2. Neurologic Complications in Pediatric Allogeneic Stem Cell Transplantation: Analysis of Risk Factors and Outcome.

- Abstract
Neurologic complications following stem cell transplantation are of utmost importance owing to their high morbimortality. Although many studies have been performed in the adult population, reports in children are scarce. Our objective was to determine the most common neurologic complications in a pediatric population and to analyze possible risk factors for their development. We performed an exploratory retrospective study of neurologic complications in pediatric patients who had allogeneic stem cell transplantation over the last 18 years. We identified 66 neurologic complications in 178 allogeneic stem cell transplantations. The most frequent neurologic complications were those involving the peripheral nervous system and those related to drug toxicity. Survival decreased significantly in the presence of neurologic complications. Multivariate logistic regression analysis showed that independent risk factors for developing neurologic complications were development of chronic extensive graft-vs-host disease requiring treatment, cytomegalovirus reactivation, and central nervous system radiation. Prompt diagnosis and preemptive treatment, if possible, are necessary to avoid long-term sequelae or mortality.

- Abstract
Introduction: Neurological complications are some of the most important complications that can occur in a patient undergoing haematopoietic stem cell transplantation (HSCT), not only because of the high mortality rate, but also because of the sequelae that appear in survivors. The causes of such complications are manifold and very often coexist in the same patient: toxicity of the conditioning regimen, graft-versus-host disease and its treatment, infections and their treatment, platelets and coagulation disorders, liver failure or arterial hypertension with low platelet count., Aims: The aim of the present study is to provide a clinical description and to describe the risk factors for complications involving the central nervous system that may occur during the course of HSCT, in order to assist in the early detection of these disorders that may have a negative influence on the morbidity and mortality of these patients., Development: The following types of neurological complications are described: central nervous system infections, vascular complications, pharmacological toxicity, metabolic complications, immune-mediated disorders and post-HSCT carcinogenesis, and effects of graft-versus-host disease and thrombotic microangiopathy on the nervous system., Conclusions: The patient undergoing HSCT is at particular risk for the development of neurological complications. Early diagnosis and treatment are needed to try to reduce the high morbidity and mortality in these patients.

4. [Evaluation of Chromalbicans Agar for presumptive identification of Candida albicans]

- Abstract
The utility of Chromalbicans Agar (Biolife Italiana, Milano, Italy) was evaluated with 723 clinical isolates and type culture collection strains from different genera including Candida, Cryptococcus, Pichia, Rhodotorula, Saccharomyces, Trichosporon y Zygosaccharomyces. Presumptive identification was confirmed by germ tube test and carbohydrate assimilation on API-ATB ID 32C (bioMerieux, France). Growth on Chromalbicans Agar was very useful for the presumptive identification of C. albicans isolates, and sensitivity and specificity values were significantly high (97%), since a very low number of isolates were found to be false negative or false positive.

This study further evaluated the in vitro activity of anidulafungin (VER002, Versicor Inc.) (LY303366) against 460 clinical yeast isolates. MICs of anidulafungin, fluconazole and itraconazole were determined by following the NCCLS M27-A guidelines. Minimum fungicidal concentrations (MFCs) of anidulafungin were determined for 230 isolates of Candida spp. The activity of anidulafungin in vitro was significantly superior (P < 0.05) to those of itraconazole and fluconazole against Candida albicans, Candida tropicalis, Candida glabrata and Candida krusei, but anidulafungin was less active for Candida famata and Candida parapsilosis. The differences were not significant for the other species evaluated.

The in vitro susceptibilities of 183 clinical yeast isolates to sertaconazole (STZ) were compared to their susceptibilities to clotrimazole (CTZ), econazole (ECZ), ketoconazole (KTZ), miconazole (MNZ), fluconazole (FLZ), itraconazole (ITZ), tioconazole (TCZ), amphotericin B (AMB) and flucytosine (5FC) by using a commercial agar diffusion method. Strains were isolated from vaginal and other superficial clinical samples (18 species of Candida and five strains belonging to other yeast genera). Only one strain (0.5%) was resistant to STZ out of 87.4% of susceptible strains (n=160). The percentage of susceptible strains was higher than those obtained with the other agents evaluated and the percentage of resistant strains was lower than for most of the other antifungals. The pattern of susceptibility of C. albicans to STZ, TCZ, ITZ and CLZ was similar and superior to the pattern of susceptibility of this species to MNZ, ECZ, FLZ, 5FC and KTZ. C. dubliniensis was more susceptible to STZ, MNZ, MNZ, FLZ, ITZ, CLZ than to TCZ, ECZ, 5FC, AMB or KTZ. Ten susceptible strains to STZ were resistant to FLZ and one strain was resistant to ITZ. The overall antifungal activity of STZ in vitro against a wide range of clinically important yeasts from vaginal and cutaneous samples indicates the therapeutic potential of this agent for the treatment of infections caused by these fungi. However, the activity of STZ and the clinical value of in vitro data need to be verified in human clinical trials.

In general, the MIC did not vary with time of incubation, except in a Z. fumigatus strain in which the MIC went from 2 to 16 mg/l. The geometric mean of the MIC and MIC(90) of itraconazole for Aspergillus spp. was 0.44 mg/l and 0.5 mg/l, respectively; and for Fusarium spp. it was 14.1 mg/l and 16 mg/l, respectively. With 0.5 mg/l 75% of the Aspergillus spp. strains were inhibited, and 100% of these strains were inhibited with 2 mg/l. A. niger and A. fumigatus were the most resistant species (MIC(90) 2 mg/l). The MIC of all the Fusarium strains essayed was between 4 and 16 mg/l.

Introduction and aim. Ciclopirox has been treating fungal infections for decades. Recent studies suggest ciclopirox may be repurposed to treat cancer, viral infections, and neurological disorders. Ciclopirox exerts anticancer by inhibiting multiple pathways of cancer cell growth and survival and anti-viral actions by reducing viral replication and altering the host immunological response to viral infection. Recent research suggests that ciclopirox may protect against neurodegenerative illnesses including Alzheimer's and Parkinson's. This narrative review shows ciclopirox's potential to treat cancer, viral infections, and neurological diseases. Material and methods. Current relevant research publications focused on ciclopirox and its repurposing medicinal potential, therefore a well-designed technique was used to find them. „Ciclopirox", „Anti-fungal", „Anti-cancer", „Repurposing", and „Therapeutic potential" were used to search PubMed, Web of Science, EMBASE, and Google Scholar. Analysis of literature. Ciclopirox may reduce oxidative stress and inflammation, which may cause several illnesses. Overall, the repurposing of ciclopirox for the treatment of cancer, viral infections, and neurodegenerative disorders represents a promising avenue of research that warrants further investigation. Conclusion. It was concluded that CPX and olamine derivatives as outstanding antifungal medications, as well as provide information on ongoing research to use them for other illnesses. Clinical needs for novel antifungal agents have increased due to the increase of people with a compromised immune system, the appearance of resistant fungi, and infections by unusual yeasts. The search for new molecular targets for antifungals has generated considerable research, especially usingmodern omicsmethods (genomics, genome-wide collections of mutants, and proteomics) and bioinformatics approaches. Recently, micro- and nanoscale approaches have been introduced in antifungal drug discovery. Microfluidic platforms have been developed, since they have a number of advantages compared to traditional multiwell-plate screening, such as low reagent consumption, the manipulation of a large number of cells simultaneously and independently, and ease of integrating numerous analytical standard operations and large-scale integration. Automated high-throughput antifungal drug screening is achievable bymassive parallel processing. Various microfluidic antimicrobial susceptibility testing (AST) methods have been developed, since they can provide the result in a short time-frame, which is necessary for personalized medicine in the clinic. New nanosensors, based on detecting the nanomotions of cells, have been developed to further decrease the time to test antifungal susceptibility to a few minutes. Fusarium species are known to cause disseminated cutaneous lesions in immunocompromised patients. Some cases of fusariosis are reported in patients infected with the human immunodeficiency virus. There are two reports in such patients with systemic comorbidities like lymphoma, neutropenia and infective port-a-catheter. Another reported patient had systemic fusariosis, without skin involvement. Diagnosis and treatment of cutaneous fusariosis is difficult and resistance to antifungals is a problem. Our patient was at an advanced human immunodeficiency virus infection stage with disseminated cutaneous fusariosis, without any systemic involvement, who responded completely to oral itraconazole. Purpose: To compare the safety and efficacy of intravitreal anidulafungin injection with voriconazole and amphotericin B (Amp B) in an experimentalCandidaendophthalmitis (CE) model. Methods: Intravitreal 1 × 105CFU/0.1 mlCandida albicanswas injected into the right eyes of 24 New Zealand rabbits, which were divided into 4 groups. Voriconazole 50 μg/0.1 ml, Amp B 10 μg/0.1 ml, and Anidulafungin 50 μg/0.1 ml were injected by intravitreal injection 72 h after inoculation. The control group was injected with 0.1 ml 0.9% NaCl. Clinical scoring was performed by assessing the cornea, conjunctiva, iris, and vitreous on days 3 and 7 of therapy. At the end of the study, the right eyes of all rabbits were enucleated and histopathological evaluation was performed. Therapy groups were compared according to the clinical, histopathological, and microbiological analysis scores. Results: Total clinical scores were significantly different between treatment groups and the control group (p< 0.05). On day 7 of the therapy, clinical scores of the anidulafungin group were found to be significantly lower when compared with the other therapy groups, while a significant improvement was observed in the eyes of rabbits in the anidulafungin group (p< 0.05). Also, microbiological scores of the anidulafungin group were lower than those of the control group (p< 0.05). Histopathological scores of the anidulafungin treatment group were significantly better than the voriconazole and control groups. Inflammation was evidently suppressed and marked retinal toxicity was not observed with anidulafungin. Conclusions: This is the first study comparing the efficacy of anidulafungin with other antifungal agents. In this CE model, an intravitreal single dose of anidulafungin was shown to be noninferior to voriconazole and Amp B. As an alternative to Amp B or voriconazole, intravitreal anidulafungin is suggested as an effective antifungal agent for the treatment of CE. [ABSTRACT FROM PUBLISHER]

Context: Ciclopirox olamine (CPO) is indicated in the treatment of vaginal fungal infections. The frequent and large dosing of available vaginal CPO creams gives rise to poor compliance amongst females. In such a situation a delivery system capable of providing sustained release of CPO is warranted and can be realized through incorporation of its liposomal formulation into a mucoadhesive gel base. The liposomal formulation would offer sustained release whereas mucoadhesive gel would prolong the contact with vaginal wall; thus avoiding frequent and large dosing. Objective: The present study aimed at investigating mucoadhesive liposomal CPO gel for vaginal use. Method: The study embarked on evaluating liposomal CPO and its Carbopol 974
® P gel for stability at vaginal pH, release profile, rheological characteristics, mucoadhesive behavior and finally antifungal activity. Results: The results revealed that CPO liposomes were stable at vaginal pH; its Carbopol gel released 58.75 ± 6.4% of CPO at the end of 24 h which suggested sustained release. Rheology via viscometric, oscillatory stress sweep and oscillatory frequency sweep testing of the gel, studied at different temperatures and under different dilutions with vaginal fluid simulant testified pseudoplastic behavior of the gel. It also pointed towards the predominance of elastic behavior of the gel at all the dilutions. The gel exhibited good mucoadhesivity to sheep vaginal tissue. Furthermore, CPO entrapped in liposome too displayed antifungal activity. Conclusion: The study undertaken recommended Carbopol 974® P gel loaded with CPO liposomes as a potential delivery system for treatment of fungal vaginal infections. [ABSTRACT FROM AUTHOR]- Published

Zudem konnten durch die Untersuchung der Schreibprozesse neue Erkenntnisse über Beethovens Arbeitsorganisation und seine Schreibstrategien gewonnen werden. Dies wird durch neue Forschungen zur Provenienz des Skizzenbuchs ergänzt: Wie zuvor nicht bekannt, beschäftigte sich bereits Johannes Brahms mit diesem Skizzenbuch.

Ciò premesso, il disegno sarà un tracciato preciso e uniforme, concepito nella mente, eseguito per mezzo di linee ed angoli, e condotto a compimento da persona dotata d'ingegno e di cultura”. Leon Battista Alberti, De re aedificatoria | La tradizione accademica fiorentina nell'ambito del DISEGNO, quale mezzo per l'analisi, l'espressione e la figurazione dell'architettura e del suo ambiente, vanta per privilegio storico molti illustri rappresentanti. Ne sono testimonianza due antiche istituzioni: l'Accademia delle Arti del Disegno ed il Gabinetto Disegni e Stampe degli Uffizi, dove sono raccolti esempi magistrali di disegni con caratteristiche e intenzioni mirate nella rappresentazione. Dunque le geometria degli architetti rinascimentali e gli studi della prospettiva del Brunelleschi e di Piero della Francesca hanno indicato una strada sapiente e che nel solco della tradizione si è evoluta in particolare nel concetto della rappresentazione, operazione mai disgiunta da una scientificità controllata e controllabile. Il Convegno UID Le ragioni del disegno parte da questa realtà per un confronto con la complessità degli strumenti attuali e la elaborazione degli esiti ottenuti impiegando le nuove tecnologie nelle elaborazioni grafiche ormai “oltre” le convenzioni bidimensionali e tridimensionali. Questi prodotti sono spesso sorprendenti, ma talvolta risultano impoveriti nel lessico espressivo del disegno e nella costruzione corretta del necessario Modello di riferimento che permetta di indagare, di analizzare e conoscere la realtà materiale e le intenzioni immateriali contenute nella forma visibile. È nozione riconosciuta dagli esperti che approfondendo la costruzione di questi elaborati innovativi si ritrovino regole geometriche e analitiche sempre derivate da quella geometria ordinatrice di controllo e di ricerca che era ed è la base culturale del disegno. | Il problema che si apre è la pluralità dei linguaggi e dei metodi scientifici di elaborazione che ogni campo, nel caso, tecnologico possiede e che occorre coordinare e saper bene interpretare verso le finalità prestabilite. Nell'ultimo dopoguerra l'area disciplinare del Disegno ha assunto una configurazione precisa, non solo didattica, ma teorica e di studio. Gli ambiti di interesse sono riconducibili alla elaborazione e determinazione degli elementi fondanti di ricerca progettuale nel senso più largo, dalla documentazione alla conoscenza critica, con l'uso del mezzo grafico quale veicolo insostituibile per chi, anche se in campi diversi, interviene a modificare, capire e conservare l'ambiente. La vitalità della ricerca si è poi trasferita nell'organizzazione della didattica con nuovi profili e declinazioni di successo. Questo connubio fra ricerca e docenza deve essere riconosciuto come testimonianza di vivacità della disciplina che ha saputo gestire tradizione ed innovazione e ha messo a disposizione, ad un livello alto, le proprie conquiste scientifiche in ambito didattico e interdisciplinare. Dalle considerazioni effettuate emerge il carattere di un'area che, orientata su obiettivi interdisciplinari, muove sempre dalle competenze specifiche del Disegno. Va preso atto che si tratta di un'area assai ricca di contenuti e di tradizione scientifica e sperimentale che, ponendosi come campo di analisi la matrice formale e l'elaborazione di verifica, può diventare fonte di
