This is the Routine Groundwater Monitoring Report, as required under Part I.F.1 of State of Utah Groundwater Discharge Permit No. UGW370004 (the "GWDP") for the fourth quarter of 2018 for Energy Fuels Resources (USA) Inc's. White Mesa Uranium Mill. As required under Parts I.E. 1 , I.E.2 and I.E.5 of the GWDP this Report includes all recorded field measurements and laboratory analyses for well monitoring conducted during the quarter.
