Abstract Spinal cord injury (SCI) results in rapid, severe osteoporosis and an increased risk of lower extremity fractures. Despite the medical complications associated with these fractures, there is no standard of care to prevent osteoporotic fractures following SCI. Functional electrical stimulation‐ (FES‐) assisted rowing is a promising intervention to improve bone health in SCI because of its ability to generate a muscular contraction in conjunction with mechanical loading of the lower extremity long bones. Combination therapy consisting of FES‐rowing plus zoledronic acid (ZA) may be a superior treatment via inhibition of bone resorption and stimulation of new bone formation. We studied participants enrolled in a randomized clinical trial comparing FES‐rowing alone with FES‐rowing plus ZA to improve bone health in SCI. Volumetric CT scans at the distal femur and proximal tibial metaphyses were performed. Bone geometric properties (cortical thickness index [CTI], cortical compressive strength index [CSI], buckling ratio [BR], bending strength index) and mineral (cortical bone volume [CBV], cortical bone mineral density, cortical bone mineral content) indices were determined. In models adjusting for baseline values, we found that the CBV (p = 0.05 to 0.006), the CTI (p = 0.009), and the BR (p = 0.001) at both the distal femoral and proximal tibial metaphyses were greater in the ZA plus rowing group compared with the rowing‐only group. Similarly, there was a significant positive association between the total rowing work completed and the BR at the proximal tibia (p = 0.05). A subgroup analysis of the rowing‐only arm showed that gains in the CSI at the tibial metaphysis varied in a dose‐dependent fashion based on the total amount of exercise performed (p = 0.009). These findings demonstrate that the osteogenic response to FES‐rowing is dose‐dependent. Combination therapy with ZA and FES‐row training has therapeutic potential to improve bone quality, and perhaps reduce fracture risk at the most common fracture site following SCI. © 2019 The Authors. Spinal cord injury is associated with skeletal unloading, sedentary behavior, decreases in skeletal muscle mass, and exercise intolerance, which results in rapid and severe bone loss. To date, monotherapy with physical interventions such as weight-bearing in standing frames, computer-controlled electrically stimulated cycling and ambulation exercise, and low-intensity vibration are unsuccessful in maintaining bone density after SCI. Strategies to maintain bone density with commonly used osteoporosis medications also fail to provide a significant clinical benefit, potentially due to a unique pathology of bone deterioration in SCI. In this review, the available data is discussed on evaluating and monitoring bone loss, fracture, and physical and pharmacological therapeutic approaches to SCI-associated disease of the skeleton. Many researchers have investigated femur fractures using 3D models created with finite element (FE) software; however, these models need validation. Cadavers are used in experiments to validate the FE model. Nevertheless, there are several restrictions and obstacles to experimenting on the cadaver femur bone. The aim of this study was to investigate the effect of loading direction on the stress distribution and fracture risk of a proximal femur bone under quasi-static loading in a sideways fall condition. A validated 3D FE model of the proximal femur was developed by employing the results obtained from a quasi-static experimental test. Instead of cadaver, 3D-printed proximal femur bone was used. Various fall loading configurations were used to simulate a sideways fall with inclination angles from 0° to 20° and rotational angles from −15° to 15°. The highest von Mises stress is due to sideways falls distributed in the femur neck region. This study provides new information on FE model construction and medical FE analysis. Spinal cord injury (SCI) is associated with bone fragility and fractures around the knee. The purpose of this investigation was to validate a computed tomography (CT) based finite element (FE) model of the proximal tibia and distal femur under biaxial loading, and to retrospectively quantify the relationship between model predictions and fracture incidence. Twenty-six cadaveric tibiae and femora (n = 13 each) were loaded to 300 N of compression, then internally rotated until failure. FE predictions of torsional stiffness (K) and strength (T ult ) explained 74% (n = 26) and 93% (n = 7) of the variation in experimental measurements, respectively. Univariate analysis and logistic regression were subsequently used to determine if FE predictions and radiographic measurements from CT and dual energy X-ray absorptiometry (DXA) were associated with prevalent lower-limb fracture in 50 individuals with SCI (n = 14 fractures). FE and CT measures, but not DXA, were lower in individuals with fracture. FE predictions of T ult at the tibia demonstrated the highest odds ratio (4.98; p = 0.006) and receiver operating characteristic (0.84; p = 0.008) but did not significantly outperform other metrics. In conclusion, CT-based FE model predictions were associated with prevalent fracture risk after SCI; this technique could be a powerful tool in future clinical research.

A small scale mechanical loading and testing device was designed and fabricated to apply in vivo loads to mouse tibiae and to mechanically test the bones in three-point bending. A linear actuator and load cell were used to apply accurately controlled forces to the samples. The device was tested and validated using standard materials of known properties to quantify accuracy and precision. The device had accuracy and precision errors of less than 5% for most of the material tests, an accuracy error of less than 10% and precision error of less than 5% for in vivo loading, and a precision error of less than 5% for ex vivo bone testing. Our results showed that the device was capable of implementing loading interventions and could be used as a tool to assess the response of these interventions. This device represents one solution to the lack of portability and a high setup cost that prevent some research groups from adopting in vivo animal loading. [ABSTRACT FROM AUTHOR]

Slowing the decline in walking mobility in the elderly is critical for maintaining the quality of life. Wearable assistive devices may 1 day facilitate mobility in older adults; however, we need to ensure that such devices do not impair stability in this population that is predisposed to fall-related injuries. This study sought to quantify the effects of untethered ankle exoskeleton assistance on measures of stability, whole-body dynamics, and strategies to maintain balance during normal and perturbed walking in older adults. Eight healthy participants (69-84 years) completed a treadmill-based walking protocol that included perturbations from unexpected belt accelerations while participants walked with and without ankle exoskeleton assistance. Exoskeleton assistance increased frontal plane range of angular momentum (8-14%, p ≤ 0.007), step width (18-34%, p ≤ 0.006), and ankle co-contraction (21-29%, p ≤ 0.039), and decreased biological ankle moment (16-27%, p ≤ 0.001) during unperturbed and perturbed walking; it did not affect the anteroposterior margin-of-stability, step length, trunk variability, or soleus activity during unperturbed and perturbed walking. Metatarsal bone stress injuries (BSIs) are common in athletic populations. BSIs are overuse injuries that result from an accumulation of microdamage that exceeds bone remodeling. Risk for metatarsal BSI is multifactorial and includes factors related to anatomy, biology, and biomechanics. In this article, anatomic factors including foot type, metatarsal length, bone density, bone geometry, and intrinsic muscle strength, which each influence how the foot responds to load, are discussed. Biologic factors such as low energy availability and impaired bone metabolism influence the quality of the bone. Finally, the influence of biomechanical loads to bone such as peak forces, load rates, and loading cycles are reviewed. General management of metatarsal BSI is discussed, including acute care, rehabilitation, treatment of refractory metatarsal BSI, and evaluation of healing/return to sport. We hypothesize that variations of body anthropometry, conjointly with the bone strength, determine the risk of hip fracture. To test the hypothesis, we compared, in a simulated sideways fall, the hip impact energy to the energy needed to fracture the femur. Ten femurs from elderly donors were tested using a novel drop-tower protocol for replicating the hip fracture dynamics during a fall on the side. The impact energy was varied for each femur according to the donor's body weight, height and soft-tissue thickness, by adjusting the drop height and mass. The fracture pattern, force, energy, strain in the superior femoral neck, bone morphology and microarchitecture were evaluated. Fracture patterns were consistent with clinically relevant hip fractures, and the superior neck strains and timings were comparable with the literature. The hip impact energy (11 – 95 J) and the fracture energy (11 – 39 J) ranges overlapped and showed comparable variance (CV = 69 and 61%, respectively). The aBMD-based definition of osteoporosis correctly classified 7 (70%) fracture/non-fracture cases. The incorrectly classified cases presented large impact energy variations, morphology variations and large subcortical voids as seen in microcomputed tomography. Objectives. The purpose of this experimental study was to investigate the effects of nonelastic taping and dual task on ankle kinematics and kinetics in gait analysis of healthy adults. Methods. A total of 21 healthy adults completed trials of gait analysis using a Vicon system combining ground walking with different cognitive task conditions (none, modified Stroop color/character naming, and serial-7 subtraction), with or without nonelastic taping. Ankle kinematics and kinetics including speed, ankle plantarflexion and inversion angle, ground reaction force (GRF), and stride time variability (STV) under all conditions of taping (YES or NO) and cognitive task (none, naming, and subtraction) were characterized and analyzed with repeated-measures ANOVA. Results. As regards cognitive performance, the serial-7 subtraction performance under walking conditions with and without taping was significantly poorer than simple sitting condition (P < 0.001). For kinematics and kinetics, STV showed statistically significant decrease (P = 0.02) when subjects underwent taping application. Vertical GRF was significantly greater under taping than barefoot (P = 0.001). Ankle plantarflexion at initial contact (IC) under the dual-task walking was significantly more than under simple walking (P = 0.008). Conclusions. Applications of nonelastic taping and dual task may lead to the STV, vertical GRF, ankle plantarflexion, and speed alterations because of restricted joint range of motion and changed sensorimotor neural circuit. During walking in neurologically-intact controls, larger mediolateral pelvis displacements or velocities away from the stance foot are accompanied by wider steps. This relationship contributes to gait stabilization, as modulating step width based on pelvis motion (hereby termed a “mechanically-appropriate” step width) reduces the risk of lateral losses of balance. The relationship between pelvis displacement and step width is often weakened among people with chronic stroke (PwCS) for steps with the paretic leg. Our objective was to investigate the effects of a single exposure to a novel force-field on the modulation of paretic step width. This modulation was quantified as the partial correlation between paretic step width and pelvis displacement at the step’s start (step start paretic $\rho _{\text {disp}}$). Following 3-minutes of normal walking, participants were exposed to 5-minutes of either force-field assistance (n = 10; pushing the swing leg toward mechanically-appropriate step widths) or perturbations (n = 10: pushing the swing leg away from mechanically-appropriate step widths). This period of assistance or perturbations was followed by a 1-minute catch period to identify after-effects, a sign of altered sensorimotor control. The effects of assistance were equivocal, without a significant direct effect or after-effect on step start paretic $\rho _{\text {disp}}$. In contrast, perturbations directly reduced step start paretic $\rho _{\text {disp}}$ (p = 0.004), but were followed by a positive after-effect (p = 0.02). These results suggest that PwCS can strengthen the link between pelvis motion and paretic step width if exposed to a novel mechanical environment. Virtual Reality is a versatile platform to study human behavior in simulated environments and to develop interventions for functional rehabilitation. In this work, we designed a dual-task paradigm in a virtual environment where both tasks demand motor skills. Twenty-one healthy adults (mean age: 24.1 years) participated in this study. The experiment involved three conditions - normal overground walking, catch and throw a ball while standing, and catch and throw a ball while walking overground —all in the virtual environment. We investigated the dual-task gait characteristics and their correlations with outcomes from cognitive assessments. Results show that subjects walk conservatively with smaller stride lengths, larger stride widths and stride time while catching and throwing. However, they are able to throw the balls more accurately at the target and achieve higher scores. During the dual-task throw, we observed that the participants threw more balls during the stance phase of the gait when the foot was in the terminal stance and pre-swing region. During this region, the body has forward momentum. In addition, the changes in gait characteristics during dual-task throw correlate well with outcome measures in standardized cognitive tests. This study provides a new and engaging paradigm to analyze dual-motor-task cost in a virtual reality environment and it can be used as a basis to compare strategies adopted by different population groups with healthy young adults to execute coordinated motor tasks. Summary: This study investigates the effectiveness of a robot-mediated strategy aimed at promoting balance recovery after multidirectional slippages. Six older adults were asked to manage anteroposterior and mediolateral slippages while donning an active pelvis orthosis (APO). The APO was set up either to assist volunteers during balance loss or to be transparent. The margin of stability, in sagittal and frontal planes, was the main metric to assess the effectiveness of balance recovery. Results showed that the assistive strategy is effective at promoting balance recovery in the sagittal plane, for both perturbing paradigms; however, it is not effective at controlling stability in the frontal plane. Objective: This study sought to determine the changes in muscle activity about the ankle, knee, and hip in able-bodied people walking at steady state on surfaces with different degrees of slipperiness. Methods : Muscle activity was measured through electromyographic signals from selected lower limb muscles and quantified to directly compare changes across surface conditions. Results: Our results showed distinct changes in the patterns of muscle activity controlling each joint. Muscles controlling the ankle showed a significant reduction in activity as the surface became more slippery, presumably resulting in a compliant distal joint to facilitate full contact with the surface. Select muscles about the knee and hip showed a significant increase in activity as the surface became more slippery. This resulted in increased knee and hip flexion likely contributing to a lowering of the body's center of mass and stabilization of the proximal leg and trunk. Conclusion : These findings suggest a proximal-distal gradient in the control of muscle activity that could inform the future design of adaptable prosthetic controllers. Significance: Walking on a slippery surface is extremely difficult, especially for individuals with lower limb amputations because current prostheses do not allow the compensatory changes in lower limb dynamics that occur involuntarily in unimpaired subjects. With recent advances in prosthetic control, there is the potential to provide some of these compensatory changes; however, we first need to understand how able-bodied individuals modulate their gait under these challenging conditions. The objective of this study was to examine the relationship between slip, trip and fall injuries and obesity in a population of workers at the Idaho National Laboratory (INL) in Idaho Falls, Idaho. INL is an applied engineering facility dedicated to supporting the US Department of Energy's mission. An analysis was performed on injuries reported to the INL Medical Clinic to determine whether obesity was related to an increase in slip, trip and fall injuries. Records were analysed that spanned a 6-year period (2005–2010), and included 8581 employees (mean age, 47 ± 11 years and body mass index [BMI], 29 ± 5 kg/m2; 34% obesity rate). Of the 189 people who reported slip, trip and fall injuries (mean age, 48 ± 11 years), 51% were obese (P < 0.001 compared with uninjured employees), and their mean BMI was 31 ± 6 kg/m2(P < 0.001). Obesity in this population was associated with a greater rate of slip, trip and fall injuries. The objective of the study was to design a virtual reality (VR) training to induce perturbation in older adults similar to a slip and examine the effect of the training on kinematic and muscular responses in older adults. Twenty-four older adults were involved in a laboratory study and randomly assigned to two groups (VR training and control). Both groups went through three sessions including baseline slip, training, and transfer of training on slippery surface. The training group experienced 12 simulated slips using a visual perturbation induced by tilting a VR scene while walking on the treadmill and the control group completed normal walking during the training session. Kinematic, kinetic, and electromyography data were collected during all the sessions. Results demonstrated the proactive adjustments such as increased trunk flexion at heel contact after training. Reactive adjustments included reduced time to peak activations of knee flexors, reduced knee coactivation, reduced time to trunk flexion, and reduced trunk angular velocity after training. In conclusion, the study findings indicate that the VR training was able to generate a perturbation in older adults that evoked recovery reactions and such motor skill can be transferred to the actual slip trials. Summary: Computed tomography and finite element modeling were used to assess bone mineral and stiffness loss at the knee following acute spinal cord injury (SCI). Marked bone mineral loss was observed from a combination of trabecular and endocortical resorption. Reductions in stiffness were 2-fold greater than reductions in integral bone mineral. Introduction: SCI is associated with a rapid loss of bone mineral and an increased rate of fragility fracture. The large majority of these fractures occur around regions of the knee. Our purpose was to quantify changes to bone mineral, geometry, strength indices, and stiffness at the distal femur and proximal tibia in acute SCI. Methods: Quantitative computed tomography (QCT) and patient-specific finite element analysis were performed on 13 subjects with acute SCI at serial time points separated by a mean of 3.5 months (range 2.6-4.8 months). Changes in bone mineral content (BMC) and volumetric bone mineral density (vBMD) were quantified for integral, trabecular, and cortical bone at epiphyseal, metaphyseal, and diaphyseal regions of the distal femur and proximal tibia. Changes in bone volumes, cross-sectional areas, strength indices and stiffness were also determined. Results: Bone mineral loss was similar in magnitude at the distal femur and proximal tibia. Reductions were most pronounced at epiphyseal regions, ranging from 3.0 % to 3.6 % per month for integral BMC ( p < 0.001) and from 2.8 % to 3.4 % per month ( p < 0.001) for integral vBMC. Trabecular BMC decreased by 3.1-4.4 %/month ( p < 0.001) and trabecular vBMD by 2.7-4.7 %/month ( p < 0.001). A 3.8-5.4 %/month reduction was observed for cortical BMC ( p < 0.001); the reduction in cortical vBMD was noticeably lower (0.6-0.8 %/month; p ≤ 0.01). The cortical bone loss occurred primarily through endosteal resorption, and reductions in strength indices and stiffness were some 2-fold greater than reductions in integral bone mineral. Individuals with acute SCI experienced a marked loss of bone mineral from a combination of trabecular and endocortical resorption. Changes in bone mineral content (BMC) and volumetric bone mineral density (vBMD) were quantified for integral, trabecular, and cortical bone at the femoral neck, trochanteric, and total proximal femur regions. Changes in bone volumes, cross-sectional areas, and surrogate measures of compressive and bending strength were also determined. Results: During the acute period of SCI, subjects experienced a 2.7-3.3 %/month reduction in integral BMC ( p < 0.001) and a 2.5-3.1 %/month reduction in integral vBMD ( p < 0.001). Trabecular BMC decreased by 3.1-4.7 %/month ( p < 0.001) and trabecular vBMD by 2.8-4.4 %/month ( p < 0.001). A 3.9-4.0 %/month reduction was observed for cortical BMC ( p < 0.001), while the reduction in cortical vBMD was noticeably lower (0.8-1.0 %/month; p ≤ 0.01). Changes in bone volume and cross-sectional area suggested that cortical bone loss occurred primarily through endosteal resorption. Declines in bone mineral were associated with a 4.9-5.9 %/month reduction in surrogate measures of strength. With the elderly population increase, osteoporosis and its consequences have become not just a health issue but also a serious economic burden. The trabecular bone structure plays a very important role for the bone quality and mechanical competence of the scaffold. Currently, it is claimed that the trabecular microarchitecture understanding can improve the fracture risk prediction above 65%. Several parameters seem to be correlated providing structural details of the trabecular bone network. However, the tortuosity of the trabeculae has not yet been systematically taken into account and its contribution has not been fully investigated and understood. In this paper, we discuss the relationship between the trabecular tortuosity, connectivity, volume fraction, and elasticity, and provide a unified parameter to estimate the mechanical competence of the structure. It is shown that the trabecular network tortuosity presents high linear correlation with the other parameters and that the trabeculae tend to get aligned in the direction where the structure is mostly submitted to stress, corresponding to higher stiffness orientation. This new parameter will help to integrate the relevant information of bone microarchitecture quality and assess more directly the real trabecular fragility in osteoporotic patients. Approximate entropy (ApEn) and sample entropy (SampEn) are mathematical algorithms created to measure the repeatability or predictability within a time series. Both algorithms are extremely sensitive to their input parameters: m (length of the data segment being compared), r (similarity criterion), and N (length of data). There is no established consensus on parameter selection in short data sets, especially for biological data. Therefore, the purpose of this research was to examine the robustness of these two entropy algorithms by exploring the effect of changing parameter values on short data sets. Data with known theoretical entropy qualities as well as experimental data from both healthy young and older adults was utilized. Our results demonstrate that both ApEn and SampEn are extremely sensitive to parameter choices, especially for very short data sets, N ≤ 200. We suggest using N larger than 200, an m of 2 and examine several r values before selecting your parameters. Extreme caution should be used when choosing parameters for experimental studies with both algorithms. Based on our current findings, it appears that SampEn is more reliable for short data sets. SampEn was less sensitive to changes in data length and demonstrated fewer problems with relative consistency. [ABSTRACT FROM AUTHOR]

Recent advances in design of powered artificial legs have led to increased potential to allow lower limb amputees to actively recover from stumbles. To achieve this goal, promptly and accurately identifying stumbles is essential. This study aimed to 1) select potential stumble detection data sources that react reliably and quickly to stumbles and can be measured from a prosthesis, and 2) investigate two different approaches based on selected data sources to detect stumbles and classify stumble types in patients with transfemoral (TF) amputations during ambulation. In the experiments, the normal gait of TF amputees was perturbed by a controllable treadmill or when they walked on an obstacle course. The results showed that the acceleration of prosthetic foot can accurately detect the tested stumbling events 140-240 ms before the critical timing of falling and precisely classify the stumble type. However, the detector based on foot acceleration produced high false alarm rates, which challenged its real application. Combining electromyographic (EMG) signals recorded from the residual limb with the foot acceleration significantly reduced the false alarm rate but sacrificed the detection response time. The results of this study may lead to design of a stumble detection system for instrumented, powered artificial legs; however, continued engineering efforts are required to improve the detection performance and resolve the challenges that remain for implementing the stumble detector on prosthetic legs. Falling is a frequent cause of serious injury in older adults and trips are a dominant cause of falls in this rapidly growing population. Although there are few laboratory protocols that induce actual trips, there are many protocols that utilize surrogate tasks. These surrogate tasks, which are time-critical but do not involve an obstacle, appear to share a number of biomechanical characteristics with stepping responses following a trip. However, although rapid and safe negotiation of the obstacle and restoration of dynamic equilibrium are common requisites for success, we expected that stepping response kinematics during a successful recovery from a trip over a previously unseen obstacle would be substantially different than those of surrogate tasks without an obstacle. Unexpected trips were induced in 13 older men and women by an obstacle, the presence of which the subjects were previously unaware. Selected kinematics of the leading and trailing limb stepping responses during recovery from the induced trip were compared to those of two surrogate tasks that did not involve an obstacle. Multivariate analysis of variance (MANOVA) revealed that step height, step length, peak horizontal velocity, and peak vertical velocity of the leading and trailing limbs were significantly different during recovery from the induced trip compared to the surrogate tasks. These between-task performance differences may limit the extent to which performance of the surrogate tasks accurately and precisely reflect the potential to recover dynamic equilibrium following a trip. Spinal cord injury (SCI) results in rapid, severe osteoporosis and an increased risk of lower extremity fractures. Despite the medical complications associated with these fractures, there is no standard of care to prevent osteoporotic fractures following SCI. Functional electrical stimulation‐ (FES‐) assisted rowing is a promising intervention to improve bone health in SCI because of its ability to generate a muscular contraction in conjunction with mechanical loading of the lower extremity long bones. Combination therapy consisting of FES‐rowing plus zoledronic acid (ZA) may be a superior treatment via inhibition of bone resorption and stimulation of new bone formation. We studied participants enrolled in a randomized clinical trial comparing FES‐rowing alone with FES‐rowing plus ZA to improve bone health in SCI. Volumetric CT scans at the distal femur and proximal tibial metaphyses were performed. Bone geometric properties (cortical thickness index [CTI], cortical compressive strength index [CSI], buckling ratio [BR], bending strength index) and mineral (cortical bone volume [CBV], cortical bone mineral density, cortical bone mineral content) indices were determined. In models adjusting for baseline values, we found that the CBV (p = 0.05 to 0.006), the CTI (p = 0.009), and the BR (p = 0.001) at both the distal femoral and proximal tibial metaphyses were greater in the ZA plus rowing group compared with the rowing‐only group. Similarly, there was a significant positive association between the total rowing work completed and the BR at the proximal tibia (p = 0.05). A subgroup analysis of the rowing‐only arm showed that gains in the CSI at the tibial metaphysis varied in a dose‐dependent fashion based on the total amount of exercise performed (p = 0.009). These findings demonstrate that the osteogenic response to FES‐rowing is dose‐dependent. Combination therapy with ZA and FES‐row training has therapeutic potential to improve bone quality, and perhaps reduce fracture risk at the most common fracture site following SCI. © 2019 The Authors.
