Abstract

The presence of seismic threat multiplies the environmental hazard, especially for oil and gas production and transport facilities in water areas. Currently, there are no normative maps of general seismic zoning of the water areas of the inland and marginal seas of Russia, especially since there are practically no maps of detailed seismic zoning and seismic microzoning of even individual parts of the water areas. Taking into account the fact that intensive development of offshore oil and gas fields and the Northern Sea Route has begun, the development of such maps becomes a very urgent scientific and practical task. The seismic hazard assessment for the submerged crossing was carried out in 2008. The initial seismic effects were calculated using a probabilistic seismic hazard analysis based on five models of seismic zones and three types of models of attenuation of peak and spectral accelerations. The results of the performed calculations, including deaggregation, have shown that the initial seismicity of the area of the gas pipeline crossing route through the Nevelskoy Strait for a return period of 1,000 years is lower than indicated on the OSR-2016-B map, where the eastern end of the crossing route through the strait is characterized by the initial seismicity equal to 9 points. The soil conditions (seismic microzoning) have been taken into account by three different methods: the seismic rigidity method, the calculation method, and the method considering the thickness of Quaternary deposits. The present studies show that seismic effects vary along the pipeline route from the mainland to Sakhalin Island from 8.4 to 8.9 on the MSK-64 scale for the recurrence period of seismic shaking T = 1,000 years and from 9.3 to 9.7 points for T = 5,000 years.

Abstract

Background: The SUCCOR cohort was developed to analyse the overall and disease-free survival at 5 years in women with FIGO 2009 stage IB1 cervical cancer. The aim of this study was to compare the use of adjuvant therapy in these women, depending on the method used to diagnose lymphatic node metastasis. Patients and Methods: We used data from the SUCCOR cohort, which collected information from 1049 women with FIGO 2009 stage IB1 cervical cancer who were operated on between January 2013 and December 2014 in Europe. We calculated the adjusted proportion of women who received adjuvant therapy depending on the lymph node diagnosis method and compared disease free and overall survival using Cox proportional-hazards regression models. Inverse probability weighting was used to adjust for baseline potential confounders. Results: The adjusted proportion of women who received adjuvant therapy was 33.8% in the sentinel node biopsy + lymphadenectomy (SNB+LA) group and 44.7% in the LA group (p = 0.02), although the proportion of positive nodal status was similar (p = 0.30). That difference was greater in women with negative nodal status and positive Sedlis criteria (difference 31.2%, p = 0.01). Here, those who underwent a SNB+LA had an increased risk of relapse [hazard ratio (HR) 2.49, 95% confidence interval (CI) 0.98–6.33, p = 0.056] and risk of death (HR 3.49, 95% CI 1.04–11.7, p = 0.042) compared with those who underwent LA. Conclusions: Women in this study were less likely to receive adjuvant therapy if their nodal invasion was determined using SNB+LA compared with LA. These results suggest a lack of therapeutic measures available when a negative result is obtained by SNB+LA, which may have an impact on the risk of recurrence and survival.

Abstract

Objective: Based on the SUCCOR study database, our primary objective was to identify the independent clinical pathological variables associated with the risk of relapse in patients with stage IB1 cervical cancer who underwent a radical hysterectomy. Our secondary goal was to design and validate a risk predictive index (RPI) for classifying patients depending on the risk of recurrence. Methods: Overall, 1116 women were included from January 2013 to December 2014. We randomly divided our sample into two cohorts: discovery and validation cohorts. The test group was used to identify the independent variables associated with relapse, and with these variables, we designed our RPI. The index was applied to calculate a relapse risk score for each participant in the validation group. Results: A previous cone biopsy was the most significant independent variable that lowered the rate of relapse (odds ratio [OR] 0.31, 95% confidence interval [CI] 0.17–0.60). Additionally, patients with a tumor diameter >2 cm on preoperative imaging assessment (OR 2.15, 95% CI 1.33–3.5) and operated by the minimally invasive approach (OR 1.61, 95% CI 1.00–2.57) were more likely to have a recurrence. Based on these findings, patients in the validation cohort were classified according to the RPI of low, medium, or high risk of relapse, with rates of 3.4%, 9.8%, and 21.3% observed in each group, respectively. With a median follow-up of 58 months, the 5-year disease-free survival rates were 97.2% for the low-risk group, 88.0% for the medium-risk group, and 80.5% for the high-risk group (p < 0.001). Conclusion: Previous conization to radical hysterectomy was the most powerful protective variable of relapse. Our risk predictor index was validated to identify patients at risk of recurrence.

Abstract

In order to ensure trouble-free operation of offshore oil production and transportation facilities, it is necessary to consider seismic hazard in their design. As part of this work, seismic hazard calculations were performed for the Zhenis oil and gas field facilities located in the Kazakhstan sector of the Middle Caspian. According to this project, the initial seismicity and seismic microzoning were clarified, i.e., accounting for real soil conditions made by different methods. Calculations showed that the intensity of seismic shocks in the Zhenis field can be 8.31 points on the MSK-64 scale, which corresponds of acceleration of a sea bottom ground of 0.253 g. Abstract

ObjectiveTo evaluate disease-free survival of cervical conization prior to radical hysterectomy in patients with stage IB1 cervical cancer (International Federation of Gynecology and Obstetrics (FIGO) 2009).MethodsA multicenter retrospective observational cohort study was conducted including patients from the Surgery in Cervical Cancer Comparing Different Surgical Aproaches in Stage IB1 Cervical Cancer (SUCCOR) database with FIGO 2009 IB1 cervical carcinoma treated with radical hysterectomy between January 1, 2013, and December 31, 2014. We used propensity score matching to minimize the potential allocation biases arising from the retrospective design. Patients who underwent conization but were similar for other measured characteristics were matched 1:1 to patients from the non-cone group using a caliper width ≤0.2 standard deviations of the logit odds of the estimated propensity score.ResultsWe obtained a weighted cohort of 374 patients (187 patients with prior conization and 187 non-conization patients). We found a 65% reduction in the risk of relapse for patients who had cervical conization prior to radical hysterectomy (hazard ratio (HR) 0.35, 95% confidence interval (CI) 0.16 to 0.75, p=0.007) and a 75% reduction in the risk of death for the same sample (HR 0.25, 95% CI 0.07 to 0.90, p=0.033). In addition, patients who underwent minimally invasive surgery without prior conization had a 5.63 times higher chance of relapse compared with those who had an open approach and previous conization (HR 5.63, 95% CI 1.64 to 19.3, p=0.006). Patients who underwent minimally invasive surgery with prior conization and those who underwent open surgery without prior conization showed no differences in relapse rates compared with those who underwent open surgery with prior cone biopsy (reference) (HR 1.94, 95% CI 0.49 to 7.76, p=0.349 and HR 2.94, 95% CI 0.80 to 10.86, p=0.106 respectively).ConclusionsIn this retrospective study, patients undergoing cervical conization before radical hysterectomy had a significantly lower risk of relapse and death.

Abstract

This study examined burnout dynamics among Ukrainian academic staff during the full-scale war. A cross-sectional study was conducted twice: the first wave in July 2022 and the second in January 2023. To assess the burnout syndrome as the final outcome, the self-reported Maslach Burnout Inventory-Human Services adapted for socioeconomic professions by Vodopyanova was used and correlated with different characteristics. The results showed a significant positive trend in emotional exhaustion among Ukrainian academic staff, with females being more sensitive to war-related factors. The results demonstrate that prolonged stressful situations associated with the ongoing war and constant changes in professional conditions lead to significant depersonalization dynamics among academic staff of both sexes. For male academic staff, factors such as age and academic position become less significant for depersonalization. However, university relocation and migration processes were significant factors affecting female academic staff's' perception of effectiveness and accomplishment in their professional activities. The alarming dynamic of burnout levels detected among Ukrainian academic staff urges the national and institutional levels to take prompt actions to enhance the academic staff's mental health in the workplace for preserving not only the quality of higher education, but also human capital in war times for postwar recovery in Ukraine., (© 2023. Abstract

Abstract—In 2007–2015, Shirshov Institute of Oceanology of the Russian Academy of Sciences (IO RAS) carried out seismological observations in the water area and on the coast of the Baltic Sea using autonomous seismic stations. Here, in the area of the Sambia Peninsula, previously considered aseismic, a strong perceptible earthquake with a magnitude of about M = 4.6 occurred in 2004. The most interesting data were obtained by IO RAS in 2008–2009 from seismological monitoring using autonomous bottom and coastal seismic stations. The data obtained in 2010–2015 turned out to be unsuitable for full-scope processing due to several causes (losses of bottom seismographs, high noise level at coastal stations, etc.). Seismological monitoring in the west of the Kaliningrad region and in the adjoining area of the Baltic Sea detected weak earthquakes with magnitudes ML = 2.5–3 whose sources are confined to the development are of the Kravtsovskoe offshore oil field. Some of these earthquakes have been recorded by the stations of the Norwegian seismic array NORSAR and by the seismic stations of Sweden. The Kravtsovskoe oil field is located on the shelf northwest of the unique natural object, the Curonian Spit. The detected weak earthquakes are likely to be anthropogenic, induced by reservoir pressure disturbances as a result of intensive hydrocarbon production, and are probably precursors of a strong man-made earthquake. Abstract

The vagina and its microbiota form a balanced ecosystem; any movement outside the normal range for this ecosystem of obligate and/or facultative microbes, termed dysbacteriosis, can lead to infection and disease. This review summarizes recent research on the vaginal ecosystem, the Lactobacillus species that dominate it, and the means by which they suppress the growth, development, and/or proliferation of pathogenic microbial species. Lactobacilli colonization is believed to be beneficial since it prevents other microorganisms from colonizing the vaginal epithelium via competitive adhesion, interactions with the local immunity and plasminogen-plasmin system, and the production of lactic acid, hydrogen peroxide, and antibacterial substances. In addition, lactobacilli also constitute an important part of the urogenital tract microbiota and the lactobacilli-dominated vaginal microbiome is a major determinant for female urogenital health.

Abstract

Thrombophilia is associated with increased risks of venous thrombosis in women taking oral contraceptive preparations. Universal thrombophilia screening in women prior to prescribing oral contraceptive preparations is not supported by current evidence. The case is presented of a 23 year-old women with a personal history of interruption and on the same day started with oral contraceptive (0.03 microg ethynil estradiol - 0.075 microg gestodene), which due on a 18 pill/day to acute headache, increasing vomiting and speaking defects. Physical/neurologic/gynecologic examinations observed a normal status. The MRI and CT revealed thrombosis of the left internal jugular vein and brain ischemia with cerebral infarction and ischemic stroke. The acute therapy of thrombotic findings was accompanied with many tests. The thrombophilia PCR-Real time - test finds heterozygous carrier of the Factor V (Leiden). This case shows the need of large prospective studies that should be undertaken to refine the risks and establish the associations of thrombophilias with venous thrombosis among contraceptive users. The key to a prompt diagnosis is to know the risk factors. The relative value of a thrombophilia screening programme before contraceptive using needs to be established.

Abstract

The distribution and prevalence of 33 high-, medium-, and low-risk HPV genotypes were surveyed in women aged between 15 and 54 years and with normal cytology in the biggest cities and regions in Bulgaria (viz., Sofia, Plovdiv, Varna, and Burgas) and in two smaller communities (viz., Pleven and Vidin). This survey was also intended to update previous data collected in 2012 and to assess the results of a national campaign to immunize all 12-year-old girls against HPV, which was also started in 2012. The GenoFlow HPV Array Test Kit was used to analyze HPV status in cervical samples collected in 2012-2016 from 5277 women who had visited 47 gynecological clinics across the study sites. HPV infection was confirmed in 1570 (29.8%). The most common infecting genotype was HPV-16, which was detected in 687 women (13%), followed by HPV-56 in 333 women (6.3%), HPV-33 in 209 cases (4%), and HPV-31 in 190 subjects (3.6%). Abstract

YCr1-xFexO3 materials with x = 0; 0.125; 0.25; 0.33; 0.5; 0.67; 0.75; 0.875 and 1 have been prepared by solution combustion method followed by thermal treatment at 800° C in air. X-ray powder diffraction spectra of samples confirmed the formation of solid solutions within the whole concentration interval. All spectra were successfully indexed in orthorhombic Pnma space group. Neutron diffraction measurements at room temperature have confirmed that some of the samples studied are antiferromagnetically ordered. This is consistent with the asymptotic Curie temperatures determined from the dependence of the reverse magnetic susceptibility on temperature. The effective magnetic moments were determined from the Rietveld refinement of the neutron powder diffraction data. [ABSTRACT FROM AUTHOR]
