Background: The surprising increase observed in recent years in the use of minimally invasive sacroiliac joint arthrodesis techniques as a treatment for low back pain justifies an objective review of this results., Purpose: carry out a systematic review of the literature to evaluate the clinical results of patients with low back pain treated with percutaneous arthrodesis of the SIJ., Study Design: Systematic review., Methods: Systematic search of the medical literature. The words used in the Search were: "Hollow screw system", "percutaneous sacroiliac joint stabilization", "sacroiliac joint", "minimally invasive", "sacroiliac pain", "sacroiliac dysfunction". With logical connectors such as "and", "not" and "or"., Databases: Pubmed, Scopus, Embase, Ovid, EBSCO host and google scholar. The search extended from the beginning of the databases until September 2024. The "Rayyan" program was used to collect the information and facilitate the analysis process., Inclusion Criteria: Systematic review from the literature, clinical trials, observational studies and case series. They followed PRISMA principles., Results: 661 articles were found, of which 108 articles were duplicates. The criteria (Inclusion/Exclusion) were applied to the 553 articles identified based on the independent reading of the summaries by each of the authors on the Rayyan platform and 434 articles were excluded. The 119 selected articles were completely reviewed to finally obtain 102 articles included in the review. Epidemiological data were extracted into an Excel table, methodological and related to clinical results and safety of procedures made. The Prisma checklist for systematic reviews was applied in each article. The epidemiological quality of the articles was evaluated based on the checklists STROBE and CONSORT. The results regarding clinical improvement were overwhelming. However, most of the studies were sponsored by industry and with a limited population and follow-up not always prolonged., Conclusions: Although the clinical results regarding the effectiveness of SIJ fusion are forceful for their effectiveness, we recommend considering some aspects for their analysis and especially long-term studies., Competing Interests: Declarations. We aimed to study the quality of communication between health care providers and patients with low back pain with emphasis on information giving in a back pain clinic, including if characteristics of patients could be associated with communication quality. We videotaped 79 encounters in which 21 providers informed patients about the results of magnetic resonance imaging of the back. Background information about the patients was collected by questionnaires and interview after the encounter. Videotapes were analysed with the Four Habits Coding Scheme (4HCS) in which higher scores mean better communication. There were strong negative correlations between 4HCS scores and the duration of back pain, and patient age. The results were significant for all professional subgroups (doctors, physiotherapists, chiropractors). Communication quality in encounters with back pain patients is worse, the longer the patient has suffered pain. The objective of the research was to construct a brief coping questionnaire designed to assess the approach-avoidance dichotomy. A 20-item questionnaire was designed and tested in samples of 206 students and 93 patients. Based on empirical analyses and interviews with patients, 12 items were chosen for the final scale, which was named the Brief Approach/Avoidance Coping Questionnaire (BACQ). BACQ was then tested in a clinical sample of 299 primary care patients. A Cronbach's alpha of 0.68 was found in a primary care sample. In testing concurrent validity, BACQ indexes correlated significantly with relevant COPE sub-scales in a 0.34 to 0.57 range. A factor structure based on a two-factor solution gave one bipolar factor, ranging from active approach to resignation and withdrawal, and a second factor with items indicating diversion. In conclusion, the 12-item BACQ is a brief measure of coping strategies with satisfactory psychometric properties. The instrument is designed to measure a general concept of approach versus avoidance oriented coping, but the findings also point to two sub-dimensions of avoidant coping, resignation/withdrawal and diversion. Eighty-four male patients with a mean age of 56.4 years (range 34 to 65 years) were subjected to a semistructured interview 12–21 weeks after acute myocardial infarction in order to elucidate possible positive changes in family or spouse relationships induced by or following on from the disease. There were appreciable positive alterations in the love and caring domains and in communication, especially in those patients (one third of the total) who considered their total life situation to be improved after the myocardial infarction. A majority of the patients judged their family's adaptation to the myocardial infarction as good. The authors argue that it is important to discover, and reinforce, possible positive changes within the family after a myocardial infarction in one of its members and not merely to focus on negative effects. AIMS: Low back pain (LBP) is a major public health problem in both Norway and Sweden. The aim of the study was to estimate the prevalence of LBP and sickness absence due to LBP in two neighbouring regions in Norway and Sweden. The two areas have similar socioeconomic status, but differ in health benefit systems. METHODS: A representative sample of 1,988 adults in Norway and 2,006 in Sweden completed questionnaires concerning LBP during 1999 and 2000. For this study only individuals in part or full time jobs, (n = 1,158 in Norway and n = 1,129 in Sweden) were included. RESULTS: In Norway the lifetime prevalence was 60.7% and in Sweden 69.6%, the one-year prevalence was 40.5% and 47.2%, and the point prevalence 13.4% and 18.2% respectively. There was a significantly higher risk of reporting LBP in Sweden, even after controlling for gender, age, education, and physical workload. There was no difference in risk of self-certified short-term sickness absence (1-3 days), but it was a 40% lower risk of sickness absence with medical sickness certification in Sweden compared with Norway. CONCLUSION: The prevalence of LBP was higher in the Swedish area than in the Norwegian. The risk of self-certified sickness absence, however, showed no differences and the risk of medically certified sickness absence was lower in the Swedish area. Objective: To identify core elements of what patients with chronic low back pain perceive as good clinical communication and interaction with a specialist ('The Good Back-Consultation').Design: Qualitative study including observation of consultations and a subsequent patient interview. Quantitative data were also recorded.Subjects: Thirty-five patients with chronic low back pain referred to a specialist.Methods: Thirty-five consultations were observed with respect to history-taking, clinical examination and interaction between patient and doctor. Patients were subsequently interviewed about how they perceived the consultation. Fourteen specialists with various specialty branches and 35 patients (18 males) participated. For 3 of the specialists a positive effect (return to work) on patients with chronic low back pain had been documented in previous randomized controlled trials. Qualitative data analysis was performed using a template method.Results: Most patients thought that the history-taking and clinical examination had been thorough and satisfactory. Patients emphasized the importance of being given an explanation during the examination of what was being done and found, of receiving understandable information on the causes of the pain, of receiving reassurance, discussing psychosocial issues and discussing what can be done. The most important characteristic of 'The Good Back-Consultation' was that the specialist took the patient seriously.Conclusion: The findings may represent an important potential for enhancing clinical communication with patients. Study Design: A randomized controlled trial with stratified block design.Objectives: To evaluate a treatment program focusing on whether specific stabilizing exercises for patients with pelvic girdle pain after pregnancy reduce pain, improve functional status, and improve quality of life. SUMMARY OF BACKGROUND DATA. : The evidence of effectiveness of treatment for pelvic girdle pain is weak. Recent research has focused on the importance of activation of muscles for motor control and stability of the lumbopelvic region. To the authors' knowledge, the efficacy of applying these principles for pelvic girdle pain has not previously been evaluated in a randomized controlled trial.Methods: Eighty-one women with pelvic girdle pain were assigned randomly to two treatment groups for 20 weeks. One group received physical therapy with a focus on specific stabilizing exercises. The other group received individualized physical therapy without specific stabilizing exercises. Assessments were administered by a blinded assessor, at baseline, after intervention and 1 year post partum. Main outcome measures were pain, functional status and quality of life.Results: There were no dropouts. After intervention and at 1 year post partum, the specific stabilizing exercise group showed statistically and clinically significant lower pain intensity, lower disability, and higher quality of life compared with the control group. Group difference in median values for evening pain after treatment was 30 mm on the Visual Analog Scale. Disability was reduced by more than 50% for the exercise group; changes were negligible in the control group. Significant differences were also observed for physical tests, in favor of the specific exercise group.Conclusion: An individualized treatment approach with specific stabilizing exercises appears to be more effective than physical therapy without specific stabilizing exercises for women with pelvic girdle pain after pregnancy. The Patient Perspective Survey (PPS) is designed to enhance consultation outcome and patient participation among general practice patients with complex health problems. This article presents the final version of the survey. This version is more condensed, tested for psychometric and clinical properties, and with increased emphasis on coping. The development and testing was performed in two phases with two groups of GPs and patients. Indications for use and criteria for rejection of items were defined, based on comprehensive data analysis and clinical judgement. The number of items was reduced to 38 items. The majority of diagnoses were related to psychosocial or musculoskeletal disorders. For the survey's main dimensions (psychological and somatic items, everyday life situation and coping) sound psychometric properties were identified. Seven open-ended questions were found useful for obtaining important new information and stimulating processes, often related to coping aspects. Background: Overweight and obesity are a global pandemic, contributing to death and disability-adjusted life-years. Obesity is a major factor in the onset of chronic inflammatory diseases (CIDs). Yet, several knowledge gaps remain: For several CIDs, inconsistent results have been reported, relating to their obesity-imposed risk, data on most rare CIDs remain unavailable, sex differences and racial disparities remain mostly unaddressed. Methods: A large-scale cohort study compared the risk of developing 46 CIDs in individuals with overweight/obesity (n=3,101,824) to an equal number of non-overweight/obese individuals. Propensity score matching optimized between-group comparability, and sensitivity analyses assessed study robustness. Results: The risk of developing any CID was 28.48% in overweight/obese individuals versus 17.55% in non-overweight/obese controls, with a hazard ratio (95%-confidence interval) of 1.52 (1.509-1.521, p<0.0001). This risk was consistent across all sensitivity, sex-, and race-stratified analyses. Overweight and obesity were associated with an increased risk for 24 of 46 CIDs in the primary analysis and all sensitivity analyses. For 12 diseases, increased risks were confirmed to one of the two sensitivity analyses, while for 10 diseases, results were discordant. No increased risk was observed for one disease. In sex-stratified analysis, overweight and obesity posed a more pronounced risk for four CIDs in female individuals. In race-stratified analysis, overweight and obesity were linked to a higher risk for seven CIDs in White individuals and to one CID in "Black or African American" individuals. Purpose: Diabetes Mellitus (DM) management is increasingly focusing on patient-centered care, making patient-reported experience measures (PREMs) critical for understanding the subjective aspects of diabetes treatment and self-management. These measures differ based on cultural contexts and individual perspectives, leading different countries to the development of country-specific tools to assess care quality from the patient's viewpoint. This review aimed to identify available instruments for assessing patient-reported experiences in individuals with diabetes and examine the different domains, items, and the validity and reliability of these instruments. Methods: Following PRISMA-ScR guidelines, databases including PubMed, Embase, CINAHL, Cochrane, and Scopus were searched for English-language articles without year limitations. This scoping review focused on PREMs that evaluate the quality of diabetes care among adolescent and adult patients with type 1 and type 2 DM. Studies that used patient expectation questionnaires, involved individuals not receiving care, or focused on patient-reported outcomes rather than experiences were excluded. Results: Eight articles from six countries representing different healthcare settings were included, mostly from developed countries. A variety of methodologies were used to develop these PREM instruments, with unique domains and items. Content analysis revealed five commonly measured domains: (1) care planning, (2) patient education, (3) professionalism, (4) quality of care, and (5) hospital care and transition, reflecting diverse patient experiences across healthcare services. Conclusions: This scoping review identifies a limited number of tools for evaluating PREMs in diabetes care, highlighting variability in their development and domain coverage. Five core domains are proposed across different settings, with an emphasis on culturally adapted measures to enhance the accuracy of patient experience capture in diverse populations. Background: Pregnancy-related posterior pelvic girdle pain (PPGP) is a common cause of back pain and disability in the postpartum period. The objective of this study was to investigate the efficacy of orthotic support on pain, disability, and motor control in women with pregnancy-related PPGP. Methods: Eighty-four women with a clinical diagnosis of pregnancy-related PPGP participated in this randomized controlled trial (RCT). Participants were randomly allocated into three groups (with a ratio of 1:1:1): the pelvic support group, the lumbar support group, and the control group (patient-education leaflet). Pain severity, disability, effort during active straight leg raising test (ASLR), maximum isometric muscle force (hip flexion and trunk rotation), and joint position reproduction (JPR) of hip abduction were assessed as study outcomes. These variables were measured at four time points —before the intervention, immediately after the intervention, at the 4-week follow-up (at this time, the intervention period was terminated), and at the 5-week follow-up (one week after discontinuing the interventions)— to evaluate the possible effects of wearing support. Repeated-measures multivariate analysis of variance (MANOVA) was applied to determine the statistical significance between groups. Bonferroni post-hoc correction was used to identify significant differences between groups at different study time points. Results: There was a significant interaction effect for group × time for the study outcomes, including pain severity, disability, effort during ASLR, and maximum isometric muscle force between groups (p < 0.001), except JPR of hip abduction (p = 0.13). There were statistically significant differences in post hoc comparisons for pain intensity and effort during ASLR in lumbar support versus control condition and for maximum isometric muscle force in orthotic interventions versus control conditions immediately after the intervention (P < 0.008). Post hoc tests demonstrated statistically significant differences in orthotic interventions versus control conditions after 4-week and 5-week follow-ups (P < 0.008). None of the interventions significantly changed the JPR of hip abduction compared to the control group (p > 0.008). The effect sizes for study outcomes were large, except for the JPR of hip abduction. Conclusions: For women with pregnancy-related PPGP, both lumbar and pelvic supports were beneficial for decreasing pain and disability symptoms. Objective: To enable measurement of coping in the general Danish population the aims of this study are to 1) describe the translation and cultural adaption of the Danish Brief Approach/Avoidance Coping Questionnaire (BACQ) and 2) investigate the psychometric properties of the Danish BACQ. Design: The BACQ was translated and adapted into Danish, and the psychometric properties tested in two samples of adult Danish citizens: Sample A = 167, used for exploratory factor analysis (EFA), and Sample B = 330 persons, used for confirmatory factor analysis (CFA). Internal consistency was evaluated by Cronbach's Alpha, item-to-rest correlation, and scale-to-scale Pearson correlation. Results: The EFA suggested reasonable fits for both a three-factor and four-factor model, confirmed by the CFA with acceptable goodness-of-fit indices for both models. Using the four-factor-model would require a re-evaluation of the scale. The three-factor model had admissible internal consistency with an overall Cronbach's alpha of 0.66. Individuals with low self-rated health, extreme concern about current health and poor physical fitness, respectively, had lower Approach and higher Diversion and Resignation scores. Conclusion: The psychometric properties showed that the Danish BACQ could be used as a three-factor model. Subjects and design: The study was based on the total patient sample (n =110) of a randomized controlled trial comparing two intervention methods advising cardiovascular high-risk men of lifestyle changes in general practice. Behaviour and risk factor changes during the one-year intervention study were analysed using multiple regression and logistic regression analyses with the above-mentioned independent variables. Setting: Twenty-two general practice centres in the county of Hordaland, western Norway. Results: Self-efficacy of increased physical exercise was the only variable significantly related to exercise change. Age and self-efficacy were statistical significant predictors of smoking cessation success. None of the independent variables was statistically significantly related to blood pressure or cholesterol change. Educational level related negatively, although statistically insignificantly, with total risk change. Conclusion: The study confirms the importance of self-efficacy in both human behaviour and motivation for behaviour change. Purpose: To compare sociodemographic, life style and clinical characteristics in patients operated for lumbar disc herniation in public and private clinics in Norway, and evaluate whether selection for surgery and surgical treatment were different across the two settings., Methods: A cross-sectional multicenter study of patients who underwent elective surgeries for lumbar disc herniation at 41 (31 public and 10 private) hospitals. Data were included in the Norwegian Registry for Spine Surgery., Results: Of the 5,308 elective surgical procedures, 3,628 were performed at public hospitals and 1,680 at private clinics. Patients in the private clinics were slightly younger, more likely to be man, have higher level of education, and more likely to be employed. Disability and retirement pensions were more than double in the public as compared to the private clinics. Mean duration of sick leave was 24 weeks (SD 36.4) in the public and 15 weeks (20.7) in the private clinics. There were minor differences in pain, disability and quality-of life, number of verified disc herniations and radiological findings. Number of days at hospital, total operation time and proportion of complications were significantly higher in the public than in the private clinics., Conclusion: Patients having elective surgery due to lumbar disc herniation in public and private clinics were different with respect to many sociodemographic and life style variables. There were minor differences with respect to clinical variables and selection of patients for surgery, but substantial differences related to aspects of the surgical treatment.
