Abstract

Hand-held three dimensional models of the human anatomy and pathology, tailored-made protheses, and custom-designed implants can be derived from imaging modalities, most commonly Computed Tomography (CT). However, standard DICOM format images cannot be 3D printed; instead, additional image post-processing is required to transform the anatomy of interest into Standard Tessellation Language (STL) format is needed. This conversion, and the subsequent 3D printing of the STL file, requires a series of steps. Initial post-processing involves the segmentation-demarcation of the desired for 3D printing parts and creating of an initial STL file. Then, Computer Aided Design (CAD) software is used, particularly for wrapping, smoothing and trimming. Devices and implants that can also be 3D printed, can be designed using this software environment. The purpose of this article is to provide a tutorial on 3D Printing with the test case of complex congenital heart disease (CHD). While the infant was born with double outlet right ventricle (DORV), this hands-on guide to be featured at the 2015 annual meeting of the Radiological Society of North America Hands-on Course in 3D Printing focused on the additional finding of a ventricular septal defect (VSD). The process of segmenting the heart chambers and the great vessels will be followed by optimization of the model using CAD software. A virtual patch that accurately matches the patient's VSD will be designed and both models will be prepared for 3D printing.

Abstract

Introduction Most vertical root fractures occur in root canal treated teeth, and they usually run in a buccolingual direction. The butterfly effect is an optical phenomenon seen in some sections of tooth roots. The aim was to investigate the microhardness of dentin in mesiodistal and buccolingual cross sections of roots exhibiting the effect. Methods Thirty extracted single-rooted teeth were allocated according to patient age: group 1, 15–24 years; group 2, 25–44 years; and group 3, 45 years and older. Roots were embedded in acrylic and cut into ten 1-mm-thick cross sections. Sections were viewed under a light microscope and coded (1 or 2) according to presence or absence of the butterfly effect. A root scored 20 when all levels featured the butterfly appearance. The 2 teeth with the highest score from each group and 2 control teeth with the minimum score (10) were selected. Two adjacent, consecutive cross sections were chosen from the middle of the roots. Vickers microhardness testing was carried out on the dentin walls. Results Mean hardness scores were highest mesiodistally (83.7 kgf/mm2) and lowest buccolingually (56.4 kgf/mm2), a significant difference (P = .028). This trend was found across all age groups. Conclusions Root sections with the butterfly effect are harder mesiodistally. This might explain the high prevalence of vertical root fractures that run buccolingually.

Abstract

Trismus refers to any condition inducing limited mouth opening and may present as a result of acquired or congenital pathology. We present the case of a newborn who presented with severe, congenital trismus due to brainstem dysgenesis. We describe the course of his investigations, and a multidisciplinary approach to the management of his care and follow-up. To our knowledge, this is one of the earliest reported cases of congenital trismus attributable to brainstem dysgenesis. A literature review was conducted to provide an overview of the differential pathogenesis as it presents in congenital cases and discuss the complexity of managing congenital trismus due to brainstem dysgenesis in a neonate and infant. Eccrine poroma is a benign adnexal tumor that originates from the uppermost portion of the intraepidermal eccrine duct. It usually presents as a solitary tumor. Histopathology shows a monomorphic proliferation of cuboidal cells, which radially extend from the basal layer to the dermis. Here, we present a rare multilesional eruption of eccrine poroma after chemotherapy and bone marrow stem cell transplantation for acute promyelocytic leukemia along with a description of clinical, pathological, and dermoscopic findings. Abstract

Using a genetic screen in yeast we found that Mycobacterium tuberculosis PE-PGRS62 was capable of disrupting yeast vacuolar protein sorting, suggesting effects on endosomal trafficking. To study the impact of PE-PGRS62 on macrophage function, we infected murine macrophages with Mycobacterium smegmatis expressing PE-PGRS62. Infected cells displayed phagosome maturation arrest. Phagosomes acquired Rab5, but displayed a significant defect in Rab7 and LAMP-1 acquisition. Macrophages infected with M. smegmatis expressing PE-PGRS62 also expressed two- to threefold less iNOS protein when compared with cells infected with wild-type bacteria. Consistent with this, cells infected with a Mycobacterium marinum transposon mutant for the PE-PGRS62 orthologue showed greater iNOS protein expression when compared to cells infected with wild-type organisms. Complementation restored the ability of the mutant to inhibit iNOS expression. No differences in iNOS transcript levels were observed, suggesting that PE-PGRS62 effects on iNOS expression occurred post-transcriptionally. Marked differences in colony morphology were also seen in M. smegmatis expressing PE-PGRS62 and in the M. marinum transposon mutant,suggesting that PE-PGRS62 may affect cell wall composition. These findings suggest that PE-PGRS62 supports virulence via inhibition of phagosome maturation and iNOS expression, and these phenotypes may be linked to effects on bacterial cell wall composition. Abstract

We use proprietary data on intraday transactions at a futures brokerage to analyze how implied leverage influences trading performance. Across all investors, leverage is negatively related to performance, due partly to increased trading costs and partly to forced liquidations resulting from margin calls. Defining skill out‐of‐sample, we find that relative performance differentials across unskilled and skilled investors persist. Unskilled investors' leverage amplifies losses from lottery preferences and the disposition effect. Leverage stimulates liquidity provision by skilled investors, and enhances returns. Although regulatory increases in required margins decrease skilled investors' returns, they enhance overall returns, and attenuate return volatility. [ABSTRACT FROM AUTHOR]

Abstract

The commercial services market offers significant revenue potential and opportunities to expand business beyond the consumer space. Cable operators are well positioned to address a broad customer base by leveraging existing infrastructure presence, but must consider network enhancements to provide commercial services effectively. MPLS provides the means to architect a converged network offering multiple services, while reducing capital and operational expenditures. This architecture enables operators to create a service delivery network that can differentiate traffic flows and guarantee strict service-level commitments. Implementation of MPLS may be immediate with native deployment over dedicated physical infrastructure or phased with integration into existing IP infrastructure. This implementation enables cable operators to migrate associated network operations gracefully as deployment of next-generation architecture often requires operational ramp-up. Commercial customers demand the highest levels of reliability, availability and responsiveness. These attributes are the top criteria enterprise customers consider when selecting a service provider. True carrier-class network elements and automated service provisioning are necessary to build a network that meets these expectations. Service flexibility is another factor that adds value for potential customers. While the focus of commercial services is implementation of Ether-net-based access, the ability to service customer locations that connect via legacy frame relay, ATM, or TDM wide area network (WAN) circuits further expands the market opportunity for cable operators. While infrastructure technology is just one component in the cable operator's overall strategy to vie for the promising commercial services market, it provides the foundation for success in capturing market share. Marketing, sales, service and operations are other important factors that must be addressed, but with a solid network infrastructure and focus on value, cable operators are in a prime position to grow their revenue significantly with commercial services.
