Introduction: Bowel regimens (BR) before radical cystectomy (RC) are currently not recommended by Enhanced Recovery After Surgery (ERAS) protocols, as prior studies have shown BRs lead to worsened outcomes. However, many of those studies have used historic literature before recent surgical advancements such as minimally invasive RC and have not investigated the impact BRs have by type of urinary diversion. Our goal is to determine the outcomes of preoperative BR in patients undergoing RC based on diversion type using a modern patient cohort., Methods: RCs performed between 2019 and 2020 with BR information available were identified in the American College of Surgeons' National Surgical Quality Improvement Program (NSQIP). Patients were grouped by type of BR received: no BR, both mechanical bowel preparation (MBP) and preoperative oral antibiotic BR (OABR), MBP only, and OABR only. We conducted propensity score matching based on factors influencing the operative approach. Baseline demographics and 30-day complication rates were compared between matched groups. We analyzed hospital length of stay (LOS) via multivariate regression with a Poisson distribution., Results: In total, 2054 RCs were identified with 2.4% receiving OABR, 21.3% receiving MBP, 5.3% receiving both, and 71.0% receiving no BR. For patients with ileal conduit diversions, outcomes with BRs appeared mixed, as OABR leads to increased LOS. For patients with neobladder diversions, BRs were not associated with any worsened outcomes and were associated with reduced length of stay., Conclusions: BRs such as OABR may associated with improved outcomes in patients receiving RC with neobladder diversion, a finding that warrants further investigation., (© 2025 The Japanese Urological Association.)

Introduction: Placing ureteral stents at the ureteroileal anastomosis for radical cystectomy with ileal conduit (RCIC) has long been common practice. Recently, some providers have begun omitting stents. We sought to investigate differences in perioperative and 30-day outcomes between patients who underwent RCIC with and without stents placed at the ureteroileal anastomosis., Methods: We identified RCICs performed between 2019 and 2021 in the National Surgical Quality Improvement Program database and corresponding Cystectomy-Targeted Participant Use File. Baseline demographics, comorbidities, and operative parameters were compared via Pearson's χ 2 and t tests between stented and stentless RCICs. Outcomes of interest, including rates of UTIs, acute kidney injury, renal failure requiring dialysis, ileoileal anastomotic leaks, ureteral obstruction, urinary leak or fistula formation, reoperations, and 30-day hospital readmissions, were compared using Pearson's χ 2 . All statistical tests were 2-tailed with P < .05 considered significant., Results: A total of 5418 RCICs were identified. Four hundred ninety-eight (9.2%) were stentless. There were no differences in baseline demographics or comorbidities. Significantly fewer stented patients had robotic-assisted operations (23% vs 29%, P < .01). Stented patients had lower rates of urinary leak or fistula formation (3.1% vs 4.8%, P = .04). There was no significant difference in 30-day rates of UTIs, acute kidney injuries, renal failure, ileoileal anastomotic leaks, ureteral obstruction, reoperations, and readmissions. Limitations include retrospective design and lack of longitudinal tracking past 30 days., Conclusions: Stentless patients had noninferior outcomes compared to stented patients in most important 30-day outcomes. Our analysis suggests that stents may not be necessary in ileal conduit urinary diversion procedures.

Von Hippel-Lindau (vHL) is a hereditary disease characterized by the development of benign and malignant tumors across multiple organ systems. It is seen in approximately 1 in 36,000 live births. Given that vHL is a rare disease, studies that seek to characterize vHL are often hampered by small sample sizes. The TriNetX database, which contains data from over 100 million patients, may offer the ability to define and describe a large number of vHL patients. The primary objectives of this study were to describe the prevalence of vHL-associated conditions and investigate clinical outcomes using TriNetX. The secondary objective was to compare the results of this analysis to what has been reported in the published vHL literature. TriNetX was queried to establish a cohort of patients with a diagnosis of vHL. This cohort was then used to define the prevalence of the following conditions: reproductive organ (epididymal and broad ligament) cystadenomas, renal cell carcinoma (RCC), pheochromocytomas, endolymphatic sac tumors (ESLTs), central nervous system (CNS) and retinal hemangioblastomas, and pancreatic neuroendocrine tumor (pNETs). A total of 1232 patients in TriNetX had a recorded diagnosis of vHL. Of this, 34 (6.0% of males) patients had epididymal cystadenoma, 21 (3.4% of females) had broad ligament cystadenoma, 352 (28.6%) had RCC, 251 (20.4%) had pheochromocytoma, <10 had ELST, 171 (13.9%) had CNS hemangioblastoma, 34 (2.8%) had pNETs, and 66 (5.4%) had retinal hemangioma. Objective: To analyze antibiotic prescribing trends, particularly the decline in fluoroquinolone use for various genitourinary infection (GUI) and procedures and the growing preference for alternative treatments., Methods: The TriNetX database was queried for antibiotic utilization rates from 2016 to 2022. We analyzed cases of cystitis, pyelonephritis, prostatitis, and other GUIs, as well as periprocedure utilization for benign prostatic hyperplasia, urethral stricture disease, pelvic organ prolapse/stress urinary incontinence, and overactive bladder/interstitial cystitis (OAB/IC). We determined the top four to six antibiotics prescribed within 5 days of diagnosis or procedure. Statistical analysis included linear regression modeling., Results: From 2016 to 2022, fluoroquinolone use decreased in cystitis (P <.001), pyelonephritis (P <.001), and prostatitis (P <.01). These infections were increasingly treated with beta-lactams (P <.0001, P <.01, P <.01, respectively). Additionally, we found a decrease in fluoroquinolone utilization in procedures for benign prostatic hyperplasia (P <.001), urethral stricture (P <.01), pelvic organ prolapse/stress urinary incontinence (P <.001), and OAB/IC (P <.001). All cohorts except for OAB/IC showed a significant increase in beta-lactam use (P <.001, P <.01, P <.01, respectively)., Conclusion: We noted a significant decline in fluoroquinolone use for GUI and procedures, accompanied by increased beta-lactam use, reflecting changes in prescribing practices. Introduction: ChatGPT is an artificial intelligence platform available to patients seeking medical advice. Traditionally, urology patients consulted official provider-created materials, particularly the Urology Care Foundation™ (UCF). Today, men increasingly go online due to the rising costs of health care and the stigma surrounding sexual health. Online health information is largely inaccessible to laypersons as it exceeds the recommended American sixth to eighth grade reading level. We conducted a comparative assessment of patient education materials generated by ChatGPT vs UCF regarding men's health conditions., Methods: All 6 UCF men's health resources were identified. ChatGPT responses were generated using patient questions obtained from UCF. Adjusted ChatGPT responses were generated by prompting, "Explain it to me like I am in sixth grade." Textual analysis was performed using sentence, word, syllable, and complex word count. Six validated formulae were used for readability analysis. Two physicians independently scored responses for accuracy, comprehensiveness, and understandability. Statistical analysis involved Wilcoxon matched-pairs test., Results: ChatGPT responses were longer and more complex. Both UCF and ChatGPT failed official readability standards, although ChatGPT performed significantly worse across all 6 topics (all P < .001). Conversely, adjusted ChatGPT readability typically surpassed UCF, even meeting the recommended level for 2 topics. Qualitatively, UCF and ChatGPT had comparable accuracy, although ChatGPT had better comprehensiveness and worse understandability., Conclusions: When comparing readability, ChatGPT-generated education is less accessible than provider-written content, although neither meets the recommended level. Our analysis indicates that specific artificial intelligence prompts can simplify educational materials to meet national standards and accommodate individual literacy.

Recent advances are allowing computed tomography (CT) and magnetic resonance imaging (MRI) to add diagnostic information derived from microscopic-scale brain structure, pathology, and physiology to the gross pathologic information that has been the core of brain imaging diagnosis since the 1980s. Physiologic imaging with MR perfusion weighted imaging has joined MR diffusion imaging as an essential components of stroke and brain tumor MRI. At the same time, the volume scanning revolution in CT technology has dramatically decreased the radiation doses required for CT perfusion imaging by allowing routine simultaneous CT perfusion and noninvasive dynamic bone subtracted CT angiography (CTA) without a significant increase in radiation dose over conventional head CT-CTA alone. Although ongoing research and clinical trials is needed to define more precisely how these techniques can best be exploited to improve clinical care and patient outcomes, in the acute stroke and subarachnoid hemorrhage populations the radiation risk associated with CT perfusion imaging is negligible and the physiologic information promises significant patient safety benefits., (Thieme Medical Publishers 333 Seventh Avenue, New York, NY 10001, USA.)

Objective: The purpose of this article is to evaluate the accuracy of preliminary on-call radiology resident interpretation of CT angiography (CTA) compared with digital subtraction angiography (DSA) in detecting cerebral aneurysms in subarachnoid hemorrhage (SAH)., Materials and Methods: A retrospective review compared resident interpretations of head CTA performed after hours for SAH to the results of DSA. The sensitivity and specificity of resident interpretations were classified on a per-patient and per-aneurysm basis. The accuracy of resident interpretations was also determined according to aneurysm location and number., Results: Between January 2007 and December 2009, 83 patients with SAH underwent both CTA and DSA. DSA documented an aneurysm in 53 of 83 patients. Per patient, residents identified at least one aneurysm in 46 of 53 patients (87%). Per aneurysm, resident sensitivity and specificity for detecting aneurysms of any size were 62% and 91%, respectively, which improved for aneurysms 3 mm or larger to 73% and 97%, respectively. The posterior communicating and intracranial internal carotid arteries were resident "blind spots," with aneurysms 3 mm or larger detected with sensitivities of 33% and 50%, respectively. In contrast, anterior communicating artery aneurysms were correctly identified 95% of the time. In only 35% of cases with multiple aneurysms did residents correctly identify more than one aneurysm., Conclusion: The sensitivity of on-call resident interpretation of CTA for aneurysms in SAH is lower than expected, with a potential for delay in diagnosis and management in a small number of patients. Focused training to carefully review apparent blind spots and the frequency of multiple aneurysms may reduce inaccuracies.

Objective: The purpose of this study was to show the value of 2D and 3D reformations of CT data from abdominal and pelvic CT performed immediately at the workstation using a PACS-based software program to evaluate the lumbar spine and pelvis in patients with blunt abdominal trauma., Materials and Methods: We reviewed the abdominopelvic CT scans and conventional radiographs of the lumbar spine or pelvis of 156 consecutive patients with blunt abdominal trauma. The CT data were compared with the radiographic findings and also with the findings of dedicated repeat CT of the spine or pelvis, when performed., Results: CT depicted 80 fractures of the lumbar spine and 178 pelvic fractures. Radiography showed 40 fractures of the lumbar spine and 138 pelvic fractures. No additional fractures were identified on dedicated repeat CT., Conclusion: Conventional radiographs to clear the lumbar spine are no longer required when abdominopelvic CT data are available. CT and reformatted CT data show more fractures than radiography and miss no fractures compared with dedicated CT of the lumbar spine or pelvis. Having these images immediately available through the PACS workstation saves time for the trauma team in the management of critically ill patients.

Background Diffuse correlation spectroscopy (DCS) is a non-invasive optical technique that enables continuous blood flow measurements in various organs, including the brain. DCS quantitatively measures blood flow from temporal fluctuations in the intensity of diffusely reflected light caused by the dynamic scattering of light from moving red blood cells within the tissue. Methods We performed bilateral cerebral blood flow (CBF) measurements using a custom DCS device in patients undergoing neuroendovascular interventions for acute ischemic stroke. Experimental, clinical, and imaging data were collected in a prospective manner. Results The device was successfully applied in nine subjects. There were no safety concerns or interference with the standard angiography suite or intensive care unit workflow. Six cases were selected for final analysis and interpretation. DCS measurements with photon count rates greater than 30 KHz had sufficient signalto- noise to resolve blood flow pulsatility. We found an association between angiographic changes in cerebral reperfusion (partial or complete reperfusion established in stroke thrombectomy cases; temporary flow arrest during carotid artery stenting) and those observed intraprocedurally with CBF measurements via DCS. Limitations of the current technology included sensitivity to the interrogated tissue volume under the probe and the effect of local changes in tissue optical properties on the accuracy of CBF estimates. Conclusion Our initial experience with DCS in neurointerventional procedures showed the feasibility of this non-invasive approach in providing continuous measurement of regional CBF brain tissue properties. In the outpatient clinic setting, AI offers promising improvements in efficiency through Chatbots, streamlined medical documentation, and personalized patient education materials. On the billing side, AI technologies hold potential for optimizing the selection of appropriate billing codes, automating prior authorizations, and enhancing healthcare fraud detection. Objective: To use an artificial intelligence (AI)-powered large language model (LLM) to improve readability of patient handouts., Study Design: Review of online material modified by AI., Setting: Academic center., Methods: Five handout materials obtained from the American Rhinologic Society (ARS) and the American Academy of Facial Plastic and Reconstructive Surgery websites were assessed using validated readability metrics. The handouts were inputted into OpenAI's ChatGPT-4 after prompting: "Rewrite the following at a 6th-grade reading level." The understandability and actionability of both native and LLM-revised versions were evaluated using the Patient Education Materials Assessment Tool (PEMAT). Results were compared using Wilcoxon rank-sum tests., Results: The mean readability scores of the standard (ARS, American Academy of Facial Plastic and Reconstructive Surgery) materials corresponded to "difficult," with reading categories ranging between high school and university grade levels. Conversely, the LLM-revised handouts had an average seventh-grade reading level. LLM-revised handouts had better readability in nearly all metrics tested: Flesch-Kincaid Reading Ease (70.8 vs 43.9; P < .05), Gunning Fog Score (10.2 vs 14.42; P < .05), Simple Measure of Gobbledygook (9.9 vs 13.1; P < .05), Coleman-Liau (8.8 vs 12.6; P < .05), and Automated Readability Index (8.2 vs 10.7; P = .06). PEMAT scores were significantly higher in the LLM-revised handouts for understandability (91 vs 74%; P < .05) with similar actionability (42 vs 34%; P = .15) when compared to the standard materials., Conclusion: Patient-facing handouts can be augmented by ChatGPT with simple prompting to tailor information with improved readability. Purpose: Large language models (LLMs) are a form of artificial intelligence (AI) that uses deep learning techniques to understand, summarize and generate content. The potential benefits of LLMs in healthcare is predicted to be immense. The objective of this study was to examine the quality of patient information leaflets (PILs) produced by 3 LLMs on urological topics., Methods: Prompts were created to generate PILs from 3 LLMs: ChatGPT-4, PaLM 2 (Google Bard) and Llama 2 (Meta) across four urology topics (circumcision, nephrectomy, overactive bladder syndrome, and transurethral resection of the prostate). PILs were evaluated using a quality assessment checklist. PIL readability was assessed by the Average Reading Level Consensus Calculator., Results: PILs generated by PaLM 2 had the highest overall average quality score (3.58), followed by Llama 2 (3.34) and ChatGPT-4 (3.08). PaLM 2 generated PILs were of the highest quality in all topics except TURP and was the only LLM to include images. Medical inaccuracies were present in all generated content including instances of significant error. Readability analysis identified PaLM 2 generated PILs as the simplest (age 14-15 average reading level). Llama 2 PILs were the most difficult (age 16-17 average)., Conclusion: While LLMs can generate PILs that may help reduce healthcare professional workload, generated content requires clinician input for accuracy and inclusion of health literacy aids, such as images. LLM-generated PILs were above the average reading level for adults, necessitating improvement in LLM algorithms and/or prompt design. Objectives: Advancements in Artificial Intelligence(AI) have made platforms like ChatGPT increasingly relevant in medicine. This study assesses ChatGPT's utility in addressing bacterial infection-related questions and antibiogram-based clinical cases., Methods: This study involved a collaborative effort involving infectious disease (ID) specialists and residents. A group of experts formulated six true/false, six open-ended questions, and six clinical cases with antibiograms for four types of infections (endocarditis, pneumonia, intra-abdominal infections, and bloodstream infection) for a total of 96 questions. The questions were submitted to four senior residents and four specialists in ID and inputted into ChatGPT-4 and a trained version of ChatGPT-4. A total of 720 responses were obtained and reviewed by a blinded panel of experts in antibiotic treatments. They evaluated the responses for accuracy and completeness, the ability to identify correct resistance mechanisms from antibiograms, and the appropriateness of antibiotics prescriptions., Results: No significant difference was noted among the four groups for true/false questions, with approximately 70% correct answers. The trained ChatGPT-4 and ChatGPT-4 offered more accurate and complete answers to the open-ended questions than both the residents and specialists. Regarding the clinical case, we observed a lower accuracy from ChatGPT-4 to recognize the correct resistance mechanism. ChatGPT-4 tended not to prescribe newer antibiotics like cefiderocol or imipenem/cilastatin/relebactam, favoring less recommended options like colistin. Both trained- ChatGPT-4 and ChatGPT-4 recommended longer than necessary treatment periods (p-value = 0.022)., Conclusions: This study highlights ChatGPT's capabilities and limitations in medical decision-making, specifically regarding bacterial infections and antibiogram analysis. While ChatGPT demonstrated proficiency in answering theoretical questions, it did not consistently align with expert decisions in clinical case management. Despite these limitations, the potential of ChatGPT as a supportive tool in ID education and preliminary analysis is evident. Background: Generative Pretrained Model (GPT) chatbots have gained popularity since the public release of ChatGPT. Studies have evaluated the ability of different GPT models to provide information about medical conditions. To date, no study has assessed the quality of ChatGPT outputs to prostate cancer related questions from both the physician and public perspective while optimizing outputs for patient consumption., Methods: Nine prostate cancer-related questions, identified through Google Trends (Global), were categorized into diagnosis, treatment, and postoperative follow-up. These questions were processed using ChatGPT 3.5, and the responses were recorded. Subsequently, these responses were re-inputted into ChatGPT to create simplified summaries understandable at a sixth-grade level. Readability of both the original ChatGPT responses and the layperson summaries was evaluated using validated readability tools. A survey was conducted among urology providers (urologists and urologists in training) to rate the original ChatGPT responses for accuracy, completeness, and clarity using a 5-point Likert scale. Furthermore, two independent reviewers evaluated the layperson summaries on correctness trifecta: accuracy, completeness, and decision-making sufficiency. Public assessment of the simplified summaries' clarity and understandability was carried out through Amazon Mechanical Turk (MTurk). Participants rated the clarity and demonstrated their understanding through a multiple-choice question., Results: GPT-generated output was deemed correct by 71.7% to 94.3% of raters (36 urologists, 17 urology residents) across 9 scenarios. GPT-generated simplified layperson summaries of this output was rated as accurate in 8 of 9 (88.9%) scenarios and sufficient for a patient to make a decision in 8 of 9 (88.9%) scenarios. Stroke mimics constitute a significant proportion of patients with suspected acute ischemic stroke. These conditions may resemble acute ischemic stroke and demonstrate abnormalities on perfusion imaging sequences. The most common stroke mimics include seizure/epilepsy, migraine with aura, brain tumors, functional disorders, infectious encephalopathies, Wernicke's encephalopathy, and metabolic abnormalities. Brain perfusion imaging techniques, particularly computed tomography perfusion and magnetic resonance perfusion, are being widely used in routine clinical practice for treatment selection in patients presenting with large vessel occlusion. At the same time, the utilization of these imaging modalities enables the opportunity to better diagnose patients with stroke mimics in a time‐sensitive setting, leading to appropriate management, decision‐making, and resource allocation. In this review, we describe patterns of perfusion abnormalities that could discriminate patients with stroke mimics from those with acute ischemic stroke and provide specific case examples to illustrate these perfusion abnormalities. In addition, we discuss the challenges associated with interpretation of perfusion images in stroke‐related pathologies. In general, perfusion imaging can provide additional information in some cases—when used in combination with conventional magnetic resonance imaging and computed tomography—and might help in detecting stroke mimics among patients who present with acute onset focal neurological symptoms. Hyperparasitoids are some of the most diverse members of insect food webs. True hyperparasitoids parasitize the larvae of other parasitoids, reaching these larvae with their ovipositor through the herbivore that hosts the parasitoid larva. During pupation, primary parasitoids also may be attacked by pseudohyperparasitoids that lay their eggs on the parasitoid (pre)pupae. By attacking primary parasitoids, hyperparasitoids may affect herbivore population dynamics, and they have been identified as a major challenge in biological control. Over the past decades, research, especially on aphid- and caterpillar-associated hyperparasitoids, has revealed that hyperparasitoids challenge rules on nutrient use efficiency in trophic chains, account for herbivore outbreaks, or stabilize competitive interactions in lower trophic levels, and they may use cues derived from complex interaction networks to locate their hosts. This review focuses on the fascinating ecology of hyperparasitoids related to how they exploit and locate their often inconspicuous hosts and the insect community processes in which hyperparasitoids are prominent players. Cutworms (Lepidoptera: Noctuidae) are economically important pests of multiple field crops, but are difficult to control with insecticides. Biological control with native parasitoids may offer a sustainable alternative for pest management. Cover crops improve the availability of resources required by parasitoids to increase their effectiveness as biocontrol agents. Copidosoma spp. (Hymenoptera: Encyrtidae) have the potential to control several species of cutworms in the Canadian Prairies. However, their foraging preferences for non‐host food items are unknown. In this study, we tested flower color and odor preferences of Copidosoma bakeri (Howard) across nine potential cover crop species and investigated how these resources affect survival and longevity of the female wasps. Results indicated that the wasps were most attracted to plants with yellow flowers in the family Brassicaceae (i.e., camelina, mustard, and canola). In multiple‐choice tests, the wasps preferred camelina over all other flower species, and showed an intermediate preference for mustard and canola. In dual‐choice tests with color paper, the wasps preferred yellow over white, and the light yellow of camelina over the bright yellow of canola and mustard, whereas they showed a preference for the odors of mustard and canola. Life expectancy was increased when the wasps were provided floral resources compared to the absence of a food source. Later planting dates and mixed‐species plantings of cover crops resulted in flowering times that better coincided with native wasp emergence. Camelina is known to suppress weeds and may be used as a habitat management tool for enhancing parasitism of cutworms by C. bakeri in the Canadian Prairies. However, further testing is needed to achieve late flowering well into August and early September and to ensure planting additional Brassica spp. does not attract more late‐season pests to cash crops such as canola. Host–parasitoid dynamics is critically influenced by variability in parasitism risk among hosts, and the host preference of the parasitoid is one of the factors causing such variability. This study examined the host preferences of the pupal parasitoid Dirhinus giffardii (Silvestri) (Hymenoptera: Chalcididae) on pupae of the oriental fruit fly Bactrocera dorsalis (Hendel) (Diptera: Tephritidae) in the laboratory. Dirhinus giffardii prefers large hosts and avoids hosts nearing eclosion when hosts are exposed to the parasitoid. This study reexamined the preferences when host pupae are in the soil (simulated by peat moss) because tephritid hosts, including B. dorsalis, pupate in the soil. Results showed that D. giffardii preferentially parasitized large hosts when hosts were exposed, but host size‐dependent parasitism was absent when hosts were in the soil. Preference on host age was absent even when hosts were exposed, contradicting the results of previous studies. The discrepancy was examined by performing a follow‐up experiment to test the hypothesis that D. giffardii cannot distinguish host age, but eggs that are laid on older hosts cannot develop successfully. Biological control efficiency can be improved by developing effective mass‐rearing systems to produce large numbers of high‐quality parasitoids. This study explored an alternative host for rearing Sclerodermus brevicornis (Kieffer) (Hymenoptera: Bethylidae), a potential biocontrol agent for the suppression of exotic and invasive wood‐boring longhorn beetle (Coleoptera: Cerambycidae) populations in the European agroforestry ecosystems. We tested larvae of the rice moth, Corcyra cephalonica Stainton (Lepidoptera: Pyralidae), as host for the parasitoid. We quantified the probability and timing of host attack and parasitism as well as reproductive success, offspring production, and the characteristics of adult offspring. As S. brevicornis is a quasi‐social species (multiple females, communally produced offspring broods), we also explored the effects of varying the number of females to which individual hosts were presented, with the aim of determining the optimal female‐to‐host ratio. As time to host attack can be a limiting factor in S. brevicornis rearing protocols, we tested the use of adult females of another bethylid species, Goniozus legneri Gordh, to paralyse C. cephalonica larvae prior to presentation. We identified the conditions within our experiment that maximized offspring production per host and offspring production per adult female parasitoid. We found that C. cephalonica is suitable as a factitious host and, as it is considerably more straightforward for laboratory rearing than cerambycid species, it is a good candidate for adoption by future S. brevicornis mass‐rearing and release programmes. Diagnostic imaging technology in vascular disease has long focused on large vessels and the pathologic processes that impact them. With improved diagnostic techniques, investigators are now able to uncover many underlying mechanisms and prognostic factors for microvascular disease. In the heart and brain, these pathologic entities include coronary microvascular disease and cerebral small vessel disease, both of which have significant impact on patients, causing angina, myocardial infarction, heart failure, stroke, and dementia. In the current paper, we will discuss parallels in pathophysiology, classification, and diagnostic modalities, with a focus on the role of magnetic resonance imaging in microvascular disease of the heart and brain. Novel approaches for streamlined imaging of the cardiac and central nervous systems including the use of intravascular contrast agents such as ferumoxytol are presented, and unmet research gaps in diagnostics are summarized. Purpose: In subarachnoid hemorrhage, noncontrast CT features are used to guide the localization of ruptured aneurysms on CT angiography and DSA. Multiplanar CT may improve the localization of aneurysm rupture sites over axial plane CT alone., Methods: Multiplanar CT in three orthogonal planes was used to evaluate 94 cases of SAH. Two investigators independently evaluated each imaging plane for focal thick SAH with mass effect, intracerebral hemorrhage, focal edema, filling defect, subdural hemorrhage, and dominant intraventricular hemorrhage. Also, rupture site was qualitatively identified by combining these variables in each plane and combination of three planes. DSA served as the gold standard to locate the rupture sites., Results: Interobserver agreement was k 0.60 to 0.79 for axial, k 0.43 to 0.86 for coronal and k 0.43 to 0.74 for sagittal planes. Good to substantial agreement was observed for the localization of rupture site in three planes (focal SAH with mass effect - k 0.78 to 0.85; filling defect - k 0.95 to 1.0; intracerebral hemorrhage - k 1.0; focal edema k 1.0; subdural hemorrhage - k 0.61 to 0.83). Dominant intraventricular hemorrhage revealed significant association with DSA to locate ruptured aneurysms (Fisher's exact test - Pr <  = P (< 0.001)). With non-missing data, frequency of correct ratings to locate rupture site was 66/67 (99%) in axial plane, 59/66 (89%) in coronal plane, 64/67 (96%) in sagittal plane and 77/77 (100%) in combined 3 planes., Conclusions: Multiplanar CT head is more successful than axial plane CT alone for the localization of aneurysm rupture sites in SAH., (© 2022. The parasitoid wasp family Braconidae is likely the second-most species-rich family in the animal kingdom. Braconid wasps are widely distributed and often encountered. They constitute one of the principal groups of natural enemies of phytophagous insects, of which many are serious pest species. The enormous biological diversification of braconid wasps has led to many homoplasies, which contributed widely to instabilities in historical classifications. Recent studies using combinations of genetic markers or total mitochondrial genomes allow for better founded groupings and will ultimately lead to a stable classification. Abstract: Maternal rearing conditions are expected to affect fitness components of progeny so that maternal effects effectively protect offspring from environmental stressors. The objective of this study was to elucidate the interaction between maternal rearing condition, age, and offspring rearing condition in the parasitoid wasp Lysiphlebus fabarum (Marshall) (Hymenoptera: Braconidae, Aphidiinae). The effects of maternal age, body size, and host instar used for development on fitness components of progeny were evaluated. Host was the black bean aphid, Aphis fabae Scopoli (Hemiptera: Aphididae). Results indicated that small and young mothers (developing in first‐instar hosts) produced progeny with a longer developmental time compared to large and old mothers (developing in second‐instar hosts). Progeny that hatched from larger eggs also had a larger body size. There were, however, no significant differences in the size of progeny that developed in different host instars. Progeny that hatched from large eggs had a higher initial egg load, and progeny produced by younger mothers had larger eggs. These differences were significant only when progeny developed in the first host instar, whereas in the second and fourth instar no differences were observed. Our findings indicate that progeny with larger size at the egg stage had superior fitness. Age‐specific maternal effects were not as strong as the effect of the mother's body size. Although the interaction between host instar of progeny development and maternal rearing condition was not statistically significant, developing in suboptimal instars led to trade‐offs between fitness components in progeny. The global burden of stroke remains high, and of the various subtypes of stroke, large vessel occlusions (LVOs) account for the largest proportion of stroke-related death and disability. Several randomized controlled trials in 2015 changed the landscape of stroke care worldwide, with endovascular thrombectomy (ET) now the standard of care for all eligible patients. With the proven success of this therapy, there is a renewed focus on penumbral sustenance. In this review, we describe the ischemic penumbra, collateral circulation, autoregulation, and imaging assessment of the penumbra. Blood pressure goals in acute stroke remain controversial, and we review the current data and suggest an approach for induced hypertension in the acute treatment of patients with LVOs. Finally, in addition to reperfusion and enhanced perfusion, efforts focused on developing therapeutic targets that afford neuroprotection and augment neural repair will gain increasing importance. ET has revolutionized stroke care, and future emphasis will be placed on promoting penumbral sustenance, which will increase patient eligibility for this highly effective therapy and reduce overall stroke-related death and disability. Objectives: To investigate whether physical exposure parameters such as the dose index (CTDI), dose length product (DLP), and size-specific dose estimate (SSDE) are predictive of DNA damage.Methods: In vitro, we scanned a phantom containing blood samples from five volunteers at CTDI 50, 100, and 150 mGy. One sample was not scanned. We also scanned samples in three different-size phantoms at CTDI 100 mGy. In vivo, we enrolled 45 patients and obtained blood samples before and after cardiac CT. The γ-H2AX foci were counted.Results: In vitro, in the control and at CTDI 50, 100, and 150 mGy, the number of γ-H2AX was 0.94 ± 0.24 (standard error, SE), 1.28 ± 0.30, 1.91 ± 0.47, and 2.16 ± 0.20. At SSDE 180, 156, and 135 mGy, it was 2.41 ± 0.20, 1.91 ± 0.47, and 1.42 ± 0.20 foci/cell. The γ-H2AX foci were positively correlated with the radiation dose and negatively correlated with the body size. In vivo, the γ-H2AX foci were significantly increased after CT (from 1.21 ± 0.19 to 1.92 ± 0.22 foci/cell) and correlated with CTDI, DLP, and SSDE.Conclusions: DNA damage was induced by cardiac CT. Parasitoids have the potential to suppress populations of their hosts and thus may play an important role in influencing the temporal and spatial dynamics of pest arthropods. Behavioural interactions between foraging females, collectively constituting 'mutual interference', can reduce host suppression. We use laboratory microcosms to assess the prevalence and consequences of mutual interference behaviour in a bethylid wasp, Goniozus nephantidis (Muesebeck) (Hymenoptera: Bethylidae), which is known to brood guard and to engage in agonistic contests for individual hosts and which is also an agent of biological pest control. We hold host and parasitoid numbers constant and vary the degree of female-female contact that can occur. Mutual interference is manifest in a considerable reduction in the number of offspring produced when females are not fully isolated from each other, due to effects operating at the early stages of offspring production. This mutual interference may contribute towards the limited degree of host population suppression achieved when some species of bethylids are deployed as agents of biological pest control and also has clear potential to influence the efficiency of mass rearing of parasitoids prior to field release. Although parasitoid wasps are important regulators of herbivorous insect populations, very little is known regarding their movement under field conditions. Here, we examined the vertical and cardinal directionality of dispersal by Binodoxys communis ( Gahan) ( Hymenoptera: Braconidae: Aphidiinae), an exotic Asian parasitoid released as a biological control agent of the soybean aphid, Aphis glycines Matsumura ( Hemiptera: Aphididae: Aphidinae), in North American soybeans, Glycine max (L.) Merr. ( Fabaceae). The patterns are consistent with time-dependent, sex-specific dispersal strategies; whereas males show flight that is devoid of vertical or cardinal directionality, females consistently move at or above soybean canopy height toward the east and north. Male capture rates also appear to be more correlated with local conditions than those of females. These results suggest that females actively cross the flight boundary layer (the space above which insect flight is largely wind-driven as opposed to self-directed) and follow air currents away from soybean fields, whereas males engage in more localized movement. The data also suggest differing response potentials of the sexes to changing local resource conditions (host availability, female availability, host plant cultivar). We discuss the implications for sex-specific movement on the basic biology of introduced parasitoid species, and their applied role as potential agents in importation biological control programs. Biological control of insect pests relies on the ability of natural enemies to limit pest populations. The behaviours expressed by natural enemies against their prey or hosts are modulated by a number of factors and a better understanding of these factors is key to obtaining more efficacious pest control. We propose here that optimality models based upon a behavioural ecology approach can provide a framework that should enable optimisation of biological control practices. We limit our discussion to parasitoid natural enemies and review the factors known to influence the behaviour of these insects. The most important areas that have been studied extensively in the behavioural ecology of insect parasitoids are addressed here: (1) residence time in a host patch, (2) clutch size, (3) sex ratio, (4) host and patch marking, and (5) diet choice. We discuss the implications of the incorporation of these optimality models into efficacious biological control practices and suggest areas where a better knowledge of the behavioural ecology of these insects could improve the efficacy of parasitoid-based pest control. Background: Neurological emergencies often pose diagnostic challenges for emergency physicians because these patients often present with atypical symptoms and standard imaging tests are imperfect. Misdiagnosis occurs due to a variety of errors. These can be classified as knowledge gaps, cognitive errors, and systems-based errors. The goal of this study was to describe these errors through review of quality assurance (QA) records. Methods: This was a retrospective pilot study of patients with neurological emergency diagnoses that were missed or delayed at one urban, tertiary academic emergency department. Cases meeting inclusion criteria were identified through review of QA records. Three emergency physicians independently reviewed each case and determined the type of error that led to the misdiagnosis. Proportions, confidence intervals, and a reliability coefficient were calculated. Results: During the study period, 1168 cases were reviewed. Forty-two cases were found to include a neurological misdiagnosis and twenty-nine were determined to be the result of an error. The distribution of error types was as follows: knowledge gap 45.2% (95% CI 29.2, 62.2), cognitive error 29.0% (95% CI 15.9, 46.8), and systems-based error 25.8% (95% CI 13.5, 43.5). Cerebellar strokes were the most common type of stroke misdiagnosed, accounting for 27.3% of missed strokes. Conclusions: All three error types contributed to the misdiagnosis of neurological emergencies. Misdiagnosis of cerebellar lesions and erroneous radiology resident interpretations of neuroimaging were the most common mistakes. Parasitoids of the hymenopterous family Encyrtidae are one of the most important groups of natural enemies of soft scale insects and have been used extensively in biological control. We summarize existing knowledge of the biology, ecology, and behavior of these parasitoids and how it relates to biological control. Soft scale stage/size and phenology are important determinants of host range and host utilization, which are key aspects in understanding how control by these parasitoids is exerted. Furthermore, the nutritional ecology of encyrtids and their physiological interactions with their hosts affect soft scale insect population dynamics. Lastly, the interactions among encyrtids, heteronomous parasitoids, and ants shape parasitoid species complexes and consequently have a direct impact on the biological control of soft scale insects. The advent of flow dynamics and the recent availability of perfusion analysis software have provided new diagnostic tools and management possibilities for cerebrovascular patients. To this end, we provide an example of the use of colorcoded angiography and its application in a rare case of a patient with a pure middle cerebral artery (MCA) malformation. A 42-year-old male chronic smoker was evaluated in the emergency room due to sudden onset of severe headache, nausea, vomiting and left-sided weakness. Head computed tomography revealed a right basal ganglia hemorrhage. Cerebral digital subtraction angiography (DSA) showed a right middle cerebral artery malformation consisting of convoluted and ectatic collateral vessels supplying the distal middle cerebral artery territory-M1 proximally occluded. An associated medial lenticulostriate artery aneurysm was found. Brain single-photon emission computed tomography with and without acetazolamide failed to show problems in vascular reserve that would indicate the need for flow augmentation. Twelve months after discharge, the patient recovered from the left-sided weakness and did not present any similar events. A follow-up DSA and perfusion study using color-coded perfusion analysis showed perforator aneurysm resolution and adequate, albeit delayed perfusion in the involved vascular territory. We propose a combined congenital and acquired mechanism involving M1 occlusion with secondary dysplastic changes in collateral supply to the distal MCA territory. Angiographic and cerebral perfusion work-up was used to exclude the need for flow augmentation. Nevertheless, the natural course of this lesion remains unclear and long-term follow-up is warranted. Background and purpose The difference in the relationship between the size of intracranial aneurysms (IAs) and their risk of rupture in patients with singe IAs versus those with multiple IAs is unclear. We sought to retrospectively analyze the size of ruptured IAs (RIAs) in patients with single and multiple IAs in order to study this relationship further. Methods We retrospectively measured the size and location of RIAs in all patients who presented to our institute with an acute subarachnoid hemorrhage between 1 January 2005 and 31 December 2010. The IAs were classified by size into very small IAs or VSAs (≤3 mm), small IAs or SAs (>3 mm but ≤7 mm) and others (>7 mm). Results 379 patients (281 with a single IA, Group 1 and 98 with multiple IAs, Group 2) with 419 treated RIAs were included in the study. VSAs and SAs constituted the majority of RIAs in both groups (33.5% and 45.2% in Group 1 and 24.6% and 50.7% in Group 2) and the mean size of the RIAs was not different between the two groups. VSAs constituted almost two-thirds of all RIAs in certain locations whereas IAs > 7 mm in size did not constitute more than a third of the RIAs at any of the arterial locations. Conclusions The high incidence of VSAs, particularly in certain locations in both patient subgroups, suggests that current diagnostic, prognostic and therapeutic options in the management of IAs should be more tailored towards the management of these difficult-to-treat lesions. Although ovipositing insects may predominantly use resources that lead to high offspring quality, exceptions to this rule have considerably aided understanding of oviposition decisions. We report the frequency of host species use by a solitary facultative hyperparasitoid, Brachymeria subrugosa Blanchard ( Hymenoptera: Chalcididae). In our samples, the wasp attacks the large pupae of the moth Gonioterma indecora Zeller ( Lepidoptera: Elachistidae), as well as the considerably smaller, and rarer, pupae of two of its other parasitoids. Consistent with conditional sex allocation models, the wasp produced mainly female offspring on the largest (moth) host, an unbiased sex ratio on the middle-sized (parasitoid) host, and only males on the smallest (parasitoid) host. Adult offspring size was correlated with the size of the host attacked. These features strongly suggest that the two smaller, primary parasitoid, hosts produce lower-quality offspring. Despite being more common, the proportion of hosts from which parasitoids emerged was lowest (14%) on the largest host species, and highest on the rarer middle-sized (34%) and smallest (30%) hosts. This suggests that costs or constraints on attacking high-quality primary hosts may be a selective force favouring the evolution of hyperparasitism. [ABSTRACT FROM AUTHOR]

The way an invasion progresses through space is a theme of interest common to invasion ecology and biological pest control. Models and mark-release studies of arthropods have been used extensively to extend and inform invasion processes of establishment and spread. However, the extremely common single-scale approach of monitoring initial spread leads to misinterpretation of rate and mode. Using the intentional release of a novel biological control agent (a parasitic hymenoptera, Eretmocerus hayati Zolnerowich & Rose (Hymenoptera: Aphelinidae), we studied its initial dispersal and spread at three different spatial scales, the local scale (tens of metres), field scale (hundreds of metres) and landscape scale (kilometres) around the release point. We fit models to each observed spread pattern at each spatial scale. We show that E. hayati exhibits stratified dispersal; moving further, faster and by a different mechanism than would have been concluded with a single local-scale post-release sampling design. In fact, interpretation of each scale independent of other scales gave three different models of dispersal, and three different impressions of the dominant dispersal mechanisms. Our findings demonstrate that using a single-scale approach may lead to quite erroneous conclusions, hence the necessity of using a multiple-scale hierarchical sampling design for inferring spread and the dominant dispersal mechanism of either human intended or unintended invasions. Many aspects of a parasitoid’s biology may be affected by its host. Host size, for example, could affect parasitoid fitness, especially in gregarious parasitoids, in which the resource is used by multiple siblings. Oomyzus sokolowskii (Kurdjumov) (Hymenoptera: Eulophidae) is a gregarious larval–pupal endoparasitoid of the diamondback moth, Plutella xylostella L. (Lepidoptera: Plutellidae), a major pest of crucifers worldwide, and is able to superparasitize the host. This study focuses on the hypothesis that because resource availability is higher in larger hosts, parasitoids developing in larger hosts will fare better. However, superparasitized hosts are expected to yield larger numbers of parasitoid offspring of smaller body size. Results showed that superparasitism increased the number of parasitoid offspring produced per host and increased offspring longevity, but decreased offspring body size. However, developmental time and sex ratio of parasitoid offspring was similar among hosts parasitized once, twice, or three times. Regardless of superparasitism, parasitoids emerging from larger hosts that were fed honey solution lived longer than similarly fed progeny from smaller hosts (36.4 vs. 22.1 days). The results partially support the hypothesis that Oomyzus gained fitness from an increase in host size; moreover, superparasitism seems advantageous for Oomyzus due to increased offspring numbers and longevity. In natural systems, pre-adult stages of some insect herbivores are known to be attacked by several species of parasitoids. Under certain conditions, hosts may be simultaneously parasitized by more than one parasitoid species (= multiparasitism), even though only one parasitoid species can successfully develop in an individual host. Here, we compared development, survival, and intrinsic competitive interactions among three species of solitary larval endoparasitoids, Campoletis sonorensis (Cameron) (Hymenoptera: Ichneumonidae), Microplitis demolitor Wilkinson, and Microplitis croceipes (Cresson) (Hymenoptera: Braconidae), in singly parasitized and multiparasitized hosts. The three species differed in certain traits, such as in host usage strategies and adult body size. Campoletis sonorensis and M. demolitor survived equally well to eclosion in two host species that differed profoundly in size, Pseudoplusia includens (Walker) and the larger Heliothis virescens (Fabricius) (both Lepidoptera: Noctuidae). Egg-to-adult development time in C. sonorensis and M. demolitor also differed in the two hosts. Moreover, adult body mass in C. sonorensis (and not M. demolitor) was greater when developing in H. virescens larvae. We then monitored the outcome of competitive interactions in host larvae that were parasitized by one parasitoid species and subsequently multiparasitized by another species at various time intervals (0, 6, 24, and 48 h) after the initial parasitism. These experiments revealed that M. croceipes was generally a superior competitor to the other two species, whereas M. demolitor was the poorest competitor, with C. sonorensis being intermediate in this capacity. However, competition sometimes incurred fitness costs in M. croceipes and C. sonorensis, with longer development time and/or smaller adult mass observed in surviving wasps emerging from multiparasitized hosts. Females of insect endoparasitoids inject substances along with the egg at oviposition, which can regulate or induce changes in a number of host physiological processes to benefit the developing parasitoid. These changes can be caused by substances such as venoms, calyx fluids, or symbiotic-associated virus particles (polydnavirus), which are injected by females along with their eggs, and by substances secreted by parasitoid-derived tissues (teratocytes) or the developing parasitoid larvae. Teratocytes (dissociated cells of the serosal membrane after parasitoid eclosion) release substances that have roles (i) in parasitoid nutrition, (ii) in the digestion of host tissues, and (iii) in the regulation of host development. Teratocytes of Toxoneuron nigriceps (Viereck) (Hymenoptera: Braconidae) have been implicated in the arrestment of the host development and in the regulation of circulating levels of host ecdysteroids. Here we describe the cDNA of a teratocyte-secreted chitinase and its expression during parasitoid development, and discuss its putative role in this host–parasitoid association. The establishment and maintenance of suitable habitat on-farm or in the surrounding landscape can enhance the survival of beneficial parasitic Hymenoptera, thus improving the control of pest species. Both endemic and weedy non-crop plant species across a highly modified agricultural landscape supported species-rich and abundant parasitic wasp assemblages with diverse biology and host associations. It was also shown that isolated, recently planted, single-species stands of plants can rapidly accumulate diverse assemblages of parasitoids. Chalcidoidea was the most species-rich and abundant group, egg and larval parasitoids were the most speciose and abundant guilds, and parasitoids of herbivorous insects feeding on and inside plant tissue were the most species-rich and abundant functional groups. The hymenopteran assemblages associated with the majority of plant species were dominated by three parasitoid species: a Trichogrammatidae, a Scelionidae ( Telenomus sp.) and a Eulophidae ( Ceranisus sp.), all genera that contain many important biocontrol agents of pest Lepidoptera, Hemiptera and Thysanoptera. Behavioural interactions among relatives may have consequences for many other traits. We tested the hypothesis that solitary parasitoids (displaying siblicidal behaviour in their larvae) have narrower host ranges than gregarious parasitoids (with tolerant larvae). In laboratory experiments, we compared parasitization success in two sister species of braconid wasp [ Aphaereta genevensis (Fischer), solitary, and Aphaereta pallipes (Say), gregarious (Hymenoptera: Braconidae: Alysiini)] on eight Drosophila species or strains. Host species or strain was the most important factor affecting parasitization success, and some of this variation was accountable to host physiological defences. Although two hosts were more suitable for the solitary species, and one more suitable for the gregarious species, these differences were small, and there was no consistent difference across all hosts. Wasp body size was positively correlated with parasitization success in both wasp species. This may be because body size increases oviposition success, or the motivation to oviposit. In A. pallipes parasitization success peaked after 3–4 days, but later in A. genevensis. This is likely due to low life expectancy and high egg loads increasing oviposition tendency in young A. pallipes, and egg limitation decreasing oviposition tendency in old A. pallipes. These data suggest that interactions among wasp larvae do not greatly affect the size of the fundamental niche examined here. However, they show the potential for life history traits, which differ between the species as a likely consequence of larval interactions, to affect the extent of the realized niche.
