Abstract
- Leg ulcers are costly and slow to heal, and after healing, up to 70% recur. Health systems are slow to embrace an effective long-term chronic disease management and health promotion approach needed for effective management of older adults with peripheral vascular disease, who experience years of leg ulceration and recurrence cycles. This presentation will provide results of health and cost-effectiveness outcomes from three community health service models of care for adults with chronic leg ulcers resulting from peripheral venous and/or arterial disease.
