Abstract

Background: In the COVID-19 pandemic, novel regional services and communication channels emerged across all sectors of the German healthcare system. To contribute to pandemic preparedness, this study aims to describe newly established services in response to the COVID-19 pandemic from a stakeholder perspective and to examine the interprofessional communication channels, applying a nationwide cross-sectional approach. Methods: A nationwide sample of German healthcare stakeholders comprising general practitioners, associations of statutory health insurance physicians, hospital medical directors, local health departments, rescue coordination centres, medical directors of emergency services, outpatient nursing services, nursing homes, community care access centres, and hospital nursing managers was surveyed. A web-based questionnaire asked for their level of participation in newly implemented regional COVID-19 services and communication channels. Stakeholders' level of recommendation was measured using the Net Promotor Score (NPS), a metric that assesses their satisfaction towards the services surveyed. Results: In total, 1312 healthcare stakeholders participated in the survey. Diagnostic centres (23.0–90.9%), COVID-19 wards in hospitals (40.5–92.1%), emergency medical vehicles designated solely for COVID-19 patients (16.5–68.4%), and crisis intervention teams (11.6–30.6%) exhibited the highest rates of engagement. The services receiving the highest recommendation for future use were COVID-19 focus practices (NPS: 33.4–43.7), COVID-19 wards in hospitals (NPS: 47.6–84.4), transportation of COVID-19 patients exclusively by predefined professional groups (NPS: 12.5–36.4), and newly implemented digitally supported nursing services (NPS: 58.3–100.0). Telephones emerged as the most frequently used communication channel (58.0–96.7%), while email was the primary digital channel (23.7–81.5%). Conclusions: During the COVID-19 pandemic, Germany experienced significant variation in the implementation of pandemic-related services across healthcare sectors, with stakeholders prioritising services built on existing healthcare structures. Abstract

The National Pressure Injury Advisory Panel, European Pressure Ulcer Advisory Panel, and the Pan Pacific Pressure Injury Alliance are commencing a new (fourth) edition of the Prevention and Treatment of Pressure Ulcers/Injuries: Clinical Practice Guideline. The fourth edition of the International Pressure Injury (PI) Guideline will be developed using GRADE methods to ensure a rigorous process consistent with evolving international standards. Clinical questions will address prevention and treatment of PIs, identification of individuals at risk of PIs, assessment of skin and tissues, and PI assessment. Implementation considerations supporting application of the guidance in clinical practice will be developed. The guideline development process will be overseen by a guideline governance group and methodologist; the guideline development team will include health professionals, educators, researchers, individuals with or at risk of PIs, and informal carers.This article presents the project structure and processes to be used to undertake a systematic literature search, appraise risk of bias of the evidence, and aggregate research findings. The methods detail how certainty of evidence will be evaluated; presentation of relative benefits, risks, feasibility, acceptability, and resource requirements; and how recommendations will be made and graded. The methods outline transparent processes of development that combine scientific research with best clinical practice. Strong involvement from health professionals, educators, individuals with PIs, and informal carers will enhance the guideline’s relevance and facilitate uptake. This update builds on previous editions to ensure consistency and comparability, with methodology changes improving the guideline’s quality and clarity.

25. Publikationen aus dem deutschsprachigen Raum in Zeitschriften mit hohem Impact Factor: Entwicklung und Validierung eines Suchfilters

Abstract

Zusammenfassung.Hintergrund:Das Publikationsaufkommen in Zeitschriften mit hohem Impact Factor ist ein Indiz für die Teilhabe einer Disziplin am internationalen Diskurs. Ein Suchfilter ermöglicht es, definierte Publikationen zuverlässig und reproduzierbar zu recherchieren. Ziel:Entwicklung und Validierung eines geografischen Suchfilters für Publikationen in pflegewissenschaftlichen Zeitschriften mit hohem Impact Factor mit Beteiligung von Pflegewissenschaftlerinnen / -wissenschaftlern aus dem deutschsprachigen Raum. Methode:Der Suchfilter wurde objektiv in mehreren Stufen entwickelt und geprüft: (i) Bildung eines Entwicklungs- und eines Validierungssets, jeweils bestehend aus relevanten und nicht relevanten Publikationen, (ii) Generierung des Suchfilters mittels Textanalyse des Entwicklungssets, (iii) interne Validierung anhand des Entwicklungssets und (iv) externe Validierung anhand des Validierungssets. Die Validität wurde anhand mehrerer Genauigkeitsparameter geprüft, z. B. Sensitivität, Spezifität, positiver prädiktiver Wert (PPW) und Number needed to read (NNR). Ergebnisse:Der Suchfilter identifizierte 22 von 30 relevanten und 16 von 21 nicht relevanten Publikationen korrekt im Entwicklungsset: Sensitivität 80 % (95 % KI 66 − 94), Spezifität 76 % (95 % KI 58 − 94), PPW 83 % (95 % KI 69 − 97). Die externe Validierung ergab ähnliche oder bessere Ergebnisse: Sensitivität 81 % (95 % KI 67 − 96), Spezifität 88 % (95 % KI 71 − 100), PPW 88 % (95 % KI 75 − 100). Die NNR betrug 1,2 bzw. 1,1. Schlussfolgerungen:Der Suchfilter hat das Potenzial, die intendierten Publikationen zu identifizieren.

Abstract

Existing literature reviews on technology and cancer care cannot be easily applied for the development of nurses’ training programs as they either focus on a specific form of cancer, specific technologies, specific use cases of the technology, or specific phases of cancer treatment. Currently, there is no general, overarching overview on the use of new technologies in oncology nursing. Therefore, the overall aim of this review is to scope the literature on all types of nursing technologies used in cancer care, their application and their effects on the patients, the nurses as well as the organisation. Furthermore, the feasibility of considered nursing technologies in cancer care is to be investigated in order to draw conclusions about barriers and facilitators and necessary skills of oncology nurses. Thus, this scoping review will address the following research questions: 1. What kinds of nursing technologies and applications are available and used in cancer care? 2. What effect does the use of new nursing technologies have in cancer care for patients, nurses and health care organisations? 3. How feasible is the implementation of these nursing technologies? Which barriers and facilitators of implementation are reported? Method The search will be carried out in three databases (MEDLINE via PubMed, Cochrane Library and CINAHL). All results of the final search will be transmitted and screened by two independent reviewers. All settings of the cancer care continuum will be considered, as long as implementation of a nursing technology is described and evaluated. Outcomes concerning the patient, the nurse and the organisation and the feasibility will be taken into account. Referring to the Cochrane Effective Practice and Organisation of Care, (cluster) randomized trials, non-randomized trials, controlled before-after studies, interrupted time series studies, repeated measures studies, prospective cohort studies and non-concurrent cohort studies will be included. Studies need to be published between 2011 and 2021 in English or German with an abstract available. Studies will be excluded if they do not fit the inclusion criteria mentioned above. Moreover, reported multi-component interventions including nursing technology, but without providing separate data on technology effects, will be excluded as well. For each study, information about authors, year, study design, aim, setting, participants, type of nursing technology used, provided service of technology, content of technology, duration of intervention/technology use, patient safety and effectiveness of technology on patient, nurse and organisational related outcomes as well as feasibility outcomes will be extracted. Results will be summarised narratively, including tables and graphs, e.g. joint displays about effects on patient related and feasibility outcomes.
