Abstract

Abstract Background New drugs to tackle the next pathway or mutation fueling cancer are constantly proposed, but 97% of them are doomed to fail in clinical trials, largely because they are identified by cellular or in silico screens that cannot predict their in vivo effect. Methods We screened an Adeno-Associated Vector secretome library (> 1000 clones) directly in vivo in a mouse model of cancer and validated the therapeutic effect of the first hit, EMID2, in both orthotopic and genetic models of lung and pancreatic cancer. Results EMID2 overexpression inhibited both tumor growth and metastatic dissemination, consistent with prolonged survival of patients with high levels of EMID2 expression in the most aggressive human cancers. Mechanistically, EMID2 inhibited TGFβ maturation and activation of cancer-associated fibroblasts, resulting in more elastic ECM and reduced levels of YAP in the nuclei of cancer cells. Conclusion This is the first in vivo screening, precisely designed to identify proteins able to interfere with cancer cell invasiveness. EMID2 was selected as the most potent protein, in line with the emerging relevance of the tumor extracellular matrix in controlling cancer cell invasiveness and dissemination, which kills most of cancer patients.

2. 424 Deciphering the Immune Landscape in Benign Breast Disease: Implications for Risk Stratification and Breast Cancer Prevention

Abstract

OBJECTIVES/GOALS: The objective of our research is to define unique molecular and immune markers in benign breast tissue to better identify women at risk of node-positive breast cancer (BC). The goal of the work is to improve individualized risk assessment, to guide targeted prevention and screening recommendations, and to reduce disease incidence and mortality. METHODS/STUDY POPULATION: From the Mayo Clinic’s Benign Breast Disease (BBD) cohort, we matched women who developed node-positive breast cancer after a BBD biopsy (cases; n=42) with women who remained cancer-free (controls; n=37), considering patient age and biopsy date. We used NanoString gene expression profiling to identify differentially expressed genes (DEGs) between cases and controls. We optimized a multiplex immunofluorescence (mIF) approach to simultaneously detect multiple markers within single FFPE tissue slides to correlate cells expressing DEGs in relation to innate and adaptive immune effectors. We used tissue segmentation, cell phenotyping, and spatial relationships to define molecular and immune differences between cases and controls. RESULTS/ANTICIPATED RESULTS: We discovered higher gene expression levels of IRF8 (interferon regulatory factor 8, a factor involved in immune cell differentiation) in controls as compared to cases (p = 0.0024) and found that IRF8 expression is also associated with longer cancer onset times among cases (p = 0.0012). Our pilot mIF experiments revealed higher frequencies of CD4+, CD8+, CD68+, CD20+ and CD11c+ cells in controls compared to cases. We predict that higher IRF8 expression and increased frequencies of immune cells in BBD biopsies indicate a proactive immune environment that may act to prevent cancer development. Furthermore, we predict that our analyses of the spatial localization of these markers by mIF may offer further predictive insight. DISCUSSION/SIGNIFICANCE: Deciphering the relationship between immune alterations in BBD and risk of node positive BC has the potential to improve individualized risk prediction. These insights will foster improved surveillance and informed screening and prevention, ultimately reducing BC incidence and mortality.

Abstract

Background There is mounting evidence on the existence of a Pediatric Inflammatory Multisystem Syndrome-temporally associated to SARS-CoV-2 infection (PIMS-TS), sharing similarities with Kawasaki Disease (KD). The main outcome of the study were to better characterize the clinical features and the treatment response of PIMS-TS and to explore its relationship with KD determining whether KD and PIMS are two distinct entities. Methods The Rheumatology Study Group of the Italian Pediatric Society launched a survey to enroll patients diagnosed with KD (Kawasaki Disease Group – KDG) or KD-like (Kawacovid Group - KCG) disease between February 1st 2020, and May 31st 2020. Demographic, clinical, laboratory data, treatment information, and patients’ outcome were collected in an online anonymized database (RedCAP®). Relationship between clinical presentation and SARS-CoV-2 infection was also taken into account. Moreover, clinical characteristics of KDG during SARS-CoV-2 epidemic (KDG-CoV2) were compared to Kawasaki Disease patients (KDG-Historical) seen in three different Italian tertiary pediatric hospitals (Institute for Maternal and Child Health, IRCCS “Burlo Garofolo”, Trieste; AOU Meyer, Florence; IRCCS Istituto Giannina Gaslini, Genoa) from January 1st 2000 to December 31st 2019. Chi square test or exact Fisher test and non-parametric Wilcoxon Mann-Whitney test were used to study differences between two groups. Results One-hundred-forty-nine cases were enrolled, (96 KDG and 53 KCG). KCG children were significantly older and presented more frequently from gastrointestinal and respiratory involvement. Cardiac involvement was more common in KCG, with 60,4% of patients with myocarditis. 37,8% of patients among KCG presented hypotension/non-cardiogenic shock. Coronary artery abnormalities (CAA) were more common in the KDG. The risk of ICU admission were higher in KCG. Lymphopenia, higher CRP levels, elevated ferritin and troponin-T characterized KCG. KDG received more frequently immunoglobulins (IVIG) and acetylsalicylic acid (ASA) (81,3% vs 66%; p = 0.04 and 71,9% vs 43,4%; p = 0.001 respectively) as KCG more often received glucocorticoids (56,6% vs 14,6%; p Conclusion Our study suggests that SARS-CoV-2 infection might determine two distinct inflammatory diseases in children: KD and PIMS-TS. Older age at onset and clinical peculiarities like the occurrence of myocarditis characterize this multi-inflammatory syndrome. Our patients had an optimal response to treatments and a good outcome, with few complications and no deaths.

Abstract

Health-related quality of life (HRQOL) after intestinal transplantation (IT) is important, as many psychological troubles have been reported in these patients on the long term. Our aim was to assess and compare HRQOL of patients after IT to patients after liver transplantation (LT) or on home parenteral nutrition (HPN) for intestinal failure. A cross-sectional study included patients and their parents between 10 and 18 years of age, on HPN for more than 2 years, or who underwent IT or LT, with a graft survival longer than 2 years. Quality of life was explored by Child Health Questionnaire. Thirteen children-parents dyads after IT, 10 after LT, and eight children on HPN completed the survey. Patients were a median age of 14 years old, a median of 10 years post-transplantation or on HPN. Patients after IT scored lower than patients after LT or on HPN in "social limitations due to behavioral difficulties" and in "behavior." They scored higher than those on HPN in "global health." Parents of children after IT scored lower than those after LT in many domains. No relevant correlation with clinical data was found. Our study showed the multi-level impact of IT on quality of life of patients and their parents. It highlights the importance of a regular psychological follow-up for patients, but also of a psychological support for families. Helping the patients to overcome the difficulties at adolescence may improve their mental health in adulthood.

Abstract

Electrochemiluminescence (ECL) is a leading technique in bioanalysis.[1] It is a luminescent phenomenon where the excited species are produced with an electrochemical stimulus rather than with a light excitation source. For this reason, ECL displays improved signal-to-noise ratio compared to photoluminescence, with minimized effects due to light scattering and luminescence background.[2] Chasing ever-increasing sensitivities, ECL can ideally be coupled to nanotechnology to develop new strategies for clinical applications and analyte determination also in real and complex matrices.[3] In particular, dye-doped silica nanoparticles (DDSNs) present many advantages: they can be easily synthesized, are hydrophilic and are prone to bioconjugation thanks to silica chemistry. With this approach we were able to achieve extremely bright systems and DDSNs allow the 8.5 fold increase of ECL intensity.[4] In DDSNs, light emission is influenced by the combination of several factors that make DDSNs complex multichromophoric structures. However, when ECL comes into play, the scenario is even more complicated due to the coreactant−NP interactions effect. Such complex scenario was approached at the theoretical level by developing suitable mechanistic models for ECL generation[5] while, at the same time, the influence of doping level on ECL efficiency was evaluated. Our results showed that the ECL intensity of a nanosized system cannot be merely incremented acting on doping, since other parameters come into play. These studies support the application in ECL microscopy providing valuable indications for the design of more efficient ECL nano- and micro-sized labels towards more sensitive ECL-based immunoassay for bioanalysis applications. Simple Summary: Multiplex digital spatial profiling is a newly emerging approach for investigating multiple RNA transcripts and proteins simultaneously in the context of intact tissue. This review describes the different spatial technologies for the study of breast cancer, providing examples of their applicability in research and clinical settings to guide diagnosis and prognosis and to inform treatment decisions. While RNA sequencing and multi-omic approaches have significantly advanced cancer diagnosis and treatment, their limitation in preserving critical spatial information has been a notable drawback. This spatial context is essential for understanding cellular interactions and tissue dynamics. Multiplex digital spatial profiling (MDSP) technologies overcome this limitation by enabling the simultaneous analysis of transcriptome and proteome data within the intact spatial architecture of tissues. In breast cancer research, MDSP has emerged as a promising tool, revealing complex biological questions related to disease evolution, identifying biomarkers, and discovering drug targets. This review highlights the potential of MDSP to revolutionize clinical applications, ranging from risk assessment and diagnostics to prognostics, patient monitoring, and the customization of treatment strategies, including clinical trial guidance. We discuss the major MDSP techniques, their applications in breast cancer research, and their integration in clinical practice, addressing both their potential and current limitations. Abstract

Background: New drugs to tackle the next pathway or mutation fueling cancer are constantly proposed, but 97% of them are doomed to fail in clinical trials, largely because they are identified by cellular or in silico screens that cannot predict their in vivo effect. Methods: We screened an Adeno-Associated Vector secretome library (> 1000 clones) directly in vivo in a mouse model of cancer and validated the therapeutic effect of the first hit, EMID2, in both orthotopic and genetic models of lung and pancreatic cancer. Results: EMID2 overexpression inhibited both tumor growth and metastatic dissemination, consistent with prolonged survival of patients with high levels of EMID2 expression in the most aggressive human cancers. Mechanistically, EMID2 inhibited TGFβ maturation and activation of cancer-associated fibroblasts, resulting in more elastic ECM and reduced levels of YAP in the nuclei of cancer cells. Conclusion: This is the first in vivo screening, precisely designed to identify proteins able to interfere with cancer cell invasiveness. EMID2 was selected as the most potent protein, in line with the emerging relevance of the tumor extracellular matrix in controlling cancer cell invasiveness and dissemination, which kills most of cancer patients. Abstract

Background: There is mounting evidence on the existence of a Pediatric Inflammatory Multisystem Syndrome-temporally associated to SARS-CoV-2 infection (PIMS-TS), sharing similarities with Kawasaki Disease (KD). The main outcome of the study were to better characterize the clinical features and the treatment response of PIMS-TS and to explore its relationship with KD determining whether KD and PIMS are two distinct entities. Methods: The Rheumatology Study Group of the Italian Pediatric Society launched a survey to enroll patients diagnosed with KD (Kawasaki Disease Group – KDG) or KD-like (Kawacovid Group - KCG) disease between February 1st 2020, and May 31st 2020. Demographic, clinical, laboratory data, treatment information, and patients' outcome were collected in an online anonymized database (RedCAP®). Relationship between clinical presentation and SARS-CoV-2 infection was also taken into account. Moreover, clinical characteristics of KDG during SARS-CoV-2 epidemic (KDG-CoV2) were compared to Kawasaki Disease patients (KDG-Historical) seen in three different Italian tertiary pediatric hospitals (Institute for Maternal and Child Health, IRCCS "Burlo Garofolo", Trieste; AOU Meyer, Florence; IRCCS Istituto Giannina Gaslini, Genoa) from January 1st 2000 to December 31st 2019. Chi square test or exact Fisher test and non-parametric Wilcoxon Mann-Whitney test were used to study differences between two groups. Results: One-hundred-forty-nine cases were enrolled, (96 KDG and 53 KCG). KCG children were significantly older and presented more frequently from gastrointestinal and respiratory involvement. Cardiac involvement was more common in KCG, with 60,4% of patients with myocarditis. 37,8% of patients among KCG presented hypotension/non-cardiogenic shock. Coronary artery abnormalities (CAA) were more common in the KDG. The risk of ICU admission were higher in KCG. Lymphopenia, higher CRP levels, elevated ferritin and troponin-T characterized KCG. KDG received more frequently immunoglobulins (IVIG) and acetylsalicylic acid (ASA) (81,3% vs 66%; p = 0.04 and 71,9% vs 43,4%; p = 0.001 respectively) as KCG more often received glucocorticoids (56,6% vs 14,6%; p < 0.0001). SARS-CoV-2 assay more often resulted positive in KCG than in KDG (75,5% vs 20%; p < 0.0001). Short-term follow data showed minor complications. Comparing KDG with a KD-Historical Italian cohort (598 patients), no statistical difference was found in terms of clinical manifestations and laboratory data. Conclusion: Our study suggests that SARS-CoV-2 infection might determine two distinct inflammatory diseases in children: KD and PIMS-TS. Older age at onset and clinical peculiarities like the occurrence of myocarditis characterize this multi-inflammatory syndrome. Abstract

The combination of highly sensitive techniques such as electrochemiluminescence (ECL) with nanotechnology sparked new analytical applications, in particular for immunoassay‐based detection systems. In this context, nanomaterials, particularly dye‐doped silica nanoparticles (DDSNPs) are of high interest, since they can offer several advantages in terms of sensitivity and performance. In this work we synthesized two sets of monodispersed and biotinylated [Ru(bpy)3]2+‐doped silica nanoparticles, named bio‐Triton@RuNP and bio‐Igepal@RuNP, obtained following the reverse microemulsion method using two different types of nonionic surfactants. Controlling the synthetic procedures, we were able to obtain nanoparticles (NPs) offering highly intense signal, using tri‐n‐propylamine (TPrA) as coreactant, with bio‐Triton@RuNps being more efficient than bio‐Igepal@RuNP. New research from the Mayo Clinic aims to identify unique molecular and immune markers in benign breast tissue to better identify women at risk of node-positive breast cancer. The study used gene expression profiling and immunofluorescence techniques to analyze tissue samples from women who developed breast cancer after a benign breast disease biopsy, as well as those who remained cancer-free. The researchers discovered higher gene expression levels of interferon regulatory factor 8 (IRF8) in the cancer-free group, suggesting a proactive immune environment that may prevent cancer development. Abstract

No livro'O terrível flagelo da humanidade: discursos médico-higienistas no combate à sífilis na Paraíba (1921-1940)', Rafael Nóbrega problematiza com brilhantismo os enunciados médicos sobre a doença, dando especial atenção às práticas educativas tecidas a partir da preocupação com o cuidado da higiene corporal e, principalmente, sexual. A análise do discurso, de inspiração foucaultiana, é utilizada como metodologia, o que possibilitou ao autor problematizar os interesses e as relações de poder que produziram enunciados sobre a doença na Paraíba. Ao discutir como os discursos sobre a doença na Paraíba foram elaborados, Rafael se apropria dos sentidos e sensibilidades presentes nas fontes e produz novos significados para a sífilis e para os sifilíticos ao estabelecer interpretações dos acontecimentos. Romper com as antigas leituras que o homem da sociedade escravista fazia do corpo, do sexo, do trabalho e da moral não se constituía em uma'destruição'da tradição, mas na desqualificação de determinados hábitos tradicionais que a ciência sexual e a ciência médica instituíram, no século XX, como degenerativos à família e ao contexto modernos, limpando as novas gerações das influências mórbidas, como a sífilis, que era legitimada pelos pais da elite do engenho como uma marca de virilidade. O discurso eugênico deslegitimaria essa doença como signo de potência viril e, em nome do corpo atlético, saudável e musculoso, legitimaria os esportes como formadores do corpo enrijecido. Esse é um livro de vital importância para a historiografia da saúde e das doenças, seja em nível local ou nacional.

Di recente sono tornati ad affacciarsi molti interrogativi a proposito della figura di santa Chiara d'Assisi. Perché la giovane seguì Francesco, fuggendo nottetempo dalla casa paterna? Si era forse invaghita di lui o voleva davvero imitare Gesù, secondo l'esempio del Poverello? Perché anche altri familiari, addirittura la madre di Chiara, lasciarono la dimora patrizia per una vita di strettissima povertà? In questo testo Gianluigi Pasquale ha rivisitato la storia di questa santa che si distinse per tenace umiltà. La narrazione, dai toni vividi, proietta il lettore nello scenario del XIII secolo e gli permette di assaporarne i colori e i suoni mentre svela le imprese e i drammi attraverso i quali Chiara divenne «donna di luce». Una donna – la prima a scrivere una regola di vita monastica – che fu capace di contrapporsi persino al Papa pur di ottenere il privilegio della povertà assoluta, appunto la sua «luce». Con una scelta innovativa l'autore somma alla piacevolezza della rievocazione storica il fascino della consultazione diretta delle fonti, arricchendo il racconto con una fondamentale selezione di documenti originali.

32. La Galleria dei Carracci in Palazzo Farnese a Roma : Eros, Anteros, Età dell'Oro

Abstract

Ad una'auspicabile pubblicazione'della magistrale ricerca di Stefano Colonna sulla Galleria dei Carracci in Palazzo Farnese accennavo già nel 1999 e nell'occasione citavo dalla tesi del Colonna i versi di Onorio Longhi da lui ritrovati, dai quali si evince con chiarezza l'identificazione di Arianna con Margherita Aldobrandini, che nel 1600 andò sposa a Ranuccio Farnese; che questo sia il tema principale dell'affresco centrale della Galleria, è stato pienamente riconosciuto dalla critica successiva. L'ampiezza del giro di orizzonte consente di entrare nel vivo della concezione neo-platonica che è sottesa alla volta e di acquisire certezze interpretative. Queste sono confortate del resto dal fondamentale ritrovamento di quattro rari epitalami relativi alle nozze Farnese-Aldobrandini: scoperta vistosa anche per altre implicazioni relative alla raffinata cultura del grande amico del Caravaggio. L'identificazione di Arianna con Margherita Aldobrandini, la nipote del pontefice che si univa in nozze all''eroico'Ranuccio, vi è chiaramente rivelata e stabilisce un altro, fondamentale punto fermo per intendere il messaggio della Galleria. [dalla presentazione di Maurizio Calvesi] Il volume mette in luce la complessità del clima culturale farnesiano da Achille Bocchi in poi, con i contributi sia di figure poco note come l'umanista ungherese Janos Zsamboky e l'olandese Stephen Wynkens Pigge, sia del celebre collezionista-archeologo Fulvio Orsini. Viene riletta la figura di Agostino Carracci in rapporto a letterati, filosofi e musicisti coevi e gli vengono attribuite alcune piccole raffinate incisioni. La Galleria dei Carracci è presa in esame in relazione al matrimonio di Ranuccio Farnese e Margherita Aldobrandini, alla luce degli'inediti'epitalami che provano per la prima volta in maniera inequivocabile il rapporto tra le nozze e gli affreschi, ponendo fine alla vexata quaestio del programma iconografico della Galleria stessa e del valore di Anteros come Amor Virtutis o Amore reciproco. La Galleria viene infine valutata nella sua articolazione concettuale con una lettura critica storico-artistica proiettata in avanti verso il XVII secolo, anche attraverso il raffronto con il pensiero controriformistico del filosofo Pomponio Torelli e gli affreschi di François Perrier in Palazzo Peretti in Roma. Una vasta bibliografia ed una ricca Appendice di documenti manoscritti ed a stampa relativi all'ambiente farnesiano, quasi tutti inediti, completano il libro corredato da più di 100 illustrazioni, in gran parte relative agli affreschi della Galleria Farnese e del Camerino del Cardinale Odoardo che hanno rappresentato, per la novità e ricchezza del linguaggio figurativo dei Carracci e lo splendore delle forme e dei colori, la fine del Manierismo alle soglie del Barocco nascente ed il ritorno elegiaco e meditato a Raffaello, Correggio e Michelangelo.
