Background: Monitoring of renal function after kidney transplantation (KT) is performed by measuring serum creatinine (SCr), urine volumes (UV), and glomerular filtration rate (GFR). Other methods based on oxygen metabolism, such as the renal venous oxygen pressure (P rv O 2 ), may be useful. The aim of this study was to explore the correlation between P rv O 2 and SCr, UV, and GFR 5 days after KT (SCr5, UV5, and GFR5, respectively)., Methods: We conducted a prospective cohort study in adults scheduled for living donor KT. A venous blood sample was taken from the renal vein after declamping the renal artery, and blood gas determinations were made. Correlation analyses between P rv O 2 and SCr5, UV5, and GFR5 were done by calculating Spearman's correlation coefficient with generalized linear models (GLM). A Spearman's correlation analysis was performed between the percentage decrease in SCr (%ΔSCr) and P rv O 2 . A GLM was also performed to determine the association of P rv O 2 with slow graft function (SGF)., Results: The study included 42 patients, of whom 67% were men. The median age was 31 years (IQR, 27-43.5). P rv O 2 was negatively correlated with SCr5 (ρ = -0.53, P = .003), and positively correlated with GFR5 (ρ = 0.49, P = .001) and %ΔSCr (ρ = 0.47, P = .002). A higher P rv O 2 was associated with an increase in GFR in univariable (β = 1.24, 95% CI, 0.56-1.93, P = .001) and multivariable (β = 1.24, 95% CI, 0.53-1.94, P = .001) analyses. Introduction: Degloving soft-tissue injuries are serious and potentially devastating medical conditions where an early recognition is a crucial step for a favorable outcome. One of the most important types is Morel-Lavallée lesions (MLL); a significant soft-tissue injury associated with pelvic trauma (30%) and thigh (20%), located over the greater trochanter., Material and Methods: In this retrospective study we selected adult patients diagnosed with MLL between 2010 and 2019 at our trauma center. We then identified 9 cases and followed them up for a minimum of two years. CT scans were performed to measure the size of the degloved zone., Results: we did not found direct relationship between greater dimensions of MLL injury and the need for an increase of days to return to work. We rather identified an association between bigger dimensions of MLL injury and higher energy trauma. These patients waited an average of 133 days to return to work after being injured; which is a longer period compared to non-op patients., Discussion: MLL lesions generally take several days to develop and many may be missed on initial evaluation. Once identified, compression dressings should be applied, especially when diagnosed acutely. Early identification would lead to early operative debridement. Also, drainage should be performed, since the pathophysiology of the injury will result in the failure of observation or simple aspiration., Conclusion: MLL diagnosis and treatment must be identified as early as possible. We didn't find a correlation between MLL size and the treatment performed. In our study all patients returned to their jobs and normal life. Aim: Morel-Lavallée lesion (MLL) is the separation of the skin from the deep fascia caused by shear stress after trauma and the formation of a fluid-filled cavity between the fascia layers. After the inflammation develops, a fibrous capsule develops around the serosanguineous fluid, preventing its absorption. Cavity infection, skin necrosis, and muscle, tendon, bone, and implant exposure were observed in untreated MLLs. This study presents vacuum-assisted surgical treatment for large and complex MLLs and the effects of treatment on the functionality of lower extremities. Methods: 23 patients who were diagnosed with post-traumatic MLL in the lower extremities and underwent vacuum-assisted surgical treatment were examined. Demographic characteristics, comorbidities, smoking status, body mass index (BMI), type of trauma, anatomical location, skin characteristics of the cavity, additional injuries, duration of treatment delay, medical and surgical procedures, return to work, and social life were investigated. All cavities were evaluated using ultrasonography, and cavity volumes were measured. All patients underwent percutaneous incision drainage and radical debridement of necrotic tissues and cavities. Lower extremity function was evaluated using the lower extremity functional scale (LEFS). Results: A total of 82.6% of patients had chronic MLLs. All patients had high-volume MLLs and functional impairment due to MMLs. The vacuum-assisted wound closure provided compression, drainage, and obliteration of the cavity alone in the treatment of MLL. Additionally, the positive effects of negativepressure wound therapy (NPWT) on wound healing prevented complications such as recurrent MLLs, seroma, dehiscence, and graft loss. This method has also positively affected lower-extremity functionality during subsequent periods (p<0.001). Conclusions: In treating large-volume acute and chronic MLLs, percutaneous drainage, radical debridement, cavity closure with quilting sutures, and pressure support with NPWT are reliable methods that ensure problem-free healing of MLLs.
