The present paper concerns the linear fate of transverse perturbations in a gravity-driven, thin-film flow over a soluble substrate. We propose a reduced-order model, based on a boundary-layer treatment of the solute transport and a depth-integration of the Stokes equations, using two extended lubrication methodologies found in the literature. We obtain a closed-form dispersion relation, which we compare to a previous, fully resolved analytical investigation (Bertagni and Camporeale, J. Fluid Mech., vol. 913, 2021, A34). The aim of this study was to evaluate the mental health consumption among patients with early-stage breast cancer in two radiation oncology departments in two countries (USA and Italy). Data were extracted from the medical records of consecutive patients treated between 2014 and 2015 in two centers. Extracted data included patient's demographics, treatment, referral to psychological supportive care programs, and prescribed psychotropic drugs. Data from the two centers were compared using Student's t, Wilcoxon, Fisher's exact, and Jonckheere-Terpstra tests. Adjusted relative risks (RR) were estimated using Poisson regression. A total of 231 (Italy = 110, USA = 121) patients were included, with a mean age of 60 years. The crude rate of psychological supportive care visits was similar in the US versus the Italian cohort (28.9 vs. 21.8%, p = 0.23). The crude rate of prescribed psychotropic drug was higher in the US cohort versus Italian cohort (43.8 vs. 18.2%, p < 0.0001). These differences remained significant after adjusting for breast cancer subtype, stage, and treatment (RR 1.8, 95 CI 1.17-2.76). Between 20 and 30% of patients receive psychological supportive care during treatment for breast cancer. The use of psychotropic medication was higher in the US cohort than the cohort from Italy. The reasons for these differences might be related to social and cultural differences and the method of prescribing medication.

Background: Head and neck cancer (HNC) treatments cause severe toxicities, leading to high rates of emergency department (ED) visits and unplanned hospital admissions (UHAs). 
., Objectives: The study aimed to evaluate a quality improvement project to reduce ED visits and UHAs.
., Methods: A weekly nurse/nurse practitioner-led symptom management clinic was created for patients with HNC receiving radiation therapy deemed at high risk for an ED visit or UHA. Primary quality metrics were rate of visits or unplanned admissions. Time-to-event actuarial analysis and log-rank tests (two-tailed) were performed to compare observed to historic rates. 
., Findings: Rates of ED visits and UHAs were 11% and 16%, respectively. Having more symptom management visits marginally correlated with reduction in UHA. The authors estimate six ED visits and four UHAs were prevented, which is a $176,848 cost reduction.

Purpose: Unplanned hospital admissions in cancer patients undergoing treatment is an understudied area with important implications for both health care costs and patient outcomes. The goal of this retrospective study was to evaluate the rate, reasons for, and predictors of unplanned hospital admissions during or soon after palliative or curative radiation therapy for cancer, with or without chemotherapy., Methods and Materials: A total of 1116 consecutive patients who received external beam radiation therapy for a malignancy at the University of North Carolina at Chapel Hill from January 1 through December 31, 2010, were studied. The primary outcome was unplanned hospitalization within 90 days of starting radiation therapy (ie, during or soon after). Multivariable logistic regression was used to examine patient and treatment factors associated with admissions., Results: Twenty percent of patients experienced an unplanned admission, which was especially likely in patients with lung (25% of such patients admitted), head and neck (22%), and gastrointestinal (21%) cancers, as well as those treated with palliative intent (31%). The most common causes for admission were gastrointestinal symptoms, neurologic symptoms, respiratory symptoms, pain, and fever or infection. Forty-seven percent of admitted patients were seen in the clinic within 2 weeks of unplanned hospital admission, and 61% of those patients had a related complaint in the clinic. Multivariate analysis showed that married patients (odds ratio [OR] = 0.58; P < .001), curative intent (OR = 0.38; P < .001), and no concurrent chemotherapy (OR = 0.55; P < .001) were associated with decreased odds for admission., Conclusions: Unplanned admissions are relatively common during or soon after radiation therapy in our patient series. Lung cancer is the most common type of cancer, excluding nonmelanoma skin cancer, and is the leading cause of cancer death in the United States. Notable carcinogens involved in the development of lung cancer include smoking, secondhand smoke, and radon. Lung cancer is divided into 2 major types: non-small-cell lung cancer, the most prevalent, and small-cell lung cancer. Treatment includes surgery, chemotherapy, radiation, or a combination of the same. Medical nutrition therapy is often required for nutrition-related side effects of cancer treatment, which include but are not limited to anorexia, nausea and vomiting, and esophagitis. The best protection against lung cancer is avoidance of airborne carcinogens and increased consumption of fruits and vegetables. Studies have shown that smokers taking large amounts of beta-carotene and vitamin A supplements had increased lung cancer incidence and mortality. However, ingestion of beta-carotene from foods, along with a diet rich in fruits and vegetables, has a protective role against lung disease. The use of complementary and alternative medicine by lung cancer patients is prevalent; therefore, clinicians should investigate whether complementary and alternative therapies are used by patients and advise them on the use of these therapies to avoid any potential side effects and interactions with conventional therapies. The article concludes with a case study of a patient with non-small-cell lung cancer and illustrates the use of medical nutrition therapy in relation to cancer treatment side effects.

Purpose: Orbital manifestations of non-Hodgkin's lymphoma (NHL) are rare and accounts for only 1% of all cases of NHL. There have been no reports of treating orbital lymphoma using intensity-modulated radiotherapy (IMRT)., Materials and Methods: Four patients were treated at our institution for orbital lymphoma using IMRT. Radiotherapy (RT) plans using wedged pair fields were developed for comparison. Clinical results using IMRT are presented and a dosimetric analysis between IMRT and RT was performed., Results: All patients had a complete response based on their physical examinations and post-IMRT imaging. Symptoms that had been present at initial presentation resolved in all patients during the course of the treatment. All four patients experienced only grade 1 dry eye syndrome and keratitis. The average dose to the contralateral orbit, lacrimal gland, and lens were all significantly reduced (P < 0.01) in IMRT patients as compared to the RT patients. IMRT reduced the V5 and V10 for the contralateral lens, orbit, and lacrimal gland and the optic chiasm (P < 0.05)., Conclusion: IMRT is feasible when treating orbital lymphoma and reduces dose to critical structures while providing excellent dose coverage of target volumes. IMRT offers patients with orbital lymphoma excellent clinical outcomes, similar to conventional RT, with no increased toxicity.

Background/Objectives: Nasopharyngeal carcinoma (NPC) is commonly treated with radiotherapy (RT) or concurrent chemoradiotherapy (CCRT). However, unplanned emergency department (ED) visits during treatment can disrupt therapy and impact patient outcomes. This study aims to identify the risk factors associated with unplanned ED visits in patients with NPC receiving RT or CCRT. Methods: We retrospectively analyzed 2111 patients with NPC treated between 2001 and 2019 at Chang Gung Memorial Hospital. Patients were categorized based on whether they experienced an unplanned ED visit during or up to three months post-treatment. Demographic and clinical variables were compared using the Chi-squared test, and survival outcomes were assessed using Kaplan-Meier analysis. Results: Among the cohort, 573 patients (27.2%) experienced at least 1 unplanned ED visit. Risk factors for unplanned ED visits included older age (p < 0.001), hypertension (p < 0.001), higher Charlson Comorbidity Index (p = 0.001), and advanced clinical stage (T stage, p = 0.0046; N stage, p = 0.0034; M stage, p = 0.0008). No significant difference in ED visit rates was observed between RT alone and CCRT groups. Conclusions: Unplanned ED visits were common during NPC treatment, with risk factors primarily related to patient age, comorbidities, and disease stage. Identifying high-risk patients may enable interventions to reduce ED visits, improve survival outcomes, and alleviate healthcare costs. [ABSTRACT FROM AUTHOR]

Today's home care nurse faces many challenges in providing patient-centered care while working within the guidelines set forth by the payor. A patient with Type 1 diabetes who was blind and lived alone was referred to our agency. A multidisciplinary approach for diabetes education along with tools for the visually impaired led our patient to independent diabetes management while keeping within the managed care guidelines.

Dusty plasmas are electrically quasi-neutral media that, along with electrons, ions, neutral gas, radiation, and electric and/or magnetic fields, also contain solid or liquid particles with sizes ranging from a few nanometers to a few micrometers. These media can be found in many natural environments as well as in various laboratory setups and industrial applications. As a separate branch of plasma physics, the field of dusty plasma physics was born in the beginning of 1990s at the intersection of the interests of the communities investigating astrophysical and technological plasmas. An additional boost to the development of the field was given by the discovery of plasma crystals leading to a series of microgravity experiments of which the purpose was to investigate generic phenomena in condensed matter physics using strongly coupled complex (dusty) plasmas as model systems. Finally, the field has gained an increasing amount of attention due to its inevitable connection to the development of novel applications ranging from the synthesis of functional nanoparticles to nuclear fusion and from particle sensing and diagnostics to nano-contamination control. The purpose of the present perspectives paper is to identify promising new developments and research directions for the field. As such, dusty plasmas are considered in their entire variety: from classical low-pressure noble-gas dusty discharges to atmospheric pressure plasmas with aerosols and from rarefied astrophysical plasmas to dense plasmas in nuclear fusion devices. Aim: The aim of the study was to investigate the incidence of sleep disturbance and its relationship with anxiety and depression symptoms, social support and hope in breast cancer patients in China during chemotherapy. Design: A single‐centre cross‐sectional study. Methods: A total of 329 breast cancer patients were selected via convenience sampling method before they began chemotherapy (n = 115), before the 5th week of chemotherapy (n = 117) or 1 month after chemotherapy ended (n = 97) and administered paper‐and‐pencil questionnaires to evaluate sleep quality, depression and anxiety symptoms, social support and hope. Risk factors significantly associated with sleep disturbance during bivariate were incorporated in the multivariate analysis. Bivariate analyses showed that age, menopausal status, depression and anxiety symptoms, emotional/informational support, tangible support, affectionate support, positive social interaction and total support were predictors of sleep disturbance. Results: Sleep disturbance was prevalent in breast cancer patients before (27.0%), during (32.5%) and after (39.2%) chemotherapy, with 37.4%, 41.9% and 52.6% of participants, respectively, reporting sleeping below the recommended 7 h. Only 8.6%–15.5% of patients reported taking sedative‐hypnotic drugs during the chemotherapy. Multivariate analyses found that participants reporting clinically significant anxiety (HADS > 8) were 3.5 times more likely to report sleep disturbance (PSQI > 8) than participants without clinically significant anxiety, and each increment in emotional/informational support was associated with a 9.04% reduced risk of sleep disturbance. Purpose: Malnutrition is highly prevalent in head and neck cancer (HNC) patients, with weight loss being one of the major nutritional indicators. The objective of this study was to investigate the impact of weight loss on treatment interruptions and unplanned hospital admissions in HNC patients undergoing radiotherapy (RT) with or without chemotherapy. Methods: In this retrospective cohort study, consecutive HNC patients who started RT between January 2011 and December 2019 were included. Data from a total of 1086 subjects with 747 (68.8%) nasopharyngeal carcinomas (NPCs) and 31.2% (N=339) non-NPC patients were analysed. Body weight (BW) was measured before, during, and after RT treatment. Factors associated with ≥10% weight loss, treatment interruption, and unplanned admissions were analysed using multivariate logistic regression. Results: The prevalence of ≥10% weight loss was 26.8% (N=288), with 32.7% (N=243) in NPC and 13.5% (N=45) in non-NPC patients. The prevalence of RT delay in patients with ≥10% vs. <10% weight loss was 6.2% vs. 7.0% (p=0.668) in NPC patients and 42.2% vs. 50.5% (p=0.300) in non-NPC patients. The prevalence of unplanned admissions in patients with ≥10% vs. <10% weight loss was 51.9% vs. 25.3% (p<0.001) in NPC patients and 68.9% vs. 27.0% (p<0.001) in non-NPC patients. Conclusion: In our study, ≥10% weight loss was found to be associated with a higher rate of unplanned admissions but not with RT delay or chemotherapy interruption. Clinical implications: With the knowledge of the impact of weight loss on hospital admissions and the characteristics of patients with weight loss, nutritional intervention can be effectively focused on the stratification of patients for intensive nutritional support to reduce weight loss. Aims The aim of this systematic review was to critique research examining the use of nurse-led clinics for head and neck cancers. Method This review was conducted using the Pickering systematic literature review framework. Searching during November 2021, research published between 2011--2021 from the databases MEDLINE, Scopus, PsychINFO, Cochrane and CINAHL was appraised using the Mixed Methods Appraisal Tool (MMAT), descriptive quantitative analysis and thematic analysis. Results A total of 134 articles were recovered, with 13 studies included for analysis. Of these, seven were cohort studies, four were observations-based and five were randomised control trials. Furthermore, 12 of these studies highlighted the role of an outpatient clinicbased nurse practice, and four were based on the generalised role of an advanced practice nurse. Of all 13 studies, only one was based in Australia, with the majority based in either Europe (7) or the USA (8). Conclusion Nurse-led clinics for head and neck cancer follow ups show promising results to improve patients' quality of life. All 13 studies showed an increase in patient-reported quality of life. Further research may be required to study the financial and resource availability for the utilisation of nurse-led clinics in the future. Impact This study addressed a gap in knowledge of the success and distribution of nurse-led clinics in the field of head and neck cancers. Overall, there is a strong outcome of feasibility and quality patient outcomes for nurse-led clinics. However, there are low sample sizes in the articles found. This may have future impacts on policy and resource funding for further development of nurse-led services; however, further research will be required. [ABSTRACT FROM AUTHOR]

Aim: Non-surgical treatment for head and neck cancer (HNC) often results in severe toxicities, which are detrimental to a patient’s health and quality of life. There is limited published UK data on unplanned hospital admissions and reasons associated with admission. We aim to identify frequencies and reasons for unplanned hospital admissions, highlighting those patient groups who are most vulnerable. Methods: A retrospective study of unplanned hospital admissions of HNC patients receiving non-surgical treatment was completed. An inpatient admission was defined as ≥ 1 night spent in the hospital. To test potential demographic and treatment predictors of inpatient admission, a multiple regression model was constructed using the endpoint measure (unplanned admission), as the dependent variable. Results: A cohort of 216 patients was identified over a 7-month period, and 38 of these patients (17%) required an unplanned admission. Treatment type was the only statistically significant predictor of in-patient admission. The majority of admissions were patients receiving chemoradiotherapy (CRT) (58%) with predominant reasons for admission being nausea and vomiting (25.5%) and decreased oral intake/dehydration (30%). Of the patients admitted, 12 had a prophylactic PEG placed pre-treatment, and 18 of 26 admitted without prophylactic PEG required nasogastric tube feeding during their admission. Discussion: Almost one-fifth of HNC patients over this time period required hospital admission; the majority of which can be attributed to treatment toxicities when receiving CRT. This is concurrent with other studies which review the impact of radiotherapy versus CRT. Increased support and monitoring, particularly focused on nutrition, are required for patients with HNC who receive CRT. Key message: This article describes a retrospective review of a patient undergoing non-surgical treatment for head and neck cancer. These patients frequently require unplanned hospital admission. The results indicate that patients undergoing (chemo)radiotherapy are most vulnerable to deterioration and additional support focused on nutrition for these patients is indicated. Purpose: This study examined the pattern of psychosocial care in breast cancer survivors. Methods: In a prospective study with measurements before surgery, 1 month, 8 months, and 5 years thereafter, we examined the proportion of breast cancer survivors who were aware about, had been offered and received various types of psychosocial services from psychologists, social workers, doctors, self-help groups etc. The degree of helpfulness per service among users was ascertained with Likert scales. Determinants of awareness, offer and use were investigated using binary logistic regression analyses. How the services are inter-related was tested with principal component analyses. Results: Among 456 breast cancer survivors who participated until 5 years, psychological services were known by 91%, offered to 68%, and used by 55% of patients. Social services were known by 86%, offered to 65%, and used by 51%. Women ≥ 65 years were less likely to be informed about (odds ratio (OR) 0.2) and get offers for psychosocial services (OR 0.4 for social and 0.5 for psychological services) than women < 65 years. The services rated most helpful were social services in the hospital, psychological counselling by a consultant and psychotherapy in private practices. Conclusion: These findings underline the importance of psychosocial support by physicians in addition to the "professional" mental health and social care providers. Purpose: The aim of this study was to identify at what time patients with cancer and their relatives want to be informed about and receive psychosocial counselling. Methods: In a multi‐centre prospective study, both users and non‐users of psychosocial cancer counselling were asked to participate in a qualitative interview about their preferences for psychosocial counselling. A second interview was scheduled 3–6 months thereafter. Interviews were transcribed verbatim and coded according to content analysis. Results: Altogether, 61 patients and 42 relatives participated, resulting in 183 interviews (103 at t1, 80 at t2). Most often, the optimal time‐point for cancer counselling was defined by the participants according to the treatment trajectory, and the most frequently mentioned preferred time‐point was 'as early as possible': at the time of cancer diagnosis (mentioned in 22% of interviews with men and 10% of interviews with women). In particular, men also said that they wanted psychosocial counselling while waiting for test results (5%), at the point of returning to work and/or when under the threat of unemployment (3%). Women especially preferred this support during therapy (4%) and 'when one has the capacity for it' (4%), in terms of time and emotional capacity. Conclusions: Healthcare providers should inform their cancer patients and the relatives about the possibilities of professional psychosocial support early on in the disease trajectory (i.e. at the time of diagnosis) and repeatedly thereafter, especially in times of uncertainty, while ensuring the patients' capacity to process information at each time‐point is taken into consideration. The term "cancer" refers to the state in which cells in the body develop mutations and lose control over their replication. Malignant cancerous cells invade in various other tissue sites of the body. Chemotherapy, radiation, and surgery are the first-line modalities for the majority of solid cancers. These treatments work by mitigating the DNA damage of cancerous cells, but they can also cause harm to healthy cells. These side effects might be immediate or delayed, and they can cause a high rate of morbidity and mortality. Dietary interventions have a profound impact on whole-body metabolism, including immunometabolism and oncometabolism which have been shown to reduce cancer growth, progression, and metastasis in many different solid tumor models with promising outcomes in early phase clinical studies. Dietary interventions can improve oncologic or quality-of-life outcomes for patients that are undergoing chemotherapy or radiotherapy. In this chapter, we will focus on the impact of nutritional deficiencies, several dietary interventions and their proposed mechanisms which are used as a novel therapy in controlling and managing cancers., (© 2024. Background: There is a growing need for palliative care for patients near the end of life and their caregivers. Palliative and end-of-life care (EoLC) education are recommended for all health care (e.g., physicians, nurses, and allied health practitioners) and social care professionals (e.g., social workers) to ensure the quality of services. However, less attention has been afforded to generic, in contrast to specialized, EoLC education. This study evaluated the effectiveness of a series of short-term generic EoLC educational programs for health and social care professionals. Method: A pre-post survey design was adopted, focusing on different EoLC core competences. Results: Significant improvement was observed in all perceived competences after the educational programs, regardless of participants' occupation or EoLC experience. Perceived competence in self-care was rated significantly higher than all other competences prior to the programs. Healthcare professionals rated significantly higher on competence in symptom management than social workers. Scores on communication skill and self-care competences were significantly higher following longer (i.e., 16-24 hours) than shorter (i.e., 4-8 hours) programs. Conclusion: Generalist palliative/EoLC educational programs may enable health and social care professionals to refresh and extend their knowledge and skills and enhance their perceived competence in providing EoLC. Further research on generalist palliative/EoLC education is needed to examine the impact of continuing training on professionals' actual practice in EoLC and palliative care. [ABSTRACT FROM AUTHOR]

Introduction: Lung cancer (LC) is highly prevalent worldwide, with elevated mortality. In this population, taste and smell alterations (TSAs) are frequent but overlooked symptoms. The absence of effective therapeutic strategies and evidence-based guidelines constrain TSAs' early recognition, prevention and treatment (Tx), promoting cancer-related malnutrition and jeopardizing survival outcomes and quality of life. Objectives: To systematically review the literature on TSAs in LC patients, understand the physiopathology, identify potential preventive and Tx strategies and to further encourage research in this area. Methods: Literature search on English language articles indexed to PubMed, CINALH, SCOPUS and Web of Science using MeSH terms "Lung neoplasms","Dysgeusia", "Olfaction Disorders", "Carcinoma, Small Cell","Carcinoma, Non- Small-Cell Lung "Adenocarcinoma of Lung","Carcinoma, Large Cell", and non-MeSH terms "Parageusia", "Altered Taste", "Smell Disorder", "Paraosmia", "Dysosmia","Lung Cancer" and "Oat Cell Carcinoma". Results: Thirty-four articles were reviewed. TSAs may follow the diagnosis of LC or develop during cancer Tx. The estimated prevalence of self-reported dysgeusia is 35-38% in treatment-naïve LC patients, and 35-69% in those undergoing Tx, based on studies involving LC patients only. One prospective pilot trial and 1 RCT demonstrated a clinically significant benefit in combining flavor enhancement, smell and taste training and individualized nutritional counselling; a systematic review, 1 RCT and 1 retrospective study favored using intravenous or oral zinc-based solutions (150mg 2-3 times a day) for the prevention and Tx of chemotherapy (CT) and radiotherapy (RT) -induced mucositis and subsequent dysgeusia. Conclusions: This is the first review on dysgeusia and dysosmia in LC patients to our knowledge. We propose combining taste and smell training, personalized dietary counselling and flavor enhancement with oral zinc-based solutions (150mg, 2-3 times a day) during CT and/or RT in this population, in order to prevent and help ameliorate Tx-induced dysgeusia and mucositis. However due to study heterogeneity, the results should be interpreted with caution. Developing standardized TSA measurement tools and performing prospective randomized controlled trials to evaluate their effect are warranted. [ABSTRACT FROM AUTHOR]

Monitoring the plasmasphere is an important task to achieve in the Space Weather context. A consolidated technique consists of remotely inferring the equatorial plasma mass density in the inner magnetosphere using Field Line Resonance (FLR) frequencies estimates. FLR frequencies can be obtained via cross‐phase analysis of magnetic signals recorded from pairs of latitude separated stations. In the last years, machine learning (ML) has been successfully applied in Space Weather, but this is the first attempt to estimate FLR frequencies with these techniques. We survey several supervised ML algorithms for identifying FLR frequencies by using measurements of the European quasi‐Meridional Magnetometer Array. Our algorithms take as input the 2‐hour cross‐phase spectra of magnetic signals and return the FLR frequency as output; we evaluate the algorithm performance on four different station pairs from L = 2.4 to L = 5.5. Results show that tree‐based algorithms are robust and accurate models to achieve this goal. Their performance slightly decreases with increasing latitude and tend to deteriorate during nighttime. The estimation error does not seem to depend on the geomagnetic activity, although at high latitudes the error increases during highly disturbed geomagnetic conditions such as the main phase of a storm. Our approach may represent a prominent space weather tool included into an automatic monitoring system of the plasmasphere. This work represents only a preliminary step in this direction; the application of this technique on a more extensive data set and on more pairs of stations is straightforward and necessary to create more robust and accurate models. Numerical schemes that preserve the structure of the kinetic equations can provide stable simulation results over a long time. An electromagnetic particle-in-cell solver for the Vlasov-- Maxwell equations that preserves at the discrete level the noncanonical Hamiltonian structure of the Vlasov--Maxwell equations has been presented in [Kraus et al., J. Plasma Phys., 83 (2017)]. While the original formulation has been obtained for Cartesian coordinates, we extend the formulation to curvilinear coordinates in this paper. For the discretization in time, we discuss several (semi-)implicit methods either based on a Hamiltonian splitting or a discrete gradient method combined with an antisymmetric splitting of the Poisson matrix and discuss their conservation properties and computational efficiency. Purpose: The objective of this scoping review was to examine the effectiveness of supportive care interventions designed to reduce ED visits among patients receiving active cancer treatment. Methods: Literature search involving nine electronic databases and grey literature. Inclusion criteria considered studies assessing the impact of any intervention to reduce ED utilization among patients with active cancer. Dichotomous and continuous outcomes were summarized as risk ratio (RR) or mean difference (MD) with 95% confidence intervals (CIs) using a random-effects model, wherever appropriate. Results: A total of 25 studies were included. Interventions identified in these studies comprised the following: routine and symptom-based patient follow-up, oncology outpatient clinics, early symptom detection, comprehensive inpatient management, hospital at home, and patient navigators. Six out of eight studies assessing oncology outpatient clinics reported a decrease in the proportion of patients presenting to the ED. A meta-analysis of three of these studies did not demonstrate reduction in ED utilization (RR 0.78; 95% CI 0.56 to 1.08; I2 = 77%) when comparing oncology outpatient clinics with standard care; however, sensitivity analysis supported a decrease in ED visits (RR 0.86; 95% CI 0.74 to 0.99; I2 = 47%). Three studies assessing patient follow-up interventions showed no difference in ED utilization (RR 0.69; 95% CI 0.38 to 1.25; I2 = 86%). Conclusion: A variety of supportive care interventions designed to mitigate ED presentations by patients receiving active cancer treatment have been developed and evaluated. Limited evidence suggests that an oncology outpatient clinic may be an effective strategy to reduce ED utilization; however, additional high-quality studies are needed. [ABSTRACT FROM AUTHOR]

Introduction: Breathlessness is the most common and refractory symptom in lung cancer patients. Even though various educational programmes have been developed, only a few were intended for implementation in the home setting for its management. Aim: Feasibility of a study for implementing a nurse-led educational programme for breathlessness management of lung cancer patients at home. Method: A randomized feasibility study was undertaken between February 2017 and October 2018. Patients were recruited through referral from oncologists from two oncology centers in Cyprus under certain inclusion and exclusion criteria. Patients were randomized in the intervention or control group via a computer programme, and their named family caregivers (f.c.) were allocated in the same group. Participants were not blinded to group assignment. The intervention consisted of a PowerPoint presentation and implementation of three non-pharmacological interventions. The control group received usual care. Patients were assessed for breathlessness, anxiety, and depression levels, whereas f.c. were assessed for anxiety, depression, and burden levels. F.c. also assessed patients' dyspnea level. The duration of the study process for both the intervention and control group was over a period of 4 weeks. Results: Twenty-four patients and their f.c. (n = 24) were allocated equally in the intervention and control group. Five patients withdrew, and the final sample entered analysis was 19 patients and 19 family caregivers. In the intervention group n = 11 + 11, and in the control group n = 8 + 8. In the intervention group patients' breathlessness and anxiety levels showed improvement and their f.c.s in the anxiety and burden levels. Major consideration was the sample size and the recruitment of the patients by the referring oncologists. Attrition was minor during the study process. No harm was recorded by the participants of the study. Conclusions: The study provided evidence of the feasibility of the implementation of the educational programme. For the future definitive study major consideration should be patients' recruitment method in order to achieve adequate sample size. Moreover, qualitative data should be collected in relation to the intervention and the involvement of f.c. There is not currently a consensus on the process responsible for producing the waiting time distribution of solar flares. This study presents an information theoretical approach to determining whether solar flare data are significantly distinguishable from a nonstationary Poisson process. A study of solar flares stronger than C1 class detected by the Geostationary Operational Environmental Satellite from 1975 to 2017 was performed. A sequence of waiting times (time elapsed between adjacent X-ray flare peaks) was constructed from the data. Surrogate waiting time sequences were produced using a time-varying Poisson firing rate from the Bayesian block procedure. Utilizing Shannon entropy, the mutual information of time-lagged waiting time distributions was computed for both the original data and the surrogates using a method of discretization by binning. When the entire period is considered, we see that when compared to carefully constructed surrogates, there is a significant elevation of mutual information on a timescale of approximately 30 hr, demonstrating that flares are confidently related to subsequent flares, contradicting the null hypothesis that flares are produced by a nonstationary Poisson process. When only 4 yr subsets of the data are considered, we see that at relatively small timescales (on the order of 10–30 hr), solar flare waiting times have a significant impact on subsequent flares. When corrected for the number of points in each considered time window, there is no correlation between the magnitude of significance and position in the solar cycle. [ABSTRACT FROM AUTHOR]

A novel numerical method has been developed that incorporates electrically conducting objects into particle-in-cell simulations of electrostatic plasma. The method allows multiple objects connected by voltage and current sources in an arbitrary circuit topology. Moreover, by means of an unstructured mesh, the objects can have arbitrary shapes. The electric potential of the objects is solved self-consistently by incorporating charge constraints into the finite element discretization of the Poisson equation. This method has been implemented in a new code, Particles-in-Unstructured-Cells (PUNC), suitable for rapid prototyping. The flexibility of this code has proven convenient to survey various methods, and an issue of reduced convergence rate of today’s unstructured plasma–object interaction codes is highlighted. Modeling the transport of "dust" particles in a magnetically confined plasma device is an area of active research and requires a detailed understanding of the forces experienced by dust immersed in a plasma. One of the most significant of these is the "ion drag force." Dust transport codes employ a model of this force that was not specifically designed for fusion plasmas and so does not consider the relevance of strong magnetic fields. However, it is shown here that the effect of magnetic fields on the ion drag force is significant for such plasmas. In this work, the Monte Carlo code DiMPl is employed to perform the first detailed characterization of the dependence of the ion drag force on magnetic fields. A semi-empirical model of this dependence is fitted onto the simulation data, so that these magnetic effects may be straightforwardly captured by dust transport codes. The limiting behavior of the ion drag force in the case of very strong fields is derived analytically and shown to be consistent with the simulation results. The validity of the results is further motivated through a novel theoretical treatment of the ion drag force at intermediate magnetic field strengths. [ABSTRACT FROM AUTHOR]

Determining the equilibrium charge of conducting spheres in plasmas is important for interpreting Langmuir probe measurements, plasma surface interactions, and dust particle behavior. The Monte Carlo code Dust in Magnetised Plasmas (DiMPl) has been developed for the purpose of determining the forces and charging behavior of conducting spheroids under a variety of conditions and benchmarked against previous numerical results. The floating potentials of spheres in isothermal, collisionless, hydrogen plasmas as a function of magnetic field strength and size relative to the Debye length are studied using DiMPl and compared with new results from the N-body tree code (pot) and recent particle in cell measurements. The results of all three simulations are similar, identifying a small range at modest ion magnetization parameters over which the electron current is reduced relative to the ion current. The potential as a function of magnetic field strength is found to be relatively insensitive to dust size for dust smaller than the Debye length. The potential of large dust is found to depend less strongly on flow speed for modest magnetic field strengths and to decrease with increasing flow speed in the presence of strong magnetic fields for smaller dust. A semi-empirical model for the potential of small dust in a collisionless plasma as a function of magnetic field strength is developed, which reproduces the expected currents and potentials in the high and low magnetic field limit. Studies indicate that inhibiting a single signaling pathway or one single product of a gene is insufficient for the prevention and treatment of cancer. This is due to the fact that dysregulation must occur in more than 500 genes in order to produce a cancerous phenotype. Despite this evidence, available drugs used for cancer treatment focus on a single target. Meanwhile, berberine as a nutraceutical is capable of targeting various processes involved in tumor development including proliferation, invasion, angiogenesis, and metastasis. In comparison with synthetic agents, berberine is cheaper, safer, and more available. Berberine has shown anti-inflammatory properties which make it an ideal option in order to prevent inflammation-associated cancers. Colorectal cancer is one of the most common cancers all over the world and its incidence is increasing each day. Therefore, further investigations about berberine could be helpful in the discovery of novel agents for preventing and/or treating colorectal cancer. This review emphasizes the studies investigating the roles of berberine in colorectal cancer such as controlling cell signaling pathways, inducing apoptosis, regulating microRNAs, attenuating oxidative stress, and affecting inflammation. Background: Radiation therapy (RT) is the treatment of choice in patients with low-grade ocular adenexal mucosa-associated lymphoid tissue lymphoma (OAML) and many of them experience post-RT dry eye with varying severity. The purpose of the present study was to investigate ocular effects of RT on meibomian glands and dry eye by directly visualizing structural changes. Secondly, we focused on the comparison of two groups of patients according to tumor location and radiation technique.Methods: Sixty-four eyes with OAML of conjunctiva, orbit, lacrimal gland, or lacrimal sac were grouped into conjunctival lymphoma and "orbital-type" lymphoma (i.e., orbit, lacrimal gland, and lacrimal sac). Subjects were investigated for morphological changes in meibomian glands by meiboscore grading system. Radiation technique was examined and Ocular Surface Disease Index (OSDI) questionnaire, Schirmer's test, tear film break-up time (TBUT), slit lamp examination of corneal surface and lid margin abnormality were conducted before and after RT.Results: The increase in meiboscore was statistically significant over time after RT in both groups (P < 0.001). The extent of increase in meiboscore was significantly greater in the "orbital-type" lymphoma group than in the conjunctival lymphoma group (P < 0.001). The changes in OSDI, TBUT, corneal fluorescein staining score and lid margin abnormality score after RT were significantly different across two groups (P = 0.042, 0.001, 0.035 and 0.001, respectively). Schirmer's value decreased after RT in both groups. Dry eye symptoms were most severe right after RT in both groups, but a gradual resolution was noted in most patients with conjunctival lymphoma, whereas symptoms persisted in "orbital-type" lymphoma patients. The OSDI score and corneal fluorescein staining score were positively correlated with meiboscore in "orbital-type" patients at post-RT 6 months (r = 0.43, P = 0.04; r = 0.39, P = 0.03, respectively).Conclusions: Patients with OAML had different degrees of morphological changes in meibomian glands according to tumor location and radiation technique. "Orbital-type" lymphoma patients are more likely to experience severe injury to meibomian glands, which eventually leads to persistent dry eye. The electric charge transport through the two-grid systems of the Alternative Low Power Ion Engine is studied using a particle-in-cell (PIC) code. In this plasma thruster design, the ionizing electrons from the only cathode and the ions extracted from the ionization chamber flow in opposite directions through the open spaces of its grids. This configuration differs from classical gridded ion engines where only positive ions are accelerated. The electric charge transport is simulated, and the results are in agreement with previous experimental observations. The numerical simulations are performed for a single beamlet domain in a 2D axial symmetry, and only two aligned holes of the grid system are studied. The extracted ions move outwards from the ionization chamber reaching supersonic velocities of 43–51 km/s which are controlled by an acceleration voltage. The ionizing electrons from the only cathode are accelerated along the opposite direction and achieve energies in the range of 400–600 eV inside the ionization chamber. Additionally, the PIC simulations predict the development of an electric field close to the exit section of the thruster not observed previously because of the practical limitations of experiments. The imbalance of electric charge in the region where the electrons are injected from the only cathode creates a positive field that further accelerates the electric charges, preventing low energy ions from the plasma stream from returning to the plasma thruster. Background: This work aims to compare the dosimetric performance of three-dimensional conformal radiotherapy (3D-CRT), a relatively available technique in developing countries, to intensity-modulated radiotherapy (IMRT) in the treatment of different stages of nasopharyngeal carcinoma (NPC). Materials and Methods: According to the diagnostic stages, 40 NPC patients were divided into two equal groups. Three planning techniques such as 3D-CRT, seven-field IMRT (7F-IMRT) and nine-field IMRT (9F-IMRT) were compared. Dose prescriptions of 70 and 66 Gy were delivered in 35 fractions to gross planning target volume (PTV1) and bilateral retropharyngeal carcinoma (PTV2), respectively. Results: Stage I dose data for almost all of the three investigated planning techniques obey the international recommendations. The dose delivered to PTV1 and PTV2 for 3D-CRT and 7F-IMRT are statistically similar, whereas 9F-IMRT is significantly better than 3D-CRT. For organs at risk (OARs), the delivered dose is significantly better for 9F-IMRT compared with the other two techniques, whereas 7F-IMRT is significantly better than 3D-CRT. Conclusions: 3D-CRT is an acceptable alternative treatment technique for stage I NPC patients in developing countries suffering from the lack of advanced radiotherapy treatment techniques. 3D-CRT and 7F-IMRT have comparable performance in PTVs, while 9F-IMRT is superior in PTVs and OARs. Introduction: In recent years, the prognosis for low-grade lymphoma in the ocular region has improved markedly, whereas high-grade lymphoma is still a challenging disease to manage. Numerous strategies are available for treating ocular adnexal lymphoma, and these include chemotherapy, radiotherapy, and immune-modulating agents. Areas covered: This review provides an overview of the available treatment modalities for ocular adnexal lymphoma, including novel treatment approaches and recent advances. Furthermore, management guidelines for ocular adnexal lymphoma are provided based on existing guidelines described for lymphoma outside the ocular region. This review was based on an extensive MEDLINE search using keywords such as ‘Ocular adnexal lymphoma AND treatment’, ‘Ocular adnexal lymphoma AND management’. Expert commentary: Ocular adnexal lymphoma is a rare disease and the treatment modalities of ocular adnexal lymphoma are numerous. Therefore, we suggest that the treatment should be centralized in centers in each country and placed in a small number of experienced hands. This, in combination with large multicenter studies, will provide new information and improve the prognosis in the future. The treatment of low-grade low-stage lymphoma mainly includes radiotherapy alone. High-stage or high-grade lymphoma should be managed with chemotherapy or immunochemotherapy, potentially in combination with radiotherapy. In recent years, many new therapy approaches have occurred, and expectations for therapies such as chimeric antigen receptor-modified T-cells are high. [ABSTRACT FROM AUTHOR]

A new model describing dust charging and heating in unmagnetized plasmas in the presence of large electron emission currents is presented. By accounting for the formation of a potential well due to trapped emitted electrons when the dust is positively charged, this model extends the so-called OML+ approach, thus far limited to thermionic emission, by including electron-induced emission processes, and in particular low-energy quasi-elastic electron reflection. Revised semi-analytical formulas for the current and heat fluxes associated with emitted electrons are successfully validated against particle-in-cell simulations and predict an overall reduction of dust heating by up to a factor of 2. When applied to tungsten dust heating in divertor-like plasmas, the new model predicts that the dust lifetime increases by up to 80%, as compared with standard orbital-motion-limited estimates. There are still many missing elements to complete the physical picture at the basis of the Hall thruster functioning. The origin of the anomalous electron cross-field transport often ascribed to azimuthal electron E × B drift instability remains decoupled from self-consistent ion axial acceleration and radial boundary conditions, at the same time. This study represents the first attempt to correlate the different mechanisms contributing to the electron transport by means of a fully kinetic three-dimensional Particle-in-Cell model. A geometrical scaling scheme has been used to make the simulation possible. This scheme irremediably changes what are some salient characteristics of the discharge, such as the wall interaction and the axial component of the electric field. For this reason, a critical assessment of the effects of reducing dimensions has been addressed. The present paper deals with the physics of discharge channel. Results confirm the occurrence of E × B drift instability along the azimuthal direction. The modulation is almost standing wave: it moves back and forth travelling only a short distance before being axially convected away. In addition, the dielectric floating potential nature of the lateral walls gives to the azimuthal modulation an important radial component creating an oblique pattern in the radial-azimuthal plane. As a consequence, the azimuthal electric field presents a double alternating structure: two phase-opposing waves are present in the first and second half of the radial extension between the two lateral walls. Finally, the effect of secondary electron emission from walls is not sufficient to guarantee the right electron current to neutralize the ion beam, but rather it works as an auxiliary mechanism (together with ion heating and azimuthal rotation) to saturate the electron drift instability leading to smaller amplitude oscillations.
