Abstract

HAMPI is a computerized clinical decision support system focused on ensuring efficacy and / or avoiding the toxicity of narrow therapeutic drugs, renal elimination and those nephrotoxic agents that may aggravate renal damage; so, that it performs a personalized calculation of the dose for each patient based on their renal function. Renal function is estimated by creatinine clearance, obtained by the modified Cockcroft-Gault formula based on ideal body weight and body surface area; withclearance, the drug dose is calculated through the Dettli nomogram which handles drug variables as Qo and the standard dose for patients with normal renal function, data found in the program database. At the same time, it allows the notification of Adverse Drug Reactions (ADR) through the yellow card form proposed by the Ministry of Public Health of Ecuador (MSP), which was integrated with the Karch and Lasagna algorithm to evaluate the relationship of Causality between medication and ADR. For the validation of the doses calculated by the program, a retrospective study was carried out on patients from the year 2015 diagnosed with CRS at the Vicente Corral Moscoso Hospital, who were prescribed at least one drug susceptible of adjustment, where it was compared the dose calculated by HAMPI with the dose indicated in the international guidelines versus the dose administered in this health home.
