Background: Work-related low back pain (LBP) is a common musculoskeletal disorder related to work and contributes to decreased employee health and productivity. Physical therapists (PTs) are more susceptible to developing LBP than other healthcare employees. Although LBP might contribute to decreased quality of life (QoL) among PTs, factors underpinning correlations between work-related musculoskeletal disorders and QoL among PTs are not substantiated. This study aimed to investigate correlations between work-related LBP and QoL among PTs. Methods: This cross-sectional study used an online survey completed by PTs. The survey form was based on a previously published questionnaire for assessing the experience of work-related LBP and the 36-Item Short Form Health Survey for evaluating QoL. The dependent variable was QoL; the independent variables were LBP rating, how LBP impacted the work of the PTs, weekly hours of direct patient contact, experience as a PT, and income. Results: A total of 690 PTs, consisting of males (375) and females (315), completed the survey form. Correlation analysis revealed significant correlations between QoL and the following variables: income (r=0.42, p < 0.01), LBP impact on participant's job performance (r=0.41, p < 0.01), experience as a PT (r=0.17, p < 0.01), weekly hours of direct patient contact (r=0.11, p < 0.01), rating of LBP (r=-0.64, p < 0.01) and area of PT specialty (r=-0.15, p < 0.01). Conclusions: The study showed correlations related to multiple factors between QoL and work-related LBP among PTs. Further investigation is needed to obtain more knowledge about these correlations.

During the 1920s and 1930s, Fay M. Jackson broke traditional barriers by serving as the first African American foreign correspondent for the Associated Negro Press (ANP). Jackson was the only African American female reporter of the ANP who covered the coronation of King George VI in 1937 and used the opportunity to report on the sociopolitical affairs of Blacks in Europe while specifically underscoring the Italo-Ethiopian conflict. While in Europe, Jackson set out to meet with various political figures and activists of color to emphasize the parallel treatment between Blacks in the U.S and other communities of color outside the U.S. Little research has been done on the role of African American female journalists in American history. Therefore, Jackson's importance is further accentuated by the fact that she was one of few women who forged a way into the Black press. Abstract In modern era, deficiency of Vitamin D3 is predominantly due to limited exposure to sunlight and UV radiation resulting from indoor lifestyles. Several studies have revealed that vitamin D deficiency can lead to chronic vascular inflammation, diabetes mellitus, hypertension, congestive left ventricular hypertrophy, and heart failure. This study introduces a green synthesis of novel bimetallic nanoporous composite, CuO/Ag using lemon extract. The synthesized nanoporous material, CuO/Ag@lemon extract was characterized using several analytical techniques, including X-ray diffraction (XRD), transmission electron microscopy (TEM), scanning electron microscopy (SEM), and energy-dispersive X-ray spectroscopy (EDX). The CuO/Ag@lemon extract nanoparticles were immobilized on glassy carbon electrode (GCE) to prepare modified CuO/Ag@lemon extract–GCE interface. The electrocatalytic and electrochemical properties investigation was carried out on the modified electrode. using cyclic voltammetry (CV), differential pulse voltammetry (DPV), and amperometry for detecting of Vitamin D3. The DPV method displayed a linear response range of 0.02–22.5 µM with a detection limit of 2.62 nM, while the amperometric method showed a broader linear range of 0.25–23.25 µM with a detection limit of 2.70 nM with 82% modified electrode stability. The designed electrode exhibited a positive response to the inclusion of Vitamin D3 with electro-oxidation, reaching steady-state within 3.4 s, with 87% reproducibility within a day. The proposed method offers a rapid and sensitive platform for detection of Vitamin D3 with minimal interference from other molecules. The early diagnosis of Vitamin D3 deficiency using modified electrodes allows for early treatment, thereby preventing severe health complications.

Cell models of cardiac ischemia-reperfusion (IR) injury are essential to facilitate understanding, but current monolayer cell models poorly replicate the in vivo IR injury that occurs within a three-dimensional tissue. Here we show that this is for two reasons: the residual oxygen present in many cellular hypoxia models sustains mitochondrial oxidative phosphorylation; and the loss of lactate from cells into the incubation medium during ischemia enables cells to sustain glycolysis. To overcome these limitations, we incubated isolated adult mouse cardiomyocytes anoxically while inhibiting lactate efflux. These interventions recapitulated key markers of in vivo ischemia, notably the accumulation of succinate and the loss of adenine nucleotides. Upon reoxygenation after anoxia the succinate that had accumulated during anoxia was rapidly oxidized in association with extensive mitochondrial superoxide/hydrogen peroxide production and cell injury, mimicking reperfusion injury. This cell model will enable key aspects of cardiac IR injury to be assessed in vitro.

We present a case of acute-onset orbital inflammation with rapidly progressive proptosis, episcleral venous stasis with raised intraocular pressure and loss of vision in a patient with a recent diagnosis of chronic myelomonocytic leukaemia (CMML). The patient's orbital inflammation and ocular hypertension showed no response to topical and systemic pressure-lowering agents and non-steroidal anti-inflammatory agents but resolved rapidly after the commencement of intravenous steroids. The patient was subsequently treated with the hypomethylating agent azacitidine with good systemic control of CMML with no further orbital inflammation. CMML is strongly associated with systemic inflammatory disease, possibly due to the upregulation of inflammatory pathways in the abnormal monocytes. Background: Neutrophilic cells are among the most abundant immune populations within the head and neck tumor microenvironment (TME) and harbor multiple mechanisms of immunosuppression. Despite these important features, neutrophilic cells may be underrepresented in contemporary studies that aim to comprehensively characterize the immune landscape of the TME due to discrepancies in tissue processing and analysis techniques. Here, we review the role of pathologically activated neutrophilic cells within the TME and pitfalls of various approaches used to study their frequency and function in clinical samples., Methods: The literature was identified by searching PubMed for "immune landscape" and "tumor immune microenvironment" in combination with keywords describing solid tumor malignancies. Key publications that assessed the immune composition of solid tumors derived from human specimens were included. The tumor and blood processing methodologies in each study were reviewed in depth and correlated with the reported abundance of neutrophilic cells., Results: Neutrophilic cells do not survive cryopreservation, and many studies fail to identify and study neutrophilic cell populations due to cryopreservation of clinical samples for practical reasons. Introduction: Establishing a diagnosis is paramount in medical practice as it shapes patients' experiences and guides treatment. Patients grappling with rare diseases face a triple challenge: prolonged diagnostic journeys, limited responses to existing therapies, and the absence of effective monitoring tools. Genetic diagnosis often provides crucial diagnostic and prognostic information, opening up possibilities for genotype-targeted treatments and facilitating counselling and relative testing. The NIHR BioResource - Rare Diseases (NBR) Study and the Cohort Study in Idiopathic and Hereditary Pulmonary Arterial Hypertension (PAH Cohort study) aimed to enhance diagnosis and treatment for PAH, successfully identifying the genetic cause in 25% of idiopathic cases. However, the diagnostic and therapeutic odyssey in patients with PAH remains largely unexplored., Methods: Stakeholders from the NBR and PAH Cohort studies were recruited using purposive sampling. In-depth interviews and focus groups were recorded, transcribed, anonymised, and analysed thematically using MAXQDA software., Results: The study involved 53 interviews and focus groups with 63 participants, revealing key themes across five stages of the diagnostic odyssey: initial health concerns and interactions with general practitioners, experiences of misdiagnosis, relief upon receiving the correct diagnosis, and mixed emotions regarding genetic results and the challenges of living with the disease. Following the diagnosis, participants embarked on a therapeutic journey, facing various challenges, including the disease's impact on professional and social lives, the learning curve associated with understanding the disease, shifts in communication dynamics with healthcare providers, therapeutic hurdles, and insurance-related issues. Building on these insights, we identified areas of unmet needs, such as improved collaboration with primary care providers and local hospitals, the provision of psychological support and counselling, and the necessity for ongoing patient education in the ever-evolving realms of research and therapy., Conclusions: The study highlights the significant challenges encountered throughout the diagnostic and therapeutic journey in PAH. To enhance patient outcomes, it is crucial to raise awareness of the disease, establish clear diagnostic pathways, and seamlessly integrate genetic diagnostics into clinical practice. Streamlining the diagnostic process can be achieved by utilising existing clinical infrastructure to support research and fostering better communication within the NHS. Abstract The decision of the UK to leave the EU has far‐reaching, and often shared, implications for agriculture and fisheries. To ensure the future sustainability of the UK’s agricultural and fisheries systems, we argue that it is essential to grasp the opportunity that Brexit is providing to develop integrated policies that improve the management and protection of the natural environments, upon which these industries rely. This article advances a stakeholder informed vision of the future design of UK agriculture and fisheries policies. We assess how currently emerging UK policy will need to be adapted in order to implement this vision. Our starting point is that Brexit provides the opportunity to redesign current unsustainable practices and can, in principle, deliver a sustainable future for agriculture and fisheries. Underpinning policies with an ecosystem approach, explicit inclusion of public goods provision and social welfare equity were found to be key provisions for environmental, agricultural and fishery sustainability. Recognition of the needs of, and innovative practices in, the devolved UK nations is also required as the new policy and regulatory landscape is established. Achieving the proposed vision will necessitate drawing on best practice and creating more coherent and integrated food, environment and rural and coastal economic policies. Our findings demonstrate that “bottom‐up” and co‐production approaches will be key to the development of more environmentally sustainable agriculture and fisheries policies to underpin prosperous livelihoods. However, delivering this vision will involve overcoming significant challenges. The current uncertainty over the nature and timing of the UK’s Brexit agreement hinders forward planning and investment while diverting attention away from further in‐depth consideration of environmental sustainability. In the face of this uncertainty, much of the UK’s new policy on the environment, agriculture and fisheries is therefore ambitious in vision but light on detail. Full commitment to co‐production of policy with devolved nations and stakeholders also appears to be lacking, but will be essential for effective policy development and implementation. This historical article analyses the work of an under-recognised black female journalist, Fay M. Jackson, who wrote as the Associated Negro Press Hollywood correspondent during the 1930s. Drawing from the literature on celebrity studies, the black press and black feminist thought, this study reveals how Hollywood’s first black female correspondent brought visibility and personhood to black celebrities while employing conventional journalistic strategies. The analysis highlights two primary themes that emerged in Jackson’s celebrity news coverage: Jackson’s employment of gossip news and biographical profiles to bring personhood to black female celebrities; and Jackson’s coverage of the ‘browning’ of Hollywood to highlight the importance of racial representation within the entertainment industry. This study addresses how a black feminist approach to analysing race and gender politics in black media and Hollywood extends previous understandings about the history of celebrity studies. A tissue resident-like phenotype in tumor infiltrating T cells can limit systemic anti-tumor immunity. Enhanced systemic anti-tumor immunity is observed in head and neck cancer patients after neoadjuvant PD-L1 immune checkpoint blockade (ICB) and transforming growth factor β (TGF-β) neutralization. Using T cell receptor (TCR) sequencing and functional immunity assays in a syngeneic model of oral cancer, we dissect the relative contribution of these treatments to enhanced systemic immunity. The addition of TGF-β neutralization to ICB resulted in the egress of expanded and exhausted CD8 + tumor infiltrating lymphocytes (TILs) into circulation and greater systemic anti-tumor immunity. This enhanced egress associated with reduced expression of Itgae (CD103) and its upstream regulator Znf683 . Circulating CD8 + T cells expressed higher Cxcr3 after treatment, an observation also made in samples from patients treated with dual TGF-β neutralization and ICB. These findings provide the scientific rationale for the use of PD-L1 ICB and TGF-β neutralization in newly diagnosed patients with carcinomas prior to definitive treatment of locoregional disease., Competing Interests: The authors declare no competing interests.

Background: Increase in presentations of self-harm to primary care, a risk factor of suicide, has led to a growing interest in identifying at-risk populations. Aim: To examine whether osteoporosis or fractures are risk factors for self-harm, suicidal ideation, and suicide. Design and setting: This was a systematic review of observational studies in adults (>18 years) that had examined the role of osteoporosis and/or fractures in subsequent self-harm, suicidal ideation, and/or suicide. Method: Six databases were searched from inception to July 2019. Additional citation tracking of eligible studies was undertaken in November 2022. Screening, data extraction, and quality assessment of full-text articles were performed independently by at least two authors. Where possible, meta-analysis was run on comparable risk estimates. Results: Fifteen studies were included: two examined the outcome of self-harm, three suicidal ideation, and 10 suicide. In approximately half of studies on osteoporosis, the risk of suicidal ideation and suicide remained significant. However, pooling of adjusted odds ratios from three studies indicated no association between osteoporosis and suicide (1.14, 95% confidence interval = 0.88 to 1.49). Nine studies examined the risk of a mixture of fracture types across different outcomes, limiting comparisons. However, all studies examining vertebral fracture (n = 3) reported a significant adjusted negative association for self-harm and suicide. Conclusion: Patients with vertebral fractures, a risk potential factor for suicide, may benefit from clinical case finding for mood disorders with personalised primary care management. There is little knowledge of how policymakers manage governance networks ("metagovern") within climate policy and especially at non-executive levels of public management. One strategy to metagovern is through using intermediary actors such as funding bodies. However, as novel actors within climate governance, such "climate intermediaries" are under-researched. We address these gaps by exploring the metagovernance through an intermediary actor, namely the Norwegian "Klimasats" Fund. We find that the logic of funding bodies lends itself to "carrots" as opposed to "sticks", weakening the potential for transformation. Funding bodies can also increase existing differences in climate action between larger and smaller local authorities. However, funding bodies have a beneficial bi-directional functionality, incentivising local innovation whilst feeding lessons both up to and across government. Funding bodies also have the power to make local actors into intermediaries in their own right and can influence policy discourses. Thus, in assessing metagovernance at the non-executive level and using intermediary actors such as funding bodies, we reveal significant challenges, but also surprising opportunities, for the low-carbon transition. Protein capsules are promising drug delivery vehicles for cancer research therapies. Apoferritin (AFt) is a self-assembling 12 nm diameter hollow nanocage with many desirable features for drug delivery, however, control of drug retention inside the protein cage remains challenging. Here we report the encapsulation of copper(ii)-1,10-phenanthroline (Cu(phen)) within the horse spleen AFt (HSAFt) nanocage, by diffusion of the metal through the pores between the protein subunits. Transmission electron microscopy revealed the formation of organised copper adducts inside HSAFt, without affecting protein integrity. These structures proved stable during storage (>4 months at -20 °C). Exposure to physiologically relevant conditions (37 °C) showed some selectivity in cargo release after 24 h at pH 5.5, relevant to the internalisation of AFt within the endosome (60% release), compared to pH 7.4, relevant to the bloodstream (40% release). Co-encapsulation of temozolomide, a prodrug used to treat glioblastoma multiforme, and Cu(phen) enabled entrapment of an average of 339 TMZ molecules per cage. In vitro results from MTT and clonogenic assays identified cytotoxic activity of the Cu(phen), HSAFt-Cu(phen) and HSAFt-Cu(phen)-TMZ adducts against colorectal cancer cells (HCT-116) and glioblastoma cells (U373V, U373M). Genetic research and testing are increasingly important for understanding and treating pulmonary arterial hypertension. We aimed to explore how attitudes toward genetic research among clinical and research teams impacted the engagement in genetic research and the integration of genetic insights into clinical practice. We conducted 53 semistructured interviews and focus groups with patients, clinicians, and researchers from nine UK Pulmonary Hypertension centers, who had genetic research experience. Transcripts were thematically coded using inductive analysis. In this study, we focus on the researchers', clinicians', and study team's perspectives. From the interview data, several key themes emerged, ranging from study design, recruitment, and consent procedures to the return of individual genetic results. Additionally, participants reflected on both the successes of these studies and the future directions of genetic research. The analysis highlighted the critical importance of fostering collaborative networks firmly rooted in existing clinical and research infrastructure in rare disease study setups. Furthermore, the significance of trust-building, personalized communication, and transparency among stakeholders was underscored. The study offered valuable insights into the motivating and hindering factors to participant recruitment and consent procedures. Lastly, the findings gathered from processes surrounding the return of individual genetic results, genetic counselling, and the recruitment of relatives provided invaluable lessons regarding the integration of genetics into clinical practice.
