Klebsiella pneumoniae (KP) is the most common cause of endogenous endophthalmitis (EE) in Asia, but data in Europe are scarce. We describe eight cases of KP EE compared to a cohort of EE in a French center. EE cases were retrospectively studied between January 2014 and January 2021. KP EE cases were analyzed to assess clinical, microbiological features, and outcome. Among the 33 EE cases identified, the first causative agent (24%, n = 8) was KP, mainly (7/8) with hypervirulent phenotype (hvKP). All but one of these cases occurred from December 2019 to January 2021. Contrary to non-KP patients, KP patients had multiple extraocular infective foci (p =.006), all presented with liver abscesses (p <.001), 50% had cerebral involvement (p =.13). Visual outcome was poor in both groups. KP is an emerging cause of EE in a French center, consistently associated with liver abscesses, frequent cerebral involvement, and predominance of hvKP strains. [ABSTRACT FROM AUTHOR]

Background: Improving timeliness of pathogen identification is crucial to allow early adaptation of antibiotic therapy and improve prognosis in patients with pneumonia. We evaluated the relevance of a new syndromic rapid multiplex PCR test (rm-PCR) on respiratory samples to guide empirical antimicrobial therapy in adult patients with community-acquired pneumonia (CAP), hospital-acquired pneumonia (HAP), and ventilator-acquired pneumonia (VAP).Methods: This retrospective multicenter study was conducted in four French university hospitals. Respiratory samples were obtained from patients with clinical and radiological signs of pneumonia and simultaneously tested using conventional microbiological methods and the rm-PCR. A committee composed of an intensivist, a microbiologist, and an infectious diseases specialist retrospectively assessed all medical files and agreed on the most appropriate antimicrobial therapy for each pneumonia episode, according to the results of rm-PCR and blinded to the culture results. The rm-PCR-guided antimicrobial regimen was compared to the empirical treatment routinely administered to the patient in standard care.Results: We included 159 pneumonia episodes. Most patients were hospitalized in intensive care units (n = 129, 81%), and episodes were HAP (n = 68, 43%), CAP (n = 54, 34%), and VAP (n = 37, 23%). Conventional culture isolated ≥ 1 microorganism(s) at significant level in 95 (60%) patients. The syndromic rm-PCR detected at least one bacteria in 132 (83%) episodes. Based on the results of the rm-PCR, the multidisciplinary committee proposed a modification of the empirical therapy in 123 (77%) pneumonia episodes. The modification was a de-escalation in 63 (40%), an escalation in 35 (22%), and undetermined in 25 (16%) patients. In microbiologically documented episodes (n = 95), the rm-PCR increased appropriateness of the empirical therapy to 83 (87%), as compared to 73 (77%) in routine care.Conclusions: Use of a syndromic rm-PCR test has the potential to reduce unnecessary antimicrobial exposure and increase the appropriateness of empirical antibiotic therapy in adult patients with pneumonia. According to ADQI criteria, AKI was defined as rapidly reversed in 42 patients (25.3% of AKI patients), persistent AKI in 16 patients (9.7%) and as acute kidney disease (AKD) in 64 patients (38.6%), remaining patients couldn't be classified (n = 44). 108 (61.0%) patients died in intensive care, 76 (43.4%) patients had a hemorrhagic complication, 46 patients (26%) had a severe hemorrhagic complication defined by death or/and massive transfusion or/and hemostatic procedure (surgery or endoscopy), 87 (49.1%) patients had thrombopenia < 100 G/L, 50 (28.2%) patients had thrombopenia < 50 G/L, 53 patients (30.1%) and 43 patients (24.6%) required antiaggregation and anticoagulation withdrawal. Tracheal disorders patterns were: tracheal ring fractures (15 patients), granulomas (11 patients), tracheomalacia (7 patients), subglottic edema (4 patients), tracheal stenosis (2 patients). Concerning the other 31 patients, antibiotics prescribed were: amoxicillin/clavulanic acid in 17 patients (47.2%), carbapenems in 17 patients (47.2%), aminosids in 13 patients (36.1%), glycopeptides in 10 patients (27.8%), colistin in 9 patients (25%), fluoroquinolones in 7 patients (19.4%), cephalosporins in 6 patients (16.6%), tigecycline in 4 patients (11.1%). For the modalities of BT: the cross match was done for 7 patients in ICU (18% of cases), and for 2 patients of CVS in block A2 (7% of patients), we had 2 anti-B agglutinations (2.34%): one in ICU patient AB+, and one in patient block A+, for each one blood bags have been changed.
