This paper quantitatively assesses the effect of value chain development projects on market linkages of small-scale farmers of a staple crop and agricultural income. We focus on Nicaraguan bean producers participating in a value chain development project. Using a panel data set, we apply difference-in-differences and instrumental variables estimators to identify causal effects of programme participation. We first quantify the positive direct effect of training on bean sales in terms of quantity and percentage of beans sold in linked markets. Second, we show that intensified commercialisation contributes to total agricultural income, suggesting a positive indirect effect of the intervention. Our results indicate that market linkage of smallholder farmers requires different sets of intervention tools than farm technical assistance. [ABSTRACT FROM AUTHOR]

Radiation protection for interventional radiology (IR) physicians is very important. Current IR X-ray systems tend to use flat-panel detectors (FPDs) rather than image intensifiers (IIs). The purpose of this study is to test the hypothesis that there is no difference in physician-received scatter radiation (PRSR) between FPD systems and II systems. This study examined 20 X-ray systems in 15 cardiac catheterisation laboratories (11 used a FPD and 9 used an II). The PRSR with digital cineangiography and fluoroscopy were compared among the 20 X-ray systems using a phantom and a solid-state-detector electronic pocket dosemeter. The maximum PRSR exceeded the minimum PRSR by ∼12-fold for cineangiography and ∼9-fold for fluoroscopy. For both fluoroscopy and digital cineangiography, the PRSR had a statistically significant positive correlation with the entrance surface dose (fluoroscopy, r = 0.87; cineangiography, r = 0.86). There was no statistically significant difference between the average PRSR of FPDs and IIs during either digital cineangiography or fluoroscopy. There is a wide range of PRSR among the radiography systems evaluated. The PRSR correlated well with the entrance surface dose of the phantom in 20 X-ray units used for IR. At present, interventional radiology (IVR) tends to involve long procedures (long radiation duration), and physicians are near to the source of scattered radiation. Hence, shielding is critical in protecting physicians from radiation. Protective aprons and additional lead-shielding devices, such as tableside lead drapes, are important means of protecting the physician from scattered radiation. The purpose of this study was to evaluate whether non-lead aprons are effective in protecting physicians from radiation during IVR procedures. In this study, the radiation protection effects of commercially available protective lead and non-lead aprons, when exposed to diagnostic X rays, are compared. The performance of these non-lead and lead aprons was similar for scattered X rays at tube voltages of 60-120 kV. Properly designed non-lead aprons are thus more suitable for physicians because they weigh ∼20% less than the lead aprons, and are non-toxic. [ABSTRACT FROM AUTHOR]

This review identifies evidence on supply-side interventions to change the practices of antibiotic prescribers and gatekeepers in low- and middle-income countries (LMICs). A total of 102 studies met the inclusion criteria, of which 70 studies evaluated interventions and 32 provided insight into prescribing contexts. All intervention studies were from human healthcare settings, none were from animal health. Only one context study examined antibiotic use in animal health. The evidence base is uneven, with the strongest evidence on knowledge and stewardship interventions. The review found that multiplex interventions that combine different strategies to influence behaviour tend to have a higher success rate than interventions based on single strategies. Evidence on prescribing contexts highlights interacting influences including health system quality, education, perceptions of patient demand, bureaucratic processes, profit, competition, and cultures of care. Most interventions took place within one health setting. Very few studies targeted interventions across different kinds of providers and settings. Interventions in hospitals were the most commonly evaluated. There is much less evidence on private and informal private providers who play a major role in drug distribution in LMICs. We examine the effects of a subsidy program in Haiti that provided smallholders subsidies for inputs (rice seeds, fertilizer, pesticides, and specific labor tasks) using a randomized control trial. The program led to lower input use and lower yields in the year subsidies were received, and the decline in input use and yields persisted through the following year. Using data from a complementary information intervention in which randomly selected farmers were provided clarification regarding their future receipt of vouchers, we find evidence suggesting that incorrect expectations of future transfers partially explain the disappointing outcomes. In addition, instead of increasing input use, the subsidies seem to have led farmers to pay off their loans and take fewer new ones. In complex post‐emergency environments such as the one in which this program took place, input subsidies may need to be avoided, as they require considerable information to optimally design and careful coordination by many actors to achieve the expected gains. The Cooperative Law, passed in 2007, aims to support and regulate farmer cooperatives in China. The adoption of its regulations is uneven, with most regulations followed except the one-person, one-vote rule. We model investment decisions of Chinese cooperatives and measure the efficiency of capital allocation under heterogeneous membership and alternative governance policies. We show that relative membership heterogeneity in equity and patronage holdings and a cooperative's dividend policy are crucial in determining the optimal level of governance centralization. Centralizing the governance to major equity holders and following other regulations reflect a trade-off faced by cooperatives between obtaining financial support from the government and achieving efficient decision-making.
