Surgical treatment of carotid artery bifurcation disease has evolved considerably over the years, and has probably now reached maturity. Since the original procedures on the carotid vessel, an enormous number of operations have been performed, generating criticism for which randomized trials were necessary to establish clinical value. Progressive refinements in the surgical techniques with shunting and patching, intraoperative monitoring, and postoperative hemodynamic assessment have consistently improved the results of the operative treatment.
