Abstract

Surgical treatment of carotid artery bifurcation disease has evolved considerably over the years, and has probably now reached maturity. Since the original procedures on the carotid vessel, an enormous number of operations have been performed, generating criticism for which randomized trials were necessary to establish clinical value. Progressive refinements in the surgical techniques with shunting and patching, intraoperative monitoring, and postoperative hemodynamic assessment have consistently improved the results of the operative treatment. Endovascular procedures now offer a new, interesting challenge. As far as the postoperative results of carotid endarterectomy are concerned, the gold standard has been identified, and the centers and surgeons who want to adopt this surgical procedure should conform
