Objective Assessment of human joint cartilage is a crucial tool to detect and diagnose pathological conditions. This exploratory study developed a workflow for 3D modeling of cartilage and bone based on multimodal imaging. New evaluation metrics were created and, a unique set of data was gathered from healthy controls and patients with clinically evaluated degeneration or trauma. Design We present a novel methodology to evaluate knee bone and cartilage based on features extracted from magnetic resonance imaging (MRI) and computed tomography (CT) data. We developed patient specific 3D models of the tibial, femoral, and patellar bones and cartilages. Forty-seven subjects with a history of degenerative disease, traumatic events, or no symptoms or trauma (control group) were recruited in this study. Ninety-six different measurements were extracted from each knee, 78 2D and 18 3D measurements. We compare the sensitivity of different metrics to classify the cartilage condition and evaluate degeneration. Results Selected features extracted show significant difference between the 3 groups. We created a cumulative index of bone properties that demonstrated the importance of bone condition to assess cartilage quality, obtaining the greatest sensitivity on femur within medial and femoropatellar compartments. We were able to classify degeneration with a maximum recall value of 95.9 where feature importance analysis showed a significant contribution of the 3D parameters. Conclusion The present work demonstrates the potential for improving sensitivity in cartilage assessment. Indeed, current trends in cartilage research point toward improving treatments and therefore our contribution is a first step toward sensitive and personalized evaluation of cartilage condition.

This paper describes the interconnections and predictive value between Body Mass Index (BMI), Isometric Leg Strength (ISO) and soft tissue distribution from mid-thigh Computed Tomography (CT) scans using Machine Learning (ML) regression and classification algorithms. A novel methodology for soft tissue patient specific CT profile called Nonlinear Trimodal Regression Analysis (NTRA) was developed using radiodensitomentric distribution from a CT scan. This method defines 11 parameters used as input features for Tree-Based ML algorithms in order to apply regression and classification on BMI and ISO. K_fold Cross-Validation with k = 10 is applied to obtain several models to choose the best one using the higher coefficient of determination (R2) as an evaluator of the quality of regression prediction. Following this, BMI and ISO are divided into 3 and 5 classes and the same methodology is used to classify them. For this analysis, an accuracy parameter is calculated to evaluate the quality of the results. The max R2 is 88.9 for the BMI and it is obtained using the Gradient-Boosting Algorithm. The best accuracy was 76.1 for 3 classes and 73.1 for 5 classes. The best results obtained for ISO are R2 = 66.5 and an accuracy of 65.5 for the 3 classes classification. Furthermore, the connective tissue assumes high importance in the prediction process. In this methodological study the feasibility of a ML approach was tested with good results, in order to show a novel approach to study the correlation between physiology parameters and imaging.

ObjectiveAnterior cruciate ligament reconstruction (ACLR) predisposes footballers for subsequent ACL and hamstring (HS) injury. This case series examines HS muscle activation patterns during the running in ACLR patients (bone-patellar tendon-bone (BTB) and (HS) graft) after completion of functional criteria allowing return to training.MethodsElectromyography (EMG) recorded from medial and lateral HS bilaterally during treadmill running (12, 14 and 16 km/hour) from 21 male ACLR patients on average 7 months from surgery (5-9) that underwent (HS) (n=12) or BTB reconstruction (n=9) were compared with 19 healthy runners. Main outcome measures: EMG signal was normalised to peak during the running. Pairwise comparisons were made for each muscle group examining stance and swing activation for mean and peak EMG for each patient group and leg.ResultsSignificantly lower relative peak activation in stance (not swing) phase for medial HS was seen for all conditions with effect sizes ranging from −0.63 (controls, BTB non-injured leg) to −1.09 (HS injured). For lateral HS only BTB injured were significantly lower in stance phase (−1.05)ConclusionACLR patients show neuromuscular alterations during different phases of running. The finding of reduced medial HS activity in stance phase might have implications for knee instability and HS muscle injury on resumption of sport.

Bone remodelling after total hip arthroplasty has been largely observed and investigated. Most studies rely on projective images and only few obtain 3D information with limited spatial resolution. This study proposes a method to provide quantitative, 3D high-resolution data about femur bone density variations, by means of CT volume processing. This would offer a tool for further research and clinical studies. Five patients subjected to primary, cementless total hip arthroplasty were considered. Calibrated CT volumes were acquired before, just after surgery and 1 yr later. Bone remodelling hinders accurate alignment of femur volumes acquired after a year; instead, prosthesis stem remains unchanged. Thus, after metal artefact reduction, prosthesis was segmented, and stem-based accurate alignment was obtained. A test to exclude prosthesis migration was performed by considering specific femur anatomical landmarks. Bone density error due to artefact reduction and realignment was estimated. Quantitative differences in bone mineral density were computed for each voxel, providing a resolution of about 1 mm. Preliminary results showed that the femur underwent consistent remodelling after a year. Widespread bone density losses appeared in those areas where strain-adaptive remodelling is normally expected, particularly about the calcar. Conversely, distal areas with clear stem-bone contact showed considerable density gains.

Total Hip Arthroplasty has been one of the most successful surgical procedure in terms of patient outcomes and satisfaction. However, due to increase in life expectancy and the related incidence of age-dependent bone diseases, a growing number of cases of intra-operative fractures lead to revision surgery with high rates of morbidity and mortality. Surgeons choose the type of the implant, either cemented or cementless prosthesis, on the basis of the age, the quality of the bone and the general medical conditions of the patients. Generally, no quantitative measures are available to assess the intra-operative fracture risk. Consequently, the decision-making process is mainly based on surgical operators&rsquo, expertise and qualitative information obtained from imaging. Motivated by this scenario, we here propose a mechanical-supported strategy to assist surgeons in their decisions, by giving intelligible maps of the risk fracture which take into account the interplay between the actual mechanical strength distribution inside the bone tissue and its response to the forces exerted by the implant. In the presented study, we produce charts and patient-specific synthetic &ldquo, traffic-light&rdquo, indicators of fracture risk, by making use of ad hoc analytical solutions to predict the stress levels in the bone by means of Computed Tomography-based mechanical and geometrical parameters of the patient. We felt that if implemented in a friendly software or proposed as an app, the strategy could constitute a practical tool to help the medical decision-making process, in particular with respect to the choice of adopting cemented or cementless implant.

Background The aim of the study was to compare the bone mineral density changes between unmatched patients undergoing total hip arthroplasty receiving uncemented and cemented type of implants. Previous studies have used DEXA or a two dimensional analysis to estimate the bone quality following total joint replacement, whereas this study presents the changes in three dimensions. Methods Fifty subjects both male and females receiving both cemented and uncemented type of implant were recruited. Two CT scans were taken of each subject, the first at 24 h post surgery and the second one 1 year after surgery. The scans were calibrated using a phantom converting the Hounsfield units to bone mineral density values in g/cm3. The two scans were registered together using anatomical landmarks and resliced to compare the two femurs in the identical frame of reference. The bone density gain and loss was calculated by comparing density values between the two sets of scans. Findings The results showed that most of the bone loss was located around the Lesser Trochanter and some bone density gain at the distal tip of the implant. The three dimensional density changes occur differently between individuals and the study showed no correlation of bone loss with age. Interpretation The bone loss occurred mostly at the proximal femur, which is in agreement with previously presented studies. By carrying out three dimensional analysis on the bone gain and loss on the femur, it is possible to identify the patients that are showing high degree of bone loss.

Sarcopenia, the progressive degeneration of aging muscle, is identified as an independent risk factor for significant morbidity, disability, and mortality in elderly individuals. In this paper Artificial Intelligence technologies, in particular Machine Learning (ML) supervised algorithms, are adopted to predict physiological parameter starting from muscle, fat and connective tissue distribution values of a mid-thigh Computer Tomography (CT) images. We developed and validated a novel method for soft tissue radiodensitometric distribution profiling, which is entitled nonlinear trimodal regression analysis (NTRA) method for soft tissue CT profiling. The work shows a comparative analysis using the NTRA method and standard soft tissue CT analysis modalities which was implemented on parameters assemblies from the 3,157 patients AGES-Reykjavik dataset. Furthermore, ML approach is used to find connections between amplitude, location, width and skewness in fat, muscle, and connective tissue and link these data to biomechanical measurements, Body Mass Index (BMI) and Cholesterol. The results highlight the specificities of each muscle quality metric to Lower Extremity functions and sarcopenic comorbidities. ML approach shows good predictive values for BMI having as most significant features connective and fat amplitude. Standardizing a quantitative methodology for myological assessment in this regard would allow for the generalizability of sarcopenia research to the indication of compensatory targets for clinical intervention.

There are two surgical approaches to performing total hip arthroplasty (THA): a cemented or uncemented type of prosthesis. The choice is usually based on the experience of the orthopaedic surgeon and on parameters such as the age and gender of the patient. Using machine learning (ML) techniques on quantitative biomechanical and bone quality data extracted from computed tomography, electromyography and gait analysis, the aim of this paper was, firstly, to help clinicians use patient-specific biomarkers from diagnostic exams in the prosthetic decision-making process. The second aim was to evaluate patient long-term outcomes by predicting the bone mineral density (BMD) of the proximal and distal parts of the femur using advanced image processing analysis techniques and ML. The ML analyses were performed on diagnostic patient data extracted from a national database of 51 THA patients using the Knime analytics platform. The classification analysis achieved 93% accuracy in choosing the type of prosthesis, the regression analysis on the BMD data showed a coefficient of determination of about 0.6. The start and stop of the electromyographic signals were identified as the best predictors. This study shows a patient-specific approach could be helpful in the decision-making process and provide clinicians with information regarding the follow up of patients.

In this regard, the NTRA method defines 11 subject-specific soft tissue parameters and has illustrated high sensitivity to changes in skeletal muscle form and function. The present work further explores the use of these 11 NTRA parameters in the construction of a machine learning (ML) system to predict body mass index and isometric leg strength using tree-based regression algorithms. Results obtained from these models demonstrate that when using an ML approach, these soft tissue features have a significant predictive value for these physiological parameters. In this study Machine Learning supervised regression and classification algorithms are used to predict Body Mass Index (BMI), starting from Computed Tomography scans (CT). From each patient CTs, 11 parameters describing muscle, connective tissue and fat, are extracted creating a patient specific soft tissue profile called Nonlinear Trimodal Regression Analysis (NTRA). Regression and classification are applied in order to predict and classify BMI using Tree-Based algorithms. A proper Train-Test division of the dataset is applied using k_fold Cross-Validation. Various combinations of features are employed with k_fold division in order to obtain the best coefficient of determination (R2) as evaluator of the quality of regression’s prediction. Afterward, BMI is divided into 3 and 5 classes and the same methodology is used to classify it. For this analysis, the accuracy parameter is calculated to evaluate the quality of the results. The max R2 is 0,83 and it is obtained using the 11 NTRA parameters as regressors, k_fold = 16, and the Gradient-Boosting Algorithm. The amplitude of the connective and fat tissue always covers more than 50% of all the feature importance. The best accuracy was 0,80 for 3 classes and 0,74 for 5 classes. The results prove that the 11 NTRA parameters can have a very significant predictive value and the same methodology can be applied in future works to predict other physiological parameters and comorbidities.

This article outlines the methods and applications of threshold-based techniques to assess in vivo muscle and bone tissue distribution in normal and pathological conditions using computed tomography imaging. The approaches described here use medical imaging processing techniques and computational methods to study bone mechanical proprieties, analyze and quantify muscle morphology, visualize changes with 3-D models, develop subject-specific numerical profiles, and assess muscle and bone changes during clinical treatments.

International audience; Objective. Maintaining upright posture is a complex task governed by the integration of afferent sensorimotor and visual information with compensatory neuromuscular reactions. The objective of the present work was to characterize the visual dependency and functional dynamics of cortical activation during postural control. Approach. Proprioceptic vibratory stimulation of calf muscles at 85 Hz was performed to evoke postural perturbation in open-eye (OE) and closed-eye (CE) experimental trials, with pseudorandom binary stimulation phases divided into four segments of 16 stimuli. 64-channel EEG was recorded at 512 Hz, with perturbation epochs defined using bipolar electrodes placed proximal to each vibrator. Power spectra variation and linearity analysis was performed via fast Fourier transformation into six frequency bands (Δ, 0.5-3.5 Hz; θ, 3.5-7.5 Hz; α, 7.5-12.5 Hz; β, 12.5-30 Hz; γ_low, 30-50 Hz; and γ_high, 50-80 Hz). Finally, functional connectivity assessment was explored via network segregation and integration analyses. Main Results. Spectra variation showed waveform and vision-dependent activation within cortical regions specific to both postural adaptation and habituation. Generalized spectral variation yielded significant shifts from low to high frequencies in CE adaptation trials, with overall activity suppressed in habituation; OE trials showed the opposite phenomenon, with both adaptation and habituation yielding increases in spectral power. Finally, our analysis of functional dynamics reveals novel cortical networks implicated in postural control using EEG source-space brain networks. In particular, our reported significant increase in local θ connectivity may signify the planning of corrective steps and/or the analysis of falling consequences, while α band network integration results reflect an inhibition of error detection within the cingulate cortex, likely due to habituation. Significance. Our findings principally suggest that specific cortical waveforms are dependent upon the availability of visual feedback, and we furthermore present the first evidence that local and global brain networks undergo characteristic modification during postural control.

Muscle degeneration has been consistently identified as an independent risk factor for high mortality in both aging populations and individuals suffering from neuromuscular pathology or injury. While there is much extant literature on its quantification and correlation to comorbidities, a quantitative gold standard for analyses in this regard remains undefined. Herein, we hypothesize that rigorously quantifying entire radiodensitometric distributions elicits more muscle quality information than average values reported in extant methods. This study reports the development and utility of a nonlinear trimodal regression analysis method utilized on radiodensitometric distributions of upper leg muscles from CT scans of a healthy young adult, a healthy elderly subject, and a spinal cord injury patient. The method was then employed with a THA cohort to assess pre- and postsurgical differences in their healthy and operative legs. Results from the initial representative models elicited high degrees of correlation to HU distributions, and regression parameters highlighted physiologically evident differences between subjects. Furthermore, results from the THA cohort echoed physiological justification and indicated significant improvements in muscle quality in both legs following surgery. Altogether, these results highlight the utility of novel parameters from entire HU distributions that could provide insight into the optimal quantification of muscle degeneration., This work was partly funded by the European Commission in scope of the project PASSPORT. Total Hip Arthroplasty requires pre-surgical evaluation between un-cemented and cemented prostheses. A Patient with intra-operative periprosthetic fracture and another with a successful outcome were recruited, and their finite element models were constructed by processing CT data, assuming elastic-plastic behavior of the bone as function of the local density. To resemble the insertion of the prosthesis into the femur, a fictitious thermal dilatation is applied to the broach volume. Strain-based fracture risk factor is estimated, depicting results in terms of the total mechanical strain expressed using a simple "traffic lights" color code to provide immediate, concise, and intelligible pre-operative information to surgeons.

Total hip arthroplasty is a ubiquitously successful orthopedic surgical procedure, whose prevalence is rising worldwide. While many investigations focus on characterizing periprosthetic pathophysiology, the objective of our research is to develop and describe multi-metric assemblies as a first step toward creating a patient-specific mobility index that rehabilitators and orthopedic surgeons can utilize for prescribing their respective procedures. In total, 48 total hip arthroplasty patients (both cemented and uncemented) undergoing unilateral, primary surgery went through computed tomographic scans and gait analysis measurements both before and 1 year following their surgery. Altogether, the reported quantitative metrics include 11 spatial and temporal gait parameters, muscle density, and electromyography signals from the rectus femoris, vastus lateralis, and vastus medialis, and bone mineral density values from bioimage analysis around the implant stem. We found that measured parameters from a subgroup were sensitive to changes observed during patient recovery, implicating the predictive sensitivity of these patient conditions. Most post-operative gait parameters changed significantly, while electromyography data indicated few significant differences. Moreover, results from bioimage analyses indicate a general reduction of periprosthetic bone mineral density after 1 year, in association with increasing density of the quadriceps muscles. Furthermore, this work identifies which quantitative metrics undergo the greatest variation after total hip arthroplasty and demonstrates the clinical feasibility of a multimodal approach to mobility assessment that may ultimately support decision-making for post-surgical rehabilitation protocols.

A new sophisticated method that uses video analysis techniques together with a Maillon Rapide Delta to determine the tensile properties of the transverse carpal ligament–carpal tunnel complex has been developed. Six embalmed cadaveric specimens amputated at the mid-forearm and aged (mean (SD)): 82 (6.29) years were tested. The six hands were from three males (four hands) and one female (two hands). Using trigonometry and geometry the elongation and strain of the transverse carpal ligament and carpal arch were calculated. The cross-sectional area of the transverse carpal ligament was determined. Tensile properties of the transverse carpal ligament–carpal tunnel complex and Load–Displacement data were also obtained. Descriptive statistics, one-way ANOVA together with a post-hoc analysis (Tukey) and t-tests were incorporated. A transverse carpal ligament–carpal tunnel complex novel testing method has been developed. The results suggest that there were no significant differences between the original transverse carpal ligament width and transverse carpal ligament at peak elongation (P= 0.108). There were significant differences between the original carpal arch width and carpal arch width at peak elongation (P=0.002). The transverse carpal ligament failed either at the mid-substance or at their bony attachments. At maximum deformation the peak load and maximum transverse carpal ligament displacements ranged from 285.74 N to 1369.66 N and 7.09 mm to 18.55 mm respectively. The transverse carpal ligament cross-sectional area mean (SD) was 27.21 (3.41)mm2. Using this method the results provide useful biomechanical information and data about the tensile properties of the transverse carpal ligament–carpal tunnel complex.

This chapter outlines the methods and applications of X-ray computed tomography (CT) imaging to analyze soft tissue and skeletal muscle density and volume in the context of modern challenges in the field of translational myology. The approaches described here use medical imaging processing techniques and computational methods to quantify muscle morphology, illustrate changes with 3D models, develop numerical profiles specific for each individual, and assess muscle changes due to targeted medical treatment.

Little is known about the mechanics of in vivo loading on total wrist prostheses where many studies have looked at the mechanics of other types of arthroplasty such as for the hip and the knee which has contributed to the overall success of these types of procedures. Currently surgeons would prefer to carry out arthrodesis on the wrist rather than consider arthroplasty as clinical data have shown that the outcome of total wrist arthroplasty is poorer than compared to the hip and knee. More research is needed on the loading mechanisms of the implants in order to enhance the design of future generation implants. This study looks at the load transfer characteristics of the Universal 2 implant using a finite element model of a virtually implanted prosthesis during gripping. The results showed that the loading on the implant is higher on the dorsal and ulnar aspect than on the volar and radial aspect of the implant. The whole load is transmitted through the radius and none through the ulna.

Background: The high risk of fracture associated with chronic spinal cord injury (SCI) is attributed to extensive disuse-related bone loss in previously weight-bearing long bones. Changes in bone mineral density (BMD) after SCI have been documented extensively for the epiphyses of the tibia and femur, fracture-prone sites in this patient group. Less attention has been given to patterns of cortical bone loss in the diaphyses, but variability in BMD distributions throughout the long bones may contribute to some patients’ increased susceptibility to shaft fractures in chronic SCI. Aim: A cross-sectional study was carried out to determine whether BMD distributions along the tibia differ between individuals with chronic SCI and healthy able-bodied (AB) controls, in both the trabecular and cortical bone compartments. The effects of time post-injury and gender on BMD distribution were also explored. Methods: Individuals with chronic (≥ 6 months post-injury) motor-complete SCI were recruited from the Queen Elizabeth National Spinal Injuries Unit (Glasgow, UK). AB control subjects were recruited to achieve similar age and gender profiles for the SCI and control groups. Multi-slice pQCT (XCT3000, Stratec) was performed along the length of the tibia (2mm thickness, 0.5mm voxel size), at 1% intervals in the epiphyses and 5% intervals in the diaphysis (34 slices in total). This paper illustrates the feasibility and utility of combining cranial anatomy and brain function on the same 3D-printed model, as evidenced by a neurosurgical planning case study of a 29-year-old female patient with a low-grade frontal-lobe glioma. We herein report the rapid prototyping methodology utilized in conjunction with surgical navigation to prepare and plan a complex neurosurgery. The method introduced here combines CT and MRI images with DTI tractography, while using various image segmentation protocols to 3D model the skull base, tumor, and five eloquent fiber tracts. This 3D model is rapid-prototyped and coregistered with patient images and a reported surgical navigation system, establishing a clear link between the printed model and surgical navigation. This methodology highlights the potential for advanced neurosurgical preparation, which can begin before the patient enters the operation theatre. Moreover, the work presented here demonstrates the workflow developed at the National University Hospital of Iceland, Landspitali, focusing on the processes of anatomy segmentation, fiber tract extrapolation, MRI/CT registration, and 3D printing. Understanding the load mechanics of orthopaedic implants is important to be able to predict their behaviour in-vivo. Much research, both mechanical and clinical, has been carried out on hip and knee implants, but less has been written about the mechanics of wrist implants. In this paper, the load mechanics of the Universal 2 wrist implant have been measured using two types of measuring techniques, strain gauges and Fibre Bragg Grating measurements to measure strains. The results were compared to a finite element model of the implant. The results showed that the computational results were in good agreement with the experimental results. Better understanding of the load mechanics of wrist implants, using models and experimental results can catalyse the development of future generation implants.

Medical imaging is of particular interest in the field of translational myology, as extant literature describes the utilization of a wide variety of techniques to non-invasively recapitulate and quantity various internal and external tissue morphologies. In the clinical context, medical imaging remains a vital tool for diagnostics and investigative assessment. This review outlines the results from several investigations on the use of computed tomography (CT) and image analysis techniques to assess muscle conditions and degenerative process due to aging or pathological conditions. Herein, we detail the acquisition of spiral CT images and the use of advanced image analysis tools to characterize muscles in 2D and 3D. Results from these studies recapitulate changes in tissue composition within muscles, as visualized by the association of tissue types to specified Hounsfield Unit (HU) values for fat, loose connective tissue or atrophic muscle, and normal muscle, including fascia and tendon. We show how results from these analyses can be presented as both average HU values and compositions with respect to total muscle volumes, demonstrating the reliability of these tools to monitor, assess and characterize muscle degeneration., This research has been supported by the University Hospital Landspitali Scientific fund.

Total Hip Arthroplasty (THA) is one of the most utilized and successful orthopedic surgical procedures, and with increasing life expectancies in many populations worldwide, THA rates are projected to continue to rise accordingly. Despite the procedure’s rising prevalence, many periprosthetic fracture and unloading events are still reported. While many investigations have recently focused on potential assessment modalities and metrics to observe and characterize periprosthetic pathophysiology in THA patients, there is no extant, reliable method for the quantitative assessment of patients prior to THA. In most cases, respective opinions of the physicians involved dictate this decision, and procedures are therefore founded upon both the surgeon’s own experiences and qualitative generalizations based on suggested indicators of bone quality (gender, age, and qualitative assessment of CT images). There is therefore a great need for a quantitative, multimodal gold standard to securely choose the appropriate implant on a patient-specific basis. The objective of the re- search presented herein was to describe a novel assembly of such data from a 72-patient cohort as a first step towards eventually creating a patient-specific, presurgical application that orthopedic surgeons can utilize for determining the optimal THA prosthesis procedure. Here, we report the use of 3D soft tissue segmentation of the Rectus femoris, Vastus lateralis, and the Vastus medialis muscles, the use of 3D FEA to compute Fracture Risk Indices (FRI), the pre-operative measurement and user-friendly assembly of 11 gait parameters, and the measurement and analyses of EMG activation data – all of which are presented herein as a comprehensive patient report for a representative 60 year-old female patient.

Sarcopenic muscular degeneration has been consistently identified as an independent risk factor for mortality in aging populations. Recent investigations have realized the quantitative potential of computed tomography (CT) image analysis to describe skeletal muscle volume and composition; however, the optimum approach to assessing these data remains debated. Current literature reports average Hounsfield unit (HU) values and/or segmented soft tissue cross-sectional areas to investigate muscle quality. However, standardized methods for CT analyses and their utility as a comorbidity index remain undefined, and no existing studies compare these methods to the assessment of entire radiodensitometric distributions. The primary aim of this study was to present a comparison of nonlinear trimodal regression analysis (NTRA) parameters of entire radiodensitometric muscle distributions against extant CT metrics and their correlation with lower extremity function (LEF) biometrics (normal/fast gait speed, timed up-and-go, and isometric leg strength) and biochemical and nutritional parameters, such as total solubilized cholesterol (SCHOL) and body mass index (BMI). Data were obtained from 3,162 subjects, aged 66–96 years, from the population-based AGES-Reykjavik Study. 1-D k-means clustering was employed to discretize each biometric and comorbidity dataset into twelve subpopulations, in accordance with Sturges’ Formula for Class Selection. Dataset linear regressions were performed against eleven NTRA distribution parameters and standard CT analyses (fat/muscle cross-sectional area and average HU value). Parameters from NTRA and CT standards were analogously assembled by age and sex. Analysis of specific NTRA parameters with standard CT results showed linear correlation coefficients greater than 0.85, but multiple regression analysis of correlative NTRA parameters yielded a correlation coefficient of 0.99 (P, This work was funded by the Landspítali Scientific Fund, PI: Paolo Gargiulo, A-2014-072 (http://www.landspitali.is/).

The aim of this work was to create an anatomically accurate three-dimensional finite element model of the wrist, applying subject-specific loading and quantifying the internal load transfer through the joint during maximal grip. For three subjects, representing the anatomical variation at the wrist, loading on each digit was measured during a maximal grip strength test with simultaneous motion capture. The internal metacarpophalangeal joint load was calculated using a biomechanical model. High-resolution magnetic resonance scans were acquired to quantify bone geometry. Finite element analysis was performed, with ligaments and tendons added, to calculate the internal load distribution. It was found that for the maximal grip the thumb carried the highest load, an average of 72.2 ± 20.1 N in the neutral position. Results from the finite element model suggested that the highest regions of stress were located at the radial aspect of the carpus. Most of the load was transmitted through the radius, 87.5 per cent, as opposed to 12.5 per cent through the ulna with the wrist in a neutral position. A fully three-dimensional finite element analysis of the wrist using subject-specific anatomy and loading conditions was performed. The study emphasizes the importance of modelling a large ensemble of subjects in order to capture the spectrum of the load transfer through the wrist due to anatomical variation.

The variability in patient outcome and propensity for surgical complications in total hip replacement (THR) necessitates the development of a comprehensive, quantitative methodology for prescribing the optimal type of prosthetic stem: cemented or cementless. The objective of the research presented herein was to describe a novel approach to this problem as a first step towards creating a patient-specific, presurgical application for determining the optimal prosthesis procedure. Finite element analysis (FEA) and bone mineral density (BMD) calculations were performed with ten voluntary primary THR patients to estimate the status of their operative femurs before surgery. A compilation model of the press-fitting procedure was generated to define a fracture risk index (FRI) from incurred forces on the periprosthetic femoral head. Comparing these values to patient age, sex, and gender elicited a high degree of variability between patients grouped by implant procedure, reinforcing the notion that age and gender alone are poor indicators for prescribing prosthesis type. Additionally, correlating FRI and BMD measurements indicated that at least two of the ten patients may have received nonideal implants. This investigation highlights the utility of our model as a foundation for presurgical software applications to assist orthopedic surgeons with selecting THR prostheses.

The disuse-related bone loss that results from immobilisation following injury shares characteristics with osteoporosis in post-menopausal women and the aged, with decreases in bone mineral density leading to weakening of the bone and increased risk of fracture. The aim of this study was to use the finite element method to: (i) calculate the mechanical response of the tibia under mechanical load and (ii) estimate of the risk of fracture; comparing between two groups, an able-bodied group and spinal cord injury patients group suffering from varying degrees of bone loss. The tibiae of eight male subjects with chronic spinal cord injury and those of four able-bodied age-matched controls were scanned using multi-slice peripheral quantitative computed tomography. Images were used to develop full three-dimensional models of the tibiae in Mimics (Materialise) and exported into Abaqus (Simulia) for calculation of stress distribution and fracture risk in response to specified loading conditions – compression, bending and torsion. The percentage of elements that exceeded a calculated value of the ultimate stress provided an estimate of the risk of fracture for each subject, which differed between spinal cord injury subjects and their controls. The differences in bone mineral density distribution along the tibia in different subjects resulted in different regions of the bone being at high risk of fracture under set loading conditions, illustrating the benefit of creating individual material distribution models. A predictive tool can be developed based on these models, to enable clinicians to estimate the amount of loading that can be safely allowed onto the skeletal frame of individual patients who suffer from extensive musculoskeletal degeneration (including spinal cord injury, multiple sclerosis and the ageing population). The ultimate aim is to reduce fracture occurrence in these vulnerable groups.

A finite element model of the wrist was developed to simulate mechanical changes that occur after surgery of the wrist. After partial arthrodesis, the wrist will experience altered force transmission during loading. Three different types of partial arthrodesis were investigated — radiolunate, radioscaphoid, and radioscapholunate — and compared with the healthy untreated wrist. The results showed that the compressive forces on the radiocarpal joint decreased compared with the untreated wrist with both radiolunate and radioscaphoid fusions. The load transmission through the midcarpal joints varied depending on arthrodesis type. The forces in the extrinsic ligaments decreased with the fusion, most noticeably in the dorsal radiotriquetral ligament, but increased in the dorsal scaphotriquetral ligament. From the results of the study it can be concluded that the radioscapholunate fusion shows the most biomechanically similar behaviour out of the three fusion types compared with the healthy wrist. The modelling described in this paper may be a useful approach to pre-operative planning in wrist surgery.

In this study, the first normative database of movement control in the cervical spine has been established. For this purpose the Fly Test was used, which is a reliable and valid clinical test capable of detecting deficient movement control of the cervical spine in patients with neck pain and its associated disorders. One hundred and eighty-two asymptomatic persons, eighty-three men and ninety-nine women, aged 16-74 years, divided into six age groups, were recruited. The Fly Test, using a 3-space Fastrak device, recorded the accuracy of cervical spine movements when tracking three incrementally difficult movement patterns. Amplitude accuracy (AA), directional accuracy (DA), and jerk index (JI) were compared across patterns and age groups. A multivariate analysis of variance revealed a significant effect for age (p0.001) but not gender (p0.05). Lower accuracy for AA and DA in all three movement patterns was observed in the groups of subjects aged 55-64 and 65-74 years, and also for JI in the easy and medium patterns. Knowledge of normative values for the Fly Test is important and useful in identifying impaired movement control and monitoring the effectiveness of treatment interventions in patients with neck pain of traumatic and non-traumatic origin.

Introduction Anatomically, the Transverse Carpal Ligament (TCL) attaches to the carpal bones distally and proximally. Beneath the TCL, during finger and hand movements the median nerve and tendons move in the longitudinal, transverse, volar and dorsal directions [Ugbolue et al , 2005] and become compressed as they move in the dorsal and volar directions [Armstrong, 1979]. Within the carpal tunnel complex, the tendons and median nerve together with the TCL form a pulley system [Brooks et al , 2003; Stecco et al , 2010]. Biomechanically, the TCL has been studied to determine its compressive [Holmes, 2011] and tensile [Li et al , 2009; Sucher et al , 1998] properties. While these methods have involved either excising the TCL or determining the biomechanical properties of the TCL intact / transected, experimentally there is still no widely accepted method designed to specifically evaluate the tensile properties of the TCL and carpal tunnel complex. That is to date, there are no known methods to test the TCL to failure in-situ . Methods Six embalmed cadaveric specimens amputated at the mid forearm and aged 82 ± 6.29 years were tested. The six hands were from four individuals (two pairs and two individual hands), three males (four hands) and one female (two hands). The tensile properties of the TCL were determined using a commercial Maillon Rapide Delta (S3i Ltd, Bawtry, England, UK) fastened to a steel work piece (Figure 1). The Maillon Rapide Delta (S3i Ltd, Bawtry, England, UK) was fastened on to the TCL and then attached to the upper hydraulic tensile grips connected to the 1000 N load cell. The specimen was adjusted until the TCL was aligned and perpendicular to the Maillon Rapide Delta (S3i Ltd, Bawtry, England, UK). The test protocol started with a preconditioning cycle (10 cycles of 0.5 N loading at a rate of 2Hz) before testing the ligament to failure at a deformation rate of 20 mm/s. Descriptive statistics and tensile properties of the TCL – carpal tunnel complex and Load – Displacement data were also obtained. Results and discussion The TCL failed either at the mid-substance or at their bony attachments. At maximum deformation the peak load and maximum TCL displacements ranged from 285.74N to 1,369.66N and 7.09 mm to 18.55 mm respectively. The load at tensile strength ranged from 272.09N to 1293.36N and the ultimate tensile strength mean (SD) was 23.99 (10.68) Nmm -2 . A unique method of tensile testing the TCL and carpal tunnel complex that is uncomplicated and practical has been developed. The methodology has been validated and is capable of generating highly repeatable data. The finite element method has been used with considerable success to simulate the behaviour of various joints such as the hip, knee and shoulder. It has had less impact on more complicated joints such as the wrist and the ankle. Previously published finite element studies on these multi bone joints have needed to introduce un-physiological boundary conditions in order to establish numerical convergence of the model simulation. That is necessary since the stabilising soft tissue mechanism of these joints is usually too elaborate in order to be fully included both anatomically and with regards to material properties. This paper looks at the methodology of creating a finite element model of such a joint focussing on the wrist and the effects additional constraining has on the solution of the model. The study shows that by investigating the effects each of the constraints, a better understanding on the nature of the stabilizing mechanisms of these joints can be achieved.

A small number of cadaveric studies have been carried out looking at the force transmission through the radiocarpal joint. In this study subject specific finite element models were created of the whole wrist joint using measured biomechanical data to capture the forces acting on the wrist with the hand generating a maximum gripping force.

In paraplegia of Spinal Cord Injury (SCI), bone loss can occur rapidly and extensively in the paralysed lower limbs. It was reported that some patients develop osteoporosis in the tibia and femur even within the first year of injury [Coupaud et al, 2011]. Weakening of the bones after SCI is accompanied by a substantially increased risk of fracture. It is important to predict the most likely locations of fracture at regions of weakness along the bone in SCI. There is a clinical need to understand the macro-structural behaviour of the long bones in the body in order to develop potential physical treatments to tackle this musculoskeletal disorder in SCI patients. The aim of this ongoing study is to develop a modelling tool to help clinicians to quantify changes in bone structure and fracture susceptibility resulting from bone loss in the paralysed limbs after SCI.
