This work describes a preliminary study to evaluate the use of Optical Project Tomography in the diagnosis of colorectal polyps. Colonic polypoid cancers are the earliest detectable form of colorectal cancer and can potentially be cured if completely excised. The diagnosis using conventional 2D histopathology can be difficult due to epithelial displacement and 3D imaging could improve this. Optical Projection Tomography is an in vitro 2D and 3D imaging technique for small biological specimens, which produces 2D virtual sections in three orthogonal planes as well as 3D rendered images. Important diagnostic features of colorectal polyps such as dysplasia and villous morphology have been studied on OPT. Differentiating between high and low grade dysplasia was challenging due the resolution of the images but a better correlation with H&E sections was found when diagnosing cancer polyps. The 3D properties of OPT enable the surface morphology to be seen in far greater detail than we have seen before and this, understandably, shows a poor association with the 2D classification method of tubular, tubulovillous and villous adenomas which we used in this study. The 3D endoscopic Pit Pattern Classification was demonstrated well on OPT and highlighted that pit patterns frequently co-exist although the clinical relevance of this needs to be explored further. Novel findings include the detection of specific vasculature patterns in benign and cancer polyps which may help us understand why some polyps bleed and some do not leading to the growth of interval colorectal cancers. A multispectral analysis of images has further explored the potential of OPT showing enhanced images of polyps when different ultraviolet light filters are used. Inter-observer variation exists when diagnosing colorectal polyps on H&E and we have demonstrated that this is even more marked when using OPT. This is the first comprehensive study of OPT using human tissue and has brought with it many challenges for this reason. OPT has several limitations that prevent it from being marketed commercially as an independent diagnostic tool although it has potential as an adjunct to current histopathological techniques and it requires further validation.

- Abstract
Since there is no current international consensus on the optimal approach for pain management in acute pancreatitis (AP), analgesic practices may vary across different healthcare settings. This study explored global disparities in analgesic use, in particular opioids, during admission and at discharge in hospitalised AP patients. This was a post hoc analysis of the prospective PAINAP database, which included all admissions for AP between April and June 2022 with a 1‐month follow‐up. Demographic details, analgesic use, and clinical outcomes were recorded during admission and at discharge. Odds ratios (ORs) for opioid use during admission and at discharge were identified using multivariable regression analyses. Amongst the 1864 patients (52% males, median age 56 (interquartile range, 41–71)) across three different continents, simple analgesics were predominantly used as the primary analgesic (70%). Opioid use during admission was lowest in European centres (67%). Admission in Asian (OR, 2.53 (95% confidence interval (CI), 1.59–4.04), p< 0.001), and Australian (OR, 5.81 (95% CI, 3.19–10.56), p< 0.001) centres was associated with opioid administration during admission compared with European centres. Increased pain severity, longer pre‐admission pain duration, organ failure, and longer length of admission increased opioid use during admission. At discharge, Asian (OR, 2.01 (95% CI, 1.40–2.88), p< 0.001) and Australian (OR, 1.91 (95% CI, 1.28–2.85), p= 0.002) centres were associated with opioid prescription compared with European centres. Increased pain severity, longer pre‐admission pain duration, acute necrotic collections, and walled‐off necrosis also increased the likelihood of opioid prescription at discharge. There are substantial intercontinental differences in opioid use for AP pain. Accordingly, there is a need for international guidelines on pain management in AP.

Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p p Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.

This clinical consensus statement reviews the use of inotropic support in patients with advanced heart failure. The current guidelines only support use of inotropes in the setting of acute decompensated heart failure with evidence of organ malperfusion or shock. However, inotropic support may be reasonable in other patients with advanced heart failure without acute severe decompensation. The clinical evidence supporting use of inotropes in these situations is reviewed. Particularly, patients with persistent congestion, systemic hypoperfusion, or advanced heart failure with need for palliation, and specific situations relevant to implantation of left ventricular assist devices or heart transplantation are discussed. Traditional and novel drugs with inotropic effects are discussed and use of guideline‐directed therapy during inotropic support is reviewed. Finally, home inotropic therapy is described, and palliative care and end‐of‐life aspects are reviewed in relation to management of ongoing inotropic support (including guidance for maintenance and weaning of chronic inotropic therapy support). The iron deficient anaemia is a common medical condition in patients with heart failure receiving antithrombotic therapy. Especially during the COVID19 pandemic period the rate of bleeding complications associated with the antithrombotic therapy tend to be higher, as the patient’s referral to medical services is lower and the interaction doctor-patient is limited. In our retrospective observational study we included 300 consecutive patients with decompensated heat failure associating iron deficient anaemia. For defining the medical conditions we used the ESC guidelines terminology and diagnostic criteria. We assessed the association between the iron deficient anaemia and different antithrombotic therapies, recommended in concordance to ESC Guidelines. We found that aspirin 75mg/day was statistical significant associated with iron deficient anaemia (p 0.012) and anaemia severity (p 0.002), this association being assessed by Chi square and Pearson tests. Also, neither clopidogrel, ticagrelor, VKA or non-VKA were associated to the presence of anaemia. By assessing the mortality rate associated to anaemia severity, the severe anaemia was associated to higher mortality rate, meanwhile no antithrombotic therapy was associated with higher readmission or mortality rate (p<0.001). In conclusion, aspirin was the only antithrombotic therapy associated with the presence of anaemia and anaemia severity, while only severe anaemia was associated with statistic significant increase of patient’s mortality, with nonstatistical result regarding the readmission rate. This finding is concordant to the necessity of a permanent evaluation of the antithrombotic therapy in heart failure patients. [ABSTRACT FROM AUTHOR]

Exercise training (ET) and secondary prevention measures in cardiovascular disease aim to stimulate early physical activity and to facilitate recovery and improve health behaviours. ET has also been proposed for heart failure patients with a ventricular assist device (VAD), to help recovery in the patient's functional capacity. However, the existing evidence in support of ET in these patients remains limited. After a review of current knowledge on the causes of the persistence of limitation in exercise capacity in VAD recipients, and concerning the benefit of ET in VAD patients, the Heart Failure Association of the European Society of Cardiology has developed the present document to provide practical advice on implementing ET. This includes appropriate screening to avoid complications and then starting with early mobilisation, ET prescription is individualised to meet the patient's needs. Traditionally, the main indication for cardiopulmonary exercise testing (CPET) in heart failure (HF) was for the selection of candidates to heart transplantation: CPET was mainly performed in middle-aged male patients with HF and reduced left ventricular ejection fraction. Today, CPET is used in broader patients' populations, including women, elderly, patients with co-morbidities, those with preserved ejection fraction, or left ventricular assistance device recipients, i.e. individuals with different responses to incremental exercise and markedly different prognosis. Moreover, the diagnostic and prognostic utility of symptom-limited CPET parameters derived from submaximal tests is more and more considered, since many patients are unable to achieve maximal aerobic power. Repeated tests are also being used for risk stratification and evaluation of intervention, so that these data are now available. Finally, patients, physicians and healthcare decision makers are increasingly considering how treatments might impact morbidity and quality of life rather than focusing more exclusively on hard endpoints (such as mortality) as was often the case in the past. Innovative prognostic flowcharts, with CPET at their core, that help optimize risk stratification and the selection of management options in HF patients, have been developed. [ABSTRACT FROM AUTHOR]

This volume covers the cultural history of race in'the long 19th century'– the age of empire and nation-state, a transformative period during which a modern world had been forged and complex and hierarchical imperial formations were challenged by the emerging national norm. The concept of race emerged as a dominant epistemology in the context of the conflicting entanglement of empire and nation as two alternative but quite compatible forms of social imaginary. It penetrated all spheres of life under the novel conditions of the emerging mass culture and mass society and with the sanction of anthropocentric and positivistic science. Allegedly primeval and parasocial,'race'was seen as a uniquely stable constant in a society in flux amid transforming institutions, economies, and political regimes. But contrary to this perception, there was nothing stable or natural about'race.'The spread of racializing social and political imagination only reinforced the need for constant renegotiation and readjustment of racial boundaries. Therefore, avoiding any structuralist simplifications, this volume looks at specific imperial, nationalizing, and hybrid contexts framing the semantics and politics of race in the course of the long 19th century. In different parts of the globalizing world, various actors were applying their own notions of'race'to others and to themselves, embracing it simultaneously as a language of othering and personal subjectivity. Consequently, the cultural history of race as told in this volume unfolds on many levels, in multiple loci, and in different genres, thus reflecting the qualities of race as an omnipresent and all-embracing discourse of the time
