Case management issues in rural long-term care models

The article examines the Program of All-Inclusive Care for the Elderly and the Social Health Maintenance Organization for their applicability to rural long-term care settings. While some components of the programs rely on well-established provider infrastructures, other components are better suited to rural applications, such as integration of community-based, social, and medical services.
