Context: Using oxygen therapy is common in the Post Anesthesia Care Unit (PACU). However, systematic administration of oxygen exposes the patient to potential adverse effects. In 2018 and 2019, recommendations were published and objectives of SpO2 to start (SpO2 < 92%) or stop (SpO2 > 95%) oxygen therapy are proposed. Routine practices regarding oxygen therapy remain very heterogeneous and the application of these recommendations uncertain. The aim of this study was to investigate how oxygen therapy in PACU is routinely applied.Methods: A prospective, multicenter, regional, observational study was performed in 6 university and general hospitals in Auvergne area in France. Reported data were part of the usual management in the PACU. All adult patients who underwent a general anesthesia with invasive mechanical ventilation or sedation with spontaneous ventilation requiring monitoring in the PACU were included. Major outcome was the SpO2 at admission in the PACU or after extubation if performed in the PACU (T0). Main secondary outcomes were the number of patients receiving oxygen therapy and corresponding SpO2 at different predefined monitoring times in the PACU, the length of stay in the PACU including the length of stay with oxygen support, and the occurrence of oxygen weaning in the PACU.Results: Between 6 consecutive weeks in May and June 2021, 627 patients were included. Mean SpO2 at T0 was 96% with an oxygen support for 33% of them and a mean oxygen flow of 4,5L/min. SpO2 was in the ranges proposed by the BMJ for 74% of patients at this time. All along the monitoring in the PACU, 52% of patients kept a SpO2 in the ranges defined by the guidelines. 22% of patients presented at least one episode of desaturation during their surveillance. At the discharge of the PACU, 19% of patients had still an oxygen support with a mean flow of 2L/min. Median length of stay in the PACU was 75 minutes, and 45 minutes with oxygen support. For 18% of patients a target of SpO2 was prescribed.Conclusion: In this multicentric prospective observational study about postoperative oxygen management, we reported that the mean postoperative SpO2 was 96%. 52% of our population kept a SpO2 in the ranges fixed by the recommendations all along the surveillance in the PACU. Incidence of desaturation was 22%. This evaluation of the routine practice allows a better knowledge of how the oxygen is administered in the PACU and will help us to improve and personalize early postoperative care.Perspective: This study is a situational analysis of oxygen use in PACU and it will be a base to perform a training to healthcare team in guidelines. Then we will conduct a second prospective evaluation with the same modalities than the presented one, to measure the impact of the training on adequation with recommendations and improvement of postoperative oxygen management.; Introduction : L’usage de l’oxygénothérapie est courant en salle de surveillance post interventionnelle (SSPI). Une administration systématique expose pourtant le patient à une iatrogénie potentielle. En 2018 et 2019, des recommandations ont été éditées et ont défini des objectifs pour débuter (SpO2 < 92%) ou stopper (SpO2 > 95%) l’oxygénothérapie. Pour autant, les pratiques actuelles en matière d’oxygénothérapie restent très hétérogènes et l’application de ces recommandations incertaine. Le but de cette étude est d’évaluer les pratiques en termes d’oxygénothérapie en SSPI.Méthodes : Une étude prospective, multicentrique, régionale, observationnelle a été réalisée dans 6 hôpitaux de la région Auvergne. Les données étaient collectées parallèlement à la prise en charge usuelle en SSPI. Tous les patients adultes ayant bénéficié d’une anesthésie générale avec ventilation mécanique invasive ou sédation avec ventilation spontanée nécessitant une surveillance en SSPI pouvaient être inclus. Le critère de jugement principal était la SpO2 à l’arrivée en SSPI ou après extubation si celle-ci était réalisée en SSPI (T0). Les critères secondaires principaux étaient le nombre de patients recevant de l’oxygène en SSPI et les SpO2 correspondantes à différents temps prédéfinis lors de la surveillance en SSPI, la durée deséjour en SSPI et celle sous oxygène en SSPI et l’incidence du sevrage en oxygène en SSPI.Résultats : Pendant 6 semaines consécutives de mai et juin 2021, 627 patients ont été inclus. La SpO2 moyenne à T0 était de 96%, 33% des patients bénéficiaient d’une oxygénothérapie avec un débit moyen de 4,5 L/min. A T0, 74% des patients avaient une SpO2 dans les objectifs fixés par les recommandations. 52% des patients ont conservé une SpO2 dans les bornes fixées tout au long de leur surveillance en SSPI. 22% des patients ont présenté au moins un épisode de désaturation. ÀA la sortie de SSPI, 19% des patients avaient toujours une oxygénothérapie avec un débit moyen de 2L/min. La durée médiane de séjour en SSPI était de 75min et le temps passé sous oxygène en SSPI était de 45 minutes.Conclusion : Dans cette étude observationnelle multicentrique prospective, nous rapportons une SpO2 postopératoire moyenne de 96%. 52% de la population étudiée gardait une SpO2 dans les objectifs fixés par les recommandations pendant tout le séjour en SSPI. L’incidence des désaturations était de 22%. Cette évaluation des pratiques quotidiennes permet de mieux appréhender la façon dont est administré l’oxygène en SSPI et permettra d’aider à améliorer et à personnaliser la prise en charge postopératoire précoce des patients.Perspective : Cette étude se veut un point de situation de l'utilisation de l'oxygène en SSPI et servira de base à la formation des équipes soignantes aux recommandations. Une seconde évaluation des pratiques sera réalisée ensuite afin de mesurer l’impact de cette formation sur l'adéquation avec les recommandations, l'amélioration de la gestion de l'oxygène en postopératoire et le pronostic des patients.
