On the 22nd of December 2018 tsunami waves generated by the flank collapse of the Anak Krakatau volcano in Indonesia flooded the shorelines of the nearby Sumatra and Java islands. The authors conducted a field survey of the Krakatau group of islands to clarify the volume of the collapsed flank and the run-up of the tsunami around the island themselves. To aid with the simulation of this complex tsunami and document the current situation of the islands, bathymetric surveys, aerial drone and panoramic video mapping were also carried out. The results show that while much of the source directionality was towards the SE of Anak Krakatau, significant wave run-ups were also measured towards the NW, indicating that further numerical modelling is needed to fully explain this event., Journal of Coastal and Hydraulic Structures, Vol 1 (2021): Single articles

神経因性膀胱をはじめとして頻尿, 尿意切迫感および尿失禁を主訴とする患者を対象として塩酸オキシブチニンを投与し, 臨床症状を中心にその有効性, 安全性につき検討した.1)総症例数52例中44例が4週以上の投与を完了した.平均投与期間は66.8日であった.2)投与量別全般改善度を評価した成績は2 mg×3回群で"改善"以上の改善率は55%, "やや改善"以上が85%であり, 3 mg×3回群でそれぞれ68.2%, 95.5%であった.また1日2回投与でも臨床症状のコントロールが可能な症例を認めた.3)副作用は評価対象52例中2例に認められ, 2例とも口渇のみであり, 臨床上問題となる重篤なものはなかった.4)有用度は評価時において, "有用"以上67.3%, "やや有用"以上88.5%と高い有用率が得られた, The effects and the safety of oxybutynin hydrochloride were investigated in 52 patients, 17 male and 35 female, with the chief complaints of pollakisuria, urgency and urinary incontinence. Clinical responses to the drug were assessed mainly by the subjective symptoms of the patients. The diagnoses of these patients were neurogenic bladder in 17, unstable bladder in 16 and others in 19 patients. The average administration period was 66.8 days. The rate of global improvement (excellent and good) was 55% in the 2 mg dose given 3 times daily group, 68.2% in the 3 mg dose given 3 times daily group. Side effects, such as dry mouth, were observed in 2 of the 52 patients (3.8%), but no serious side effects were observed. The rate of global utility (remarkable and moderate) was 67.3%. These data indicate that oxybutynin hydrochloride seems to be useful and safe for the treatment of pollakisuria, urgency and urinary incontinence.

The clinical efficacy of norfloxacin (NFLX) was evaluated on 40 patients. They had subjective symptoms suggestive of prostate inflammations and more than five white blood cells (WBC)/hpf in their prostatic secretions (EPS) or VB3. Of these, gram negative rods were isolated from the EPS in 3 patients and gram positive cocci were obtained in 26 patients. The overall clinical efficacy was determined at the second week. The effectiveness rate of the subjective symptoms was 82.5%. The effectiveness rate of the WBC in the EPS was 47.4%. The effectiveness rate of the bacteria in the EPS was 64.3%. The overall clinical effectiveness rate was 77.8%. A subjective side effect was observed only in one patient who had skin eruption like urticaria. Mild liver dysfunction of blood chemistry analysis was shown only in two patients but they had had long standing chronic hepatitis. We conclude that NFLX is an effective drug for the patients with chronic prostatitis.

Two patients with renal or perinephric abscess were successfully managed by percutaneous drainage under ultrasonic guidance. We recommend percutaneous drainage as the first choice of treatment for a renal or perinephric abscess instead of traditional open surgery, especially in a high risk patient.

Seventy seven patients with prostatic carcinoma were treated in our clinic between 1977 and 1986. Most of them were treated by a hormonal agent as the first therapy. None of the 9 stage A1 cases showed any reactivation, but 4 of the 5 stage A2 cases relapsed to metastatic disease. The chemotherapy performed in 3 of the 4 reactivated cases had no obvious effect on the disease. Seven of the 8 patients with stage B disease were alive without relapse. Relapse was seen in the other patient who had poorly differentiated carcinoma and chemotherapy in this case resulted in stable disease for the present. Four of the 15 stage C cases including 3 poorly differentiated carcinomas were hormone resistant or reactivated. For these resistant cases radiotherapy and/or the chemotherapy were performed, but a response was seen in only one case. Consequently, the first therapy for stage A2, B and C of poorly differentiated carcinoma must be improved. Of the 40 stage D cases, 4 patients who were treated by an early combination of hormonal therapy and chemotherapy had a better prognosis than the others. These 4 patients had poorly differentiated carcinomas with multiple bone metastases. Two of these 4 patients were alive without relapse for 17 and 72 months, and one of the 2 patients with relapse was also alive for 75 months. We believe that early chemotherapy is the key for better prognosis in stage D cases.

新しい経口合成抗筋剤Ofloxacin (OFLX)を53名の尿路感染症患者に使用し, 臨床的検討を加えた.OFLX (600 mg/day)を35名の急性単純性膀胱炎患者には3日間以上, 18日の複雑性尿路感染症患者には副作用で投与を中止した1例を除いて5日間以上連日投与した.急性単純性膀胱炎:UTI薬効評価基準に合致した10例で総合有効率100%を得た.同定しえた26株の起炎菌はすべて消失し, 自覚症状も著明に改善した.全例に対する主治医判定では, 94.3%がきわめて有用または有用とされた.複雑性尿路感染症:5～7日間投与の11例中63.6%の膿尿の改善を認めた.UTI薬効評価基準に合致した4例では総合有効率100%を得た.同定しえた9株の起炎菌のうち7株が消失した.全例に対する主治医判定では, 38.9%がきわめて有用または有用と判定された.53例中4例に軽度の副作用を認めた, Fifty-three patients with urinary tract infections (UTI) were treated with Ofloxacin, a new oral synthetic antimicrobial agent, and its clinical efficacy was studied. Ofloxacin (600 mg/day) was administered to 35 patients with acute simple cystitis for more than three days, and to 17 patients with complicated UTI for more than five days except a case in which the treatment was interrupted for side effects. Acute simple cystitis: In ten cases meeting the criteria of UTI committee, overall effectiveness rate was 100%. All of the 26 strains isolated from 26 patients disappeared after the treatment. In all of acute simple cystitis cases, 94.3% were evaluated as excellent or as moderate by attending doctors. Complicated UTI: In 11 cases treated during five or seven days, 63.6% of patients showed improvement on pyuria. Seven out of nine strains isolated from seven patients disappeared. However, two strains of P. aeruginosa and P. cepacia persisted after the treatment. In all of the complicated UTI cases, 38.9% of patients were evaluated as excellent or as moderate by attending doctors. Some slight side effects were observed in four out of 53 cases. This study showed that Ofloxacin is effective against urinary tract infections.

Cefixime (CFIX) was administered orally in the treatment of 59 cases of urinary tract infection (UTI). According to the response criteria defined by the Japanese UTI committee, the rate of clinical efficacy for 26 cases of uncomplicated cystitis was 96.2%. The rate of clinical effectiveness of five patients with complicated UTI and five with uncomplicated pyelonephritis was 80.0%. In the other 23 cases which did not meet the response criteria, the efficacy rate was also high. The results indicated that CFIX was an effective drug for the treatment of UTI.

A 40-year-old male with advanced penile cancer, whose left inguinal node was ulcerated at the time of initial presentation, underwent multimodal therapy. Four cycles of chemotherapy were given from September 20, 1984 to February 8, 1985. Partial penectomy and left ilioinguinal lymphadenectomy with removal of left groin ejaculation were performed on February 20, 1985, followed by right ilioinguinal lymphadenectomy on March 20, 1985. The skin defect of the left groin was covered with tensor fascia lata myocutaneous flap. The patient is alive with no evidence of disease 30 months after the surgery.
