates., Methods: Identification of human tuberculosis (TB) cases with cultures of M. bovis was provided from the Michigan Department of Health and Human Services (MDHHS) tuberculosis database. Clinical review and interviews focused on risk factors for contact with wildlife and cattle. Whole genome sequences of human isolates were compared with a veterinary library of M. bovis strains to identify those linked to the enzootic focus., Results: Three confirmed and 1 probable human case with M. bovis disease were identified between 2019 and 2022, including cutaneous disease, 2 severe pulmonary disease cases, and human-to-human transmission. The 3 human isolates had 0-3 single-nucleotide polymorphisms (SNPs) with M. bovis strains circulating in wild deer and domestic cattle in Michigan., Conclusions: Spillover of enzootic M. bovis from deer to humans and cattle continues to occur in Michigan. Future studies should examine the routes of transmission and degree of risk to humans through expanded epidemiological surveys. 11. Wildlife vaccination strategies for eliminating bovine tuberculosis in white-tailed deer populations.

Abstract

Many pathogens of humans and livestock also infect wildlife that can act as a reservoir and challenge disease control or elimination. Efficient and effective prioritization of research and management actions requires an understanding of the potential for new tools to improve elimination probability with feasible deployment strategies that can be implemented at scale. Wildlife vaccination is gaining interest as a tool for managing several wildlife diseases. To evaluate the effect of vaccinating white-tailed deer (Odocoileus virginianus), in combination with harvest, in reducing and eliminating bovine tuberculosis from deer populations in Michigan, we developed a mechanistic age-structured disease transmission model for bovine tuberculosis with integrated disease management. We evaluated the impact of pulse vaccination across a range of vaccine properties. Pulse vaccination was effective for reducing disease prevalence rapidly with even low (30%) to moderate (60%) vaccine coverage of the susceptible and exposed deer population and was further improved when combined with increased harvest. The impact of increased harvest depended on the relative strength of transmission modes, i.e., direct vs indirect transmission. Vaccine coverage and efficacy were the most important vaccine properties for reducing and eliminating disease from the local population. By fitting the model to the core endemic area of bovine tuberculosis in Michigan, USA, we identified feasible integrated management strategies involving vaccination and increased harvest that reduced disease prevalence in free-ranging deer. Few scenarios led to disease elimination due to the chronic nature of bovine tuberculosis. 12. Evaluating Children's Use of Symbol in Some Recent Research

Abstract

This paper explores a number of examples of what appeared to be recurring symbolic expressions of spirituality found in my recent research into the spiritual experiences of children in Victorian State primary schools. These expressions appeared in drawings and in conversation. In this paper I use hermeneutic phenomenology and a multidisciplinary approach to the literature, to explore the nature of symbol, in particular, the symbols of island, snake and mountain. I examine the children's use of these symbols, with their multiple meanings and function, to gain a greater understanding of their individual and collective spirituality and well-being. This study can have applications in religious education. Through their symbolic expressions children may be enabled to explore meaning in their lives, and advance in their spiritual development. Moreover, examination by children of their own symbols may provide a bridge to understanding and exploring core ideas of religious faith, which are mainly expressed in metaphoric language. Exploring symbols can provide a way for children to exercise the imagination, grounded in bodily experience, to achieve open and enriching spiritual outcomes. (Contains 5 figures.)

Abstract

The workshop on the production of swept-area estimates for all hauls in DATRAS for biodiver-sity assessments (WKSAE-DATRAS) considered three groups of surveys for which data are sub-mitted to the Database of Trawl Surveys (DATRAS): various Beam Trawl Surveys, the Northeast Atlantic International Bottom Trawl Survey (Northeast Atlantic IBTS), and the North Sea Inter-national Bottom Trawl Survey (North Sea IBTS). All countries contributing to the above-mentioned surveys were represented by at least one par-ticipant during the workshop, apart from the Netherlands and Norway. The main objectives of the workshop were to establish tow-by-tow swept-area estimates for time-series as far back in time as possible, compare different approaches for the estimates of missing observations, and harmonize the resulting dataseries for biodiversity assessments. For all of the surveys considered, problems with data quality were detected. This included the Beam Trawl Surveys but was most pronounced for the North Sea IBTS. Outliers and potential erroneous data were listed for reporting back to the respective national institutes. In particular, missing observations or algorithms affected wing spread-based swept-area, which is needed in several applications. This workshop compared the Marine Scotland Science-MSS/OSPAR approach, which includes a data quality check for the information needed for the calculation of swept-area, and the DATRAS approach, which depends solely on correctly reported data from the national institutes. Larger data gaps were identified, in particular for several years of the North Sea IBTS. For those surveys, it is proposed that the best possible way forward at this moment is to use estimates based on the MSS/OSPAR approach. However, if dubious records (i.e. extreme outliers) were identified by the MSS/OSPAR and no other information was available, values (e.g. speed over ground or the depth at which a change from short to long sweeps should have happened) were taken from the manual. However, expe-rience has shown that the survey manuals are not followed in all instances, and so persistent country-specific and survey-specific deviations may occur. The national institutes are encouraged to check, correct, and fill in missing survey data through re-submissions to DATRAS. It is recommended that DATRAS data quality control on data sub-mission is extended for the information needed for the calculation of swept-area (e.g. distance, depth, door spread, and wing spread) and that this is done in close cooperation between the ICES Data Centre and the respective ICES survey working groups, WGBEAM (Working Group on Beam Trawl Surveys) and IBTSWG (International Bottom Trawl Survey Working Group).

Abstract

The 2019 meeting of WGECO was held at the International Council for the Exploration of the Sea, Copenhagen, Denmark from 8–16 April 2019. The meeting was attended by 16 delegates from eight countries and was chaired by Stefán Ragnarsson (Iceland) and Jeremy Collie (USA). The work conducted centred on three Terms of Reference that had been made by WGECO. In addition, a list of sensitive species prepared by WGBYC was reviewed. WGECO continued the work initiated in 2018 to examine the ecological consequences of stock rebuilding, with emphasis on benthivorous fish (ToRa). Two case studies were carried out to estimate the predation pressure of fish on benthos. In one, the consumption of benthic invertebrates by demersal fish on the US continental shelf regions was compared with their biomass in sediments. The estimated annual consumption of benthic organisms was a small proportion (<5%) of total abundance for most taxa but as high as 25% for some prey. Benthic food resources do not appear to be limiting the feeding of benthivorous fishes. The second study examined the temporal trends in diet composition and consumption rates of haddock, which has experienced a northerly shift in abundance in Icelandic waters. Some prey types showed clear trends from 2006 to 2019, while others had more variable patterns. Consumption rates of benthic invertebrates, fish and zooplankton/natantia prey types were calculated. The much greater consumption of fish prey relative to the other two prey groups was of interest. WGECO scrutinized methods to estimate density-dependent effects on fish growth and made suggestions on its estimation from stock assessment or survey data. A simulation study showed that the age-specific growth increment may not be a useful variable to test for density-dependent growth, because it depends on body size. Abstract

For effective wild salmon (Salmo salar) conservation in areas where aquaculture of salmon is practiced it is necessary to identify where the key parasite, the salmon louse (Lepeophtheirus salmonis), will have an impact on these wild salmon. A simple modelling structure is implemented in a sample system in Scotland for assessing interaction between wild salmon and salmon lice from salmon farms. The model is demonstrated for case studies of smolt sizes and migration routes through salmon lice concentration fields derived for average farm loads from 2018 to 2020. Lice modelling describes production and distribution of lice, infection rates on hosts and biological development of lice. The modelling framework allows explicit assessment of the relationships between lice production, lice concentration and impact on hosts as they grow and migrate. Lice distribution in the environment is determined using a kernel model, which summarises mixing in a complex hydrodynamic system. Smolt modelling describes their initial size, growth and migration pathways. This is illustrated for a set of parameter values applied to 10 cm, 12.5 cm and 15 cm salmon smolts. We found that salmon lice impact depends on initial size of host, smaller smolts will be more susceptible, while larger smolts are less impacted by a given number of lice encounters and migrate more rapidly. This modelling framework can be adapted to allow evaluation of threshold concentrations of lice in the water that should not be exceeded to avoid impacts on smolt populations., (Published by Elsevier B.V.)

28. Alteration in Levels of Specific miRNAs and Their Potential Protein Targets between Human Pancreatic Cancer Samples, Adjacent Normal Tissue, and Xenografts Derived from These Tumors.

Abstract

Herein, we describe the global comparison of miRNAs in human pancreatic cancer tumors, adjacent normal tissue, and matched patient-derived xenograft models using microarray screening. RNA was extracted from seven tumor, five adjacent normal, and eight FI PDX tumor samples and analyzed by Affymetrix GeneChip miRNA 4.0 array. A transcriptome analysis console (TAC) was used to generate comparative lists of up- and downregulated miRNAs for the comparisons, tumor vs. normal and F1 PDX vs. tumor. Particular attention was paid to miRNAs that were changed in the same direction in both comparisons. We identified the involvement in pancreatic tumor tissue of several miRNAs, including miR4534, miR3154, and miR4742, not previously highlighted as being involved in this type of cancer. Investigation in the parallel mRNA and protein lists from the same samples allowed the elimination of proteins where altered expression correlated with corresponding mRNA levels and was thus less likely to be miRNA regulated. Using the remaining differential expression protein lists for proteins predicted to be targeted for differentially expressed miRNA on our list, we were able to tentatively ascribe specific protein changes to individual miRNA. Particularly interesting target proteins for miRs 615-3p, 2467-3p, 4742-5p, 509-5p, and 605-3p were identified. Prominent among the protein targets are enzymes involved in aldehyde metabolism and membrane transport and trafficking. These results may help to uncover vulnerabilities that could enable novel approaches to treating pancreatic cancer.

Abstract

Background: The efficacy and safety of primary re-irradiation for MSCC are not known. Our aim was to establish the efficacy and safety of biologically effective dose-based re-irradiation., Methods: Patients presenting with MSCC at a previously irradiated spine segment, and not proceeding with surgical decompression, were eligible. A 3 Gray per fraction experimental schedule (minimum 18 Gy/6 fractions, maximum 30 Gy/10 fractions) was used, delivering a maximum cumulative spinal dose of 100 Gy 2 if the interval since the last radiotherapy was within 6 months, or 130 Gy 2 if longer. The primary outcome was a change in mobility from week 1 to week 5 post-treatment, as assessed by the Tomita score. The RTOG SOMA score was used to screen for spinal toxicity, and an MRI performed to assess for radiation-induced myelopathy (RIM)., Results: Twenty-two patients were enroled, of whom eleven were evaluable for the primary outcome. Nine of eleven (81.8%) had stable or improved Tomita scores at 5 weeks. One of eight (12.5%) evaluable for late toxicity developed RIM., Conclusions: Re-irradiation is an efficacious treatment for MSCC. The article examines Pascal's characterization of Augustinian theology as reconciling the contradictions between the erroneous alternative doctrines of Molinism and Calvinism. Augustinianism rejects what is false in each and preserves what is true in each. The article argues that Molina's theory of predestination does not in fact bear out Pascal's presentation of Molinism.

Abstract

Background: Migraine is a prevalent and chronic disease associated with high rates of disability and significant financial and socioeconomic burden. Current acute treatments for migraine attacks include both migraine-specific (e.g., triptans, ergotamines) and nonspecific (e.g., nonsteroidal anti-inflammatory drugs) medications; however, significant unmet treatment needs remain., Objectives: The authors sought to characterize the nature and drivers of unmet treatment needs in the acute treatment of migraine attacks and describe emerging migraine-specific treatments, that is, calcitonin gene-related peptide (CGRP) receptor antagonists., Data Sources: PubMed searches were conducted using search terms for studies of unmet migraine treatment needs and CGRP receptor antagonists. Additionally, studies presented at recent headache-focused congresses were included., Conclusions: Forty percent of people with migraine report at least 1 unmet treatment need. Many people are unable to use migraine-specific or nonspecific agents because of contraindications, precautions, and tolerability issues. Disease burden (disability, headache severity/frequency) remains high even in those receiving migraine-specific medications. The oral CGRP receptor antagonists, ubrogepant and rimegepant, demonstrated efficacy in reducing migraine pain, migraine-associated symptoms, and disability, with a low adverse event profile, similar to placebo., Implications for Practice: The availability and use of CGRP receptor antagonists may help reduce the extent of unmet needs in the treatment of migraine attacks, resulting in more patients receiving treatment and better outcomes for people with migraine. Nurse practitioners are well positioned to increase rates of migraine diagnosis/treatment (another key unmet need), using consensus guidelines to guide their approach., (Copyright © 2020 The Authors. Abstract

Pascal can be readily inserted into familiar characterizations of the emergence of modernity (man’s alienation from the cosmos). He himself theorized the proper relationship between innovation (legitimate in the physical sciences) and traditional authority (legitimate especially in theology). While accepting the mechanistic philosophy and in particular the rigid Cartesian distinction between immaterial and corporeal substance, he registers its divergence from our spontaneous understanding of experience; we find him encountering the view of one of his religious mentors that Cartesian science has deprived the cosmos of its spiritual significance as an image of the creator. His relationship to Latour’s conception of (non-)modernity is considered. Though many of his attitudes are uncongenial to modern secular humanism, his theologically-motivated rejection of the concept of nature as normative has surprising echoes in aspects of twenty-first century thought and culture.
