Design. A prospective, multicenter, open-label, randomized, comparative clinical study. Objective. To compare the efficacy and safety of Flucovag (sertaconazole) vaginal suppositories, 300 mg (OTCPharm JSC, Russia; manufactured by Altpharm LLC, Russia) and the brand-name drug Zalain (sertaconazole), 300 mg (Egis Pharmaceuticals PLC, Hungary) in the treatment of vulvovaginal candidiasis. Materials and methods. The study included female patients (1855 years old) with uncomplicated vulvovaginal candidiasis treated in outpatient settings. In total, 220 patients were randomized into two groups: group 1 (n=110) received a single dose of Flucovag vaginal suppositories, and group 2 (n=110) received a single dose of Zalain vaginal suppositories. Seven days after the treatment, its clinical efficacy was evaluated. In case of incomplete clinical improvement after 7 days, patients received an additional dose of Flucovag (n=4) or Zalain (n=5) suppositories, respectively. Results. At the end of therapy in the study groups, the clinical cure rates (99.1% vs. 96.3%, p0.05), microscopic, and culture results were comparable. No serious adverse events were reported in both groups throughout the study. Conclusion. The study results demonstrate a favorable safety profile and high antifungal activity of sertaconazole.

Background: Alaska Native and American Indian (ANAI) communities in Alaska are disproportionately affected by commercial tobacco use. Financial incentive interventions promote cigarette smoking cessation, but family-level incentives have not been evaluated. We describe the study protocol to adapt and evaluate the effectiveness and implementation of a remotely delivered, family-based financial incentive intervention for cigarette smoking among Alaskan ANAI people. Methods: The study has 3 phases: 1) qualitative interviews with ANAI adults who smoke, family members, and stakeholders to inform the intervention, 2) beta-test of the intervention, and 3) randomized controlled trial (RCT) evaluating intervention reach and effectiveness on verified, prolonged smoking abstinence at 6- and 12-months post-treatment. In the RCT, adult dyads (ANAI person who smokes [index participant] and family member) recruited throughout Alaska will be randomized to a no-incentives control condition (n = 328 dyads) or a 6-month incentive intervention (n = 328 dyads). All dyads will receive cessation support and family wellness materials. Smoking status will be assessed weekly for four weeks and at three and six months. Intervention index participants will receive escalating incentives for verified smoking abstinence at each time point (maximum $750 total); the family member will receive rewards of equal value. Results: A community advisory committee contributed input on the study design and methods for relevance to ANAI people, particularly emphasizing the involvement of families. Conclusion: Our study aligns with the strength and value AIAN people place on family. Findings, processes, and resources will inform how Indigenous family members can support smoking cessation within incentive interventions. Clinical Trials Registry: NCT05209451

Introduction Alaska Native and American Indian (ANAI) peoples in Alaska currently experience a disproportionate burden of morbidity and mortality from tobacco cigarette use. Financial incentives for smoking cessation are evidence-based, but a family-level incentive structure has not been evaluated. We used a community-based participatory research and qualitative approach to culturally adapt a smoking cessation intervention with ANAI families. Aims and Methods We conducted individual, semistructured telephone interviews with 12 ANAI adults who smoke, 12 adult family members, and 13 Alaska Tribal Health System stakeholders statewide between November 2022 and March 2023. Through content analysis, we explored intervention receptivity, incentive preferences, culturally aligned recruitment and intervention messaging, and future implementation needs. Results Participants were receptive to the intervention. Involving a family member was viewed as novel and aligned with ANAI cultural values of commitment to community and familial interdependence. Major themes included choosing a family member who is supportive and understanding, keeping materials positive and encouraging, and offering cash and noncash incentives for family members to choose (eg, fuel, groceries, activities). Participants indicated that messaging should emphasize family collaboration and that cessation resources and support tips should be provided. Stakeholders also reinforced that program materials should encourage the use of other existing evidence-based cessation therapies (eg, nicotine replacement, counseling). Conclusions Adaptations, grounded in ANAI cultural strengths, were made to the intervention and recruitment materials based on participant feedback. Next steps include a beta-test for feasibility and a randomized controlled trial for efficacy. Implications This is the first study to design and adapt a financial incentives intervention promoting smoking cessation among ANAI peoples and the first to involve the family system. Feedback from this formative work was used to develop a meaningful family-level incentive structure with ANAI people who smoke and family members and ensure intervention messaging is supportive and culturally aligned. The results provide qualitative knowledge that can inform future family-based interventions with ANAI communities, including our planned randomized controlled trial of the intervention. In his classic study in the Siskiyou Mountains (Oregon, USA), one of the most botanically rich forested regions in North America, R. H. Whittaker (1960) foreshadowed many modern ideas on the multivariate control of local species richness along environmental gradients related to productivity. Using a structural equation model to analyze his data, which were never previously statistically analyzed, we demonstrate that Whittaker was remarkably accurate in concluding that local herb richness in these late-seral forests is explained to a large extent by three major abiotic gradients (soils, topography, and elevation), and in turn, by the effects of these gradients on tree densities and the numbers of individual herbs. However, while Whittaker also clearly appreciated the significance of large-scale evolutionary and biogeographic influences on community composition, he did not fully articulate the more recent concept that variation in the species richness of local communities could be explained in part by variation in the sizes of regional species pools. Our model of his data is among the first to use estimates of regional species pool size to explain variation in local community richness along productivity-related gradients. We find that regional pool size, combined with a modest number of other interacting abiotic and biotic factors, explains most of the variation in local herb richness in the Siskiyou biodiversity hotspot.

Diabetes mellitus remains a huge public health concern. Anchomanes difformis Blume is an herbaceous plant with numerous folkloric uses. This study investigated the acute and sub-acute effects of hydroethanolic rhizome extracts of A. difformis (AD) on glucose level in normal and diabetic Sprague-Dawley rats. Type 2 diabetes was induced with streptozotocin (65 mg/kg) administration after nicotinamide (120 mg/kg) i.p. Diabetic rats were divided into five groups: diabetic control (distilled water 10 mL/kg, p.o.), A. difformis (125, 250 and 500 mg/kg, p.o.) and glibenclamide (2.5 mg/kg, p.o.). Group 6 was normal control (non-diabetic; distilled water 10 mL/kg, p.o.). Treatments were done for 21 days. Body weight and fasting blood glucose level of the animals were determined weekly. On day 21, blood samples were collected for serum biochemical and lipid profile assays. Vital organs were collected for assay of tissue antioxidant levels and histopathological assessment. AD produced significant (P<0.05-0.001) dose-dependent reduction in blood glucose level with peak effects at 500 mg/kg dose on day 21. 500 mg/kg AD elicited significant increase in the levels of SOD and CAT in the liver, kidney and pancreas. The level of ALP was significantly reduced in diabetic rats by AD (250-500 mg/kg). 250-500 mg/kg AD significantly reduced the levels of creatinine and urea, with significant increase in total protein levels. 500 mg/kg AD significantly reduced (P<0.05) cholesterol level. No significant histopathological distortions were observed in vital organs. The effects of AD extract were comparable to that of glibenclamide. Findings in this study suggest that AD possesses significant antidiabetic effect. [ABSTRACT FROM AUTHOR]

The beam-profile diagnostics station prototype for the superconducting rf electron linac being constructed at Fermilab at the New Muon Lab has been tested. The station uses intercepting radiation converter screens for the low-power beam mode: either a 100-\mu m thick YAG:Ce single crystal scintillator or a 1-\mu m thin Al optical transition radiation (OTR) foil. The screens are oriented with the surface perpendicular to the beam direction. A downstream mirror with its surface at 45 degrees to the beam direction is used to direct the radiation into the optical transport. The optical system has better than 20 (10) \mu m rms spatial resolution when covering a vertical field of view of 18 (5) mm. The initial tests were performed at the A0 Photoinjector at a beam energy of ~15 MeV and with micropulse charges from 25 to 500 pC for beam sizes of 45 to 250 microns. The measurement of the longitudinal behavior of the accelerated particle beams at Fermilab is crucial to the optimization and control of the beam and the maximizing of the integrated luminosity for the particle physics experiments. Longitudinal measurements in the Tevatron and Main Injector synchrotrons are based on the analysis of signals from resistive wall current monitors. This article describes the signal processing performed by a 2 GHz-bandwidth oscilloscope together with a computer running a LabVIEW program which calculates the longitudinal beam parameters.

Low intake of fruits and vegetables and high intake of sugar-sweetened beverages persists as a public health concern in rural remote Alaska Native (AN) communities. Conducting key informant interviews with 22 storekeepers in 12 communities in the Yukon-Kuskokwim region of Alaska, we explored potential factors impeding or facilitating dietary change towards healthier food choices. We selected these sites as part of a multi-level intervention aimed at introducing more traditional AN subsistence foods, increasing fruit and vegetable intake, and decreasing SSB consumption among young children enrolled in Head Start (preschool) programmes (Clinicaltrials.gov #NCT03601299). Storekeepers in these communities agreed that seasonality and flight schedules were primary factors determining commercial foods’ availability. Several storekeepers noted that federal food assistance programmes that specify which food items may be purchased with funds received from the programme and community policies that set limits on less healthy items promote customer purchases of healthier products. The fact that storekeepers are comfortable enforcing government assistance programme guidelines, company policies, and tribal resolutions suggests an important role storekeepers play in improving nutritional intake in their communities.

Background: Malaria remains an enduring public health concern in Indonesia, exacerbated by its equatorial climate that fosters the proliferation of Anopheles mosquitoes. This study seeks to assess the performance of the malaria elimination programme comprehensively., Methods: Between May and August 2022, a qualitative study was conducted in Muara Enim Regency, South Sumatra Province, involving 22 healthcare professionals from diverse backgrounds. These informants were strategically chosen for their pivotal roles in providing profound insights into various facets of the malaria elimination programme. This encompasses inputs such as human resources, budgetary allocation, and infrastructural support; processes like case identification and management, capacity enhancement, epidemiological surveillance, prevention measures, outbreak control, and enhanced communication and educational initiatives; and, notably, the programme's outcomes. Data were collected through 3-h Focus Group Discussions (FGDs) divided into two groups, each with 12 participants: healthcare professionals and programme managers. Additionally, in-depth interviews (IDIs) were conducted with ten informants. Employing the Input-Process-Output (IPO) model, this study meticulously analysed the healthcare system dynamics and the interventions' efficacy., Results: The study unveiled many challenges during the input phase, including the absence of entomologists and a shortage of diagnostic tools. Despite these obstacles, it documented remarkable accomplishments in the output domain, marked by significant advancements in the distribution of mosquito nets and the successful implementation of the Early Warning System (EWS). Despite the adversities, the programme has made substantial strides towards malaria elimination., Conclusions: Urgent action is imperative to bolster the effectiveness of the malaria elimination programme. Key measures encompass augmenting the entomologist workforce, optimizing resource allocation, and ensuring stringent adherence to regional regulations. Addressing these concerns will enhance programme efficacy, yielding enduring public health benefits. This research substantially contributes to Indonesia's ongoing malaria elimination endeavours, furnishing actionable insights for programme enhancement. Pendahuluan: CABG merupakan tindakan pembedahan membentuk pembuluh darah baru dengan melewati pembuluh darah yang menyempit, yang memakai pembuluh darah tubuh lainnya. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan pada dukungan keluarga dan tingkat kecemasan Pasien Pre Operasi CABG pada Ruang IW Bedah Jantung Rumah Sakit Polri Jakarta. Metode: Penelitian ini mengunakan analitik melalui pendekatan Cross-Sectional. Penelitian dilakukan di ruang IW Pre Operasi Bedah di Rumah Sakit Polri. Dengan Populasi yaitu seluruh pasien operasi CABG di Rumah Sakit Polri sedangkan Sampel yang diambil yaitu semua pasien pre CABG di Rumah Sakit Polri berjumlah 150 responden. Analisis penelitian menggunakan uji Chi-Square. Hasil: Hasil uji chi-square hubungan dukungan keluarga dengan kecemasan pasien pre operasi CABG pada Ruang IW Bedah Rumah Sakit Polri dengan p-value 0,037 < 0,05. Kesimpulan: Terdapat hubungan dukungan keluarga dengan kecemasan pasien pre operasi CABG pada Ruang IW Bedah Rumah Sakit Polri.

41. An In-vitro Study on Post-surgical Breast Wound Healing Activity by Zinc Oxide Dots and its Optimization using Box Behnken Design

- Abstract
In spite of the discovery of various medicines to heal post-surgical malignant wounds caused by the recurrence of breast cancer cells after chemotherapy, still it’s a serious problem to heal malignant wounds completely for surgeons and this has to be cured in order to avoid further risk of wound issues. This study is one such attempt taken to give a solution to heal surgical breast wounds for the future analysts. For this purpose, we have synthesized a novel bio-compatible fluorescent quantum dot using curcumin-loaded Zinc oxide bionanocomposite (Cur-ZnOBC). The ZnOBC is composed of neem essential oil-Aloe-vera gel complex which acted as a nanoconjugate for the formation of Cur-ZnOBC.Then it is converted into curcumin-based quantum dots (Cur-ZnOQDs) using microwave-assisted method in order to improve the drug delivery purpose. The resulting Cur-ZnOQDs was treated in the malignant wounds of breast cancer cells post-surgery for wound healing activity. The synthesized Cur-ZnOQDs was characterized to confirm its size and morphology. Thus, prepared Cur-ZnOQDs showed low cytotoxicity (IC50 0.17 µg/mL) to human breast cancer cells. Cell death was noticed at a very low concentration of 0. 6mg/mL itself. It was also found that wound closure area was increased from 22.3% at 24 h to 44.2% at 48 h which was 20 times greater cell migration when compared to control (untreated cells) and at a very low concentration of Cur-ZnOQDs (0.1mg/mL). Hence, Cur-ZnOQDs act as bio-enhancers assisting in the improvement of outcomes associated with breast cancer and, ultimately, improving the survival rate.

Eliminating malaria by 2030 is stated as goal three in the UN's Sustainable Development Goals (SDGs). However, malaria still remains a significant public health problem. This study aims to identify the factors determining malaria transmission in artisanal or small-scale miner (ASM) communities in three villages: Tanjung Agung, Tanjung Lalang, and Penyandingan, located in the Tanjung Enim District, Muara Enim, South Sumatra, Indonesia. Researchers conducted a cross-sectional study involving 92 participants from the study area. They used a logistic regression model to investigate the risk factors related to malaria occurrence. The multivariable analysis revealed that age (Adjusted Prevalence Ratio (APR) = 7.989 with 95% CI 1.724-37.002) and mosquito breeding (APR = 7.685 with 95% CI 1.502-39.309) were risk factors for malaria. On the other hand, higher education (APR = 0.104 with 95% CI 0.027-0.403), the use of mosquito repellent (APR = 0.138 with 95% CI 0.035-0.549), and the condition of house walls (APR = 0.145 with 95% CI 0.0414-0.511) were identified as protective factors. The current study highlights age and mosquito breeding sites as risk factors for malaria. Additionally, higher education, insect repellent use, and the condition of house walls are protective factors against malaria. Therefore, reducing risk factors and increasing protective measures through effective communication, information, and education are highly recommended to eliminate malaria in mining areas., (© 2023. Introduction: Malaria is a significant public health concern in Indonesia. Muara Enim Regency is one of the districts in South Sumatra with the most important number of indigenous malaria cases in the last three years (2018-2020). Therefore, this study aimed to identify determinants of indigenous malaria in the Muara Enim Regency., Methods: This study was designed as a case-control study. A stratified random sample in 2018, 2019, and 2020 was used at the Primary Health Centres (PHCs) areas of Tanjung Enim and Tanjung Agung. The sample included 49 cases and 49 controls. Indigenous malaria determinants were discovered using both bivariable and multivariable logistic regression models., Result: The multivariable logistic regression model results show that mosquito repellent reduces malaria risk by 71% (AOR = 0.29, 95% CI: 0.11-0.64). Besides, the presence of wire mesh on ventilation reduces the risk of malaria by 76% (AOR = 0.24, 95% CI: 0.10-0.57), and the distance from mosquito breeding sites near hundred meters and fewer increases the risk of malaria by 3.88 fold (AOR = 3.88; 95% CI: 1.67-8.97)., Conclusions: Multivariable analysis revealed distance from mosquito breeding sites as a risk factor for malaria. Besides, the study shows that using insect repellent, wire netting in ventilation, eliminating mosquito breeding sites, mosquito repellent or protective clothing, and improving house conditions were protective factors for indigenous malaria. Therefore, preventive and promotional efforts are essential as the first step toward malaria elimination at the study site, including avoiding direct contact between residents and vectors near mosquito breeding sites., Competing Interests: The authors have declared that no competing interests exist., (Copyright: © 2023 Hasyim et al.
