10. Increased risk of non-cancer deaths in patients with malignant brain tumors: observations from the SEER database.

Background: Research on mortality outcomes and non-cancer-related causes of death in patients with malignant brain tumors remains limited. This study aims to identify the prevalence of non-cancer-related causes of death after brain cancer (BC) diagnosis. Methods: Data of patients diagnosed with BC in the USA between 2000 and 2019 from the Surveillance, Epidemiology, and End Results (SEER) database were retrieved and analyzed. Standardized mortality ratios (SMRs) were calculated for all the reported causes of death. Results: A total of 59,663 deaths were reported during the follow-up period. The highest number of fatalities (34,677) occurred within the first year following BC diagnosis. BC-attributable deaths were the leading cause of mortality (55,482; 93%), followed by non-cancer deaths (3888; 6.5%). BC patients had an increased risk of non-cancer attributable deaths (SMR = 6.12; 95% CI 5.93–6.31) including heart diseases (N = 938; SMR = 5.3; 95% CI 4.96–5.65), cerebrovascular diseases (N = 453; SMR = 13.17; 95% CI 11.98–14.44), accidents and adverse effects (N = 227; SMR = 2.97; 95% CI 2.6–3.39), and septicemia (N = 155; SMR = 13.39; 95% CI 11.36–15.67). Conclusion: Compared to the general population, BC patients had a statistically significant higher risk of death from most of the reported causes. [ABSTRACT FROM AUTHOR]

AbstractThis study investigates the quasi-static indentation (QSI), three points bending and Izod impact performance alongside with the damage evaluation of pure glass-fiber laminates (GG) and hybrid laminates with aluminum mesh of two configurations (A1 with Al-mesh at the surface layers and A2 with Al-mesh in the middle layers). The energy absorption capacity, bending performance, and impact strength of the hybrid laminates were found to be enhanced in terms of weight ratio. Hybrid laminates exhibited better energy absorption (17.95%) and impact resistance (up to 63.7%). A1 possessed increased ductility and less damage, whereas A2 offered superior bending strength and toughness. The proposed hybrid composites proved to be an effective choice for fields requiring lightweight, energy-absorbing structures. [ABSTRACT FROM AUTHOR]

14. Role of Adjuvant Treatment in Resected Adult Extremity Soft Tissue Sarcoma.

Background: Soft tissue sarcomas (STS) are a rare and heterogeneous group of tumors, representing less than 1% of all adult malignancies. Objective: This study aimed to assess long-term outcomes of the adjuvant treatment in resected extremity soft tissue sarcoma patients including overall survival, recurrence free survival and local control. Patients and Methods: This retrospective analytical study based on hospital records of seventy-one patients with resected extremities soft tissue sarcoma who presented to Clinical Oncology and Nuclear Medicine Department, Mansoura University Hospitals, Egypt between January 1 st 2012 to December 31st 2017. Results: Most of the patients underwent upfront surgery (N: 64, 90.1%). Half of them, surgery was enough and didn’t receive any adjuvant treatments (N: 35, 49.3%), while the other half received adjuvant treatment either radiotherapy (N: 25, 35.2%), chemotherapy (N: 6, 8.5%), or both (N: 5, 7%). Only seven patients received neoadjuvant treatments (N: 7, 9.9%). The overall survival (OAS) mean in our study was 60.71 ± 26.82 months while 5-years OAS was 69 %. As regards disease free survival (DFS), mean was 48.57 ± 32.16 months and the 2-years DFS was 73.2 %. Metastasis rate was 15.5%. Disease recurrence rate was 23.9%. Conclusion: Grade of differentiation, necrosis, tumor size, adjuvant treatment, and occurrence of metastasis were the most independent prognostic factors of OAS of extremities STS, while grade of differentiation and tumor size were the most independent DFS prognostic factors. - Abstract
Objective: This study aimed to assess 30-day morbidity and mortality rates following cholecystectomy for benign gallbladder disease and identify the factors associated with complications. Summary background data: Although cholecystectomy is common for benign gallbladder disease, there is a gap in the knowledge of the current practice and variations on a global level. Methods: A prospective, international, observational collaborative cohort study of consecutive patients undergoing cholecystectomy for benign gallbladder disease from participating hospitals in 57 countries between January 1 and June 30, 2022, was performed. Univariate and multivariate logistic regression models were used to identify preoperative and operative variables associated with 30-day postoperative outcomes. Results: Data of 21,706 surgical patients from 57 countries were included in the analysis. A total of 10,821 (49.9%), 4,263 (19.7%), and 6,622 (30.5%) cholecystectomies were performed in the elective, emergency, and delayed settings, respectively. Thirty-day postoperative complications were observed in 1,738 patients (8.0%), including mortality in 83 patients (0.4%). Bile leaks (Strasberg grade A) were reported in 278 (1.3%) patients and severe bile duct injuries (Strasberg grades B-E) were reported in 48 (0.2%) patients. Patient age, ASA physical status class, surgical setting, operative approach and Nassar operative difficulty grade were identified as the five predictors demonstrating the highest relative importance in predicting postoperative complications. Conclusion: This multinational observational collaborative cohort study presents a comprehensive report of the current practices and outcomes of cholecystectomy for benign gallbladder disease. Ongoing global collaborative evaluations and initiatives are needed to promote quality assurance and improvement in cholecystectomy.

A technique for making a computer-aided design (CAD) of a complete arch restoration with a fully digital workflow is described. The virtual definitive cast was mounted on a virtual articulator, and the CAD was made in 2 stages: the anterior teeth followed by the posterior teeth. The settings of the virtual articulator were adjusted to make the CAD of anterior teeth by following Hobo and Takayama's twin-stage philosophy to ensure disclusion of the posterior teeth during eccentric movements. Posterior teeth were designed in a second stage guided by the definitive CAD of the anterior teeth, determined by the mandibular movements of the virtual articulator. [ABSTRACT FROM AUTHOR]

Background: Global developmental delay (GDD) represents a measurable lag in a young child's achievement of developmental milestones compared to age matched children. Affection of two or more developmental domains is fundamental for assumption of GDD. Many chromosomal abnormalities are responsible for developmental delay or mental retardation and can be detected using G-banded karyotyping. Aim of the work: This work aimed to determine the yield of karyotyping in children with GDD and/or dysmorphic features in Sohag University Hospital, Upper Egypt. Subjects and methods: All children presenting with GDD and/or dysmorphic features, with abnormal karyotyping or other genetic testing were included. Full history, thorough clinical and detailed neurological examinations were done. The results of other investigations done for the patients, including neuroimaging and electroencephalography (EEG), were utilized (if available). Results: The total number of patients included was 395 patients, out of 646 patients who did karyotype; the mean age of presentation was 24.7 ± 32.1 (SD) months, there were 243 (61.5%) males and 152 (38.5%) females. The positive yield of karyotyping in children with developmental delay and/or dysmorphic features, including classic Down features, was 61.1%; however, with exclusion of Down syndrome and other suspected trisomies, it became 7.4%. The most prevalent chromosomal abnormality was trisomy 21-Down syndrome (364 patients/92.2%), followed by structural chromosomal abnormalities and marker chromosome in 19 patients (4.8%) and, lastly, sex chromosome abnormalities (8 patients/2.0 %). The main complaint was GDD in half of the patients (205/51.9%), while the majority of patients had microcephaly. Conclusion: G-banded karyotyping is a useful tool with reasonable yield in evaluation of children with developmental delay and/or dysmorphic features, especially in countries with limited resources. Corneal dystrophies are inherited disorders characterised by progressive accumulation of deposits in the cornea causing visual impairment. They occur in either an autosomal dominant or recessive form and are usually manifested in the first few decades of life. The present classification is solely based on the layer or layers of the comea involved. This study aimed at better understanding of the underlying molecular basis of such disorders via linkage to specific chromosomal loci and then mutation screening of the disease genes by means of amplification of the genomic DNA using polymerase chain reaction and then sequencing and restriction enzyme digest analysis. A family with a combined comea plana and microphthalmia phenotype was mapped to the autosomal recessive comea plana (CNA2) locus on 12q21. A novel mutation in KERA gene (CNA2) has also been identified. The phenotype described is wider than that previously reported for KERA mutations and represents the first description of a stmctural protein causing microphthalmia. Mutation screening of a new carbohydrate sulfotransferase 6 gene (CHST6) in 5 families with autosomal recessive macular comeal dystrophy (MCD) type I has revealed 6 novel missense mutations, 4 homozygous and 2 compound heterozygous. This study provides further evidence that CHST6 is the MCD gene and the loss of gene function by these mutations may result in production of abnormal keratan sulfate, which would account for the MCD phenotype. Analysis of a human transforming growth factor-β-induced gene (BIGH3) in 36 patients with lattice comeal dystrophy type I (LCDI), Avellino comeal dystrophy (ACD), granular comeal dystrophy (GCD), and Reis-Bucklers' comeal dystrophy (RBCD) has revealed 4 heterozygous missense mutations. In conclusion, molecular genetics may provide a more accurate basis for the diagnosis and classification of comeal dystrophies. This work contributes to our understanding of the role of specific genes in corneal transparency and may help to identify novel therapeutic approaches for these dystrophies that cause significant visual impairment.

Water scarcity is a universal environmental constraint for agricultural sustainability and production. Two field experiments were accomplished during the 2012 and 2013 growing seasons in two sites: the experimental farm of Suez Canal University, Ismailia and Romana Province, North Sinai, Egypt to evaluate 21 genotypes of maize comprising six inbred lines and their 15 F1 crosses for their drought tolerance. The experiments were arranged as a split-plot design with three replications, where moisture levels (100 and 50% of evapotranspiration) and maize genotypes were allocated to main plots and sub-plots, respectively. Results showed reduction in performance for most measured traits in response to water stress with varying degrees with yield plant-1 being the most affected. Inversely, proline and relative water content and anthesis-silking interval were increased. Correlation results confirmed the reduced grain yield with the increasing anthesis-silking interval, and suggested kernels row-1, relative water content, peroxidase activity and rows ear-1 in Ismailia, and rows ear-1, relative water content, peroxidase activity, kernel weight in Romana were indirect selection criteria for increasing yield in water scarcity environments. Principal component (PC) analysis showed that three PCs having Eigen value >1 explained 70.67 and 70.16%; 69.79 and 71.38% of the total variability among genotypes in control and stress conditions in Ismailia and Romana, respectively. The crosses P1×P3, P4×P6, P3×P5 and P1×P5 were classified as drought tolerant under Ismailia and Romana conditions. On the other hand, P1xP4, P3xP4, and P4 were considered as drought sensitive in Ismailia conditions. In addition, P5, P2×P4, P1×P4 and P5×P6 were the most affected by water deficiency under Romana conditions.

Introduction: Acute disseminated encephalomyelitis (ADEM) is an immune mediated disease of the brain. Although it occurs in all ages, most reported cases are in children and adolescents. The aims of this study were to study the clinical pattern and outcome of ADEM in children in a tertiary center in Upper Egypt and to determine the effect of combined use of steroids and IVIg on outcome. Methods: This observational study was carried out from January 2014 through December 2014 in the Pediatric Department of Sohag University Hospital (Egypt). All children diagnosed as ADEM during a one year period were included in this study. The treatments used were IV methylprednisolone followed by oral prednisone taper and intravenous immunoglobulin for severe cases. All studied cases were followed up and reevaluated at three months and six months. We used SPSS version 10 and Chi Square, Spearman’s test and t-test for data analysis. Results: Eighteen children were included in this study (10 males and 8 females), the average age was 5.5 ± 0.9 years. Prodroma was found in 72.22% of the cases while the main complaint was encephalopathy (83.33%) followed by seizures (11.11%). The neurological findings were convulsions in 83.33%, quadriparesis (33.33%), hemiparesis (33.33), bladder involvement (both retention and incontinence) in 61.11%, and cranial nerve affection (11.11%). Demyelination patches were multifocal in 50%, mainly subcortical in 27.78%. Intelligence quotient (IQ) assessment after 6 months follow up showed that 50% were below average, 25% had mild MR while neurological evaluation showed that 75% of our patients were completely cured. The predictors of better outcome were; children related to the age group (1-4 years) (p = 0.01), children with higher GCS (6-14) (p = 0.01), and children who received steroids on the first day of symptoms and intravenous immunoglobulin in the first week (p = 0.03). Conclusion: The clinical pattern of acute disseminated encephalomyelitis is variable, and a disturbed level of consciousness was the most common presentation. The outcome is generally favorable although motor deficit and cognitive impairment were reported. The combined use of steroids and IVIg has substantial effect on the outcome in children with ADEM.

Context Chronic cough in children is a major health problem with an increased prevalence from 8 to 20.78% between 1999 and 2016. Flexible bronchoscope has become one of the most valuable tools in the diagnostic workup of unexplained chronic cough in children. Aims To assess the children with chronic cough using flexible bronchoscopy in order to diagnose the underlying causes that remain otherwise unexplained by noninvasive diagnostic tests. Settings and design Observational cohort study. Patients and methods Children with unexplained chronic cough underwent flexible bronchoscopy during the period from October 2017 to December 2019. Bronchoalveolar lavage (BAL) was done for cytologic examination and microbiologic studies. Results This study involved 60 children, aged from 2 months to 14 years. Flexible bronchoscopy demonstrated positive findings in 65% of cases. The most common finding was congenital airway anomalies in 25% of cases. The second most common finding was visible bronchitis in 21.67% of cases. Other bronchoscopic findings included endobronchial mucous plug (5%), foreign body aspiration (6.67%), extrinsic compression on the airways (10%), and adenoid hypertrophy (1.66%). Cytologic examination of BAL fluid showed that neutrophilia was the most common finding. Haemophilus influenzae was the most common isolated organism in the BAL fluid samples. Conclusions Flexible bronchoscopy is a valuable safe tool in the diagnosis of chronic cough in children. BAL helped in the diagnosis of the underlying etiology by providing samples for cytologic and microbiologic studies.

Retinal detachments are a potentially sight-threatening ophthalmic emergency that may result in significant, irreversible vision loss. The risk of developing retinal detachment increases with advancing age, myopia and trauma. Pre-existing retinal degenerations can precipitate a pre-detachment symptomatic period of photopsia or floaters, allowing clinicians to intervene early and prevent detachments. Novel imaging techniques, such as spectral-domain optical coherence tomography, and well-established topographic modalities, such as B scan, can help to elucidate the type of detachment and any underlying causes, and help with surgical management. The overarching goal of treatment is to identify and seal all retinal holes, relieve vitreoretinal traction and prevent further recurrence. Prompt prophylactic retinopexy of retinal holes and tears is crucial in preventing retinal detachment, the main treatments of which are pars plana vitrectomy, tamponading agents and silicone scleral buckle.

Background: A systematic ultrasound has been validated across a wide span of ages using both chest anatomic structures (ie, ribs , pleural line)and artifact images ,such as the B-lines. Multiple diffuse bilateral B-lines are suggestive of interstitial syndrome . In the neonate, one can accurately differentiate the aerated dry lung, appearing as uniform hypoechogenic pattern, sliding with respiration and showing horizontal (the A-lines) ,from the hyperechogenic “white” pattern of the markedly “wet” or inflamed organ. Objective To study the value of chest ultrasound in neonates with RD And its value in comparison with chest x –rays.Methods: This prospective hospital based study was conducted on 74 neonates (October 2017to September 2018 in the Neonatal intensive care unit at Sohag university hospital with any degree of RD .All cases was involved with physical examination, chest x-ray and chest US. finally data was analyzed by SPSS software.Results: In the all the 74 cases chest ultrasound was applied and the disease was further categorized into RDS 25 cases (33.78%) .Pneumonia13 cases (17.56%), (TTN) 25 cases (33.7%) , (MAS), 9cases(12.16%).Pleural effusion4 cases (5.40%). We found that there no difference between diagnosis of the cases by(clinically ,history , and chest x-rays ) versus chest sonar diagnosis (p value=0.157) which was statistically in significant . Conclusion: no difference between diagnosis of the cases by(clinically ,history , and chest x-rays ) versus chest US diagnosis.Study Limitation:: we conduct only the neonatal group suffered from limited disease as RDS,TTN,MAS ,Pneumonia, excluding cases with congenital malformation .

Background: Coma is a common in PICU, it is a state of deep, unarousable, pathological unconsciousness in which the person shows no response to environmentObjective: determine patterns of coma in patients admitted at Pediatric Intensive Care Unit, Sohag University HospitalMethods: over a one year period ,we collected data from 58 child presented with coma they were subjected to complete medical history with special concern to previous attacks of convulsions, epilepsy, history of neurological diseases, metabolic diseases .Full clinical examination with special concern to degree of consciousness by GCS, neurological examination , manifestation of increased intracranial tension and neck rigidity. Investigation include Laboratory evaluations :CBC, liver and kidney functions ,ABG,C.S.F, EEG,CT scan of the brain, MRI of the brain for all included children except critically ill patients.Results: Total 58 patients in the study. Most common causes and for coma in PICU were complicated Pneumonia in 11 patients (18.97%), followed by complicated CHD in 8 patients (13.79%),DKA in 7 patients (12.07%), Encephalitis in 7 patients (12.07%),GE 5 patients (8.62%),ICH in 4 patients (6.90%), RF in 4 patients (6.90%), Scorpion bite in 4 patients (6.90%),Sepsis in 2 patients(3.45%) , Metabolic acidosis in 2 patients (3.45%) Conclusion: infection is the first cause of comatose pediatric patients in PICU, half of them less than one year, Most common causes for coma in PICU is complicated Pneumonia .There is significant relation between the GCS grade and the outcome as increased in GCS associated with improved outcomeKey words : Children, Coma ,GCS Infection

Neutrophil gelatinase-associated lipocalin (NGAL) is a rapidly emerging biomarker for early detection of acute kidney injury (AKI). The aim of the study is to evaluate the impact of sepsis on serum NGAL in critically ill children and to investigate whether the presence of sepsis affects the ability of serum NGAL to predict AKI. Sixty-eight patients, who met criteria of sepsis and related syndromes, were classified into two groups (septic shock and severe sepsis). They were reclassified regarding who developed AKI into AKI and non- AKI groups. Twenty sex and age-matched healthy subjects served as a control group. Serum NGAL was assayed using Enzyme-linked Immunosorbent Assay (ELISA), and serum creatinine was measured using the kinetic spectrophotometric method. Serum NGAL levels were significantly high in critically ill septic patients compared to healthy controls (median: 100.4ng/ml vs. 47.1 ng/ml, P>0.0001), and were significantly higher in septic shock (median 105.1ng/ml) than in severe sepsis (median 96ng/ml) with P values of 0.005. However, there was no significant difference in the levels of serum NGAL between AKI patients and non- AKI patients (P=0.3). Receiver operating characteristic (ROC) curve analysis of serum NGAL for prediction of AKI in critically ill septic children showed an area under the curve (AUC) was 0.56 (95% C.I. =0.42-0.70) with an optimal cutoff value of 102.5ng/ml, sensitivity=56.2%, specificity 55.6%, PPV =52.9%, NPV =58.8%, and accuracy=55.9%. For serum creatinine, the AUC was 0.97 (95% C.I. =0.94- 1.00) with an optimal cutoff value of 0.6 mg/dl, sensitivity= 90.6%, specificity 91.7%, PPV=90.6%, NPV=91.7% and accuracy=91.2%. In conclusion, serum NGAL is raised in critically ill septic children and is a marker of bacterial infection and systemic inflammation. However, in AKI associated with sepsis, serum NGAL is not a specific biomarker for the prediction of AKI and it loses its early predictive property. In such patients, serum creatinine is more specific than serum NGAL.

Background: Infantile spasms (IS) are a unique form of seizure disorder that their occurrence is almost entirely limited to infancy (the first year of life) and they are refractory to conventional anticonvulsant drugs. IS usually are associated with developmental retardation or deterioration and a characteristic electroencephalographic (EEG) pattern (hypsarrhythmia) that together form a syndrome . IS involves a sudden, generally bilateral and symmetric contractions of muscles of the neck, trunk, and extremities. Patients with IS can be classified into focal IS and diffuse groups. Objective: Our main objective in this study is to evaluate the clinical profile, laboratory data, and outcome of children having infantile spasms in Sohag University Hospital. Methods: This retrospective study was conducted on 540 patient (mean age 8.4 ± 6.9 months.: ) from December 2015 to November 2016 in neuropediatric outpatient Clinic, Pediatric department at Sohag University Hospital. Detailed clinical data of the patient, Investigation done for the patient including the neuroimaging (MRI or CT) , EEG pattern, basic metabolic studies , serum enzymes and Karyotyping were retrospectively collected from files and records . Results: In this study ; ( 66.1%) of cases were males.(33,9% ) of cases were females ,mean age of presentation (8.4± 6.9 months). Positive consanguinity was found in (7.6%) of cases. (79.2%)of cases presented with flexor spasms, (8.9%) with extensor spasms and (11.9%) with mixed spasms . (8%) of cases were idiopathic, (13.5%) of cases were cryptogenic and (78.5%) of cases were symptomatic,(15.2%) of cases showed favorable outcome, and (84.8%) cases showed unfavorable outcome, of them (11.5%) of casesdeveloped othertype of seizures, (54.3%) of caseshad mental retardation, (28.8%) of cases diedand (5.2%) of cases had autistic features . thy objective of this study was to evaluate the use of central venous catheters (CVCs) in the Pediatric intensive care unit (PICU) of Sohag University Hospital . We reviewed the records of all children that had CVCs and were hospitalized between 1st of January 2016 to the end of December 2016. Patients were evaluated with respect to their age, gender, catheter type, indication for CVC insertion, site and side of the body of CVC insertion. The duration of catheter use and eventual complications were also taken into consideration. A total of 115 CVCs were inserted in 100 children. Patient age ranged from 1 month to 16 years. The average catheter insertion time was 12 days. We noted 80 (80%) CVC-related complications. Complications related to CVCs insertion were infection(40%),occlusion(10%),local edema(8%),hematoma(5%), pneumothorax(4%),catheter displacement(4%),hemothorax(4%),thrombosis(3%),air embolism(2%) were complications associated with lenght of CVCs use. We conclude that central venous catheterization is a safe and efficient procedure with many complications in pediatric patients.
