Objective Extremely low birth weight (ELBW) infants often receive transfusions of packed red blood cells (PRBCs). Long-term outcomes of infants treated with liberal versus restricted transfusion criteria have been evaluated with conflicting results. Clinicians incorporate a reticulocyte count (RC) in their transfusion decisions. There is a lack of information on reference ranges for RCs in growing ELBW infants and whether infant's chronologic age or corrected gestational age (GA) generates a specific trend in the RCs. Our aim was to evaluate the levels of RCs obtained from ELBW infants over the course of the initial hospitalization. Study Design A retrospective chart review of ELBW infants treated in the neonatal intensive care unit (NICU) and had RCs performed. We analyzed the RCs to observe trends based on the chronologic age and corrected GA. Results A total of 738 RCs were analyzed. A positive trend in RCs that reached a peak at 32 to 34 weeks' corrected GA and then experienced a downward trend was observed. Conclusion Our report examines a very common hematologic test that is theoretically helpful but is in need of guidelines concerning the appropriate frequency of testing and its utility in making transfusion decisions in ELBW infants. Background: The Unani formulation Majun Brahmi (MB), a combination of herbs, is used in India as a brain tonic and memory enhancer. Aluminium deposition in the brain is associated with the development of neurodegenerative diseases. Aims and Objectives: The present study was designed to observe that the effects of MB have been evaluated on aluminium trichloride or aluminium chloride (AlCl3)-induced cognitive impairment in an experimental rat model. Materials and Methods: Twenty male Wistar albino rats were divided into four groups of five rats each. AlCl3 was administered orally for 30 days to induce cognitive impairment. Group I received saline, Group II-AlCl3 (100 mg/kg b.wt), Group III-MB (1027.77 mg/kg b.wt), and Group IV-AlCl3 + MB (100 mg/kg b.wt+1027.77 mg/kg b. wt). At the end of the experiment, rats were subjected to behavioral and biochemical assessments. Results: Animals treated with AlCl3 showed a significant increase in time to reach the platform in the Morris water maze test (MWM), prolonged transfer latency (TL) in the elevated plus maze, and decreased step-down latency in the passive avoidance test, as compared to controls (P<0.01). Cotreatment with MB resulted in a reduced time to reach the platform in MWM, increased step-down latencies, and decreased TL. AlCl3 induction significantly increased malondialdehyde and decreased superoxide dismutase, glutathione reductase, glutathione, total antioxidant capacity, and catalase levels. Concomitant administration of MB significantly attenuated the effects AlCl3 on lipid peroxidation and restored the reduced antioxidant parameters. Conclusion: The study provides strong evidence for the potential use of MB in the treatment of neurodegenerative disorders like Alzheimer's disease. [ABSTRACT FROM AUTHOR]

This case report describes a novel mutation of the SPTB gene as a potential pathogenic cause of spherocytosis. A 3-week-old male presented with clinical and laboratory signs consistent with hemolytic spherocytosis, including jaundice, hyperbilirubinemia, anemia, reticulocytosis, negative Coombs test, no ABO or Rh incompatibility, and a peripheral blood smear notable for numerous spherocytes. His laboratory work demonstrated persistent anemia despite daily folate prompting next-generation sequencing which revealed a novel mutation in the SPTB gene resulting in a nonfunctioning protein product. Background: Vitamin D deficiency can occur when usual intake is lower than recommended levels over time, exposure to sunlight is limited, the kidneys cannot convert 25(OH)D to its active form, or absorption of vitamin D from the digestive tract is inadequate. Decreases vitamin D levels show complex relationships with diseases, which can affect the inner ear. The aim of study: To examine the level of serum 25-OH vitamin D concentrations on patients diagnosed with BPPV. Patients and Methods: This study included 60 subjects; 40 subjects with BPPV (study group) and 20 healthy persons (control group). Both groups underwent full history taking, vitamin D level assay, otological examination and audio-vestibular evaluation. Results: Subjects with BPPV (study group) have lower vitamin D levels than norms (control group). The recurrence of BPPV occurs in subjects have lowest vitamin D levels. Vitamin D deficiency more in elderly & females. Hearing may be affected. Conclusion: This study supports the relationship of lower vitamin D level with the occurrence of BPPV.
